Neoplastic angioendotheliosis: a clinicopathological entity with multifocal presentation. Case report.

Neoplastic angioendotheliosis is a rare disorder characterized by intravascular neoplastic proliferation of endothelial cells within vessels of all caliber in the meninges and neuropil. Ischemic infarcts of brain and spinal cord result from occlusion of the lumina by neoplastic cells of fibrin thrombi. Transition from reactive to neoplastic endothelium can be identified in many vessels. Steroid therapy can be beneficial in reduction of severity of symptoms, but cannot alter the course of disease. Therapeutic intervention must be undertaken promptly after the diagnosis is confirmed by meningeal and cortical biopsy if the inexorable course of the disease is to be altered.

[Investigations of pharmacokinetics and metabolic behavior of N-L-pnenylalanyl-L-2-amino-1-phenylpropane (author's transl)].

To elucidate the pharmacokinetic and metabolic behavior of N-L-phenylalanyl-L-2-amino-1-phenylpropane (7), which resembles antidepressants in its pharmacological spectrum, 14C-labelled 7 was synthesized. After i.p. application of 7 to the mouse on accumulation in the gastrointestinal tract, liver, kidney, bile and brain was shown by autoradiograms of the whole body. In the mouse 7 is excreted predominantly by the kidney (54% of the applied 14C-activity 8 h p.i.) and also considerably with the feces (11% of the applied 14C-activity 8 h p.i.). After 48 h the clearance is largely completed (85% of the applied 14C-activity). In the mouse the maximum 14C-activity in the brain (1.87% of the applied 14C-activity) is already reached after 0.25 h, 93.38% of it consisting of 7 and of amphetamine (5) liberated from 7, quantity ratio 5:7 = 1.43, and 5.00% hydroxylated metabolites of 5. With decreasing concentration of 5 and 7 the quantity ratio of 5:7 reaches a maximum value of 3.56 after 4 h. The steadily raised spontaneous motility by 7 after i.p. application lasting 4 h is attributed to 5, which is antagonized by 7. Due to pharmacokinetic and pharmacological data 5 and 7 are discussed as originators of the adrenergic effects of 7.

[Visual perception under the influence of a tranquilizer (author's transl)].

In a double blind study, the influence of the tranquilizer Camazepam on different ophthalmological parameters was examined. The 12 test persons were between 20 and 30 years of age and had entirely normal ocular findings. Visual acuity was uninfluenced by a single dose of 40 mg of Camazepam, reflecting the very slightly hypnotic effect of the drug. Roughly half of the patients showed a considerable widening of the visual field, thought to be related to the stimulation of attention caused by Camazepam. Finally the lack of muscle relaxing properties of Camazepam was confirmed as regards the extraocular muscles, since their balance remained uninfluenced as well.

Hypergammaglobulinemic purpura and Sjögren's syndrome in a child.

Hypergammaglobulinemic purpura of Waldenström and Sjögren's syndrome are rarely reported in children. This paper reports the case of a 12-year-old girl suffering from both diseases. In the serum of this patient the presence of circulating immune complexs was demonstrated, but there was no clinical sign suggesting an immune complexe disease. After a 28-month follow-up no clinical or biological change was observed.

Modification of early dimethylhydrazine carcinogenesis by colonic mucosal hyperplasia.

The interaction of colonic mucosal hyperplasia with early 1,2-dimethylhydrazine (DMH) carcinogenesis was studied in random-bred NIH Swiss mice utilizing hyperplasia-inducing Citrobacter freundii. Mice inoculated with this bacterium developed significantly more DMH focal atypia than did mice without hyperplasia following a single dose of DMH (20 mg/kg). Mice with hyperplasia also developed DMH focal atypia with diminished doses of DMH (10 and 5 mg/kg), while normal mice did not. The effect of C. freundii on early DMH carcinogenesis was shown to be due to the hyperplasia rather than to a direct interaction of the bacterium with DMH. Focal atypia arose in high incidence 1 month after a single dose of DMH (20 mg/kg) but did not appear to progress to later stages of neoplasia, since significantly fewer atypia were present at 2 to 4 months among a randomized population. Colonic focal atypia may represent a reversible preneoplastic or precursor lesion as seen in other tissues, with features more aligned to neoplasia than to hyperplasia.

Migraine thrombocytopenia, and serotonin metabolism.

Among 296 patients with thrombocytopenia, 14 had frequent migraine-like headaches, 6 of them having definite migraine. Only 1 of the 6 patients had had migraine before the onset of the blood disorder, and all 6 had migraine attacks during periods of bruising tendency and low platelet count. Control of thrombocytopenia with splenectomy or steroids was accompanied by dramatic improvement in migraine. It is suggested that migraine attacks are caused by abnormal platelet activity and abnormal serotonin metabolism. It is suggested that the immediate factor producing the abnormal platelet activity in our patients was immunological and that other cases of migraine may have an immunological cause.

[BCG vaccination, cancer and infectious mononucleosis. A retrospective study of hospitalized patients].

A retrospective study (1966--1976) of patients hospitalized for cancer was carried out to determine a possible relationship with BCG vaccination, with a control group matched for age and sex and a similar study concerning infectious mononucleosis. Among 105 patients with cancer, 54 (51.42%) had been previously vaccinated with BCG, 56 (53.33%) in the control group, and 22 (33.84%) among the 65 patients with infectious mononucleosis (p less than 0.05, x2). The mean age on admission for cancer or infectious mononucleosis among vaccinees is significantly older than among non-vaccinees, suggesting a transient protection afforded by vaccination. These results need to be confirmed by a prospective study with Epstein-Barr virus serology.

The modulating effect of human chorionic gonadotropin on lymphocyte blastogenesis.

Human chorionic gonadotropin (HCG) produced by fetal trophoblastic cells was suggested to play a role in the suppression of maternal lymphocyte response against the fetal allograft. Reports supporting this concept have demonstrated a suppressive effect of crude HCG on lymphocyte blastogenesis. However, similar doses of purified hormone preparations were unable to inhibit lymphocyte stimulation with mitogens. A modulating influence of phenol-free and immunoglobulin-free HCG preparation on lymphocyte blastogenesis is presented. Enhanced lymphocyte stimulation with various mitogens occurred in cultures supplemented with HCG at low doses. On the other hand, higher hormone concentration exerted an inhibitory effect on lymphocyte blastogenesis. An increase in lymphoid cell concentrations was demonostrated in cultures supplemented with the hormone preparation. These observations are compatible with the suggested role of HCG in the maternal tolerance toward a histoincompatible fetus.

[The "expert-killer" syndrome (author's transl)].

The "expert-killer" syndrome observed in 20 patients is a narcissistic subdivision which can be separated from the other organic neuroses. Clinically it is characterised by the triad: a) diffuse pain with numerous investigations and operations, b) absence of a diagnosis and adequate treatment, c) pathological doctor-patient relationship. The disease is based on indefinite and functional pains in the abdomen, neck and back. The failure to classify it with a clinically defined somatic disease leads to multiple diagnostic attempts which become more aggressive with increasing lack of success. Multiple operations occur in all the histories. The doctor-patient relationship is characterised by an initial idealisation of the doctor which quickly turns to rejection on account of his limited abilities. The patient only tolerates a short contact with the doctor. In the personality narcissistic traits and the persistence of an external ideal are apparent. The syndrome occurs in life at the point when the idealised partner disappoints the patient. These trigger factors are to be considered as a narcissistic insult. The functional pains are an attempt to restore the threatened self of the patient. The treatment consists of psychotherapy.
