Guillain-Barré syndrome following danazol and corticosteroid therapy for hereditary angioedema.

A case of hereditary angioedema secondary to C1 esterase inhibitor deficiency associated with lupus-like nephritis is reported. The patient was initially treated with both corticosteroids and danazol and subsequently had Guillain-Barré syndrome together with appearance of circulating immune complexes and an increase in total complement and C1q, C3, C4, B, and C1 esterase inhibitor levels. Guillain-Barré syndrome might be secondary to danazol therapy since this drug could increase both circulating immune complex production and complement synthesis, thereby providing additional substrate for the underlying immune complex disease. Normalization of complement might therefore be hazardous in lupus with underlying complement deficiency states.

Influence of acute, subchronic and chronic treatment with neuroleptic (haloperidol) on enkephalins and their precursors in the striatum of rat brain.

We examined the effects of chronic, subchronic and acute treatment with haloperidol on the ME, the MERGL and enkephalin precursor concentrations in rat brain. The changes affected primarily the striatum. The ME content was greatly increased by the treatment, the precursor level was decreased by the haloperidol treatment. The specific mRNA for proenkephalin A increased. For these reasons, we conclude that the effect of haloperidol increase both the biosynthesis and the processing of precursors of enkephalins in the striatum.

[Weber-Christian disease associated with familial alpha-1-antitrypsin deficiency. Apropos of a case].

The authors report a case of Weber-Christian disease associated with familial alpha-1-antitrypsin deficiency in a 47-year-old woman. The serum alpha-1-antitrypsin level is low and the phenotype is Pi ZZ. The histological picture was consistent with Weber-Christian panniculitis, showing foci of fat cell necrosis with lipophagic granuloma and fibrosis. The same biological trait also exists in the propositus' sister who developed emphysema without cutaneous involvement. The pathogenesis probably rests on a particular reactivity to microtraumata with exacerbation of local inflammatory events due to a partial deficient control of proteolytic activity. The prognosis of the disease varies from a chronic recurrent process to a short fulminant course with possible fatal outcome. The treatment is at the present time ill-codified and any how only symptomatic. In this case, the propositus responded well to a high dose regimen of corticosteroids, 100 mg daily, but relapsed as the treatment was reduced to 50 mg daily.

[Recklinghausen's disease with digestive localizations associated with gastric acid hypersecretion suggesting Zollinger-Ellison syndrome].

We report the case of a 49 year old male patient who had Recklinghausen's disease associated with hyperchlorhydria. The principal features of Recklinghausen's disease were cutaneous localizations and countless digestive tumors, found mainly on the small bowel. The ultrastructural aspect of these tumors was neurinoma or schwannoma. Basal acid gastric hypersecretion and a positive secretin test were highly suggestive of a Zollinger-Ellison syndrome. Gastrinoma was not found at laparotomy even though a gastrin gradient had been demonstrated by pancreatic venous sampling. In a patient with Recklinghausen's disease, neuroendocrine tumors (APUDomas) should be looked for systematically.

[Role of an anti-estrogen, tamoxifen, in the treatment of metastatic malignant melanoma].

The hormone-dependence of malignant melanoma is brought to mind by some epidemiological facts (rarely found in children, more frequently in females, higher frequency in case of prolonged use of oral contraceptives from an early age). On the other hand, estrogen receptors can be found in about 30 p. 100 of malignant melanomas. Starting from this hypothesis of a sub-population of hormone-dependent malignant melanomas, it seemed of interest to us to study the efficiency of an anti-estrogen, namely Tamoxifen (Nolvadex) in the management of metastatic malignant melanoma. It was used at a 40 mg daily dose-regimen on four patients (three post-menopausal females, one male) with multiple estrogen positive cutaneous metastases. In post-menopausal women, two cases of total regression were observed, associated with a distinct increase in progesterone receptors while under treatment and with a return to the initial stage when the treatment was stopped. This phenomenon can be explained by a double self-contradictory effect of the drug, already well known in other hormone-dependent cancers such as those of the breast or the endometrium, namely an anti-estrogen result on the tumoral growth and an estrogen result as attested by the synthesis of progesterone receptors. Certain malignant melanomas therefore behave as endocrine-dependent tumours and may so answer an anti-hormone treatment through a competitive fixation on estrogen-receptors. Our clinical results are in agreement with such a theory since Tamoxifen is at best effective on cutaneous metastases of post-menopausal women. The overall efficiency on all metastases is of the order of 10.6 p 100 (complete or partial regression).(ABSTRACT TRUNCATED AT 250 WORDS)

Humoral and cell-mediated immunity to human papillomavirus type 1 (HPV-1) in human warts.

The humoral and cell-mediated immune response to human papillomavirus type 1 (HPV-1) has been studied in 162 patients carrying papillomas of various clinical types: deep plantar wart or myrmecia, common wart, flat wart, and anogenital wart. Circulating antibodies were detected by immunodiffusion and microcomplement fixation, using purified HPV-1 particles as type-specific antigen. A significant association between myrmecia and anti-HPV-1 antibodies was found (39% of the cases). Cell-mediated immunity was evaluated by a study of delayed hypersensitivity (DH). The main capsid components of HPV-1 (HPV-1 CP), consisting mostly of a polypeptide of molecular weight 54,000, were injected intradermally. In addition to the type-specific antigens, HPV-1 CP contain other antigenic determinants shared by various types of human papilloma-viruses and masked in intact viral particles. The DH tests to HPV-1 CP showed no differences between the carriers of different papilloma types, confirming the presence of common antigenic determinants. Moreover, they gave rise to an increase or to new anti-HPV-1 antibody production mostly in myrmecia carriers (78% and 33% of the cases, respectively), and to new DH to HPV-1 CP in all groups of papilloma carriers (33% to 56%, depending on the clinical papilloma type).

[Analysis of the rebound phenomenon during outflow in plethysmography following programmed venous occlusion].

The venous outflow curve obtained using strain gauge plethysmography (Periflow JSI) frequently displays a slope change which modifies the classic exponential pattern: The semi-continuous arterial blood flow measurement together with an ECG-triggered plethysmography let the authors show the arterial origin of this slope change they called "the rebound phenomenon". Venous occlusion induces an ischemia which creates a reactive hyperemia when the venous blood current is let free. All the venous outflow curves stem from both a venous and arterial phenomenon. This latter may sometimes be of importance and has to be considered in plethysmography studies.

[Skin manifestations of Still's disease in adults. Apropos of 4 cases].

The authors report four new cases of Still's disease in adult. Each time, the clinical picture associated the following triad: fever, joint involvement and skin rashes. The latter consisted in non-pruritic but sensitive maculo-papular and disseminated lesions, the mean course of which lasted 24 to 48 hours and were readily accompanied by the two other components of the basic clinical triad. Laboratory investigations were non-specific but constantly included a polymorphonuclear leucocytosis and a raised ESR, the mean value of which reached 80 mm in the 1st hour. Antinuclear factors were not demonstrated, but circulating immune complexes were found on several occasions in three out of the four reported cases. Histopathological changes observed were those of a superficial and mid cutis edema, associated with a diffuse or perivascular infiltrate in which either mononuclears or polymorphonuclears were prominent. Dermal vessels involvement appeared rather weak, including swelling of the walls by edema with focal fibrinoid necrosis. Direct immunofluorescence showed mainly IgG, IgA and C3 vascular deposits. Based on these data, an immune pathology is discussed, besides the more classic infections pathogenesis.

Skin care in childhood.

Childhood (3 to 9 years) is a calm period of constant growth throughout which social contacts are discovered and intelligence and discretion take place. Consequently, most diseases are infectious. Microrelief was found twice shallower and skin (epidermis + dermis) twice thinner than in adults. Intervals between furrows were reduced parallel to total body surface, thus ruling out the possible existence of special shelters favouring bacterial growth. Sweating is lower, as are skin surface lipid levels, in contrast to the post-natal period, inducing dryness of skin. Although total skin stiffness in vivo was found to be identical to young adult skin, skin chapping is much more frequent. Since detergents as well as extended hot baths may reduce the stratum corneum water binding capacity, they should be used with discretion. Hence, skin care in this period of age should be based upon different criteria than in adults.

[Papular mucinosis associated with lupus erythematosus. A case presentation and review of the literature].

A 36-year-old woman presented with a widespread papulonodular eruption followed by cutaneous and systemic manifestations of lupus erythematosus. Both conditions, papular mucinosis and lupus erythematosus were investigated by histopathology, immunofluorescence and electron microscopy. Referring to seven other cases reported in the literature, the possible relationship between lupus erythematosus and cutaneous mucinosis is discussed. It seemed that the cutaneous deposits of mucine were secondary to lupus erythematosus and not a simultaneous occurrence of the two diseases.

[Neutrophil migration in patients with rheumatoid arthritis (author's transl)].

The leukocyte mobilization "in vivo" was assayed in 16 patients with rheumatoid arthritis and 10 normal subjects using the modified Senn's technique. The rate of leukocytes mobilization (LMR) is variable: in 5 cases lower than normal. No correlation is found with latex or Waaler-Rose titer, total complement in the serum, but the lowest rates of leukocyte mobilization are in previously treated patients. After 48 hours of indomethacin treatment, the LMR decreases significantly both in the patients and in the normal subjects. These results show that the abnormal "in vivo" LMR is more related to drug therapy than disease.

[Femoral paralysis caused by hematoma of the internal iliac fossa during anticoagulant treatment. Value of early surgical treatment].

The authors report two personal cases and review published cases of femoral paralysis due to hematoma of the internal iliac fossa complicating anticoagulant treatment. The interest of pelvic body scanning is emphasised both in diagnosis and for the assessment of the course of various hematomas of the internal iliac fossa. A plan for treatment is proposed in which surgery is of great importance. The latter is always effective both for the pain and the motor deficiency, a precise topographical localisation of the hematoma may be a decisive argument in favour of an early operation.

13-cis retinoic acid and acne.

Patients with very severe acne which had not responded to conventional treatment were given 13-cis retinoic acid (0.1 mg-1.0 mg/kg body-weight) in a double-blind dose response study. Sebum excretion rate was reduced by 75% at 4 weeks, and all patients had an 80% improvement in their acne severity after 4 months treatment. There were dose-related side-effects, particularly dryness of the skin and mucous membranes. No patient wished to stop treatment.
