99mTc-IDA imaging in the differential diagnosis of acute cholecystitis and acute pancreatitis.

99mTc-IDA (99mTc-dimethyl-acetanilide-iminodiacetic acid) hepato-biliary imaging was evaluated for its efficacy in distinguishing acute cholecystitis from acute pancreatitis. In a retrospective review, gallbladders were demonstrated by 99mTc-IDA in 13 of 15 patients (87%) with acute pancreatitis. This is significantly higher than reports on the frequency of gallbladder filling with oral and intravenous cholangiography in the presence of acute cholecystitis.

99mTc-IDA hepatobiliary imaging following upper abdominal surgery.

Bile flow patterns were studied with serial 99mTc-IDA images in 19 patients with cholecysto- and choledochointestinal anastomoses, gastroenteric bypasses, and combinations of the two. Complications such as anastomotic, afferent, and efferent loop obstruction and bile leakage were readily detected even in the presence of jaundice. This noninvasive technique warrants further investigation to determine its indications and weaknesses.

Assessment of the utility of gallbladder imaging with 99mTc-IDA.

Cholescintigraphy was performed in 113 patients of whom 38 had normal gallbladders, 37 had acute cholecystitis and 38 had chronic cholecystitis. The radiopharmaceuticals used in the study were either 99mTc-dimethyl acetanilide iminodiacetic acid or 99mTc-paraisopropyl acetanilide iminodiacetic acid, both of which performed equally well. All patients with non-visualized gallbladders had cholecystitis, but visualization did not exclude disease. The rapidity of obtaining the results (within one hour), the complete absence of untoward reactions to the radiopharmaceuticals, the much lower frequency of subtle or indeterminate results, the ability to render useful information in the presence of moderate jaundice and the lack of interference from overlying intestinal contents establishes these radionuclide agents as superior to both radiographic oral and intravenous cholangiography in the investigation of the acute abdomen.

Plasma insulin response to arginine stimulation in children and adolescents with constitutional short stature according to sex, age, sexual development and skinfold thickness.

The plasma insulin response to a standard arginine stimulation test was studied in 247 children and adolescents (177 males and 70 females) with constitutional growth retardation as compared with the response obtained in a matched group of 42 normal subjects and a group of 57 obese subjects. When the insulin response was related to pubertal ratings in the children and adolescents with growth retardation, it was found that the mean fasting levels and mean peak response both rose with the progression of puberty. This tendency was more pronounced in the females. When the insulin response was related to the degree of adiposity as measured by the degree of subscapular skinfold thickness, it was found that the greater the adiposity, the greater was the mean response, with the females showing higher levels than the males in each group. Even when the response in the two sexes was compared in subjects of a similar age and skinfold thickness, the females had a higher mean peak insulin response than did the males.

The radial dysplasia/imperforate anus/vertebral anomalies syndrome (the VATER association): Developmental aspects and eye findings.

The developmental evaluations of four children of different age groups with radial dysplasia/imperforate anus/vertebral anomalies syndrome are presented. These show that although the gross motor behavior is significantly delayed, intelligence, language, and social development are within normal range. Therefore, the patients with this syndrome merit every effort toward rehabilitation. Three of the patients discussed have ophthalmological abnormalities, in addition to their major malformations. It may be that congenital eye defects are another component of this syndrome of morphogenesis.

Intermittent treatment with human growth hormone (GH) in isolated GH deficiency and in multiple pituitary hormone deficiencies.

The results of intermittent GH treatment of 3-7 1/2 years duration in seven patients with isolated GH deficiency (IGHD) and five patients with multiple pituitary hormone deficiencies (MPHD) are presented. This therapeutic schedule was found to be comparably effective to those using a continuous-administration schedule. In contradistinction to the findings obtained with the latter, there was no progressive decline in growth velocity. The patients with IGHD were found to respond better than the patients with MPHD both in the first course as well as in consequent courses. In the intervals between courses, the growth velocity was less than in the pretreatment period in both groups. It is concluded that optimal results can be obtained by instituting an initial course of continuous treatment of 1 year's duration for the IGHD patients and of 2 years' duration for the MPHD patients, followed by an intermittent therapeutic schedule. This regime not only leads to the same growth achievement obtained with long-term continuous administration of GH but allows conservation of supplies of this very scarce hormone.

Effect of absorption edge filters on diagnostic x-ray spectra.

The spectra of diagnostic x-ray beams were measured with a germanium semiconductor dectector. With this system it was possible to define the K-alpha-1, K-alpha-2, K-beta-2 characteristic peaks of the tungsten target. The effects of various filters on the beam using characteristic absorption edges to create a "bandpass" effect were studied. Gadolinium filtration of the diagnostic beam used for contrast studies selectively removed both higher energy photons (greater than 50 keV) and lower energy photons (less than 33 keV) thus enhancing contst for iodine and barium while substantially reducing radiation dose.
