Cutaneous sarcoidosis in Caucasians.

One hundred eighty-eight Caucasian patients with cutaneous sarcoid lesions were studied prospectively. Twenty-five had erythema nodosum, while 163 had infiltrative cutaneous lesions. One hundred thirty-eight patients had systemic as well as cutaneous lesions, and fifty had cutaneous lesions only. All types of clinical lesions were seen among patients with cutaneous lesions only. The extent of cutaneous lesions did not correlate with the extent of systemic disease. Papular lesions were relatively uncommon and occurred as the only manifestation of the disease or were associated with hilar adenopathy and acute disease. Lupus pernio, scar infiltrates, and plaque lesions were the most common clinical lesions and were typically chronic and commonly associated with pulmonary mottling and/or fibrosis. Forty-eight of 127 had a histologically positive Kveim test. Among patients followed for more than 2 years, dinitrochlorobenzene (DNCB) sensitivity was higher among those with acute disease than among those with chronic disease. Seventy-nine patients with infiltrative cutaneous lesions were followed until the cutaneous lesions had healed. Those with lupus pernio were often left with unsightly telangiectatic scars, while the other types of lesions left either pale, slightly depressed scars or no scars at all.

Clinical and immunological studies in chronic dermatophytosis caused by Trichophyton rubrum.

Twenty-six patients with chronic dermatophytosis (CD) by Trichophyton rubrum (TR) through one year were included in a double-blind clinical trial with Levamisole/Placebo. All patients were treated with griseofulvin concomitantly. Before, during and after the trial, studies of the cellular and humoral immune response were performed. The result of treatment with griseofulvin plus levamisole (62.5% cured or improved) did not differ from that of griseofulvin plus placebo (58% improved). All patients but one showed normal general cellular immune responses. Stimulation of lymphocytes with a specific TR antigen extract showed a significantly stronger response in the patient group than in the group of 39 controls (p = 0.01-0.00003) at all investigation dates. Immediate weal reactions to the TR extract were seen in 42% of the patients and increased concentration of IgE antibodies was demonstrated in 46%. Specific IgG antibodies towards TR were observed in 42% of the patients. The presence of specific IgG antibodies and a strong specific cellular immune response were related to the severity of clinical involvement. Environmental factors and atopy were associated disorders in 81% and 77% respectively.

Cutaneous sarcoidosis: an immunofluorescence study.

Biopsy specimens from granulomatous skin lesions of 14 patients with active sarcoidosis were examined by immunofluorescence microscopy. In seven lesions, deposits of IgM, IgA or complement C3 were demonstrated in the dermal vessel walls and/or at the dermal-epidermal junction. Similar deposits were found in three of twelve biopsy specimens from clinically normal skin from a buttock of the same patients. The results support the hypothesis that deposition of circulating immune complexes in the vessel walls may be of importance for the development of granulomatous lesions in sarcoidosis.

Capillary microscopy of the nail fold in patients with psoriasis and psoriatic arthritis.

The Capillary characteristics of the nail fold area were examined and compared by capillary microscopy in 31 patients with psoriasis (PS) and 34 patients with psoriatic arthritis (PA). 70 healthy persons served as controls. Significantly shorter mean capillary loop lengths and characteristic morphological capillary changes were found both in patients with PS and in patients with PA as compared to the controls. The number of hemorrhages and sluggishness of blood flow was more pronounced in both groups of patients as compared to the control group, but only significantly so in patients with PA. The only significant difference between the PS and the PA groups was a higher subpapillary plexus visibility (SPV) index in the latter. It is concluded that most of the capillary changes demonstrable in patients with PA are clinical manifestations of PS. The difference in the SPV index, however, warrants a follow-up study to elucidate whether patients with PS and a high SPV index are likely to develop arthritis.

Lymphocyte transformation in vitro in acute dermatophytosis: a follow-up study.

Sixteen patients with recently acquired (= nonchronic) dermatophytosis were studied clinically and immunologically before, during and after treatment for 6 months. The methods applied were lymphocyte transformation in vitro (LTT) and intra-dermal test (IDT). Phytohaemagglutinin, pokeweed mitogen, purified protein derivate (PPD) and water-soluble extracts of Trichophyton (T) rubrum, T. mentagrophytes and Candida albicans were used as stimulators. In the LTT the patients in all four studies showed normal general immune responses. Compared with a control group of 22 healthy persons, lymphocytes of the patients showed significantly stronger stimulation with the T. rubrum extract (0.01 greater than p greater than 0.001) throughout the 6 months. The degree of stimulation after one month (the second study) was significantly stronger (0.05 greater than p greater than 0.01) than in the other studies. The stimulation in the last (fourth) study was the lowest (not significant). IDT showed a majority of delayed-type reactions (47-67%) in patients as compared with 7-13% in the controls, when using dermatophyte antigens. Three patients had an immediate-type reaction or mixed reactions. A correlation was found between the severity of lesions and the degree of stimulation in LTT (p less than 0.02 for T. rubrum, p less than 0.007 for T. mentagrophytes). In contrast, no relationship was found between IDT and clinical symptoms, and there was only a slight tendency toward correspondence between the results of IDT and LTT.

Circulating antibodies in rabbits against a Kveim Material.

For 1 year five rabbits were sensitized with a solubilized Kveim material. After 50 days, precipitating antibodies were demonstrated by passive immunodiffusion. Crossed immunoelectrophoresis using Kveim material against concentrated rabbit serum showed ill-defined precipitates, while two well-defined precipitates were seen when rocket immunoelectrophoresis was used. One of the precipitates probably represented an antibody against human serum protein in the K veim material, while the other may have been a specific antibody against the K veim material. No precipitates were seen when rocket immunoelectrophoresis was applied to sera from two patients with active sarcoidosis.

Capillary microscopy of the nailfold in psoriatic and rheumatoid arthritis.

The nailfold capillaries of 35 patients with psoriatic arthritis and 30 with rheumatoid arthritis were studied by capillary microscopy and the findings compared. Seventy healthy subjects served as controls. The pronounced subpapillary plexus visibility, greater number of vessels and their elongation are indicative of rheumatoid arthritis, while shorter, fewer capillaries and especially characteristic psoriatic capillaries, when present, suggest psoriatic arthritis. Sluggish blood flow, higher number of hemorrhages and higher subpappillary plexus visibility are common to both patient groups as opposed to the controls.

Psoriatic arthritis treated with oral zinc sulphate.

Twenty-four patients suffering from psoriatic arthritis participated in a double blind cross-over trial of peroral zinc sulphate versus placebo (Trial I). Eleven patients continued an open trial of peroral zinc sulphate for an additional 24 weeks (Trial II). Remission was assessed by the disappearance of symptoms (overall condition, morning stiffness, functional capacity of the joints and joint pains), and signs (mobility and swelling of the joints). Reduction of joint pains as well as increase of mobility and decrease of swelling of several joints were observed. the clinical signs of reduced inflammation were accompanied biochemically by reduction of serum immunoglobulins and an increase of serum albumin. The need for analgesics was diminished. Severe side-effects and changes in the psoriatic skin involvement were not seen. Oral zinc sulphate seems to be valuable in the treatment of psoriatic arthritis.

Superior mesenteric angiography and blood flow measurement following intra-arterial injection of prostaglandin E1.

The hemodynamic effects of intra-material prostaglandin E1 (PE1) administered via the angiographic catheter were evaluated in 14 patients. Blood flow in the superior mesenteric artery (SMA) after PGE1 infusion was compared with control SMA blood flow as measured by the "spillover" angiographic reflux method, using a specially designed injector/film-changer system. In all patients, SMA blood flow increased by more than 50% and visualization of the SMA and portal vein was enhanced.

Scopulariopsis onychomycosis treated with natamycin.

7 patients with Scopulariopsis brevicaulis onychomycosis of the toe-nails were treated locally with 1% natamycin in 60% dimethylsulphoxide. After 3-5 months treatment the fungus could no longer be isolated from the nails, and at follow-up 15 months after the completion of the treatment, 2 patients reported permanent improvement of the nails and 3 a complete cure. In 1 patient the treatment was ineffective.

Negative chemical ionization of alcohols.

Reactions of the oxygen radical anion at low pressure (ICR conditions) with aliphatic alcohols are seen to give a limited number of product anions which allows for unambiguous determination of the structure of the neutral. The reactions of OH- are also investigated and mechanisms of reactions are discussed. Analytical application of NCI is also considered. OH- negative chemical ionization mass spectra, negative metastable ion spectra, and negative collisional activation spectra (negative product from negative precursor ions) have been applied to stereochemical problems of cyclic diols. The spectra of the cis and trans isomers of 1,3- and 1,4-cyclohexanediols and 1,2- cyclopentanediols show substantial and characteristic differences. The (M-H)- alkoxide anions produced by OH- reagent ions are stabilized for the cis isomers by formation of an intramolecular hydrogen bridge involving both oxygen functions. However, trans geometry of the (M-H)- ions is indicated by abundant (M-H3)- and/or (M-H-H2O) fragment ions. The stereospecificity increases with ion internal energy (high temperature, collisional activation) or especially with ion life times (metastable ions). It is shown that in F- negative chemical ionization the relative intensities of the fluoride attachment ions MF- and of the proton abstraction ions (M-H)- reflect the stereochemistry of the cyclic diols. The results demonstrate the potential of negative ion mass spectrometry for stereochemical investigations.

Cutaneous Metastases simulating other dermatoses.

Two patients with unusual cutaneous metastases from internal cancer are presented herein. In a patient with breast cancer, cutaneous metastases developed which appeared as erythema annulare centrifugum. A patient with prostatic cancer had cutaneous metastases which mimicked pyodermas. The importance of skin biopsy as a diagnostic tool is emphasized.
