Organ donation in the Hispanic population: dondé estan ellos?

Few studies have investigated organ donation among Hispanics, although in major US cities, Hispanics, like African Americans, make up a large percentage of the general population. In fact, the 1990 census reports that of all Hispanics in the country, 90% live in urban areas. The tendency for Hispanics to use hospital emergency rooms rather than visit a physician's office may result in an inadequate exchange of information between minority patients and health-care providers. Hospitals that serve minority populations have been reported to have the lowest organ procurement rates. These hospitals are often funded by local, county, or state funds and are usually dedicated to a patient mix that is largely indigent. The heavy work load of health-care providers who often have limited resources does not lend itself to an adequate exchange of information between patients and health-care providers. The existence of this type of environment impacts on patient communication, increases the fear of complication, and may diminish the discussion of potential organ procurement. There are multiple individual and structural reasons for the high family refusal rates among Hispanics. Procurement specialists may see a proportionally small number of Hispanic families, mainly due to a lack of referrals from hospital staff concerning possible Hispanic donors. White procurement workers may not be familiar with the Hispanic family makeup and cultural attitudes. Organ procurement workers may not pursue the issue of donation with Hispanic families as much as they do with white families. Whatever the reason, if we are to increase organ donation in the Hispanic community, these complex issues will need to be addressed.

The spontaneous regression of lymphoma in AIDS.

Non-Hodgkin's lymphomas are an increasing problem in the AIDS population. They are generally aggressive, high-grade lymphomas and more commonly present at extranodal sites, particularly the central nervous system. Although chemo- and radiosensitive, the duration of response is generally short lived. Spontaneous remission of non-Hodgkin's lymphomas has been reported in immunocompetent individuals, but has not been reported in HIV disease. We would like to report the first such case.

Surface co-expression of Vibrio cholerae and Salmonella typhi O-antigens on Ty21a clone EX210.

In an attempt to construct a bivalent, live, oral cholera-typhoid vaccine, genes specifying the biosynthesis of Vibrio cholerae O-antigen have been transferred into a modified version of the attenuated, oral typhoid vaccine strain Salmonella typhi Ty21a. The present report investigates the production of V. cholerae and S. typhi O-antigens by one such clone, EX210. When cultured without galactose supplementation EX210 produces surface O-antigen of V. cholerae type, as detected by haemagglutination-inhibition and bactericidal assays, and by immuno-electron microscopy. However, the protective efficacy of Ty21a depends upon growth in the presence of exogenous galactose and under these conditions only S. typhi O-antigen is detectable on the surface of EX210. Subsequent experiments revealed that the proportion of polysaccharide of S. typhi type is dependent upon the level of galactose supplementation, and identified a limiting sugar concentration which results in surface co-expression of both O-antigens. Visualization of the two polysaccharides on silver-stained polyacrylamide gels indicates that S. typhi O-antigen subunits are polymerized into longer sidechains, suggesting that at higher galactose concentrations their predominance results in a masking of the shorter V. cholerae O-antigen.

Nonshared environmental influences and personality differences in adult twins.

The twin design was used to examine the importance of different experiences of siblings within the family and to identify relations between twins' personality differences and their differential experiences. A sample of 161 monozygotic and 74 dizygotic twin individuals between the ages of 18 and 75 years retrospectively reported on their different experiences when growing up. The Sibling Inventory of Differential Experience (SIDE) was used for the first time with a sample of twin siblings. In addition, the twins provided self-report measures of affect and personality. In contrast to results from a sibling adoption design, this study of twins showed greater evidence for genetic variance in the SIDE scales. Nevertheless, the SIDE showed significant associations with differences in personality and affect for monozygotic twins, which reflect pure environment-behavior relations.

Two amino acids in an RNA polymerase sigma factor involved in the recognition of adjacent base pairs in the -10 region of a cognate promoter.

The recognition of promoter region -10 nucleotide sequences in prokaryotes is believed to be mediated by a segment of alpha-helix in a region of RNA polymerase sigma factors called 2.4. Earlier genetic studies implicated Thr-100 in region 2.4 of the Bacillus subtilis sigma factor sigma H in the recognition of the G.C base pair at position -13 in the -10 region (GAAT) of a cognate promoter. In confirmation of this assignment, we now show that a change-of-specificity mutant of sigma H in which Thr-100 was replaced with isoleucine suppresses a G.C----A.T nucleotide substitution at position -13 but not other "promoter down mutations" (causing impaired promoter activity) at positions -13, -12, and -11. We also show that a loss-of-contact mutant created by the replacement of Thr-100 with alanine (having a short side chain) enables sigma H to tolerate three different promoter down mutations at position -13 but not down mutations at other positions. Finally, we suggest the identification of an additional amino acid involved in base-pair recognition by the demonstration that the replacement of Arg-96 with alanine specifically suppresses an A.T----G.C promoter down mutation at position -12. The identification of amino acids that are four residues apart that are involved in the recognition of adjacent base pairs may fix the orientation of region 2.4 (its NH2 terminus being proximal to the promoter transcription start site) and is consistent with a model in which the recognition of promoter region -10 nucleotide sequences is mediated by an alpha-helix in which residues involved in base-pair contact are separated by one turn and clustered on one face of the helix.

Gadolinium-enhanced magnetic resonance imaging in facial nerve lesions.

We present our 2-year experience with a contrast enhancement agent in magnetic resonance imaging (MRI) examining facial nerve pathology. Characteristics of the agent gadolinium diethylenetriamine pentaacetic acid are reviewed. The radiographic capability to differentiate pathologic vs. normal facial nerves based on enhancement and a change in signal intensity generated by the nerve is demonstrated. Experience with facial nerve disorders including Bell's palsy, facial nerve neuromas, a facial nerve graft site, postoperative facial paralysis, and traumatic facial paralysis is presented. The ability to image the facial nerve in Bell's palsy provides an entirely new means of examining this disorder, and its implications are discussed. The ability to enhance the paralyzed facial nerve in the temporal bone after posterior fossa surgery supports a previously held concept as to the pathophysiology of this problem.

Prostaglandin-E2 9-ketoreductase from swine kidney. Production of antisera and application to development of a radioimmunoassay.

Prostaglandin-E2 9-ketoreductase (PGE2-9-KR, EC 1.1.1.189), the enzyme which catalyzes the reaction from prostaglandin E2 (PGE2) to prostaglandin F2 alpha (PGF2 alpha), was purified 580-fold from swine kidney. The molecular mass of the enzyme determined by SDS-gel electrophoresis was 33 kDa. Antiserum against the purified enzyme was raised in three rabbits. The antiserum was able to precipitate PGE2-9-KR from swine kidney and to crossreact with pGE2-9-KR from several reproductive organ tissues, such as rabbit ovary, rabbit corpus luteum, rabbit endometrium and human decidua vera. When swine kidney PGE2-9-KR was labelled with 125I and incubated with affinity-purified antiserum in the presence of increasing amounts of unlabelled enzyme, competitive binding of the unlabelled enzyme to the antibody was observed. A radioimmunoassay for the quantitation of the enzyme was developed. The standard curve was linear from 5 to 500 ng enzyme. The intra- and interassay coefficients of variation were 6.4 and 13.2%, respectively. The assay may be useful for the quantitation of PGE2-9-KR in several tissues under various physiological conditions.

Studies on the prostaglandin E-2-9-ketoreductase mediated production of prostaglandin F-2 alpha and its metabolism in cultured rabbit luteal cells.

Luteal cells were isolated from pseudopregnant rabbits on days 3, 6, 9, 12 and 15 post ovulation. Prostaglandin concentration and the activities of the enzymes prostaglandin E-2-9-ketoreductase and prostaglandin-15-hydroxydehydrogenase were determined. Luteal cells from day 7 and 12 of pseudopregnancy were maintained in culture for 24 h and then exposed to a mixture of [1-14C]PGE2 (70 mumol/l) in the presence or absence of estradiol-17 beta. After a 24 h incubation period, the culture medium was adjusted to pH 3.5, immediately extracted and analysed for PG. Cultured luteal cells were able to convert exogenously applied PGE2 to PGF2 alpha and to metabolize both PGs. Primary PGs as well as their metabolites 15-keto PGE2, 15-keto PGF2 alpha, 13,14-dihydro-15-keto PGE2, and 13,14 dihydro-15-keto PGF2 alpha were detected in the culture medium and in the cells. The addition of estradiol-17 beta together with PGE2 caused a significant reduction in the PGE2-9-ketoreductase mediated PGF2 alpha synthesis, whereas the metabolism of PGE2 remained unchanged. This inhibitory effect of estradiol-17 beta was dose-dependent on day 12 of pseudopregnancy. The results demonstrate that isolated rabbit luteal cells are able to synthesize and metabolize the luteolytic factor PGF2 alpha. Whether the inhibitory effect of estradiol-17 beta may have any physiological relevance has to be examined in further studies.

Determination of the minimum time of praziquantel therapy required for the in vitro treatment of protoscoleces of Echinococcus granulosus.

Ovine and equine protoscoleces of Echinococcus granulosus were cultured for 26 days with our without praziquantel and viability assessed, by eosin exclusion, for cultures in various drug concentrations (50, 250 and 500 micrograms/l) and periods of exposure (1, 3 or 7 days (d] before removing/'rescuing' to drug-free medium. Drug efficacy was proportional to drug concentration and to length of exposure. At higher drug concentrations shorter exposures were required to produce the effect of continuous drug treatment, 1d therapy at 500 micrograms/l killing 96% ovine protoscoleces by day 14 whereas 7d therapy at 50 micrograms/l was required to produce a similar effect. Equine protoscoleces appeared marginally less susceptible than those of ovine origin. The relevance of the results in the need for peri-operative prophylaxis against spilled protoscoleces in man is discussed.

Echinococcus granulosus: the effects of praziquantel, in vivo and in vitro, on the ultrastructure of equine strain murine cysts.

Praziquantel (500 mg/kg) administered orally to BALB/c mice with secondary equine E. granulosus daily for 21, 30 or 30 + 30 days without the drug resulted in the majority of cysts, using bench criteria of turgidity and eosin exclusion, being assessed as 'alive'. Ultrastructural examination of 54 of these 'alive' cysts did not support this conclusion. They all showed increased vesiculation of the germinal layer leading, in many, to the loss of its integrity. Increased mitochondrial numbers occurred frequently. The longer drug treatments appeared to have greater effects on the germinal layer of 'alive' cysts and there was no detectable reestablishment of structural organization within 30 days after drug withdrawal. Subjectively, there was no substantial difference between cysts from 4-month and 9-month infections or between affected peritoneal and hepatic cysts. Tissue from collapsed cysts was necrotic. Peak serum levels of praziquantel (6430-6136 micrograms/l) occurred 5-10 min after drug administration (500 mg/kg) and dropped rapidly to less than 10 micrograms/l at 3 h. In an in vitro study at praziquantel concentrations of 1000 and 5000 micrograms/l over a 10-day period, most cysts were judged 'alive' by bench criteria but showed ultrastructurally a time- and concentration-dependent loss of integrity identical to that seen in vivo. Turgidity and eosin exclusion therefore underestimate the effect of praziquantel and the results indicate that in vitro experiments can fulfil a legitimate preliminary role in a hydatid chemotherapy programme.

A galE via (Vi antigen-negative) mutant of Salmonella typhi Ty2 retains virulence in humans.

We have recently described the construction of a galE derivative of Salmonella typhi Ty2 (Ty2H1) which had a 0.4-kilobase deletion in the galE gene and was sensitive to galactose-induced lysis when cultured with greater than or equal to 0.06 mM galactose (D. M. Hone, R. Morona, S. Attridge, and J. Hackett, J. Infect. Dis. 156:167-174, 1987). We now report the selection of a rifampin-resistant, via derivative of Ty2H1, EX462. Compared with the Ty2 parent strain, EX462 was serum sensitive and highly attenuated in the mouse mucin virulence assay. When four human volunteers ingested 7 X 10(8) viable EX462, two became ill and developed a typhoidlike disease with fever and bacteremia. Blood isolates from these individuals were indistinguishable from the vaccine strain by a variety of criteria. We concluded that, even in a via background, the galE mutation was not attenuating for S. typhi in humans.

Studies on prostaglandin metabolism in corpora lutea of rabbits during pregnancy and pseudopregnancy.

Corpora lutea and ovarian stromal tissue were analysed for prostaglandin (PG) concentrations and activities of enzymes involved in PG metabolism at 8, 10, 12, 13 and 15 days after induction of ovulation. In CL of pseudopregnant rabbits, the PGE-2-9-ketoreductase (PGE-2-9-KR) was highly active on Days 10, 12 and 15 when compared with Day 8 (P less than 0.01; P less than 0.001; P less than 0.05). In pregnant animals PGE-2-9-KR activity was only increased on Day 12 (P less than 0.05) but declined to basal levels on Days 13 and 15. Comparing PGE-2-9-KR activity of pseudopregnant and pregnant animals, a significant elevation was found on Day 15 of pseudopregnancy (P less than 0.025). Activities of PG-15-hydroxydehydrogenase did not exhibit any significant changes with time in pseudopregnant or pregnant rabbits. PGE-2 concentrations were increased on Days 12, 13 and 15 (P less than 0.025) when compared with Day 8. Changes in PGF-2 alpha concentrations paralleled those of PGE-2-9-KR. The concentrations of PG metabolites 13,14-dihydro-15-keto-PGE-2 and -PGF-2 alpha were lower than those of the primary PGs and did not show stage-specific changes in pseudopregnant and pregnant animals. These results demonstrate that the rabbit CL possesses enzymes to convert PGE-2 to PGF-2 alpha and to metabolize both PGs. PGE-2-9-KR may be involved in regulating the PGF-2 alpha/PGE-2 ratio and possibly in controlling the life-span of the corpus luteum.

Magnetic resonance imaging in the diagnosis of lower thoracic disc herniation.

The diagnosis of thoracic disc herniation can be difficult due to the lack of a characteristic clinical presentation. In six recent cases, magnetic resonance imaging (MRI) provided excellent noninvasive definition of the pathology and its level, in spite of atypical clinical presentation, and also provided anatomic information allowing surgical planning. Two examples are presented in which myelography was not helpful, but MRI was diagnostic. MRI scanning is the radiographic procedure of choice when thoracic disc herniation is in the differential diagnosis.

Psychosocial risk and resistance factors among children with chronic illness, healthy siblings, and healthy controls.

Psychosocial risk and resistance factors within the domains of parental functioning, family stressors, and family resources were examined as predictors of psychological adjustment and physical problems in juvenile rheumatic disease patients (N = 93), their healthy siblings (N = 72), and demographically matched healthy controls (N = 93). Family socioeconomic status and background variables showed few consistent relationships with child functioning. However, a constellation of risk and resistance factors tended to show comparable associations with functioning for patients, siblings, and controls. Higher parental depression and medical symptoms and more family stressors, sibling problems, and burden of illness on the family predicted more problems among the patients. These relationships held when disease duration and severity were controlled. For the siblings, increased parental and patient dysfunction, more family stressors, and less family cohesion and expressiveness were associated with more problems. Although the associations were not as strong, mothers' depression and lack of family cohesion and expressiveness also were related to more adjustment problems among the control children. These findings imply that there may be a general association between certain risk and resistance factors and childhood adaptation.
