Nuclear anatomy of diversionary central nervous system shunts in children.

A simple method of demonstrating shunt morphology with 99m technetium diethylene triamine penta-acetic acid (99m Tc-DTPA) is described. The anatomic abnormalities are illustrated. Demonstration of the site of obstruction is useful in selecting the neurosurgical approach. The procedure is minimally invasive, accurate, and simple to interpret.

Activation of Avena coleoptile cell wall glycosidases by hydrogen ions and auxin.

Several cell wall-bound glycosidases present in Avena sativa coleoptiles were assayed by following the hydrolysis of p-nitrophenyl-glycosides. Particular emphasis was placed on characterizing some parameters affecting the activity of beta-galactosidase. The pH optimum of this enzyme is 4.5 to 5.5; it is sensitive to copper ions and p-chloromercuribenzoate treatment and apparently has an exceptionally low turnover rate. Indoleacetic acid treatment enhanced in vivo beta-galactosidase activity of coleoptile segments by 36% over control after 60 minutes. This enhancement was prevented by abscisic acid and cycloheximide. High buffer strengths and low pH reduced the indoleacetic acid-enhanced increase in enzyme activity. These data lend support to the following proposed model of indoleacetic acid action. Indoleacetic acid enhances the release of hydrogen ions into the cell wall which promote the activities of cell wall glycosidases, some of which may participate in the cell extension process.

Physiological and Cytological Similarities between Disease Resistance and Cellular Incompatibility Responses.

Excised pea pods responded similarly to both the invasion of plant pathogenic fungi and the presence of bean tissue, bean pollen, and mouse tumor cells by synthesizing pisatin and by developing a characteristic yellow-green fluorescence. Both responses were dependent on RNA and protein synthesis. Conversely, the foreign pollen and incompatible fungi were sensitive to the pea pod tissue and were subject to abnormal development. The induction of pisatin and the yellow-green fluorescence development were mediated by multiple compounds of varying sizes released by fungi or mouse tumor cells. The incompatibility between a bean pathogen, Fusarium solani f. sp. phaseoli, and pea pod tissue was hypothesized to occur as a result of the cross contamination of such inducing compounds.

Association between administration of hepatitis B vaccine at birth and completion of the hepatitis B and 4:3:1:3 vaccine series.

CONTEXT: The association between infant age at initiation of hepatitis B vaccination and completion of the 3-dose hepatitis B vaccination series is unclear. OBJECTIVE: To assess the association between administration of the first dose of hepatitis B vaccine within 7 days of birth and completion of the hepatitis B vaccine series and the 4:3:1:3 vaccine series (4 doses of diphtheria-tetanus-pertussis vaccine, 3 doses of polio vaccine, 1 dose of measles-containing vaccine, and 3 doses of Haemophilus influenzae type b vaccine). DESIGN, SETTING, AND PARTICIPANTS: Analysis of data from the 1998 National Immunization Survey, a random-digit-dialing telephone survey (n = 34,480 completed interviews) of parents of children aged 19 to 35 months from 50 states and 28 selected urban areas in the United States that included a provider record check mail survey. MAIN OUTCOME MEASURES: Percentage of infants who received at least 3 doses of hepatitis B vaccine and percentage who received the 4:3:1:3 vaccine series, by age at receipt of the first dose of hepatitis B vaccine. RESULTS: Overall, 86.9% of children 19 to 35 months of age in 1998 received 3 or more doses of hepatitis B vaccine, and 79.9% completed the 4:3:1:3 vaccine series. Multivariate analysis indicated that, compared with children who received the first hepatitis B vaccine dose within 7 days of birth, odds ratios (ORs) for not completing the 3-dose hepatitis B vaccine series among children who received the first dose at 8 to 41 days, 42 to 91 days, 92 to 182 days, 183 to 273 days, and 274 or more days of age were 2.4 (95% confidence interval [CI], 2.0-3.0), 7.8 (95% CI, 6.5-9.3), 9.6 (95% CI, 7.0-13. 3), 18.3 (95% CI, 12.0-28.0), and 46.6 (95% CI, 33.7-64.5), respectively; ORs for not completing the 4:3:1:3 vaccine series among these same groups were 1.0 (95% CI, 0.8-1.1), 1.0 (95% CI, 0. 8-1.1), 1.7 (95% CI, 1.3-2.3), 3.8 (95% CI, 2.6-5.6), and 4.0 (95% CI, 2.9-5.5), respectively. CONCLUSION: Administration of the first dose of hepatitis B vaccine at birth is associated with increased likelihood of completion of the hepatitis B vaccination series. JAMA. 2000;284:978-983

Molluscum contagiosum: a neglected sentinel infection.

Perigenital molluscum contagiosum in adults is recognized as being a sexually transmitted infection. A retrospective casenote review was performed on patients attending a department of genitourinary medicine (GUM) who had been diagnosed as having molluscum. One or more co-existent sexually transmitted diseases (STD) were present in 30.4% of 158 men and in 32.1% of 78 women. High rates of concurrent STD were also found in patients whose only presenting symptom was molluscum-in 23.2% of men and 10.5% of women. These rates of STD would undoubtedly have been higher still had the screening of these patients been more comprehensive. In view of these findings it is advisable that adults with perigenital molluscum contagiosum be screened for other STD and that their sexual partners be contact-traced. This wil usually required referral to a department of GUM.

Optimized Monier-Williams method for determination of sulfites in foods: collaborative study.

A collaborative study was conducted of the Food and Drug Administration (FDA)-optimized Monier-Williams method for determining sulfites in foods. Twenty-one industry and government laboratories participated in the study, which was jointly sponsored by the National Food Processors Association and FDA. Familiarization samples were shipped to each collaborator. Collaborators were permitted to proceed to the main study only after they demonstrated ability to perform the method to ensure that the study tested the performance of the method itself and not that of the individual laboratories. The study design involved 3 food matrixes (hominy, fruit juice, and protein [seafood]). Each matrix was prepared at 3 sulfite levels--the regulatory level, half the regulatory level, twice the regulatory level--and as a blank. All test samples were analyzed as blind duplicates, which gave each collaborator a total of 24 test portions. Collaborative recoveries gave a reproducibility (among-laboratories) coefficient of variation that ranged from 15.5 to 26.6% for sulfite determined as SO2 by weight in the 3 foods at the 10 ppm level. The optimized Monier-Williams method has been approved interim official first action to replace the AOAC modified Monier-Williams method, 20.123-20.125.
