[Epidemiologic and clinical characteristics of Lyme borreliosis in northeastern Poland].

Lyme borreliosis (LB) caused the tick-borne spirochete B. burgdorferi, is associated with a wide variety of manifestations including dermatologic, rheumatologic, neurologic and cardiac abnormalities, that can differ from one region to another. To define clinical picture of LB in Poland, particularly in north-eastern region, we studied in 46 patients. Diagnosis of LB was established according to criterion, including serological confirmation with ELISA LB seems to be disease, which renal prevalence in Poland is higher, than it was primarily estimated. Clinical picture of the disease does not differ significantly from observed in other parts of the world. The only, but important exception is more frequent manifestation of B. burgdorferi infections with meningitis or/and encephalitis (48% of patients).

[Multiorgan changes in AIDS in a reported case].

The clinical course of HIV infection and results of autopsy examination in 49 years old patient was shown. The attention was paid to difficulties of diagnosis of opportunistic infections in a patients with advanced HIV disease. Variety of infectious factors, that could affect a patient with significant immunodeficiency was outlined.

Prevalence of antibodies against tick-borne encephalitis among residents of north-eastern Poland.

In 37 out of 613 (6%) residents of north eastern Poland, IgG antibodies to tick-borne encephalitis virus (TBEV) were detected at levels exceeding the diagnostic value of 60 VIEU/ml. The prevalence of the antibodies was not related to sex or place or residence. However, significantly higher antibody levels were found in the group of forest workers than in individuals not professionally connected with forestry.

["Prognostic markers" in serum of active and passive intravenous drug users infected with human immunodeficiency virus (HIV)].

The analyse of neopterin (NPT), beta 2-microglobulin (beta 2-M) and immunoglobulin A (IgA) in active and passive intravenous drug users was made to show the influence of intravenous drug taking on the level of so called "prognostic markers" in the serum of HIV infected persons. The intravenous drug users remained in the same, symptom-free stage of infection. The level of NPT in active drug users was higher than in passive drug users (statistically essential difference). In both analysed groups the level of NPT considerably exceeded the norm range (10 nmol/l). The differences in beta 2-M concentrations between analysed groups were not statistically essential, although higher values were observed in active drug users. The concentration of IgA in analysed persons were within the norm range. Current life conditions of the HIV-infected persons should be taken into consideration when estimating prognostic values of analysed indices.

[Seasonal prevalence of antibodies against Borrelia burgdorferi in Białowiez inhabitants].

The aim of study was evaluation of seasonal prevalence of antibodies against Borrelia burgdorferi in Białowieza area, that is Lyme borreliosis endemic region. IgM and IgG antibodies against 41-kDa antigen of Borrelia burgdorferi B31 strain were investigated with ELISA in sera of 300 Białowieza inhabitants between September 1993 and May 1994. Moreover in 40 asymptomatic persons, that were seropositive in September 1993, additional control was performed in May 1994. The highest prevalence of IgM antibodies was observed in September 1993 (43.4%), with decrease during winter and spring (down to 15.7% in April) and accompanying the highest prevalence of IgG antibodies (17.7%) in April 1994). From 33 persons, that revealed IgM antibodies in September 1993, 3 (9%) were still seropositive, 4 (12%) cleared IgM antibodies and became IgG-seropositive, and remained 26 (79%) had no antibodies. Specific IgG antibodies found in 7 persons in September, were still present in 4. Obtained results indicate necessity of taking into consideration seasonal dynamics, when results of seroconversion against Borrelia burgdorferi between different areas and different populations are compared.

Effect of misoprostol in ethanol-induced liver injury.

Prostaglandins appeared protective against acute experimental liver injury of different origin. Misoprostol, stable, orally active, synthetic derivative of PGE1 attenuates several functional alterations in liver mitochondria during ethanol administration. To study its possible hepatoprotective effects on ethanol-induced liver injury in rats we measured: serum activities of alanine aminotransferase (ALT), gamma-glutamyltranspeptidase (GGT) and concentrations of ammonia in blood and liver tissue. Histopathological evaluation of liver slices was also performed. Activities of both enzymes and ammonia values were elevated after intragastric ethanol administration for 60 days. Treatment for 30 days with misoprostol resulted in their decrease. This effect was not observed in the control group. Beneficial results were also obtained in histopathological evaluation of the liver tissue. These results indicate potential therapeutic effects of misoprostol on ethanol-induced liver injury in rats.

The assessment of functional status of blood platelets in patients infected with Giardia intestinalis after anti-parasitic treatment.

Platelets are involved in anti-parasitic mechanisms. This question is poorly recognized. Platelets may exhibit this function only after activation. Therefore we conducted this study measuring some morphological parameters of these cells which reflect activation of platelets. The experiments were done on 30 patients infected with Giardia intestinalis. Diagnosis was supported on finding trophozoits in bile, cysts and GSA 65-protein in feces. We have found some decrease of blood platelets count and normalization after anti-parasitic treatment. In the line with this observation is platelet crit. A slight change was noted in mean platelet volume before and after treatment if we compare to controls. The same observation concern platelet anisocytosis index. Nearly two-fold decrease in the count of large platelets was observed before and after treatment 3.77% and 2.07%. Our general conclusion is, the parasite is able to activate the platelets and anti-parasitic treatment normalize these changes.

[Macroscopic changes in endoscopy of upper digestive tract and Helicobacter pylori infections in patients with liver cirrhosis].

Endoscopic evaluation of upper gastrointestinal tract in 82 patients with liver cirrhosis, versus 82 controls was performed. In part of them presence of Helicobacter pylori infection was also analyzed. We found significantly higher prevalence of endoscopic changes in cirrhotic patients (87%). Diagnoses of oesophangeal candidiasis and varices, gastritis, ventricular or duodenal erosions and ulcers, duodenal deformations were more frequent in this group of patients in comparison to group of non-cirrhotic patients. Helicobacter pylori infection was found in 83% of cirrhotic patients, that was significantly higher than in non-cirrhotic control group (43%).

[Active immunization against tick-borne encephalitis virus infection in residents of North-Eastern Poland].

Group consisted of 78 persons was selected among 1587 persons vaccinated against tick-borne encephalitis. Efficiency of vaccination, controlled by measurement of specific IgM and IgG antibodies presence, was evaluated as 70.2% (47.4%-90%). The highest efficiency was observed in urban residents occasionally stayed in forest areas, that showed trace, of antibody level at the begining of study.

[Endemic threat of Lyme borreliosis in Bialowicza forestry area].

Lyme Borreliosis is a serious diagnostical and therapeutical problem in regions with high exposure of residents to ticks, which are Borrelia burgdorferi vectors. The aim of this study was to estimate occurrence of specyfic antibodies and symptoms of Lyme Borreliosis in Białowieza area, as a region of potential endemic presence of this disease. Antibodies against B. burgdorferi were found, with an immunoenzymatic assay, in 71 from among 143 of examined persons (49.7%). Diagnosis of Lyme Borreliosis was established in 54 persons (37.7% of examined population) according to criterions of Lyme Disease Foundation and Centers for Diseases Control. Symptoms of joints involvement (polyarticular or restricted to knee joints) and peripheral nervous system involvement (predominantly radiculalgia lumbosacralis) were most frequently observed in clinical picture. These results motivate to recognize Białowieza region as an endemic area of Borrelia burgdorferi infections.

[Amoebiasis--a problem in patients with ulcerative colitis].

The increasing problem is presented of Entamoeba histolytica invasion in patients with ulcerative colitis. The diagnostic-therapeutic management in five patients with amoebiasis is discussed in detail. This parasitic invasion occurred in five out of 103 patients with ulcerative colitis which accounts for 4.85%. This data is important for the treatment of chronically ill patients with ulcerative colitis.

[Organ changes in experimental strongyloidosis of rabbits provoked by Strongyloides papillosus].

Strongyloidosis was induced by percutaneous injection of rabbits; then organ changes were tested. It has been found that Strongyloides papillosus evoked changes in intestine, liver, kidney and lung. No notable changes were found in heart, spleen and suprarenal gland. These changes may be provoked not only directly by the presence of these parasites but also by the products of metabolism of S. papillosus.

[Levels of neopterin in serum of narcotics abusers infected with human immunodeficiency virus (HIV)].

Concentration of neopterin (NPT) was measured in 3-6 months intervals in the serum of HIV-infected drug users taking continuously intravenous drugs and those who stopped the habit (MONAR, ZOZ). After 6-12 months of observation the mean NPT concentration was higher in the active drug users what was statistically essential. The outcome of our study indicates that current state of the patient concerning taking of drugs must be taken under consideration when NPT concentration is assessed as marker of progression of HIV disease.

[Diphtheria--new threat or traditional disease].

Clinical and epidemiological data of the diphtheria have been described on the basis of literature findings and own observations. It have been introduced the informations presented during Meeting on Diphtheria Epidemic in Europe, Saint Petersburg, 1993. I have concentrated on the some differences between diphtheria in nowadays and in previous years and offer some suggestions concerning the treatment of this disease. It concern also plasmapheresis as a special effective procedure in toxic forms of the diphtheria.
