[New 2,4-dibromo-6-(N-cyclohexyl-N-methyl)-aminomethylanilides of derivatives of cysteine and cystine with expectorant activity].

New 2,4-dibromo-6-(N-cyclohexyl-N-methyl)aminomethylanilides of N-acetylcysteine and N-acetyl-S-carboxymethylcisteine derivatives, N-benzyloxycarbonylcysteine and N,N'-bisbenzyloxycarbonylcystine were synthesized. The compounds were tested for the expectorant activity in rabbit and mouse. N,N'-Bisbenzyloxycarbonylcystin-bis [2,4-dibromo-6-(N-cyclohexyl-N-methyl)aminomethyl]anilide (XIX) was selected for further investigations.

[Synthesis of benzofuran derivatives with H2 antagonist activity].

N-[2-(7-Dimethylaminomethyl-5-methyl-2-benzofuranmethylthio)ethyl]-N'-methyl-2-nitro-1,1-ethenediamine (III) and N-cyano-N'-[2-(7-dimethylaminomethyl-5-methyl-2-benzofuranmethylthio)ethyl]-N"-methylguanidine (IV) were synthesized. The direct transformation of 2,2-dicarbethoxy-3-hydroxy-5-methyl-7-dimethylaminomethylcoumaran (VII) and of 2,2-dicarbethoxy-3-hydroxycoumaran (XVI) to the corresponding ethyl 2-benzofurancarboxylates (IX) and (XV) is also reported. A mechanism for this reaction is proposed. Compounds (III) and (IV) showed hystamine H2 receptor antagonist activity, but were less potent than cimetidine and ranitidine.

Transcutaneous PO2 as a trend indicator of arterial PO2 in normal anesthesized adults.

The transcutaneous technique of measuring PO2 (tcPO2) was studied in 30 adults with normal cardiovascular function undergoing nitrous oxide-enflurane or nitrous oxide-fentanyl anesthesia to determine the relationship between tcPO2 and PaO2. The tcPO2 was an accurate and clinically useful trend indicator of PaO2 (r = 0.9; p less than 0.0001). The ability to detect trends was unaffected by the type of anesthetic agent used. The technique was less useful in predicting absolute values for PaO2 (r = 0.86). It is suggested that more widespread application of such monitoring awaits definitive development of a halothane-resistant electrode.

Synthesis and anticonvulsant activity of N-(benzoylalkyl)imidazoles and N-(omega-phenyl-omega-hydroxyalkyl)imidazoles.

A novel series of N-(benzoylalkyl)imidazoles and N-(omega-phenyl-omega-hydroxyalkyl)imidazoles was synthesized and evaluated for anticonvulsant activity in mice against maximal electroshock induced seizures. Some of the compounds showed an activity comparable to or better than phenytoin and phenobarbital. The N-[beta-[4-(beta-phenylethyl)phenyl]-beta-hydroxyethyl]imidazole (38) was selected for further studies; preclinical toxicology and additional efficacy evaluations are in progress. Structure-activity relationships are discussed.

Relationship between transcutaneous and arterial carbon dioxide tension in adult patients anesthetized with nitrous oxide-fentanyl and nitrous oxide-enflurane.

The transcutaneous technique of measuring carbon dioxide tension (tcPCO2) was studied in 30 adult neurosurgery patients undergoing nitrous oxide-enflurane or nitrous oxide-fentanyl anesthesia to determine the relationship between tcPCO2 and PaCO2. tcPCO2 was an accurate (r = 0.9) and clinically useful trend indicator of PaCO2. The ability to detect trends was unaffected by the type of anesthetic agent used. The technique was less useful in predicting absolute values for PaCO2 (r = 0.64). This technique may be particularly useful in situations in which control PaCO2 is essential to patient management.

Synthesis and antimycotic activity of alpha-aryl-beta,N-imidazolylalkyl benzyl ethers.

A new series of alpha-aryl-beta,N-imidazolylalkyl benzyl ethers was synthesized and tested for antimycotic and antimicrobial activity. Most of the compounds was highly active in vitro against fungi and gram-positive bacteria. The alpha,4-phenylthiophenyl-beta,N-imidazolylethyl benzyl ether (7) and the alpha,4-biphenyl-beta,N-imidazolylethyl benzyl ether (17) showed higher antifungal activity than 1-[2,4,dichloro-beta-(2,4-dichlorobenzyloxy)phenethyl]imidazole (miconazole) against Candida albicans. None of the compounds was more active than miconazole against Trichophyton mentagrophytes. The most interesting compounds of the series were assayed in vivo, but their activity was inconstant and always lower than that of miconazole.

New alpha-aryl-beta,N-imidzolylethyl benzyl and naphthylmethyl ethers with antimycotic and antibacterial activity.

A new series of alpha-aryl-beta,N-imidazolylethyl benzyl and naphthylmethyl ethers was synthesized and tested for antimycotic and antimicrobial activity. All compounds showed antifungal activity; most of them were also active against gram-positive bacteria, whereas no activity was detected against gram-negative bacteria. Structure-activity relationships are discussed. The alpha-(2,4-dichlorophenyl)-beta,N-imidazolylethyl 4-phenylthiobenzyl ether nitrate (8), which showed good skin tolerability and in vivo antimycotic activity comparable with or better than 1-[2,4-dichloro-beta-(2,4-dichlorobenzyloxy)phenethyl]imidazole (miconazole) and 1-(alpha-(o-chlorophenyl)benzhydryl]imidazole (clotrimazole), was selected for further researches.

Catheter-induced pulmonary artery perforation. Mechanisms, management, and modifications.

Six cases of pulmonary artery perforation associated with the use of Swan-Ganz catheters are reviewed. Risk factors included pulmonary hypertension, anticoagulation, and hypothermia. The mechanisms leading to perforation were clarified by the use of postmortem studies employing isolated whole lung preparations. These studies revealed that perforation results from (1) tip perforation of vasculature, (2) eccentric balloon configuration propelling the balloon through the vessel wall, and (3) balloon inflation disrupting the pulmonary artery (mean intraballoon pressure 250 mm Hg). Early clinical symptoms include hemoptysis of bright red blood and/or hypotension. Immediate evaluation may necessitate examination with a fiberoptic bronchoscope and "wedge" angiogram. If massive hemoptysis occurs, isolation of the unaffected lung by endobronchial intubation is mandatory. Pneumonectomy or lobectomy may be required. Revised guidelines for catheter insertion and pulmonary capillary wedge pressure (PCWP) measurements are presented. Finally, consideration is given to redesigning the pulmonary artery flow-guided catheter, particularly for use in patients undergoing cardiac operations with systemic anticoagulation. Modifications should be directed at (1) softer catheter tip with temperature-insensitive body, (2) low-pressure balloon, and (3) balloon pressure relief valve.

[Durene derivates. IV. N-((2,3,5,6-tetramethylbenzoyl)alkyl)imidazoles and N-((beta-(2,3,5,6-tetramethylphenyl)-beta-hydroxy)ethyl)imidazoles with anticonvulsant activity].

A series of N-[(2,3,4,6-tetramethylbenzoyl)alkyl]imidazoles and of N-([beta-(2,3,5,6-tetramethylphenyl)-beta-hydroxy]ethyl)imidazoles has been synthesized. All compounds were tested in mice for anticonvulsant activity against electroshock and pentylenetetrazole convulsions. These durene-derivatives showed good activity, whereas the corresponding benzene-derivatives were inactive.
