Influence of major surgery on plasminogen activator activity in superficial hand veins.

In 108 patients the plasminogen activator (PA) activity in superficial hand veins was measured with a fibrin slide technique before and after a major surgical operation. In the early postoperative period the PA activity was significantly decreased and the PA activity did not return to its preoperative value until the 14th postoperative day. The postoperative decrease was more than 0.5 arbitrary unit in 90 patients, resulting in an abnormally low value in 44. Overweight or an operation time of 180 min or more was found to increase the proportion of patients with a postoperative decrease in the PA activity. No sure relationship was demonstrable in the postoperative PA activity between patients with venous thrombosis and those without.

Heparinized human umbilical vein grafts. Delayed heparin desorption after alcohol treatment.

Tanned human umbilical vein grafts (Dardik Biograft, Meadox) were heparinized with tritium-labelled heparin. Two methods of heparinization were undertaken: (1) incubation with heparin followed by ethyl alcohol 50% treatment, and (2) incubation with heparin without alcohol. Grafts were placed in the carotid arteries of sheep and removed 10 and 90 days later. Heparin was lost in contact with blood but the alcohol treatment delayed heparin desorption significantly.

Influence of operative trauma on factor XII and inhibitor of plasminogen activator.

A parallel decrease of factor (F) XII and a fibrinolytic inhibitor of plasminogen activation (PA inhibitor) was found in a series of 52 patients subjected to vascular surgery. It was at a maximum on days 2-4 postoperatively. The most extensive decrease was found in patients undergoing aortoiliac bypass operation. These patients had the highest blood loss and the longest operation times indicating that the changes were correlated to the magnitude of surgical procedure. The parallel decrease of F XII and the fibrinolytic inhibitor suggests an association between these two proteins in vivo.

Blunt renal trauma. Analysis of 417 patients.

Case records of 417 patients, from a well-defined area, who were hospitalized because of blunt renal trauma during the period 1950-1979 were analyzed. The incidence increased during the final decade, corresponding to a rising incidence of motor traffic accidents and sports injuries. Young patients, between 10 and 29 years of age, were responsible for the increase and half of all patients were in this age group. Associated injuries were frequent. Emergency urography was rare during the earlier part of the investigation period, but the frequency increased strongly thereafter. Treatment was mainly conservative (nonoperative) except in patients with major renal injuries. Nephrectomy was performed in 22 patients. Reconstructive surgery was performed especially in patients with intermediate renal injuries during the final decade. The total mortality was 6.5%. Only 7 patients (1.7%) died from the renal injury. A dramatic reduction in the hospitalization time was noted.

Deep vein thrombosis during pregnancy. A prospective study.

Seventeen women with thrombosis during pregnancy were studied prospectively. The deep venous thrombosis (DVT) was diagnosed objectively with phlebography, plethysmography and thermography. In many cases (13/17) the dominant symptom was diffuse pain in the lower abdomen and/or leg which in several cases caused a delay in the diagnosis. The traditionally typical thrombotic signs were often missing. Proximal thrombi were predominant (13/17) as were left-sided ones (14/17). During pregnancy and lactation the patients were treated with heparin which, after the initial parenteral administration, was given subcutaneously by self-administration. In conclusion, DVT must be suspected and non-invasive diagnostic tests be liberally performed in cases of uncharacteristic lateral and/or declive abdominal pain or leg pain during pregnancy. The DVT must always be objectively verified before treatment. In patients with pregnancy DVT there is no indication for early labor induction and the patients can be delivered by the normal vaginal route. It is recommended that patients with DVT during pregnancy should be treated with heparin subcutaneously which can be self-administered. The post-partum treatment may be continued for 2-3 months with heparin subcutaneously or peroral anticoagulants.

Pregnancy and venous thrombo-embolism.

In 30 patients with objectively diagnosed and 13 with clinically diagnosed venous thrombosis during pregnancy the extension and localization of the thrombotic process was analysed in relation to the coagulation and fibrinolytic status at least 6 months after thrombosis. Left-sided thrombi pre-dominated, being more extensive and proximal on the left side. Most thrombi started during the last two trimesters. Few changes were found in the coagulation system. Nineteen per cent of the patients showed a defective fibrinolytic system, as diagnosed by a reduced release capacity and/or a reduced fibrinolytic activity in the vessel wall. No great differences were found between patients with clinical vs. objective diagnosis.

Decreased fibrinolytic activity in vessels and ureters of rejecting human kidneys.

Plasminogen activator (PA) activity was determined with a histochemical fibrin slide technique in renal arteries, veins and ureters in 62 transplanted kidneys and again at transplantectomy in 12 transplants who later underwent irreversible rejection. At transplantation PA activity was the same whether the grafts later rejected or not. There was a significantly decreased PA activity in the vessels and ureters in rejected transplants. The role of the fibrinolytic system as a pathogenetic factor in rejected kidneys is discussed.

Ureteral obstruction as a complication in aorto-iliac reconstructive surgery.

Two cases of ureteral obstruction secondary to aorto-iliac reconstruction with bifurcation aortic grafts are described. Twenty-eight cases have been called from the literature and analysed. There seem to exist two types of aetiology regarding this complication: development of heavy localized fibrosis, and mechanical compression owing to the ureter having been placed behind the graft. Symptoms are often diffuse, but after operative treatment the prognosis seems good. The actual incidence of this complication is not known.

Vascular complications after total hip arthroplasty.

Four cases of vascular complications in connection with total hip replacement are reported and another 25 cases from the literature summarized. Acute intraoperative injuries most often give rise to severe haemorrhage. Delayed injuries with pseudoaneurysm formation or thrombosis give rise to hip pain, distal ischaemia or haemorrhage when a prosthesis is extracted. There is a dominance of female patients and left-sided operations and cases complicated with infections and reoperations. Aetiologic and therapeutic considerations are discussed.

Development of Xa inhibitor activity in relation to postoperative deep vein thrombosis (DVT).

65 patients, undergoing hip or elective general surgery, were tested for Xa inhibitor activity using a differential assay. Classification into minor and major deep vein thrombosis (DVT) was made. This study indicates that patients with repeatedly decreased Xa inhibitor activity during the first two days after trauma are highly disposed to develop postoperative DVT. In 10 out of 14 major DVT this decreased Xa inhibitor activity was observed before any detection of DVT by objective tests was possible.

Central haemodynamic effects of dextran 70, dihydroergotamine and their combination. A study in dogs.

In a study in dogs the central haemodynamic effects of dextran 70 and dihydroergotamine, both separated and combined, were evaluated. Dextran 70 gave an increased cardiac output, and peripheral resistance was reduced. The volume of blood flow in the femoral vein increased, pressure recordings being mainly unchanged. Dihydroergotamine caused an increase in the systemic arterial, central venous and pulmonary artery pressure. Cardiac output was unchanged. Total peripheral resistance increased but pulmonary vascular resistance remained unchanged. The combination of dextran 70 and dihydroergotamine gave a rather more pronounced increase in pressure recordings, but the other parameters observed were only moderately affected.

Tissue blood flow and blood flow distribution after administration of dextran 70, dihydroergotamine and their combination. A study in dogs using the radioactive microsphere technique.

The radioactive microsphere technique was used to measure tissue blood flow after administration of dextran 70 and dihydroergotamine, both separate and in combination, in normotensive dogs. Dextran 70 increased cardiac, output and tissue blood flow to most organs. Unchanged blood flow after dextran 70 to CNS, liver (hepatic artery blood flow), adrenals, genitals, uterus and lungs (bronchial artery and arteriovenous shunts) was found. Dihydroergotamine administration did not alter cardiac output. Tissue blood flow to pancreas and thyroid was reduced and CNS-tissue blood flow increased. Heart tissue blood flow was unaffected. When dihydroergotamine was given after dextran 70 had been infused, tissue blood flow returned to baseline values--except for pancreas and thyroid, where tissue blood flow was reduced, and CNS-tissue, where blood flow was increased. When dextran 70 was infused after dihydroergotamine, cardiac output and tissue blood flow remained mainly unchanged.

Cardiac output measurements with thermodilution and radioactive microspheres. A comparative study in cats.

Thermodilution and radioactive microsphere measurements are both commonly used methods for determination of cardiac output in experimental animals. However, no comparison has been made of these methods for cardiac output determination, which is why this study was made. In cats 65 simultaneous measurements of cardiac output with the two methods were performed. No difference in mean values was found and the coefficient of correlation was 0.955 (P less than 0.001). Only eight out of the 65 measurements differed more than 10%. Thermodilution measurements are suitable for repeated determinations and when immediate data are wanted. On the other hand radioactive microsphere measurements are preferable when data on tissue blood flow and blood flow distribution are desired as well. The high correlation found seems to allow cardiac output measurements with these two methods to be grouped together.

Increased vascular plasminogen activity in patients with pheochromocytoma.

Fibrinolytic and coagulation components were assayed in six patients with pheochromocytoma. Blood samples and a specimen from a superficial hand vein were obtained before, and six months after removal of the tumour. Vascular plasminogen activator (PA) activity in the vein wall was significantly increased in all patients who all had increased concentrations of urinary adrenaline, or noradrenaline, or of both. After adrenalectomy catecholamines were normalized and the PA activity was within normal range in all patients. There were no significant differences in factor VIII, antithrombin III, fibrinolytic activity, plasminogen or inhibitors of the plasminogen activation in plasma.
