Liver blood flow and oxygen consumption during hypocapnia and IPPV in the greyhound.

Pentobarbital-anesthetized greyhounds were passively hyperventilated using intermittent positive-pressure breathing (IPPV) and the effects of raised airway pressure, accompanied by hypocapnia and then by normocapnia, on liver blood flow and oxygen consumption were studied. Electromagnetic flowmeters were used to measure hepatic arterial, portal venous, and splenic venous blood flow. Studies were carried out at three levels of raised airway pressure, both at normocapnia and hypocapnia. It was found that hypocapnic hyperventilation produced a decrease in portal venous and hepatic arterial blood flow. Normocapnic hyperventilation resulted in a restoration of portal venous blood flow but with a further decrease in hepatic arterial blood flow. A decrease in oxygen consumption with hypocapnia, returning to control values with normocapnia, was seen. It is suggested that the reduction in liver blood flow and oxygen consumption seen with passive hyperventilation is chiefly an effect of hypocapnia and is largely reversed by restoration of normocapnia.

The association between lead concentrations in teeth and domestic water lead concentrations.

Tooth lead levels have been measured in 109 children living in Glasgow. A significant association has been demonstrated between molar tooth lead concentrations and domestic drinking water lead concentrations. Tooth lead concentrations were found to be greater the longer a child had liver in older housing with lead plumbing during foetal life and following birth. Tooth lead concentrations were also found to be age-related although no difference with respect to social class could be found.

Intrapartum fetal asphyxia: a preliminary report in regard to long-term morbidity.

This is a preliminary report of a prospective follow-up study of 42 infants who had episodes of intrapartum fetal asphyxia at delivery identified by an acid-base assessment and a control group of 69 babies who had no evidence of intrapartum fetal asphyxia. The newborn infants were mature at delivery. There were no major neurologic disabilities in the asphyxia group. The pattern of physical growth and the mental and physical development indices of the babies of the asphyxia group were similar to those of the control group babies at 12 months of age. Results have not as yet indicated that the mature fetus with at least a terminal episode of asphyxia will exhibit evidence of handicap due to central nervous system injury.

Intrauterine growth retardation: a preliminary report of long-term morbidity.

This is a preliminary report of a prospective follow-up study of 88 intrauterine growth-retarded (IUGR) babies and a control group of 97 babies with weights appropriate for gestational age. The characteristic clinical features of IUGR pregnancies were observed in the obstetric patients of the IUGR group. The babies of the IUGR group have a phase of accelerated growth during the 3 months following delivery although they continue to be smaller than the babies of the control group at 12 months of age. No major neurologic abnormalities have been noted in the IUGR babies although behavioral differences were observed during the neonatal period and at 12 months of age. There were lower mental and physical development indices at 12 months of age which were due to the IUGR babies of lowest birth weight and those with a low maternal urinary estrogen index.

Components of the haemostatic mechanism in twin pregnancy.

Components of the haemostatic mechanism were measured at intervals throughout pregnancy in 11 patients with twins and compared with a group of 21 patients with singleton pregnancies. Mean plasma fibrinogen levels were consistently (but not significantly) elevated in the mothers of twins while other parameters showed no differences.

Out of hours calls.

Approximately 1000 out of hours calls performed by Auckland general practitioners are analysed. These were all general practitioners who personally or by the use of rosters care for their own patients. In a subsequent study it is intended to compare the out of hours work done by a deputising service.

The cause of the fall in serum folate in normal pregnancy.

Study of a complete antenatal population, a group of normal primigravid singleton pregnancies, and a group of normal twin pregnancies, suggests that the fall in the serum folate concentration which occurs in normal pregnancy is due principally to plasma volume expansion and should therefore be regarded as physiological.

Esophagopleural fistula after pneumonectomy.

Rupture of the esophagus into the space left after pneumonectomy is a rare and often fatal complication. Esophagopleural fistulas occurred in three patients following pneumonectomy. After previous methods failed, two patients were successfully treated by using a one-stage procedure which included (1) suture closure of the fistula, (2) buttressing the repair with a viable, pedicled, two-rib intercostal-muscle flap, and (3) performing an extensive thoracoplasty with a continuous drip infusion of neomycin. Such a procedure offers the maximum opportunity for successful treatment of this catastrophic lesion.
