Susceptibility of Rickettsia conorii and R. rickettsii to pefloxacin, in vitro and in ovo.

The activity of pefloxacin against Rickettsia conorii and R. rickettsii was determined by several methods. The mean survival time of embryonated eggs infected with R. conorii was increased by pefloxacin 50 micrograms/egg; plaque formation in Vero cells was inhibited by 1 mg/l. In a microplate assay, the MIC of pefloxacin was 0.5 mg/l for R. conorii and 1 mg/l for R. rickettsii. The results support the use of pefloxacin in treating spotted fever rickettsioses.

Enzyme secretion by human endothelial cells infected with Rickettsia conorii.

When cultured endothelial cells from the human umbilical vein were infected with rickettsia conorii, a greater increase was noticed in the inhibitor of plasminogen activator than in tissue type plasminogen activator. This could be linked to clinical observation of coagulation abnormalities in Boutonneuse fever (BF). Otherwise, transaminase levels increased significantly in the supernatant of R. conorii infected endothelial cells which could explain the increased levels of these enzyme in BF.

Incidence, clinical observations and risk factors in the severe form of Mediterranean spotted fever among patients admitted to hospital in Marseilles 1983-1984.

Seven of 142 cases (5%) of Mediterranean spotted fever admitted to hospital in Marseilles in 1983 and 1984 were severe. All of them were confirmed by specific laboratory tests. The disease resembles Rocky Mountain spotted fever with purpuric exanthem, confusion, renal insufficiency, hypoxaemia, thrombocytopenia, hyponatraemia and hypocalcaemia. Two patients died. The predisposing factors for severity were old age, alcoholism, heavy smoking, respiratory insufficiency and glucose-6-phosphate dehydrogenase deficiency.

In vitro susceptibility of Rickettsia conorii to ciprofloxacin as determined by suppressing lethality in chicken embryos and by plaque assay.

We tested the susceptibility of Rickettsia conorii to ciprofloxacin, a new quinolone antibiotic. A final concentration of 1 microgram/g of egg was effective in suppressing chicken embryo lethality, and a concentration of 0.25 micrograms/ml inhibited plaque formation in a plaque assay; however, a concentration of 0.5 microgram/ml was necessary to obtain rickettsiacidal activity. These results support the idea that ciprofloxacin could be of clinical use in treating Mediterranean spotted fever.

Ciprofloxacin therapy for Mediterranean spotted fever.

We report the treatment of five patients with Mediterranean spotted fever with the antimicrobial agent ciprofloxacin. The treatment was administered intravenously for 2 days and then perorally for 8 days. All five patients were cured. These preliminary data seem to correlate with the in vitro activity of ciprofloxacin against Rickettsia conorii.

Mediterranean spotted fever: clinical, laboratory and epidemiological features of 199 cases.

Most previous studies of Mediterranean spotted fever (MSF) have included cases that either were not laboratory-confirmed or were confirmed by the Weil-Felix test. The authors report the detailed clinical, laboratory and epidemiological features of 199 serologically-confirmed cases of MSF (by microimmunofluorescence). This work demonstrates that the disease is difficult to diagnose, especially at the beginning; that it can be fatal (2.5% of cases); and that a rapid and specific diagnosis is necessary to identify atypical cases. Epidemiological features such as season (summer essentially), presence of a dog, and travel in an endemic area (the Mediterranean Basin) are important in the diagnosis. In such cases fever associated with rash have to be considered and treated as MSF.

Meningeal penetration of apalcillin in man.

We have used high performance liquid chromatography to evaluate the meningeal penetration of apalcillin. In subjects without meningitis the penetration was poor and the levels obtained did not exceed 1.75 mg/l. In cases of meningitis, when the spinal fluid albumin level was greater than 0.60 g/l, the levels varied from 5 to 30 mg/l. These figures are in accord with those obtained in cases of experimental meningitis. A dosage of 200 mg/kg/day should allow the treatment of meningitis due to sensitive bacteria and in particular due to Pseudomonas aeruginosa.

[Serology of Mediterranean boutonneuse fever. Kinetics of antibodies detected by 3 methods: indirect immunofluorescence, indirect hemagglutination and latex agglutination].

The purpose of this work was to determine the kinetics of antibodies in mediterranean spotted fever as determined by three serologic methods: indirect fluorescent antibody test, latex agglutination test and indirect hemagglutination test. No difference was noticed in the early kinetics but after 6 months, the latex agglutination test and the indirect hemagglutination test did not detect antibodies but the indirect fluorescent antibody test was still positive. This reaction is convenient for seroepidemiologic studies.
