[Varioliform gastritis revealed by edema with hypoproteinemia (author's transl)].

The authors report the case of a woman in whom edemas with hypoproteinemia, but without digestive symptoms, were present for several months. Roentgenography, fiberoptic endoscopy, and histopathology showed typical varioliform gastritis. Hypoproteinemia was due to plasma protein leakage into the digestive tract. Edema disappeared spontaneously while gastric pain developed.

Effect of folate deficiency on vitamin B12 absorption.

In three different groups of patients presenting severe folate deficiency (purely nutritional folate deficiency, chronic alcoholism or various intestinal diseases), vitamin B12 absorption has been tested by Schilling test in order to investigate the possible effect of folate deficiency on B12 absorption. Following this study, it appears that folate deficiency in itself, even severe, cannot induce vitamin B12 malabsorption, since in the first group, consisting of malnourished old people, the Schilling test was always normal even in those who presented a lowered B12 serum level; in the group of patients with intestinal disease, the Schilling test was abnormal as expected in some patients; 3 alcoholics out of 12 presented a malabsorption of B12 on the Schilling test; the follow-up of 2 of them exhibited complete correction after normal diet and alcohol suppression. Consequently, folate deficiency does not seem responsible for a secondary B12 malabsorption unless another etiological factor is present, such as alcohol or ileopathy.

Evidence for circulating fibrin in familial Mediterranean fever.

Cryofibrinogenemia was found in 10 of 24 plasma samples (42%) from subjects with FMF. This precipitate was found during active disease as well as during intervals between crises. We found a higher incidence of cryofibrinogenemia in subjects with mild to moderately severe disease not being treated with colchicine (six of eight) as compared with colchicine-treated subjects who were in partial or complete clinical remission (four or 16; p less than 0.02). All cryofibrinogen precipitates contained fibrin, as assessed by electrophoretic analyses showing the presence of multimeric crosslinked forms of fibrin(ogen) linked by gamma-dimers. This finding in clinical specimens supports the hypothesis that fibrin in an obligatory component of cryofibrinogen. Fibrin was also found in HPF (two of six specimens) prepared from cryofibrinogen-negative FMF plasmas, thus showing that soluble forms of fibrin are even more prevalent in this disorder than is indicated by the frequent finding of cryofibrinogenemia.

[Familial Mediterranean fever, complement, cryofibrinogen, colchicine (author's transl)].

The components C1, C4, C2, C3 of the complement system were measured in a group of 24 patients with Familial mediterranean fever (F.M.F.). The presence of cryofibrinogen and its analysis was performed in the same patient. C4, C2, C3, were increased in the F.M.F. group. Cryofibrinogenemia was found in 42 p. 100 of the patients. The cryofibrinogen was composed of fibrin, fibrinogen and plasma fibronectin. The cryofibrinogen was less frequent in the group of patients treated by colchicine (p less than 0.02).

[Gamma-heavy chain disease with autoimmune abnormalities and peripheral neuropathy (author's transl)].

The authors report the case of a 72 year old woman affected by gamma heavy chain disease in whom the heavy chain was detected in the serum and the urine as well. Further study of the paraprotein showed that it belonged to the gamma 1 subclass. This observation was unusual because of the presence of a 15 year-long peripheral neuropathy and the existence of various auto-antibodies, especially intrinsic factor antibody.
