Subsequent malignancies associated with carcinoma of the uterine cervix: including an analysis of the effect of patient and treatment parameters on incidence and sites of metachronous malignancies.

The incidence and sites of metachronous malignancies were retrospectively determined from the records of 763 patients seen at Yale University medical Center and affiliated hospitals with previously untreated, invasive carcinoma of the uterine cervix from 1953-1972. These patients were treated predominantly with radiation therapy; follow-up status was known for periods of 5-25 years or until time of death in over 96% of the patients. Forty-four patients had second malignancies noted at least 6 months after the initial cervical cancer was diagnosed. The expected incidence of second malignancies was determined from the Connecticut State Tumor Registry data controlling for year of diagnosis of the cervical cancer, patient age, sex, and time at risk (person-years exposure). To correct for any error in estimation of second malignancies introduced by the existence of a latency period for the development of a second cancer, the expected incidence of malignancies was also computed for 5-year time intervals following the cervical cancer. No significant increase in second malignancies was found (observed/expected-44/36) for the entire follow-up period nor for any individual 5-year interval. However, a statistically significant increase in lung cancer and vulva-vaginal cancer was noted and a significant decrease in breast cancer was observed. Cox regression analyses were performed to study the effect of total radium exposure and total external beam treatment, adjusting for other factors. No statistically significant increased risks were found.

[Pathogenesis, clinical course and morphology of peliosis].

On the basis of two cases and a survey of literature the syndrome of the peliosis of liver, spleen and bone marrow is described and its pathogenetic factors are demonstrated. For peliosis is a syndrome which is characterized by multiple cavities filled with blood in the parenchyma. It is established that the peliosis has changed from very rare autopsy findings in a prognostically unfavorable basic disease has changed to a clinically significant and therapeutically controllable complication of the hormone therapy.

The effect of whole body heat exposure and of cooling the hypothalamus on antibody titre in the rat.

In rats implanted with chronic hypothalamic thermodes and immunized with sheep erythrocytes, body temperature was increased, 4 h per day for 2 weeks, either by exposing the animals to external heat or by cooling the preoptic area. The titre of antibodies against sheep erythrocytes was nearly tripled by preoptic cooling but was drastically decreased by heat exposure. These opposing effects of active and passive increases in body temperature indicate that factors other than the change in body temperature must also have played a significant role in modifying the humoral immune response.

Effect of aspirin treatment on chondromalacia patellae.

Twenty-nine patients (21 females and 8 males) with chondromalacia patellae diagnosed by arthroscopy were randomly allocated to receive aspirin or placebo for 3 months. Clinical and arthroscopic examination after 3 months showed no significant change in symptoms, signs, or macroscopic appearances in either group. Surgical treatment was performed in 14 patients for deteriorating symptoms.

[Histomorphological findings in the pancreas in diabetes mellitus (author's transl)].

There are some morphological signs for the clinical diagnosis of diabetes mellitus. The macroscopical findings of the skull (yellow staining, hyperostosis frontalis), of the liver (steatosis), of the pancreas (pancreatitis, atrophy) and the consistency of the brain as well as some histological changes of the pancreas may be important for the diagnosis. Our investigations of specimens form the pancreas of 100 diabetics and 50 nondiabetics allow the conclusion that amyloidosis and increased number of mast cells in the islets of Langerhans and an interstitial fibrosis of the pancreas can be used in the practice as signs for diabetes mellitus.

[Possibilities for the evaluation of receptor sites in benign prostatic hyperplasia and carcinoma of the prostate (author's transl)].

Cytoplasmic and nuclear receptor sites of the prostate will be determined for a qualitative information about the pattern of specific bindings. Their qualitative evaluation and their selective suppression will give an idea about the hormonal influence and regulation in prostatic tissue. These investigations are performed in some cases of prostatic cancer. The difficulty of tissue preparation is caused by the individual distribution of collagen tissue in prostatic cancer which will decide the yield of purified receptor sites. Besides the elemination of unspecific binding sites like SHBG and albumin is an evidence for the value of a method. According to high amount of binding sites the most effective preparation of patients before a tissue biopsy is discussed. For the clinical application the DCC test in case of cytoplasmic sites and the exchange test for nuclear sites seem to provide a suitable method. Performing gelfiltration and additional test with denatured material is necessary because of the similar size of SHBG and receptor complexes interfering the results.
