Kidney stones: current issues in diagnosis and therapy.

Kidney stones are common, and most of them are of the calcium oxalate type. Metabolic evaluation ranging from extensive to cursory has been recommended by different experts; however, some evaluation does seem warranted, even if only a single stone has occurred. Crystallographic analysis of the stone, if recovered, and measurement of urinary excretion of calcium and uric acid are indicated. Blood chemistries, measurement of urine pH, and measurement of urinary constituents besides calcium and uric acid are probably indicated as well, but controversy exists as to whether classification of hypercalciuria is necessary for management of most patients. Most, but not all, of the literature supports the use of thiazides to prevent calcium oxalate stones and the use of allopurinol in those calcium stone formers who have hyperuricosuria. In fact, allopurinol appears to be useful in patients with calcium stones even if no metabolic abnormality is discovered.

Serum creatine kinase MB isoenzyme activity in long-term hemodialysis patients.

Serum creatine kinase MB isoenzyme (CK-MB) activity was determined in 46 male long-term hemodialysis patients without evidence of myocardial infarction. Thirteen (28.3%) showed mild elevations. The abnormality persisted in seven of eight patients on repeated measurement at three- to eight-month intervals. There was a significant correlation between serum CK-MB and CK-MN activity, and the activity of both enzymes rose after intramuscular injection. The reason for the abnormality is not known. It is possible that skeletal muscle is the source of elevated enzyme activity. Caution should be exercised in the interpretation of serum CK-MB activity in the diagnosis of acute myocardial infarction in this patient population.

Adenosine Phosphate and Nicotinamide Adenine Dinucleotide Pool Sizes during Shoot Initiation in Tobacco Callus.

Shoot-forming tobacco callus is found to have high levels of adenosine phosphates and NAD(+), and a low energy charge during meristemoid and shoot primordium formation. NADH levels are low and show little change during this period. There is a decline in the content of NADPH to nondetectable levels during the process, and a transient increase of NADP(+) is observed early in culture. These patterns are indicative of a shift to a more intensive rate of metabolism during meristemoid and shoot primordium formation and apparently reflect the requirement for energy and reducing power during organ initiation.

Asystole and its treatment: the possible role of the parasympathetic nervous system in cardiac arrest.

Parasympathetic tone may be high during ventricular asystole because of reflex vagal stimulation from a number of sources. Eight patients in cardiac arrest were treated with cardiopulmonary resuscitation. All eight patients had ventricular asystole as the initial rhythm or as the result of defibrillation. Six patients failed to respond to 5 cc to 20 cc of 1:10,000 epinephrine intravenously (IV). In all eight cases a regular rhythm (sinus in seven, idioventricular in one) appeared within 30 seconds of administration of the last dose of atropine (1 mg to 2 mg IV). Five patients (62.5%) lived 12 hours, three (37.5%) were discharged from the hospital. These results suggest that atropine may be of value in the treatment of ventricular asystole.

A system for storage and retrieval of individual cells following flow cytometry.

A system has been developed to deposit cells in indexed locations on a gelatin-coated film following flow cytometry, allowing the measurements made of individual cells to be correlated with observed morphology or with subsequent microspectrophotometric measurements. Samples are deposited in a continuous track on the film by a deposition nib attached to the flow system below the observation point; laminar flow is preserved by adjusting the tape speed and the flow velocity. Locations of individual cells are indicated by etching the film with a spark triggered by the detection of a cell in the flow cytometer. After deposition, the film is dried by forced warm air. Cells on gelatin may be washed and restained with Papanicolaou and other stains with reasonable preservation of morphology. The system may be used for validation of automated cytodiagnostic procedures based on flow cytometry and for biomedical research.

Glomerulonephritis with Hodgkin's disease and herpes zoster.

A patient who had been treated for Hodgkin's disease was infected with herpes zoster. Shortly thereafter, glomerulonephritis mediated by anti-glomerular basement membrane (anti-GBM) antibody occurred. The diagnosis was confirmed by the presence of circulating anti-GBM antibody, immunofluorescence, and elution. Results of immunofluorescence of a lymph node disclosed deposits of IgG on the vascular and stromal connective tissue, in a pattern similar to that seen when heterologous anti-GBM antibody reacted with the node. Because of the known nephritogenic potential of solid lymphoid tissues, this finding suggests a possible relationship between his tumor and his glomerular disease. The role of zoster infection remains undefined.

Effects of renal failure on blood levels of cimetidine.

Serial blood samples for determination of drug levels were obtained after intravenous administration of 300 mg of cimetidine. Sixteen patients with varying degrees of renal failure were studied. There was a prolongation of drug half-life in patients with renal insufficiency compared with that in normal controls (P less than 0.001). A significant inverse relationship between the half-life and the creatinine clearance was noted (r= 0.69; P less than 0.01). The effect of hemodialysis was studied in 12 patients. Cimetidine was found to be dialyzable. This was demonstrated both by a shortening of the half-life of the drug during dialysis and by measurement of dialysance. This suggests that the dose schedule should be modified for patients with renal insufficiency and for those on hemodialysis. A single intraveno us dose of cimetidine was well tolerated by the patients. One patient developed an urticarial skin rash, believed to be allergic in nature. There was a transient, mild (but significant) rise in blood urea nitrogen and serum creatinine concentration in 5 patients with moderate renal failure.

Cytomat-R: a computer-controlled multiple laser source multiparameter flow cytophotometer system.

A multiple illumination wavelength multiparameter flow cytophotometer system, using laser sources and controlled by a small, general-purpose digital computer, has been produced for use in the development of new flow cytometric techniques. Three different laser wave-lengths can be used simultaneously to illuminate different regions of the flow chamber; as many as five measurements of light scattering at various angles, extinction, and fluorescence at one or more wavelengths can be made at each illuminated station. Cells in suspension may be examined at rates of 1000 cells/sec, with seven correlated optical measurements being recorded for each cell. A library of programs for data manipulation and statistical analysis make it possible to use the system to develop and implement cell characterization, counting and classification procedures for basic and clinical research applications.
