Control of glaucoma by reduced dosage guanethidine-adrenaline formulation.

The effect of formulations of guanethidine and adrenaline of different composition has been tested in rabbits and in patients with glaucoma. The concentrations of guanethidine and adrenaline used for the rabbits were 5.0% and 1.0%; 2.5% and 0.5%; 1.0% and 0.2%; 0.5% and 0.1%. All except the lowest combination were equally effective in the magnitude of the decrease in intraocular pressure brought about and in their duration of activity. Two formulations containing guanethidine and adrenaline at concentrations of 3.0% and 0.5% and 1.0% and 0.2% respectively (formulated as Ganda drops by Smith and Nephew Pharmaceuticals Ltd) were tested in a blind, cross-over, short-term clinical trial on 20 patients. The drops containing the lower concentration of drugs were as effective as those of higher concentration. These results lead us to believe that most patients who respond to this treatment could be put on a reduced dosage regimen, which should result in a decreased incidence and severity of side effects.

Phospholipids of the differentiating bacterium Caulobacter crescentus.

The phospholipid composition of the stalked and swarmer cell types of the differentiating, Gram-negative bacterium Caulobacter crescentus was determined. The phospholipid composition of the stalked cell type was 86.5% phosphatidylglycerol, 10.4% lysylphosphatidylglycerol, and 3.0% cardiolipin; that of the swarmer cell type was 84.1, 11.4, and 4.4% respectively. Phosphatidylethanolamine, which is a major phospholipid component of most Gram-negative bacteria, was totally absent.

Peritoneal mesotheliomas.

Eight cases of peritoneal mesothelioma have been seen at the University of Missouri Medical Center and affiliated hospitals since 1940. The incidence for our patient population is 0.24 cases per 100,000 patients. The nonspecific symptoms, physical findings, x-rays, and laboratory results usually preclude an early establishment of the diagnosis. The diagnosis was eventually established at laparotomy in seven cases and at autopsy in one. Only the benign mesothelioma was resectable; that patient is doing well 10 months after operation. The survival interval of the patients with malignant peritoneal mesotheliomas averaged 3 months, with the two patients who received chemotherapy surviving 3 and 6 months, respectively.

Keratitis due to Mycobacterium chelonei.

A case of ulcerative keratitis has been followed by ophthalmological, bacteriological and histopathological examinations. Mycobacterium chelonei has been repeatedly isolated from the lesion before and after surgical interventions.

Medical management of the sexually assaulted woman.

Public concern about the sexual assault victim demands that the medical profession be better prepared to evaluate such patients. Modern guidelines for the medical evaluation and treatment of such victims are provided. A list of supplies for a "rape tray" and guidelines for obtaining laboratory tests are included. A detailed patient consent form and laboratory chain of evidence form will facilitate care. A plea is made for legislative support to pay for lab work and the establishment of state or national laboratories equipped to handle evidentiary material.

Rupture of a gravid bicornuate uterus in a primigravida associated with clostridial and bacteroides infection.

The case is presented of a primigravid patient with spontaneous rupture of a bicornuate uterus associated with significant anaerobic infection. Thinning of the myometrium at the site of the rupture was noted in the pathologic specimen. The anaerobic infection, primarily contained in the intact amniotic sac, resolved with total abdominal hysterectomy, bilateral salpingo-oophorectomy, pelvic drainage and antibiotic therapy.

Uterine activty and plasma progesterone levels in pregnant goats.

Uterine activity was recorded during the last few weeks of pregnacy in goats, and related to changes in plasma progesterone concentration. In six of the 14 pregnancies, there was little activity until immediately pre-partum, but the remainder showed a progressive increase in uterine motility, particularly during the last seven days of pregnancy. There was a significant correlation between increased uterine activity and decline of peripheral plasma progesterone levels.

A system for assessing personal responsibility: validity, reliability and rater trainability.

Sixty-two subjects completed the California Psychological Inventory, the Rotter External-Internal locus of control scale and an audio-taped discussion of their personal problems. The audio-taped problems were rated on a five-point level of personal responsibility scale and were compared with the scores on the California Personality Inventory and the Internal-External locus of control scale in a correlation matrix which was subjected to a factor analysis. The results from these analyses supported the hypothesis that the Personal Responsibility Rating System has construct validity as a measure of psychological health. Study II assessed the trainability of the Personal Responsibility System. With a four-hour training program it was found that graduate students could be taught to rate personal responsibility in a reliable manner.

Prostaglandin levels in endometrial jet wash specimens in patients with dysmenorrhea before and after indomethacin therapy.

A patient with functional primary dysmenorrhea of over two years duration was subjected to the endometrial jet wash technique during the period of active menstrual flow. Prostaglandin F analysis of the jet washings revealed significantly elevated levels during menstruation over normal control levels. Following indomethacin therapy, jet wash prostaglandin F levels were dramatically reduced and the patient became asymptomatic. A cause and effect relationsship between prostaglandin F and dysmenorrhea is suggested by these studied.

Prostaglandin F levels in endometrial jet wash specimens during the normal human menstrual cycle.

The Gravlee endometrial jet wash technique has been used to collect uterine fluid in normal human volunteers for Prostaglandin F analysis throughout the human menstrual cycle. Uterine washings so obtained demonstrated a cyclicity in prostaglandin F content with low concentrations found during the proliferative phase and a 3-4 fold rise occurring during the secretory phase. Menstrual fluid prostaglandin F content collected with the jet wash technique gave the highest total concentrations.

Effect of adrenaline and guanethidine in reducing intraocular pressure in rabbits' eyes.

The effect of Adrenaline 0.5 per cent and Guanethidine 2.5 per cent alone and in combination in reducing intraocular pressure in rabbits has been studied. Six rabbits were used and the efficiencies of 5 different dosage protocols was estimated both as intensity and duration of activity. The 5 dosage protocols were Adrenaline alone; Guanethidine alone; Adrenaline and Guanethidine together; Adrenaline followed in 30 min by ?Guanethidine; and Guanethidine followed in 30 min by Adrenaline. The single formulation containing both drugs appeared to be at least as efficient as the others. Eye drops containing 1 per cent Adrenaline and 5 per cent Guanethidine having sufficient stability for normal use have therefore been formulated.

Effects of female sex hormones on uterine motility and reactivity to oxytocin in goats.

Uterine motility was recorded for several weeks in 10 ovariectomised goats, using small intra-uterine balloons, to study the effects of female sex hormones on uterine activity and reactivity to oxytocin. Oestradiol stimulated uterine contractions and increased both the sensitivity and intra-uterine pressure response to oxytocin. When progesterone was administered in addition to oestradiol, spontaneous uterine motility became irregular with low amplitude contrations; the sensitivity and reactivity to oxytocin was significantly depressed.
