A spatial arrangement memory testing unit.

The Spatial Arrangement Memory Testing Unit is a new apparatus which has been designed to provide a low-cost, portable instrument for assessing visuo-spatial memory performance. It is particularly useful for aiding diagnosis and planning rehabilitation programmes for patients that have suffered brain damage. The application of microprocessor technology has allowed a flexible programmable approach to the operation of the equipment. The system uses a dedicated Zilog Z80 microprocessor with associated software held in EPROM. An integral printer is used to provide a permanent record of patient performance.

The Pendine pathways project in the independent sector.

Recruitment and retention of staff, and the drive to provide quality care are challenges currently faced by nursing and residential homes in the independent sector. This article describes an innovative approach to tackling these issues. Under a teaching company scheme, a partnership was formed between the Pendine Park Nursing Home and the Centre for Learning Development to develop care and career pathways that are specific to the independent sector.

The less decade.

Although outsourcing is a relatively new buzzword, it actually amounts to contract management, a solution hospitals have used for years to manage operations areas such as dietary, housekeeping and maintenance. Many hospitals have found outsourcing a cost-effective way to meet their information system needs. Northeast Georgia Medical Center in Gainesville, Ga. is one example. Ten HealthQuest employees, including programmer Jerry Porter, work at NGMC under the medical center's facilities management agreement.

Competing for private patient revenue.

The opportunity to maximise revenue from private patients is now available to NHS hospitals. A survey of UGMs found that most felt they could compete with private hospitals but identified a number of obstacles. David Jones analyses the survey.

Nontuberculous mycobacterial infection of the head and neck in immunocompetent children: CT and MR findings.

BACKGROUND AND PURPOSE: Infections caused by nontuberculous mycobacteria (NTM) commonly manifest as cervicofacial adenitis in otherwise healthy children. The aim of this study was to characterize the imaging findings of NTM infection of the head and neck in immunocompetent children. METHODS: The medical records and imaging examinations (CT in 10, MR in two) were reviewed in 12 immunocompetent children with NTM infection of the head and neck. RESULTS: The usual presentation (n = 9) was of an enlarging, non-tender mass with violaceous skin discoloration, unresponsive to conventional antibiotics. The duration of symptoms was 6 days to 5 months. Imaging revealed asymmetric adenopathy with contiguous low-density ring-enhancing masses in all patients. There was cutaneous extension in 10 patients. Inflammatory stranding of the subcutaneous fat was minimal (n = 9) or absent (n = 2) in 11 patients. The masses involved the submandibular space (n = 3), the parotid space (n = 2), the cheek (n = 1), the anterior triangle of the neck (n = 2), the submandibular and parotid spaces (n = 2), the parotid space and neck (n = 1), and the neck and retropharyngeal space (n = 1). Surgical management included incision and drainage only (n = 2), incision and drainage with curettage (n = 2), excisional biopsy after incision and drainage (n = 1), excisional biopsy only (n = 5), superficial parotidectomy only (n = 1), and superficial parotidectomy with contralateral excisional biopsy (n = 1). All patients improved in response to surgery and long-term antimycobacterial antibiotics. CONCLUSION: NTM infection of the head and neck has a characteristic clinical presentation and imaging appearance. Recognition of this disease is important; appropriate treatment is excision and, in selected cases, antimycobacterial therapy.

A labor and delivery service unit model for a multihospital health maintenance organization.

Number of deliveries is no longer a relevant measurement for allocation of nursing resources in labor and delivery. A service unit model, which reflected the nursing effort to care for labor and delivery patients and the variability and unpredictability of these patients' needs, was designed for a major multihospital system. The development of the Labor and Delivery Service Unit Model for the Northern California Region of the Kaiser Permanente Medical Care Program is described.

The influence of mechanical stimulation on osteocyte apoptosis and bone viability in human trabecular bone.

It has been shown previously using in vivo and ex vivo animal models, that cyclical mechanical stimulation is capable of maintaining osteocyte viability through the control of apoptotic cell death. Here we have studied the effect of mechanical stimulation on osteocyte viability in human trabecular bone maintained in a 3-D bioreactor system. Bone samples, maintained in the bioreactor system for periods of 3, 7 and 27 days, were subjected to either cyclical mechanical stimulation which engendered a maximum of 3,000 microstrain in a waveform corresponding to physiological jumping exercise for 5 minutes daily or control unloading. Unloading resulted in a decrease in osteocyte viability within 3 days that was accompanied by increased levels of cellular apoptosis. Mechanical stimulation significantly reduced apoptosis (p< or =0.032) and improved the maintenance of osteocyte viability in bone from all patient samples. The percentage Alkaline Phosphatase (ALP) labelled bone surface was significantly increased (p< or =0.05) in response to mechanical stimulation in all samples as was the Bone Formation Rate (BFR/BS) (p=0.005) as determined by calcein label incorporation in the 27-day experiment. These data indicate that in this model system, mechanical stimulation is capable of maintaining osteocyte viability in human bone.
