Uncomplicated HL-A matched sibling bone marrow graft for combined immune deficiency.

When an apparently identical HL-A donor can be found and satisfactorily checked by the mixed lymphocyte reaction, it is worth attempting simple bone marrow grafting in a patient with combined immune deficiency syndrome who otherwise would die. Immunocompetence was given in this way to a baby girl by her 4-year-old brother and was confirmed by the presence of Y-chromosome in most of the transforming lymphocytes. IgA was late to emerge, presumably owing to the immaturity for IgA of the donor's bone marrow.

Effect of cigarette smoking on lipids, lipoproteins, blood coagulation, fibrinolysis and cellular components of human blood.

SUMMARY: A group of 187 volunteers aged 30-60 years were divided into heavy and light smokers and non-smokers. Heavy smokers smoked over 20 cigarettes per day and light smokers between 5-15, averaging about 7 cigarettes per day. (1) Compared with male non-smokers (N.S.), the male heavy smokers (H.S.) had a higher fasting serum turbidity, higher levels of cholesterol (mainly confined to the ester cholesterol fraction) and higher levels of serum phospholipids and triglycerides. In this H.S. group, the esterified fatty acid index (EFI) of beta and pre-beta lipoprotein were also raised as shown by paper and cellulose acetate electrophoresis. The Stypven clotting times were also shorter than for the N.S. group. (2) Increased levels of triglycerides, pre-beta EFI, longer fibrinolysis times and shorter Stypven times were recorded in the group of female heavy smokers. Changes in cholesterol levels, beta EFI, phospholipids and fasting serum turbidity were not seen in this group. (3) Again, in the male heavy smokers, haematocrit and haemoglobin levels and mean corpuscular volumes were raised. The white cell count (WBC) was very significantly raised and a differential count indicated rises in neutrophils and lymphocytes. These changes were not significant in the female H.S. group. (4) No significant changes were found in the group of light smokers but a trend was usually seen that suggested that these changes were influenced by the number of cigarettes smoked.

Protein modifications during antiviral heat bioprocessing and subsequent storage.

Antiviral heat treatment is routinely used in the bioprocessing of therapeutic proteins as a means of reducing viral load. However, in protein formulations containing sucrose this form of bioprocessing can lead to protein modifications. Using a model protein, hen egg white lysozyme, we investigated the effects of antiviral heat treatments in the presence of sucrose on protein integrity during subsequent long-term protein storage. Although heat treatment alone resulted in protein modification, subsequent medium- to long-term storage of both lyophilized and liquid samples at room temperature or above led to further protein modifications. The majority of these modifications were due to the formation of glycation and advanced glycation end products via the reaction of reducing sugars and their autoxidation products (derived from hydrolyzed sucrose) with function groups on the protein surface. These findings have implications for the improvement of therapeutic protein bioprocessing to ensure protein product quality.

Social support, knowledge of infant development, and maternal confidence among adolescent and adult mothers.

OBJECTIVE: To explore the influence of social support and knowledge of infant development on maternal confidence in performing infant care tasks among adolescents and adults. DESIGN: Descriptive, exploratory. SETTING: A large urban teaching hospital in the midwestern United States. PARTICIPANTS: 116 adolescent mothers (ages 13 to 19 years); 101 adult mothers (ages 20 to 41 years). MAIN OUTCOME MEASURES: Adult mothers reported higher levels of knowledge about infant development than did adolescent mothers. For both adolescent and adult mothers, social support and knowledge of infant development were significantly correlated with confidence in providing infant care. For adolescents, knowledge of infant development explained 15% of the variance in maternal confidence scores, and the number of people in the adolescent's household explained an additional 5%. For adults, parity explained 10%, social support explained 9%, and knowledge of infant development explained 4% of the variance in maternal confidence scores. CONCLUSIONS: An adolescent's knowledge of infant development significantly affects her confidence in providing infant care. Given the learning needs of new mothers and the time constraints for in-hospital postpartum teaching, nurses may need to adjust content and teaching methods for all new mothers, but especially to meet the needs of adolescent mothers.

Maternal screening and treatment for group B streptococcus.

Maternal group B streptococcus colonization can result in complications for both mother and fetus. There are two protocols for decreasing the risk of infection: antenatal cultures and intrapartum risk-factor screening. Compliance with guidelines improves with the active participation of nurses who monitor women at risk, notify clinicians when treatment is appropriate, and provide patient support and education.

A population-based hepatitis B vaccination coverage survey among Asian and Pacific Islander American students in Alachua County, Florida.

PURPOSE: The goal of this project was to evaluate the impact of the Florida mandatory hepatitis B vaccination law on the immunization status of Asian American and Pacific Islander (AAPI) students in Gainesville. METHODS: We (1) compared the differences of hepatitis B immunization rate between students in grades seven through nine and grades 10 through 12, (2) assessed knowledge of and attitudes towards hepatitis B vaccination of parents of AAPI students, and (3) promoted the importance of Hepatitis B vaccination among those who have not received the vaccination. PRINCIPAL FINDINGS: Two hundred and eighty-eight AAPIs were enrolled in grades seven through 12; 42 could not be contacted. Forty-seven percent of the parents (n = 116) consented to in-home interviews and gave permission to verify the immunization status. An additional 65 parents gave permission to verify the immunization status, but did not consent to be interviewed. CONCLUSIONS: As expected by state mandate, significantly more seventh, eight, and ninth graders were immunized compared to students in grades 10 through 12 (X(2) = 137.50, p < 0.0001). More than 90% of the students in the seventh, eight, and ninth grades had received all three hepatitis B vaccines. Less than 30% of the students in grades 10 through 12 received all three vaccines. Most parents (79%) said their high school-aged children had not received the vaccines because they were not required to do so. Most parents knew children needed three vaccines to be fully immunized and correctly identified the liver as the target organ. However, there was uncertainty about the mode of HBV transmission.
