Increased incidence of malignant melanoma of the skin in workers in a telecommunications industry.

In 1982 physicians at a hospital melanoma clinic in Montreal noticed that among their patients there had been seven men working in a single telecommunications company. This raised suspicions that working in that industry might be associated with development of malignant melanoma of the skin (MMS). A preliminary gross comparison with general population rates indicated that there was an increased risk in this working group. To estimate the risk of MMS more accurately, a standardised incidence ratio (SIR) was calculated based on the rates of MMS in the local population of the Greater Metropolitan Montreal Area for the years 1976-83. During that period, among workers in all plants for the company, 10 male cases of MMS were observed for an expected number of 3.7 (SIR = 2.7; 95% CI = 1.31-5.02). No cases were observed among female workers (expected = 1.3). The excess was significant among cases with a short latency (less than 20 years since beginning of employment). There was no apparent pattern of exposure based on job titles or departments.

Estimating the relationship between exposure to tar volatiles and the incidence of bladder cancer in aluminum smelter workers.

A previously reported case-referent study of 85 incident cases of bladder cancer among aluminum smelter workers and 255 matched referents revealed an excess risk among workers exposed to coal-tar pitch volatiles. For the study reported in the present investigation these data have been augmented by estimates of past workplace exposure to total tar (benzene-soluble matter) and to benzo-a-pyrene (BaP). From these new data, exposure-response relationships have been estimated by maximum likelihood. A linear relationship between cumulative exposure and relative risk and a minimum latency period of ten years were assumed on a priori grounds and found compatible with the data. Under these assumptions, relative risk increased for each year of exposure to benzene-soluble matter at a concentration of 1 mg/m3 by 13%, the 95% confidence interval being 5-31. The corresponding figure for BaP (as micrograms/m3 X year) was 2.3%. On the basis of these estimates, 40 years of exposure to benzene-soluble matter at the current exposure limit of 0.2 mg/m3 would lead to a relative risk of 2.4. There was suggestive but not conclusive evidence that relative risks due to exposure to tar volatiles and to cigarette smoke combined multiplicatively.

Induction of fetal demise in advanced pregnancy terminations: report on a funic potassium chloride protocol.

Advanced pregnancy termination with uterotonic agents with their inherent risk for occurrence of a live birth and surgical evacuation of a fetus perceived to be vigorous can both be sources of emotional anguish for women. They argue for offering women an option for induction of fetal demise prior to uterine evacuation. We report on our experience with 60 pregnancies in which lethal fetal administration of potassium chloride was performed prior to evacuation of the uterus. We describe a double-bolus technique for funic intravascular injection of potassium chloride to arrest the fetal heart. There were no maternal complications and the procedure was successful in 86.7% (n = 52) of the cases; of the remaining 8 cases, 7 had demise induced by direct fetal cardiac injection, and a live birth occurred in 1 case.

Treatment of depression in advanced Alzheimer's disease using sertraline.

Effects of sertraline on the affect and food refusal of patients with advanced dementia of the Alzheimer type (DAT are reported. Out of 10 patients with depressed affect, sertraline improved affect in eight. Six of these patients were also refusing food, and sertraline diminished food refusal in five of them. A case report of a patient who had a dramatic improvement in response to sertraline treatment is presented.

The plasticity of compensatory eye movements in rotatory tests. I. The effect of alertness and eye closure.

Fifteen voluntary subjects with a mean age of 26 years (17-39), participated in two rotatory experiments. The rotatory test used was sinusoidal harmonic acceleration (SHA) test at frequencies of 0.01-0.32 Hz. Compensatory eye movements were measured by means of EOG. Experiment A included rotation in darkness during alerting tasks (day 1), rotation in darkness with no tasks (day 2) and rotation in darkness during alerting tasks (day 3). In the alertness tests, the gain varied between 0.5 and 0.77. Phase values decreased with increasing frequency. The alertness results between days 1 and 3 did not differ in gain or phase. The gain of day 2 (no tasks) displayed significantly reduced gain at all frequencies, and a more pronounced decrease in phase than in the alertness tests. Experiment B comprised testing with eyes open (day 1) and eyes closed (day 2) during alerting tasks. These tests displayed significantly decreased gain at all frequencies with eyes closed compared with eyes open and with more negative phase at 0.08-0.32 Hz with eyes closed. The results indicate a central effect of the alertness level on compensatory eye movements, and the effect of eye closure might be explained by bio-mechanical and possibly even central effects on the eye movements. In low-frequency testing, as short a test time as possible should be used, and due to different alertness levels, the results should be evaluated with great caution.

[Could the Quebec Tumor Registry be used to identify risks of occupational cancers?].

Many countries have used their tumor registries to conduct studies on cancer and occupation. In the past, the Quebec Tumor Registry (QTR) has collected information on occupation of cases. The form currently in use has a space to include this information but it is not mandatory to do so. Among male cases of naso-sinusal, pleural and ocular cancer, diagnosed between 1975 and 1979, and aged 16 to 64, 72% had information on occupation. The occupation registered at the QTR and the occupation given by the case himself during epidemiological studies on bladder cancer and leukemia are respectively the same in 64% and 69% of cases. The association between mesothelioma and work in asbestos related industries was not found in the present study. Explanations and recommendations are given.
