Diabetes mellitus and exercise: application of research to activity planning.

Current research shows that the response to exercise is not identical for insulin dependent and non-insulin dependent diabetic clients. Contrary to what has been practiced, exercise cannot be prescribed in a general way for these individuals. Exercise offers benefits in weight reduction, lowered blood pressure and changes in lipoprotein profile for non-insulin dependent diabetics. For the insulin-dependent person, regularity of exercise must be emphasized to achieve stable metabolic control. Depending on the severity of the pathology, exercise may provoke microvascular and neurological changes or altered absorption of insulin. Illustrative of the known risks are triggering of angina pectoris, foot damage and persistent elevation of blood glucose. Optimally designed exercise programs are based on life style and are preceded by exercise stress testing for determination of cardiovascular fitness.

Correction of ascites with peritoneovenous shunting: a study of clinical management.

Although a medical regimen, with salt and water restriction and a progressive program of diuretics, ordinarily achieves relief of ascites in advanced liver disease, there has been rapid proliferation in the use of peritoneovenous shunting for persons who appear resistant to normal conservative therapy. Circumstances that justify this surgical approach have not been firmly established. Indications of a satisfactory result in the immediate postoperative period are hemodilution, increase in urine volume, decrease in body weight, and decrease in abdominal girth. Normalization of sodium excretion and serum BUN and creatinine levels are noted in patients with hepatorenal syndrome. Most commonly encountered complications are shunt malfunction, a coagulopathy that leads to generalized bleeding or to thrombosis of the shunt, ascitic fluid leak, and infection. Clarification of the risks and benefits of the procedure is yet to be determined through carefully designed clinical trials.

Action of chronic peracetic acid (Wofasteril) administration on the rabbit oral mucosa.

Peracetic acid Wofasteril) is increasingly made use of as a disinfecting fluid. Because of its high effectiveness it has found widespread use in medical practice. The action of Wofasteril on the oral mucosa during long-term action is studied by aid of an "oral tank". After a test period of 11 months peracetic acid only caused a slight ballooning of the superficial layers and enhanced vascularization of the submucous tissue. The results of the experiment suggest the intraoral administration of peracetic acid in oral surgery and stomatology.

[Otorhinologic and endoscopic findings in cleft palate patients with special reference to velopharyngoplasty].

Our findings (190 patients of whom 45 underwent follow-up examination) permit the conclusion that the velopharyngoplasty according to the principle of Sanvenero-Roselli, as modified by Heiner, does not lead to an increase in tympanic findings (preoperatively, 73%; postoperatively, 71%). The evaluation of the results from tube function testing by means of the pressure difference method reveals an improvement in tuber aeration (preoperatively, 76% of pathological results of the hearing test; postoperatively, 55.6%). This is attributed to a functional improvement of the velopharyngeal mechanisms after velopharyngoplasty. The use of postrhinoscopic nasal endoscopy in the cleft patient and the results obtained are dealth with. The procedure permits to investigate the functional processes in the velopharyngeal region under the conditions of phonation.
