Identification of HLA-DRw52 associated antigens using HLA class II allogenotyping.

The HLA-DR antigens DRw8, DRw11, DRw12, DRw13, DRw14 and DRw17 are strongly associated with the supertypic specificity DRw52. This association has been used to assist in assignment of serological specificity. However, difficulties in the identification of these antigens arise since they are serologically crossreactive. This report describes the application of restriction fragment length polymorphism (RFLP) allogenotyping to assist in the positive identification of DRw8, DRw11, DRw12, DRw13, DRw14, DRw17, DRw52a and DRw52b, and in addition describes further subtypes defined by RFLP which correlate with DQ or Dw associations. We also describe a novel TaqI/DR beta RFLP in a Caucasian family which types serologically as DRw12.

Monocyte esterase? A factor involved in the pathogenesis of lymphoproliferative neoplasia.

Monocyte esterase activity was studied in 1,000 doctor-attending patients with normal hematological indices and in 56 patients with non-Hodgkin's lymphoma (NHL) or B chronic lymphocytic leukemia (CLL). The incidence of esterase deficiency was significantly greater in the NHL-CLL patients (7.1%) than in the population group (1.7%; p less than 0.05). In the NHL-CLL group, study of the families showed the esterase deficiency to be a familial characteristic. We postulate that the presence of the anomaly may be either a factor predisposing to the development of the NHL or CLL or a factor indicating a predisposition to these disorders.

Putative immunoglobulin VH genes of the goldfish, Carassius auratus, detected by heterologous cross-hybridization with a murine VH probe.

By using a defined cDNA probe for the VH region of a murine phosphocholine-binding myeloma protein (S107) we have defined a family of distinct cross-hybridizing DNA sequences in genomic DNA of the goldfish. The estimated number of the goldfish putative VH family detectable by the S107 probe is about 36. By using two putative goldfish VH probes to analyze, by hybridization, the relationships among seven of the goldfish genomic clones, we have determined that the putative goldfish VH genes recognized by the S107 probe comprise at least several distinct families that are not closely related.

HLA-DR antigen association with proliferative diabetic retinopathy.

No significant differences could be found in the HLA-DR antigen frequency of two groups of patients with insulin dependent diabetes mellitus (IDDM). One group consisted of 49 patients with proliferative diabetic retinopathy and the other group of 31 patients with no ophthalmoscopic evidence of diabetic retinopathy.

The influence of HLA-A, B, and DR matching on graft survival in primary cadaveric renal transplantation in Belfast.

HLA-DR matching has been shown in a retrospective study of 72 renal transplant patients to significantly enhance graft survival at 12 months. HLA-A and -B antigen matching also increased the graft survival rate significantly. Analysis of combined HLA-A, -B and -DR matching suggested an improvement in graft survival rate with better matching, but this did not attain statistical significance. It is now our policy to use HLA-DR matching prospectively and to ensure that all recipients receive a kidney with a maximum of 1 HLA-DR incompatibility and a minimum of 2 HLA-A and -B antigens shared.

A new DQw1 associated antigen: HLA-DR'Br'.

A new DQw1 associated antigen, HLA-DR'Br', has been identified in three Caucasoid families. DR'Br' shows strong association with both class I and class III gene products but cannot be reliably defined in the presence of DR1, DR2, DRw6 or DRw10.

Limited joint mobility in insulin-dependent diabetes: relationship to retinopathy, peripheral nerve function and HLA status.

Eighty-eight (43 per cent) of 204 patients with insulin-dependent diabetes had limited joint mobility affecting mainly the small joints of the hands. The presence of limited joint mobility correlated with duration of diabetes and with the presence of retinopathy. Patients with longstanding diabetes were approximately 2.5 times more likely to have proliferative retinopathy if limited joint mobility was present than if it was absent, although the risk for non-proliferative or background retinopathy was not increased. In patients with longstanding diabetes and limited joint mobility nerve conduction velocity and vibration perception threshold were significantly reduced compared with patients having similar duration of diabetes but normal joints. The association between insulin-dependent diabetes and HLA-DR3 and HLA-DR4 was confirmed, but there was no difference, between patients with and without limited joint mobility, in the frequency of the various HLA types. Limitation of joint mobility appears to be another "chronic complication' of diabetes, developing in parallel with retinopathy and deteriorating peripheral nerve function, and possibly of similar aetiology.

Mismatching for HLA-A, -B antigens and renal graft survival.

One hundred and seventy five recipients of a kidney graft were analyzed to see if mismatching for certain HLA-A, -B antigens was detrimental to graft survival. No association was found between graft survival and a specific mismatched HLA-A, -B antigen.

Childhood sigmoid volvulus.

A 14-year-old boy presented to the emergency department with abdominal pain. Flat and upright abdominal films and a barium enema revealed the classic findings of sigmoid volvulus. Sigmoidoscopy allowed a soft rubber tube to be passed beyond the obstruction with immediate relief of pain. The patient subsequently underwent a descending sigmoid colectomy. Sigmoid volvulus is a rare entity in childhood. Case discussion and review of the literature is presented.

HLA antigen frequency in granuloma annulare.

The frequency of HLA-A and -B antigens in fifty-nine patients with localized granuloma annulare was compared with the frequency in 200 blood donors. The frequencies of HLA-A29, -B14 and -B15 were increased significantly in the patient group, the increase of HLA-A29 remaining significant after correction for the number of antigens compared.

HLA tissue typing and chronic vulval dystrophy.

Tissue typing was performed on 25 patients who attended the Royal Victoria Hospital with chronic vulval dystrophy (CVD). Age-matched control groups who had pernicious anaemia only and achlorhydria only were also tissue typed as these conditions had been found more frequently in our patients with CVD. When these three groups were compared with age-matched control groups of blood donors and patients with gynaecological disorders other than CVD, no specific tissue type was found in patients with CVD nor in those with pernicious anaemia or achlorhydria only.
