An electron-microscopic and cytochemical study of follicular ameloblastoma.

The ultrastructural and enzyme cytochemical features of two follicular ameloblastomas were investigated. The peripheral cells of the follicular areas in both lesions had several types of tall columnar cells which were highly polarized and showed varying degrees of cellular differentiation. These polarized cells had their nuclei situated away from the basal lamina, and often contained dilated strands of endoplasmic reticulum in the subnuclear cytoplasm. Some of these cells also contained dense-cored secretory granules, condensing granules and coated vesicles in the cytoplasm adjacent to the basal plasma membrane. These cells bore a striking resemblance to pre-ameloblasts and early secretory ameloblasts. Alkaline phosphatase and ATPase cytochemistry supported these morphologic observations. Interestingly, the central cells of the follicular areas were consistently negative for alkaline phosphatase activity as were the peripheral cells, while both cell types had ATPase activity demonstrable at their cell surface.

Megakaryocytic leukemia; identification utilizing anti-factor VIII immunoperoxidase.

Coagulation Factor VIII is produced by endothelial cells and megakaryocytes. Antibody directed against Factor VIII or its associated antigens has been shown to be a sensitive marker for both normal and neoplastic endothelial cells. Its use to identify neoplastic megakaryocytes has not been described. We used immunoperoxidase localization of Factor VIII to identify megakaryocytes and immature megakaryocytic precursors in a case of megakaryocytic leukemia.

Susceptibility of Shigella species to erythromycin.

Two of the most common causes of inflammatory enteritis are Campylobacter fetus subsp. jejuni and Shigella species. No single antimicrobial agent is recommended for treatment of both diseases. Erythromycin is used to treat C. fetus subsp. jejuni infections but has not been studied in shigellosis. For this reason, we determined the susceptibility of 22 strains of Shigella to erythromycin and found that Shigella species are susceptible to concentrations of erythromycin which are obtainable in stool.

The pulmonary manifestations of lymphomatoid granulomatosis.

The pulmonary manifestations of 5 biopsy-proved cases of lymphomatoid granulomatosis are discussed. Two patients had diffuse reticulonodular infiltration of the lungs, thought to represent infiltrates of lymphomatoid granulomatosis. Three had nodular densities of varying size and distribution, with radiologically normal intervening lung, and biopsy suggested that these represented infarcts; the actual infiltrates of lymphomatoid granulomatosis were below the limits of resolution of the chest radiograph.

Acinic cell carcinoma: its occurrence in the laryngotracheal junction after thyroid radiation.

An acinic cell carcinoma of the laryngotracheal junction developed in a 54-year-old woman 46 years after administration of radiation to her thyroid gland. Although salivary gland neoplasia has been associated with a history of radiation therapy during childhood, acinic cell carcinoma in such a setting is rare. In addition, she had parathyroid hyperplasia and multiple thyroid adenomas, lesions that have been associated with prior administration of radiation. A history of exposure to radiation should be sought in patients with salivary gland neoplasms of the larynx or trachea.

Poorly differentiated lymphocytic lymphoma with ectopic parathormone production: visulization of metastatic calcification by bone scan.

Metastatic soft tissue calcification is known to occur in hypercalcemia and is usually present in the kidneys, stomach and lungs. 1--3 This case presents two unusual features: 1) ectopic parathormone production in association with poorly differentiated lymphocytic lymphoma; and and 2) uptake of 99mTc-pyrophosphate in the liver in the absence of demonstrable abnormality at autopsy. The more usual sites of metastatic calcification also showed uptake of the radionuclide. We will discuss metastatic soft tissue calcification, ectopic parathyroid hormone production, hypercalcemia in malignancy and bone scan agent localization in soft tissues.

Phosphatase enzymes. Cytochemical study of pleomorphic adenoma and normal human salivary glands.

Human parotid glands, submandibular glands, and pleomorphic adenomas were examined by electron microscopic histochemistry. All epithelial cells of the normal salivary glands showed plasma membrane adenosine triphosphatase (ATPase) and inosine diphosphatase (IDPase) activity. However, myoepithelial cells reacted most intensely. Pleomorphic adenomas showed epithelial cells within solid and ductal portions of the tumors that were variably reactive for both ATPase and IDPase. Histochemical examination of the epithelial cells in the myxoid portions of the tumors did not provide conclusive evidence as to the nature of their progenitor cells. Surface-associated phosphatases (alkaline phosphatase, ATPase, and IDPase) cannot be reliably used as histochemical markers of salivary gland myoepithelial cells. Therefore, morphological and phosphatase histochemical studies that intend to examine the role of myoepithelial cells in salivary gland neoplasms must be interpreted with care.

Ultrastructure of the parotid duct. Cytochemical studies of the striated duct and papillary cystadenoma lymphomatosum of the human parotid gland.

To our knowledge, there are currently no reports in the literature that discuss the fine structure of the striated ducts of the human parotid gland. Papillary cystadenoma lymphomatosum (Warthin's tumor) is a benigh neoplasm found almost exclusively in the parotid gland that represents about 16% of all neoplasms of the gland. This neoplasm is believed to arise from the striated and/or excretory ducts, but there are contradictions with regard to the proposed origin and the cellular composition of some Warthin's tumors. Tissue was obtained at the time of surgery and examined ultrastructurally and cytochemically for the localization of alkaline phosphatase and adenosine triphosphatase. Myoepithelial cells were found ultrastructurally and cytochemically on the proximal aspect of the striated duct, with their cell bodies situated at the junction of the striated and intercalated ducts. Two Warthin's tumors were observed with cells that were structurally and cytochemically similar to myoepithelial cells.

Hematological changes after long-term aluminum administration to normal adult rabbits.

The effects of long-term aluminum exposure on erythroid parameters were investigated in a rabbit system. Healthy young adult male rabbits were maintained on drinking water containing five mg per L of aluminum citrate; others were treated with an intravenous solution of aluminum maltol, 0.225 mmol of aluminum per week. In the oral study, decreases in the hematocrit, hemoglobin, and red cell count were observed over a 12-month period in those animals on soft water with a low calcium content and containing aluminum citrate; however, no changes were seen in those on hard water containing aluminum citrate nor in rabbits maintained on a normal diet. Small amounts of aluminum were observed in bone marrow macrophages, usually accompanied by iron, in both the orally- and intravenously-treated animals. There was poor correlation between bone marrow aluminum content and length of exposure.

The role of fine needle aspiration cytology in the diagnosis of lymphoma.

The accuracy of fine needle aspiration (FNA) cytology for the diagnosis of lymphoma and other hematolymphoid malignancies was investigated by a review of 158 FNA specimens from 143 patients. Patients included in the study had either a diagnosis of a hematolymphoid malignancy by FNA cytology or a biopsy diagnosis of lymphoma that was preceded by FNA cytology. Biopsy specimens were obtained from 85% of the patients. Of the 158 needle aspirates, 118 (75%) were diagnosed as lymphoma, 13 (8%) as suspicious of lymphoma, 8 (5%) as myelomas, 3 (2%) as leukemias, 12 (8%) as positive for malignancy and 4 (2%) as negative for malignancy. Two of the 118 needle aspirates diagnosed as lymphoma were false positives while 3 of 13 diagnosed as suspicious for lymphoma were found to be benign. Overall, there were four false negatives. Morphologic subclassification of the lymphomas, originally attempted for 60 needle aspirates, was identical to the histologic subclassification in 51 cases (85%). FNA cytology provided the initial diagnosis of a hematolymphoid malignancy in 51% of the cases and allowed the documentation of recurrent disease in 49%. The results demonstrate the usefulness of FNA cytology for the diagnosis and management of patients with lymphoma.

Comparison of diagnostic results obtained by fine needle aspiration cytology and tru-cut or open biopsies.

In seven cases fine needle aspiration (FNA) cytology provided a diagnosis of neoplasm when the Tru-Cut (TC) tissue biopsies (four cases) and open biopsies (three cases) were negative. The specimens consisted of two breast carcinomas, two metastatic neoplasms in the liver, one metastatic melanoma in inguinal lymph nodes, a retroperitoneal mass and a pelvic mass. In the two cases of mammary carcinoma, TC biopsies were negative and FNAs were diagnostic of carcinoma. TC biopsies in the two cases of questionable hepatic metastasis were negative, but FNAs demonstrated a malignant neoplasm. Open biopsy of a retroperitoneal mass failed to diagnose a neoplasm however, subsequent ultrasound-directed FNA demonstrated a neoplasm, possibly seminoma. FNA cytology of inguinal lymph nodes in one case was diagnostic of melanoma; open biopsy showed no neoplasm. Because of the FNA diagnosis, additional sections were made and the presence of melanoma was confirmed. This series demonstrates that FNA cytology should be considered the initial diagnostic procedure more often.
