Fifty patients were hospitalised at Vientiane during May and June 1975. Infection was caused by consumption of pork meat (som-mou, lap mou and lap leuat). After an incubation period of 8 to 11 days intermittent diarrhoea followed by constant fever and orbital oedema occurred. Several days later diffuse myalgias occurred. Three patients presented neurological symptoms: left hemiplegia flexible paraplegia and limb paresis; these symptoms regressed spontaneously. 87% of the clinically suspected cases had specific serum antibodies (IFI, ID, IE). Hyperleucocytosis and hypereosinophilia were a constant factor. Anti-AH antibodies were detected by the Widal test. The histopathology characteristic of myositis was seen in the 12 muscle biopsies. Also observed was the precystic state of the larvae which is in agreement with the early timing of the samples which were obtained during the 3rd and 4rd week of infection.

Usual media are not convenient for the determination of minimal inhibitory concentrations (MIC). An agar medium without haemoglobin and seric proteins (ABNG) is proposed. Using the dilution method with less fastidious bacteria, including Neisseria sp., MIC were identical on Mueller-Hinton agar and ABNG. By using diffusion techniques, the correlation was good between MIC and zone diameters; the density of inoculum for gonococci was 2 x 10(6) CFU/ml. Dilution and diffusion techniques were used for penicillin, ampicillin, thiamphenicol, tetracycline and spectinomycin. It appears that the diffusion technique with ABNG medium would simplify the antibiotic sensitivity survey for N. gonorrhoeae, dilution technique remaining the reference method.
Polymyositis developed in a patient who had received a bone marrow transplant for the treatment of acute lymphoblastic leukemia 20 months earlier. It was a manifestation of chronic graft-versus-host disease associated with sclerodermatous changes of the skin and sicca syndrome. Polymyositis was treated successfully by polyvalent human intravenous immunoglobulin and methotrexate.

During the course of a recent epidemic of trichinosis two immunodiagnostic techniques were tested. Firstly indirect immunofluorescence (IF) using sections of the diaphram of infected mice and isolated trichine larvae. Secondly gel precipitation reactions, immunodiffusion and immunoelectrophoresis. It was observed that the induced specific antibodies appeared later than the clinical symptoms and the hypereosinophilia; the delay in appearance of the first positives detected being at least two to three weeks. Subsequently the titres rose rapidly and raised levels persisted for several months. Indirect immunofluorescence of excysted larvae remains the technique of choice, being rapid, sensitive and quantitative. The association of fluorescent and precipitation technique reduces the errors caused by cross reactions with other helminths.
A 16-month prospective study of adult patients who had been suffering from arthritis for less than one year was conducted in the Internal Medicine department of the hospital of Kigali, Rwanda. Seventy-six patients were included in the study. Out of 72 patients whose sera were tested for human immunodeficiency virus (HIV), 52 were positive, this being 72.2 percent compared with the 20.7 percent figure observed in the urban population of adults in Rwanda. Two main groups arthropathies were individualized: septic arthritis (24 patients including 19 seropositive subjects) and inflammatory rheumatism with criteria of spondyloarthropathies in some of them (34 patients, 32 tested, 28 serum positive). Thus, the finding of acute arthritis in a region with high HIV infection prevalence should suggest a possible HIV infection, especially when the arthritis itself is suggestive of this diagnosis.
