Psychiatrists' attitudes to psychosurgery. Proposals for the organization of a psychosurgical service in Yorkshire.

A survey of attitudes to psychosurgery among consultant psychiatrists in two regions of Britain was undertaken by a postal questionnaire. Most psychiatrists considered that there is still a role for psychosurgery in psychiatric practice and over 75 per cent of psychiatrists working in general adult psychiatry requested facilities for the referral of patients; the majority of those requesting preferred local facilities and thought that the psychosurgical team should carry out both the assessment and postoperative rehabilitation. Respondents to the questionnaire were conservative in their indications for considering psychosurgery. The effect that the revision of the Mental Health Act might have on psychosurgical practice was then considered and finally the organization of the authors' own practice is described.

Ferritin. Binding of beryllium and other divalent metal ions.

Rat liver homogenates in 0.1 M Tris, pH 7.5, were heated to 80 degrees C, cooled immediately, and centrifuged at 24,000 X g, and 7Be2+ was added to the supernatant. Twenty-five per cent of the radioactivity was bound to a single protein. It was purified to homogeneity and identified to be ferritin as judged by different criteria. These were sucrose density gradient centrifugation, electrophoresis in polyacrylamide gel of the native or sodium dodecyl sulfate-treated protein, reactivity to antibodies, isoelectric focusing, and total amino acid composition. Comparative study of the ability of ferritin or apoferritin to bind Cd2+, Zn2+, Cu2+, and Be2+ was conducted by using a gel equilibrium technique, Centifree micropartition technique, and microcentrifuge desalting technique. Ferritin could be saturated with Cd2+ or Zn2+ or Cu2+ but not with Be2+ even after 800 g atoms of Be2+ were bound. None of the bound Be2+ was dialyzable at 4 degrees C in 0.05 Tris acetate buffer, pH 8.5, but at pH 6.5 over 80% of the bound metal ion was dialyzed after 72 h. By contrast, apoferritin bound similar amounts of all four metal ions, some of which were dialyzable. By spectrophotometric titrations at pH 6.5 of Be2+ with sulfosalicylic acid (SSA), BeKDSSA was calculated to be 5.0 X 10(-6) M and by competition of sulfosalicyclic acid and ferritin for Be2+ the BeKDferritin was calculated to be 6.8 X 10(-6) M.

Glutamic pyruvate transaminase phenotypes in polycythaemia rubra vera.

Glutamate pyruvate transaminase (GPT) has been studied in the red cells of 46 patients with a confirmed diagnosis of polycythaemia rubra vera (PRV). The red cells of many of the patients showed low levels of enzyme activity. This activity could be restored by in vitro incubation with pyridoxal phosphate suggesting that the effect was due to low levels of B6 rather than to a primary abnormality of the GPT. Patients with the lowest levels of GPT activity were likely to have clonal chromosomal abnormalities in the bone marrow cells, particularly 20q-. Among 43 patients in whom the GPT phenotypes could be determined by an electrophoretic method there was a marked deficiency of heterozygotes. This disturbance in phenotypic expression may be related to the clonal nature of the disease in PRV, the clone showing lack of response to homeostatic controls and irregularities of gene expression.

Prolonged use of the Greenland method of treatment of gonorrhoea.

Of 1081 patients with gonorrhoea treated with probenecid 1 g orally and benzyl penicillin 5 megaunits intramuscularly, 96.7% were cured and no complications were encountered. This treatment regimen was associated with a pronounced reduction in the proportion of gonococcal strains showing diminished sensitivity to penicillin, from 51% to between 4% and 14% over a period of four years. When this treatment method was abandoned, however, the percentage of less sensitive strains rose to 33% over the following 18 months.

Chiasma frequency variation with altitude in Cepaea hortensis (můll.).

Populations of Cepaea hortensis have been sampled from an area of 121 sq km on the north slopes of the Brendon Hills, Somerset, and analysed for chiasma frequency. Variation in chiasma frequency is due to variation in a single large bivalent--the A group bivalent. In 12 populations studied during 1973 no association between chiasma frequency and ecological factors could be detected. In particular, there is no evidence of a negative correlation of population density with chiasma frequency which is known to exist in populations of C. nemoralis within the area. In 1974, 17 populations, four of which were utilised in the 1973 analysis, were studied. Negative correlations of interstitial (but not total or terminal) chiasma frequency and between-cell variance with altitude were found. It is likely that the correlation of between-cell variance with altitude is due to the strong correlation of variance with chiasma frequency which exists in these populations. There is evidence from several populations that chiasma frequency is stable between years. The implications of the negative correlation of chiasma frequency with altitude and the lack of a correlation with population density are discussed.

Klebsiella meningitis--report of nine cases.

During a serious epidemic of chest and urinary infections due to Klebsiella aerogenes in a neurosurgical unit, several patients developed klebsiella meningitis after trauma or surgery. Despite all attempts to control the epidemic and treat the meningitis with antibiotics, eight of the nine patients died. It was not until all antibiotics used to treat respiratory and urinary infections had been totally withdrawn that no further patients developed klebsiella meningitis.
