Influence of pyridoxine on the oxygen transport function of blood in the neonatal period in clinical and experimental conditions.

A significant increase of the P50 value of blood (the pO2 value of O2 half saturated blood at 37 degrees C, pH 7.40 and pCO2 of 5.33 kPa) was observed on symptom free premature newborns, mature newborns requiring intensive care and newborn rabbits treated with high dose of pyridoxine (Vitamin B6). The change seemed to be independent of the 2,3-DPG content of blood. The moderate but consistently favourable influence of vitamin B6 treatment on the O2 transport function of blood can be advantageous in early postnatal adaptation disturbances of newborns.

Hyperbilirubinemia and urinary D-glucaric acid excretion in premature infants following antepartum dexamethasone treatment.

Dexamethasone (10 mg i.m.) administered to mothers 24 hours before delivery for the prevention of respiratory distress syndrome caused a considerable elevation of the serum level of unconjugated bilirubin in premature infants throughout the first week of life. On days 3 to 4 the incidence of infants with bilirubin levels exceeding 256.56 micrometer/l (15 mg/100 ml) was found to increase (7/15 vs 2/17). The urinary excretion of D-glucaric acid, which reflects the activity of the hepatic microsomal enzyme systems, proved to be low in pre-term infants over the first 7 days of life, and it was not enhanced significantly by antenatal dexamethasone therapy. In the light of the results presented, more detailed clinical and experimental analysis of the effects of glucocorticoids on bilirubin metabolism of pre-term infants and investigation into the drug prophylaxis of steroid-induced hyperbilirubinaemia are suggested.

In vitro effects of inosine-pyruvate-phosphate on P50 values and DPG contents of fresh and stored blood from healthy neonates, symptom-free premature infants and premature infants with respiratory distress syndrome.

A study was made of the effect of a 3-hour incubation with inosine-pyruvate-phosphate (IPP) on the P50 values and 2,3-DPG contents of fresh blood and blood which had stood for 96 h, from healthy adults, term neonates, 1-2-week-old symptom-free premature infants and RDS premature infants. It was found that the P50 value and the 2,3-DPG content could be increased considerably by IPP treatment in both the fresh and the stood blood in the various groups of neonates. The question remains open as to whether the effect of IPP, in significantly improving the O2 transport in neonates, is a consequence of the 2,3-DPG alone or of some other metabolite. In RDS the stimulating effect of the IPP mixture is appreciably lower.

Favourable effect of storage at 0 degrees C on the O2 transport function of heparinized blood.

The biological value of heparinized blood was found to decrease less on storage at 0 degrees C than on storage at +4 degrees C. At 0 degrees C the P50 value decreases substantially later and less, the blood is less acidotic, and the UV absorption of the dialysate is less enhanced than at +4 degrees C. The findings justify a further search for the conditions suitable for the widespread practical application of storage at 0 degrees C.

[Ultraviolet spectrophotomerty studies of serum dialysate from patients with long term intermittent artificial kidney treatment].

In 16 chronically dialysed uraemic patients in the course of altogether 61 treatments with the artificial kidney the UV-absorption spectre of the in-vitro-dialysate of the serum was studied. In uraemia on account of the retention of the metabolic products the UV-absorption spectre of the serum dialysate is extremely increased, a decrease is to be registered after the dialysis. The method discussed is suitable for the control of the effectiveness of haemodialysis and gives a more complex picture concerning the changes during the treatment than the usual laboratory parameters.

Altered response in vitro of the human umbilical artery to sera of neonates with respiratory distress syndrome and other hypoxic conditions.

A study was made of the reaction of the human umbilical artery and the isolated rabbit auricular artery to protein-free ultrafiltrates of normal and hypoxic sera originating from neonates suffering from RDS or other acute metabolic disturbances. While a normal serum ultrafiltrate caused an enhanced contraction of the umbilical artery, hypoxic sera exerted a weaker pressor activity. At the same time, on the rabbit's isolated auricular artery the hypoxic sera exhibited a higher pressor activity. The phenomenon is in agreement with the clinical observation that spontaneous closure of the umbilical vessels is delayed in the case of fetal hypoxia.
