Evaluation of a new immunoassay for determination of phenytoin and phenobarbital: results of a European collaborative control study.

The performance of a new enzyme multiplied immunoassay technique (EMIT) was compared with other current methods, namely gas-liquid chromatography and thin-layer chromatography, for the determination of phenytoin and phenobarbital in serum. Forty-three serum samples were sent as unknowns to the participating laboratories for determination. The precision of repeated determinations was very similar for EMIT and chromatography. There was good agreement among gas chromatography, thin-layer chromatography, and EMIT results. Interlaboratory variability was lower for EMIT determinations of both antiepileptic drugs. The rapid analysis of small samples, made possible by the EMIT system, could have beneficial effects on the treatment of epilepsies.

Dopamine effects on circulation and myocardial oxygen supply.

We measured the action of dopamine given intravenously at dosage ranging from 2.5 to 320 micrograms/kg per min in closed chest anaesthetized dogs. Dopamine produced a dose-dependent increase in heart rate, cardiac index, mean arterial pressure, total peripheral resistance, pulmonary artery pressure, left ventricular end diastolic pressure, coronary flow and myocardial oxygen consumption. At dopamine dosage of 80-320 micrograms/kg per min, the coronary vascular resistance, the stroke volume index, the efficiency of heart work and the central venous pressure are all decreased. The maximum effect of dopamine on the circulation was seen at a dose between 40 and 80 micrograms/kg per min.

[Core build-ups from composites--evaluation of the technics of application].

The advantages and disadvantages of composite materials when used in direct core restoration of endodontically treated teeth is discussed. With the aid of dye penetration tests and scanning electron microscopy, in vitro fabrication of gold and amalgam cores were compared to those of 3 different composite core techniques. A compression technique used in the application of composite to the Radix -anchor post was judged successful. Neither modification of preparation nor application of a sealer before composite insertion improved margin adaptation or reduced microleakage of restoration margins. Acid etching of dentin definitely decreased marginal quality; therefore, for core construction using composites, the acid etch technique is not indicated. The proper use of composites is superior to gold and comparable to amalgam in core fabrication of endodontically treated teeth, when evaluating marginal adaptation and the reduction of microleakage at the composite-dentin interface.

Interlaboratory variability of valproic acid determinations.

19 pooled plasma specimens were sent as unknowns to 13 participating research laboratories. The interlaboratory variability between the results was very high. Only 4 out of 13 laboratories had 6--12% of their results within the 95% confidence limit for each sample. The precision of repeated determinations was fairly good. 6 out of 10 participating laboratories had a coefficient of variation of less then 5%. The reproducibility and the agreement between the different procedures for quantitative analysis of dipropylacetate is similar to that reported for other major antiepileptic drugs.

[Occipital blindness in juvenile, spongiform glio-neuronal dystrophy (author's transl)].

A 17-year-old patient with spongiform glioneuronal dystrophy is described. Symptoms of a severe infection at the onset of this disease give an insight on the unknown etiology. Occipital visual disturbances were the main signs in this case. 5 years after the onset of the disease death occured. The pathological-anatomical findings were: necrosis of ganglion cells in the visual cortex, severe loss of Purkinje- and granular cells of the cerebellum with spongy changes and cavities in the occipital lobe. Slight loss of ganglion cells was to be seen in the striata area, thalamus, dentate nucleus, and in the inferior olive. In addition, there was a demyelinisation of the posterior funiculi.

[The effect of euthyscope- and occlusion therapy in cases of convergent strabismus. Comparative, prospective examinations (author's transl)].

1. 63 children with strabismus and amblyopia (average age 7 years) were chosen at random to be treated either with occlusion or with the Euthyscope followed by occlusion. No significant difference was apparent after comparison of both forms of treatment. 2. Six more children were occluded for several months without success and were then treated for two months with the Euthyscope followed by further occlusion. In four of these cases pleoptic treatment proved to be temporarily successful. 3. In addition, 14 children with microstrabismus and amblyopia, and 17 children with amblyopia ex anisometropia showed a distinct improvement of the visual aciuty after occlusion. An 18-year-old patient with microstrabismic amblyopia achieved normal visual acuity. 4. Euthyscope treatment was used in amblyopic patients who had lost their good eye. In the case of an 18-year-old patient the distant visual acuity increased to 0.6. The increase in the near vision and the speed of reading continued to improve for several years. 5. Papilla-Macula-Relationship: 47 amblyopes were compared with 8 anisometropic and 16 microstrabismic patients as well as with 32 normal subjects. In these groups no significant difference in the papilla-maculadistance was apparent. 6. Refraction: In amblyopic eyes there were no significant refractive errors between fixing and non-fixing eyes. Conversely, in patients with microstrabismus and those with anisometropic a significant difference was found.

[A comparison of cardiovascular effects of dobutamine and dopamine (author's transl)].

The influences of dobutamine and dopamine 5-40 microgram/kg-min intravenously on hemodynamics and myocardial oxygen consumption were investigated in closed chest dogs (n=9). Heart rate (HR), cardiac index (CI), stroke volume (SVI), mean aortic pressure (MAP), pulmonary artery pressure (PAP), central venous pressure (CVP), left ventricular enddiastolic pressure (LVEDP), myocardial blood flow (MBF) and maximum dp/dt (dp/dtmax) were measured. Total peripheral resistance (TPR), coronary vascular resistance (CVR), myocardial oxygen consumption (MVO2), efficiency of heart work (EME), the ventricular volumes (EDV, ESV) and the ejection fraction (EF) were calculated. When dopamine was infused, the cardiac output rose mainly by an increase of heart rate. During dobutamine an increase of stroke volume and ejection fraction was involved in the improvement of cardiac output. Heart rate and mean aortic pressure increased to a greater extent by dopamine. Dobutamine and dopamine increase myocardial blood flow and oxygen consumption. The comparatively slight effects of dobutamine on afterload and heart rate resulted in smaller increases in myocardial oxygen consumption. The efficiency of external heart work was increased by dobutamine at the doses of 5 microgram and 10 microgram/kg-min. In the higher dose range external myocardial efficiency decreased under dopamine and dobutamine. Load data and heart rate indicate that a greater inotropic effect of dobutamine compared with equal doses of dopamine is involved in the increase of dp/dtmax. The results are discussed in relation to a clinical use of dobutamine and dopamine.

Lateral diffusion, order parameter and phase transition in phospholipid bilayer membranes containing tocopheryl acetate.

Lateral diffusion coefficient and order parameter measurements were made with pyrene excimer optical probes and fatty acid spin label probes respectively in pure dipalmitoyl phosphatidylcholine membranes and in membranes doped with tocopheryl acetate. The investigation shows, that the lateral diffusion coefficient for pyrene in dipalmitoyl pholphatidylcholine membranes is decreased whereas the order parameter of the fatty acid chains is slightly increased in the inner part of the membranes by the addition of tocopheryl acetate. The fluid-solid equilibrium phase diagram of dipalmitoyl phosphatidylcholine/tocopheryl acetate mixed membranes has been constructed from the measurements of the partition of (2,2,6,6-tetramethylpiperdine-1-oxyl) TEMPO spin labels between lipid and aqueous regions as function of temperature. In the membranes tocopheryl acetate induces a strong broadening of the temperature range of the phase transition. At low tocopheryl acetate concentrations dipalmitoyl phosphatidylcholine and tocopheryl acetate seem to be completely miscible in the solid and in the liquid crystalline state.

Measurement of diphenylhydantoin and phenobarbital by enzyme immunoassay and gas-liquid chromatography.

A new enzyme-multiplied immunoassay technique (EMIT) was compared with Kupferberg's gas-liquid chromatography (GLC) for the determination of diphenylhydantoin and phenobarbital. 116 plasma samples of patients with epilepsy were examined simultaneously by EMIT and GLC. The precision of repeated determinations was similar for both procedures. There was good agreement between the EMIT and GLC results. The rapid and precise analysis, made possible by the EMIT system, is useful for the treatment of epilepsies.

[End position nystagmus caused by prolonged lateral gaze (author's transl)].

Electronystagmograms were recorded from six normal subjects who had to maintain an extreme sideward gaze either in the dark or using a fixation target. End position nystagmus occured in three subjects immediately, in two subjects with a latency of several seconds, and in one subject with a latency of 90 seconds. The nystagmus was typically dissociated and was similar whether the subject viewed a fixation point or was recorded from in darkness.

[Clinical examination of the conception '86 of "Canoer Multistep-Therapie" (CMT) by M. v. Ardene. First communication: the standard, the control-parameters, and the course of therapy (author's transl)].

It is reported about the clinical testing of the "Cancer Multistep Therapy" (CMT) by M. von Ardenne (CMT Concept '74). It was made in 1974/75 on 42 tumour-patients. At first we demonstrate in this publication clinical and paraclinical findings, which may be relevant for the therapy-standard, but the results of the therapy-efficiency will be given later. On the base of findings and observations we verify a program-true employment of the CMT Concept '74 and we demonstrate the pathophysiological reaction-patterns in connection with the longterm-hyperglycemia (glucose-utilization, spontaneous hyperthermy with tachycardia, spontaneous leukocytosis with typical deviations in the hematogram). These reaction-patterns are described particularly as "general adaptation syndrome" (Selye). Further we give an assessment of compatibility, tolerableness, and practicability of the CMT.

[Adrenoleukodystrophy. Clinical-pathologic case history and literature review].

This article presents the clinical and pathological history of a 10-year-old boy with adrenoleukodystrophy as well as a tabular survey of 17 additional cases of which the necropsy records were published in the literature. So far as the case of adrenoleukodystrophy directly observed by the authors themselves is concerned, commencement of the disease was in the seventh year of life, with symptoms of Addison's disease and diffuse inflammatory cerebral sclerosis being observed at about the same time. The cerebral disease manifested itself as a progressive pseudobulbar and pyramidal tract symptomatology and as a progressive cerebral psychosyndrome reminiscent of an endocrinal psychosyndrome. Morphologically, the adrenoleukodystrophy was characterized by severe dystrophy of the cortex of the suprarenal gland and the brain are considered the points of action of a still unknown noxa.e suprarenal gland as well as by extensive sudanophilic processes of decomposition in the cerebromedullary substance, putamen, pallidum, and thalamus. [Porphyria variegata (author's transl)].

Porphyria variegata, one of the rarer forms of hepatic porphyria, is brought to the attention through presentation of one case. Its clinical and chemical laboratory characteristics are compared with other hepatic porphyrias and pathogenesis, hereditary transmission and therapy are gone into.
