Intra-arterial digital subtraction angiography of the spinal cord.

Digital subtraction angiography (DSA) of the spinal cord was performed in 6 patients using selective intra-arterial injections of contrast material. Two arteriovenous malformations of the spinal cord, 1 dural fistula, and 1 case of multiple hemangioblastomas were studied. Contrast and spatial resolution were satisfactory for defining normal and abnormal vascularity while reducing examination time, contrast dosage, patient discomfort, and film cost. The only significant limitation was misregistration artifacts seen on lateral views encompassing the diaphragm.

The MMPI: development of contemporary norms.

The original standardization of the MMPI was completed approximately 40 years ago. These norms were refined in 1957, but no attempt has been made to develop new standards. Therefore, a contemporary random sample of 1,408 persons living in the midwestern states who were similar to the original sample in area of residence and in the absence of significant medical problems were asked to complete the MMPI. The resulting MMPI data by age and sex are presented. In general, responses from this contemporary sample yielded higher mean scores than appeared in the original standardization group. These data suggest a somewhat more conservative approach to profile interpretation, with more careful consideration of the impact of age and sex on profile configurations.

The aging MMPI: development of contemporary norms.

Twenty-five years of experience with the Mayo Clinic computerized system for the Minnesota Multiphasic Personality Inventory (MMPI) have accrued since Swenson , Rome, and their colleagues proposed this innovation in 1959. Although it is the most respected, widely used, and thoroughly researched objective personality-assessment instrument that has been developed to date, the MMPI is aging. Work on the MMPI began in 1937, and the original normative base was established in the late 1930s and early 1940s . To develop new norms, we selected a random sample of 1,408 subjects, not under care for any physically or mentally handicapping condition and ranging in age from 18 through 99 years, from parts of Iowa, Wisconsin, and Minnesota in a 50-mile radius surrounding Rochester, Minnesota. MMPI responses obtained from this sample were used to develop normalized T-score tables for specific age ranges, and for adults in general, for the 13 basic scales of the MMPI.

CT-guided stereotactic biopsies using a modified frame and Gildenberg techniques.

Eighteen patients underwent CT guided stereotactic biopsies using either a Riechert-Mundinger stereotactic frame modified for use in the CT scanner or the CT-based intracranial landmark technique of Gildenberg and a Todd-Wells frame. In seven patients undergoing biopsy with the modified frame technique, representative tissue samples were obtained in all cases. Eleven patients had biopsies using the Gildenberg technique and representative tissue samples were obtained in 10 of the 11. No major postoperative complications occurred in either group. Both techniques allowed samples of intracranial tissue to be obtained safely and efficiently. The modified frame technique provided a higher degree of accuracy while the Gildenberg technique required significantly less CT time and provided a satisfactory degree of accuracy. In seven patients with ring enhancing lesions, biopsies of the enhancing rim provided diagnostic tissue while biopsy of the central low density center of the lesion in three patients provided necrotic material which was useful in histologically grading the tumour.

CT identification of bronchopulmonary segments: 50 normal subjects.

A systematic evaluation of the fissures, segmental bronchi and arteries, bronchopulmonary segments, and peripheral pulmonary parenchyma was made from computed tomographic (CT) scans of 50 patients with normal chest radiographs. Seventy percent of the segmental bronchi and 76% of the segmental arteries were identified. Arteries could be traced to their sixth- and seventh-order branches; their orientation to the plane of the CT section allowed gross identification and localization of bronchopulmonary segments.

Randomised, controlled study of transhepatic obliteration of varices and oesophageal stapling transection in uncontrolled variceal haemorrhage.

Percutaneous transhepatic obliteration and surgical stapling transection of the oesophagus with the EEA gun were compared prospectively in the treatment of uncontrolled oesophageal variceal haemorrhage unresponsive to conservative measures. Twenty patients with cirrhosis, with a patient portal vein and who were considered suitable for general anaesthesia and surgery, were randomised to two treatment groups (10 patients each). Immediate arrest of haemorrhage was achieved in 17 patients (nine surgery, eight obliteration). In one other patient, stapling transection succeeded where attempted transhepatic obliteration failed, and in another patient obliteration succeeded where attempted transection had failed. One patient continued to bleed and died following attempts at both procedures. Two other patients also died in hospital, without rebleeding following surgery. Variceal rebleeding during the same hospital admission occurred in two patients in the obliteration group and in none after surgery. Oesophageal stapling transection compares very favourably with a non-surgical technique such as transhepatic obliteration of varices in the emergency treatment of uncontrolled variceal haemorrhage in patients with moderate liver failure.

Experience using two CT-guided stereotactic biopsy methods.

15 patients had intracranial CT-guided stereotactic biopsies. Biopsies were performed either with a Riechert-Mundinger stereotactic frame modified for use in the CT or by using the CT scan to establish the relationship of the intracranial lesion to identifiable bony landmarks, and subsequently performing the biopsy in a standard stereotactic frame. Both systems provided safe and accurate methods for obtaining intracranial tissue.

Seminal vesiculography: limited value in vesiculitis.

Seminal vesiculograms were performed on 44 men being evaluated for chronic perineal pain consistent with seminal vesiculitis. The vesiculograms were performed prior to antibiotic instillation into the vas deferens. Vesiculograms were abnormal in 32 of the 44 patients (73%). The most common abnormality was the presence of multiple small diverticula producing a feathery appearance of the vas deferens (64%), a finding present in only 19 per cent of patients being evaluated for infertility and 20 per cent of normal patients. Asymmetry and incomplete filling of the seminal vesicles were less common abnormalities. Seventeen of these patients had seminal vesiculectomies for persistent or recurrent pain, and 15 have been followed long enough to assess the results. The histologic examination of the excised seminal vesicles was abnormal in only 3 patients (20%), despite radiographic abnormalities in 10 (67%) and improvement after surgery in 12 (80%). Seminal vesiculography may be useful to exclude anatomic abnormalities prior to instillation, but does not appear sufficiently specific to warrant use as the primary diagnostic examination for patients suspected of having seminal vesiculitis.

The role of seminal vesiculography in the evaluation of male infertility.

Although most cases of male infertility are the result of primary testicular abnormalities (either maturation arrest, decreased spermatogenesis, or germinal hypoplasia), obstruction of the genital tract is a well recognized and potentially correctable cause of infertility. Seminal vesiculography is an excellent method of evaluating the male genital tract not only for obstruction but also for other structural lesions that may cause infertility. In 27 patients undergoing vesiculography as part of an evaluation of infertility, obstruction was found in 25%. Three patients (11%) were found to have nonobstructing structural lesions as a possible cause of infertility. Patients with obstructive lesions were found to have significantly lower semen volumes than those with patent outflow tracts. The continued role of seminal vesiculography in the evaluation of male infertility is discussed.

In vivo regional quantitation of intrathoracic Tc-99m using SPECT: concise communication.

A whole-body single-photon emission computed tomographic system (SPECT) was used to quantitate the activities of a series of Tc-99m point sources in the dog's thorax and to evaluate attenuation of a uniform esophageal line source containing a known concentration of Tc-99m. A first-order attenuation correction and an empirically derived attenuation coefficient of 0.09 cm-1 were used in the SPECT analyses of the intrathoracic point sources. The relationship between SPECT measurements of multiple point-source activities and the same sources measured in air was linear over a range of 100 to 1000 muCi (slope 1.08; R2 coefficient of determination 0.97). These data are sufficiently accurate to allow an estimate of the regional activity of radiopharmaceutical in the dog's thorax and justify their use in experimental quantitation of regional pulmonary perfusion.

Rapid serial cranial computed tomography for tumor diagnosis.

Rapid serial computed tomography (RSCT) provides more information about the cerebral cortical capillary bed and leptomeningeal vessels than conventional enhanced computed tomography (CT). This increased neuroanatomical definition has potential value for separating intra- from extracerebral tumors in selected cases. RSCT offers better visualization of the angioarchitecture of highly vascular tumors than conventional enhanced CT. However, CT scans delayed several minutes after contrast administration are more useful for evaluating the extent of hypovascular tumors. Time-density curves were of limited value for tumor evaluation. However, the peak increase of Hounsfield units did correlate well with the degree of tumor vascularity assessed angiographically.
