Seven patients with hepatitis delta virus (HDV) cirrhosis underwent liver transplantation. In every case the HDV infection was florid but accompanied by an inactive hepatitis B virus (HBV) infection. The patients were given anti-HB surface antigen (HBsAg) serum globulins and HBV vaccine. Two patients cleared the HBsAg and the HDV, and are alive and well 14 and 15 months, respectively, after transplantation. HDV infection recurred in the other five patients: hepatitis developed in three, another died, and the fifth was re-transplanted for causes unrelated to viral hepatitis (reinfection was shown by the presence of HD antigen in the graft). Liver transplantation is feasible in patients with HDV disease but involves a high risk of HDV reinfection that cannot be predicted by the virological pattern of the native HBV infection or prevented by conventional HBV prophylaxis.

Quantitative information on spleen perfusion may be obtained as a byproduct of liver studies using 99Tcm colloids, by means of model analysis of dynamic data collected with a large field-of-view computerized gamma camera for about 4 min after intravenous administration of the tracer. Spleen extraction efficiency, vascular transit time, and parameters related to spleen blood flow, splenic clearance and volume of distribution of the tracer were computed, the latter three being expressed in arbitrary units. Results in 14 normal subjects and 78 patients with liver cirrhosis show good agreement with known physiopathological data. Results in five splenectomised patients and one patient undergoing ligation of the splenic artery provided further confirmation of the physiopathological meaning of the estimated parameters. Accuracy was found to be poor for spleens of small (normal) size, but was acceptable for enlarged spleens. Reproducibility of the results appears to be within 20%. It is concluded that this method, when associated with the study of liver function using a single 3-4 mCi dose of radiocolloids, may provide valuable additional information for routine assessment of splanchnic haemodynamics in patients with portal hypertension and splenomegaly.

In the this study the modification of lymphocyte subsets (T3, T4, T8) and Natural Killer (NK) cells in organ transplanted patients treated with Cyclosporin A (CyA) in the course of viral infection, have been analyzed. Different subsets have been studied with the monoclonal antibody method and infective processes have been verified by serological data of seroconversion. Our study has shown that CyA at the adopted doses does not alter NK response to viral infection; in fact, in patients with seroconversion, higher NK values and lower OKT4/OKT8 ratio values have been found with respect to patients who did not show any viral infection serologic data. Furthermore an increased incidence of reject crisis has been observed in patients with seroconversion.
In this study a sero-epidemiological investigation on 127 renal allograft recipients was examined. In these patients, treated with Cyclosporine A or conventional drugs, antibody response to various antigens (Cytomegalovirus, Herpes simplex, Varicellae/Zoster and Mycoplasma pneumoniae) was examined. The data were compared to the healthy population and dialyzed subjects.
