[Detection and evaluation of bcl-x and bcl-2 gene expression in blasts from patients with acute leukemia].

OBJECTIVE: To investigate the expression of bcl-x and bcl-2 genes in bone marrow cells of acute leukemia and their effects on response to chemotherapy. METHODS: Semi-quantitative reverse transcription polymerase chain reaction (RT-PCR) was used to detect the mRNA expressions of bcl-xL, bcl-xs and bcl-2 in blast cells from 34 acute leukemia(AL) patients. RESULTS: The expression of bcl-xL and bcl-2 or both in blast cells from 34 AL patients was higher than that in complete remission patients and normal subjects. The expression of bcl-xL and bcl-2 in relapsed patients was increased by 1.8 and 1.9 fold, respectively, compared with that in the newly diagnosed patients(P < 0.01). However, no relationship was found between the expression of bcl-xs and the relapse and chemotherapeutic response of the AL patients. There were positive correlations between the expression of bcl-xL and bcl-2 or bcl-xL and bcl-xs. The expression level of bcl-xL and bcl-2 in the patients with poor response to chemotherapy was significantly higher than that in the patients with better response. Treatment failure occurred in 80.0%, 91.7% and 100.0% of the patients with overexpression of bcl-xL and bcl-2 and both, respectively. CONCLUSION: Overexpression of bcl-xL and bcl-2 may imply the lower sensitivity of the leukemic cells to chemotherapy and higher risk of relapse.

[Correlation of eosinophil apoptosis with interleukin 5 mRNA expression in lung tissues of asthmatic guinea pigs].

OBJECTIVE: To explore the correlation of interleukin 5 mRNA expression with eosinophil apoptosis in lung tissues of asthmatic guinea pigs. METHODS: Guinea pigs were divided into asthma, asthma pretreated with dexamethasone and control groups. Guinea pigs were sensitized by exposure to aerosolized ovalbumin. Twenty four hours after the animals were challenged by aerosolized ovalbumin, the apoptosis percentages of hypodense eosinophils(HEo) and normodense eosinophils(NEo) in bronchoalveolar lavage fluid (BALF), and IL-5 mRNA expression in lung tissues were detected with terminal deoxynucleotidyl transferase mediated dUTP nick end labeling (TUNEL) technique and in situ hybridization. RESULTS: (1) In the asthmatic group, the percentages of HEo and NEo apoptosis were significantly decreased (P < 0.05), and IL-5 mRNA expression was significantly increased as compared with the control (P < 0.01). (2) In the group pretreated with dexamethasone, the apoptosis percentages of HEo and NEo were significantly increased (P < 0.01), and IL-5 mRNA expression was reduced as compared with the asthmatic group (P < 0.01). (3) IL-5 mRNA expression was negatively correlated with apoptosis percentage of HEo (r = -0.491, P < 0.05) and NEo (r = -0.492, P < 0.05). CONCLUSIONS: IL-5 mRNA expression in lung tissues closely correlated with apoptosis percentage of Eos in BALF. Inhibiting the expression of IL-5 mRNA and promoting the apoptosis of Eos in lung tissues may be one of the important mechanisms of glucocorticoids to reduce infiltration of Eos in lung tissues and their therapeutic effect in asthma.

[The changes of NO2-./NO3-. level in sputum from patients with asthma and their clinical implications].

OBJECTIVE: To evaluate the clinical applications of the detection of the stable end products of NO, nitrite and nitrate (NO2-./NO3-.) in sputum from patients with asthma. METHODS: 45 patients with asthma at remission (group I, n = 15) or exacerbation state with mild attack (group II, n = 15) or moderate to severe attack (group III, n = 15), 15 cases of chronic cor pulmonale secondary to chronic bronchitis at exacerbation (group IV), and 20, normal control subjects (group V n = 20) were included in this study. NO2-./NO3-. in sputum and serum were measured by Griess reaction, while the eosinophil cationic protein (ECP) in sputum and pulmonary function were also measured in tested patients with asthma. RESULTS: The median concentrations of sputum NO2-./NO3-. in group I, II and III were 85, 131 and 136 mumol/L respectively, all were significantly higher than that in controls (63 mumol/L); those of group II and III were significantly higher than that of group I, while that of group IV (33 mumol/L) was significantly lower than those of the asthmatic groups and the control. The level of NO derivatives in sputum was closely correlated with the degree of airflow obstruction and sputum ECP levels in patients with asthma at remission (r = -0.587, P < 0.05; r = 0.535, P < 0.01 respectively) and exacerbation (r = -0.485, P < 0.05; r = 0.563, P < 0.05 respectively). There was no significant correlation between serum and sputum NO2-./NO3-., and no significant correlation between serum NO2-./NO3-. and the degree of airflow obstruction. A significant decrease in the concentration of sputum NO2-./NO3-. was observed after therapeutic treatment of patients with moderate to severe attack (136 vs 93 mumol/L). CONCLUSION: The level of NO2-./NO3-. in sputum may be a useful marker for monitoring the state of airway inflammation and assessing the therapeutic efficacy.

[Continuous tracheal gas insufflation during pressure control ventilation with a pressure relief valve].

OBJECTIVE: To establish the method of continuous tracheal gas insufflation (CTGI) and investigate the role of pressure relief valve (PRV) in limiting high peak airway pressure induced by CTGI. METHODS: CTGI device was made by authors. Nine healthy mongrel dogs were mechanically ventilated by using Evita 2 (Drager Inc.). PRV was inserted into the ventilatory circuit for selected experiment steps and adjusted a threshold pressure equal to the preset inspiratory pressure. PCV, PCV + CTGI (TGI group) and PCV + CTGI + PRV (PRV group) were conducted in different inspiration/expiration (I/E) ratio of 1:2 and 2:1. Gas exchange, lung mechanics and hemodynamics status were monitored at the constant set inspiratory pressure of PCV. RESULTS: At the same I/E ratio, peak airway pressure in TGI group was higher than that of PRV and PCV group (P < 0.01), whereas PRV group maintained the preset inspiratory pressure as PCV group. At the different I/E ratio, peak airway pressure in TGI (2:1) group was higher than that of TGI (1:2) group(P < 0.01). PaCO2 in TGI and PRV group decreased significantly than that of PCV, TGI group was more lower than PRV group, PaO2, hemodynamics and oxygenation remained unchanged in all groups. CONCLUSIONS: The CTGI device is effective in improving alveolar ventilation with no significant influence on hemodynamics and oxygenation; A pressure relief valve is necessary to ensure the safe use of CTGI.

[Observation on effect of integrated Chinese and Western medicine in treating patients with peripheral atherosclerosis obliterans].

OBJECTIVE: To observe the therapeutic effect of integrated traditional Chinese and western medicine (TCM-WM) and its influence on plasma endothelin-1 (ET-1), nitric oxide (NO) and apoproteins. METHODS: Ninety patients of peripheral atherosclerosis obliterans (ASO) were divided into three groups randomly and treated with Salvia injection (SI), prostaglandin E1(PGE1) and SI + PGE1 respectively. The clinical manifestation of patients was observed dynamically, plasma level of ET-1, NO and apoprotein were tested before and after treatment by radioimmunoassay, Griss method, immuno-transmission method and enzyme method. RESULTS: The cure rate of the SI, PGE1 and TCM-WM group was 26.67%, 33.33% and 46.67% respectively, the effective rate of them was 26.67%, 40.00% and 43.33% respectively. The therapeutic effect of the TCM-WM group was better than that of the SI and PGE1 group, P < 0.01. The plasma level of ET-1, NO and apoprotein was positively correlated with the stage altering of ASO, it was normalized gradually when ASO was alleviating effectively. CONCLUSION: The TCM-WM therapy has good clinical effect in treating ASO. To observe the change of ET-1, NO and apoprotein level before and after treatment dynamically is helpful to judge the severity, progress of disease and effect of treatment, and has high clinical significance.

[Clinical study on treatment of rectal carcinoma with Chinese herbal medicine and high dose fluorouracil emulsion via rectal infusion].

OBJECTIVE: To study the clinical significance of rectal infusion of Chinese herbal medicine (CHM) plus high dose fluorouracil emulsion in treating rectal carcinoma. METHODS: The 86 patients of rectal carcinoma were randomly divided into CHM plus chemotherapy group and single chemotherapy group, and the changes of T-lymphocyte cell subset, natural killer (NK) activity, pathologic picture and clinical symptoms were analysed statistically before and after treatment. RESULTS: In the single chemotherapy group, the CD3 cell and NK activity decreased after treatment and most of the peri-carcinoma cellular infiltration were in grade I. In the CHM plus chemotherapy group, the CD3, CD4/CD8 and NK activity restored and CD8 lymphocyte decreased markedly after treatment in comparing with those before treatment (P < 0.05, P < 0.01), and most of the peri-carcinoma cellular infiltration concentrated in grade III. CONCLUSION: Chemotherapy alone could suppress cellular immune function significantly, significant immunoregulation and improvement of symptoms (as mucous-bloody stool, frequent defecation) could be obtained by combination therapy of CHM and chemotherapy.

[Clinical and experimental study on treatment of ankylosing spondylitis].

OBJECTIVE: To explore the effect and mechanism of Gubiyin (GBY) in treating ankylosing spondylitis (AS). METHODS: The 70 patients of AS were randomly divided into two groups, 40 patients in the GBY group were treated with GBY and 30 patients treated with indomethacin for control. Animal experiments were also conducted. RESULTS: Clinical observation showed that the markedly effective rate of the GBY group (57.5%) was higher than that of the control group (23.3%, P < 0.01), but the total effective rate of the two groups were similar. The effects of GBY in relieving symptoms of joints, improving function of joints, preventing degeneration of bone and improving laboratory parameters were better than those of indomethacin. In animal experiments, GBY showed its inhibition on granuloma formation, adjuvant arthritis in mice, and IL-1 production of abdominal macrophage and IL-2 production of spleen cell in rats. CONCLUSION: GBY has effects of anti-inflammation, analgesic, immunoregulation, inflammation mediator inhibition and hemorrheology improvement.

[Effect of nourishing kidney to inducing resuscitation method on psychic evaluation table in patients of vascular dementia].

OBJECTIVE: To probe the effect of the nourishing Kidney to inducing resuscitation (NKIR) on psychic evaluation table in patients of vascular dementia. METHODS: Modified Sanjiasan Granule (MSG), a representative recipe of NKIR, was used to treat 26 patients with vascular dementia, 7 g per day orally for one month as one therapeutic course, and the effect of treatment was evaluated by psychic evaluation table. RESULTS: MSG could increase the total scoring of Hagayakawa Dementia Scale and mini mental state examination (MMSE) as well as the single entry scoring of MMSE. It could reverse the process of vascular dementia in psychic evaluation table. CONCLUSION: MSG has good effect in improving intelligence and anti-dementia.

[Relationship between endothelins, nitric oxide and other endothelial active factors and blood stasis syndrome in patients with atherosclerosis obliterans].

OBJECTIVE: To observe the levels of plasma endothelin (ET), nitric oxide (NO), intercellular adhesion molecule (ICAM) and other endothelial active factors, and to investigate the nature of atherosclerosis obliterans (ASO)-blood stasis Syndrome (BSS) from the point of vascular endothelia and molecules. METHODS: Ninety-eight patients with ASO-BSS were divided into 3 groups (mild, moderate and severe) and 3 clinical Syndrome subtypes (Qi stagnation BSS, Qi deficiency BSS and Damp-Heat BSS). The circulating endothelial count (CEC), ET, NO and other plasma active factors were determined by density gradient method, radioimmunoassay, enzyme immunoassay method. RESULTS: The patients with ASO-BSS had high CEC and ET levels, low levels of NO, and had alterations of hemorheology, particularly in those with severe BSS and Damp-Heat BSS. CONCLUSION: ASO-BSS patients have endothelial injuries, endothelial dysfunction, and alterations of hemorheology, which may be the pathogenetic mechanism of ASO-BSS. Observation on these parameter changes will be helpful to evaluate severity, to distinguish the clinical types in TCM, and to clarify the nature of BSS.

[Effect of total flavonoids of Astragalus on nitroxide in ischemia reperfusion injury].

OBJECTIVE: To study the protective effect of total flavonoids of Astragalus (TFA) on ischemia/reperfusion injury. METHODS: Change of nitrite (NO2-), the terminal product of nitric oxide (NO) metabolism, and the effect of TFA and chloroquine (a phospholipase A2 inhibitor) on it were observed with hemorrhagic shock/reperfusion injury (S/R) rabbit model. RESULTS: Plasma NO2- content was positively correlated with blood pH and total carbon dioxide content lowering. TFA and chloroquine could block the decrease of NO in certain degree and might have some effect on maintaining acid-base balance of body. CONCLUSION: TFA has protective effect on ischemia/reperfusion injury.

[Clinical analysis of 12 cases of Chinese patients with adult T-cell leukemia/lymphoma].

OBJECTIVE: To define the clinical characteristics and differential diagnosis in Chinese patients with adult T-cell leukemia/lymphoma (ATLL) and its relation to human T lymphotropic virus type-1 (HTLV-1) infection. METHODS: Lymphocyte morphology in peripheral blood was examined in smears by Cytospin II. Lymphocyte immunophenotyping was carried out by indirect immunofluorescence method. HTLV-1 antibody was detected by indirect immunofluorescence method, proviral DNA detected by PCR method and PCR products demonstrated by Southern Blot analysis. RESULTS: The clinical manifestations in these 12 patients were fever (10/12), lymphadenopathy (8/12), skin changes (7/12), spleen and liver enlargement (7/12), jaundice (3/12), pleural effusion (1/12) and lung invasion (1/12). Patul-like cells were not easily found in routine blood smears, but easily found with Cytospin II (10/10). There was increase in lactic dehydrogenase (8/9) and soluble interleukin 2 receptor (9/9). Anemia (7/12), thrombocytopenia (6/12), pancytopenia (2/12), myelodysplasia (4/12) and eosinophilia (8/12) can be found, but no hypercalcemia. HTLV-1 antibody and proviral DNA were positive in 6/10 cases and 9/12 cases respectively. Nine cases were classified as acute type and three as lymphoma type. All patients were misdiagnosed as other diseases at the beginning. The initial presentations were skin changes (4/12), lymphadenopathy (3/12), myelodysplasia (3/12), progressive liver enlargement (1/12) and refractory pleural effusion (1/12). The complications were infections by undetermined pathogens (6/12), Streptococcal infection (1/12), gastrointestinal bleeding (3/12), acute renal failure (3/12), acute respiratory failure (2/12) and cerebral hemorrhage (1/12). Seven patients died, three still survive, and two are out of contact. CONCLUSION: ATLL may not be very rare in China. It is necessary to deepen the recognition of ATLL in Chinese.

[A study of airway reactivity and nature of chronic asthmatic bronchitis].

OBJECTIVE: To observe the airway reactivity with methacholine provocation and exercise challenge of chronic asthmatic bronchitis and to assess preliminarily the nature of chronic asthmatic bronchitis. METHODS: 42 patients with bronchial asthma, 29 with simple chronic bronchitis and 27 with chronic asthmatic bronchitis were selected. The study protocol consisted of inhalation challenge with methacholine and exercise challenge. Count of blood eosinophil cells and level of blood total IgE were studied. Serum eosinophil cationic protein (ECP) concentration was measured in the asthmatic bronchitis group. RESULTS: The positive ratio to methacholine response was 77.8% in chronic asthmatic bronchitis group, whereas the ratios were 100.0% and 41.4% in bronchial asthma and simple chronic bronchitis groups respectively. According to the response to exercise challenge, patients with chronic asthmatic bronchitis were divided into positive and negative groups. The cumulative dose of methacholine producing a 20% fall in FEV(1) (PD(20)-FEV(1)) was different between the two groups. In the positive group (9 patients), none was negative with methacholine response and PD(20)-FEV(1) being similar to the bronchial asthma group. The blood total IgE and ECP levels were significantly higher in the positive than in the negative group. CONCLUSION: The characteristics of airway reactivity in chronic asthmatic bronchitis are midway between bronchial asthma and simple chronic bronchitis. The nature of chronic asthmatic bronchitis may be different in different patients.

[A pathological study on pulmonary fibrosis caused by chronic obstructive pulmonary disease].

OBJECTIVE: To estimate the morphologic characteristics of chronic obstructive pulmonary disease (COPD) induced pulmonary fibrosis. METHODS: 41 autopsy cases of COPD were chosen as the study group and other 10 autopsy cases died from non-respiratory diseases as control group. Histochemical stain and image quantitative analysis were used to identify the type of collagen. RESULTS: In 36 cases (88%) of study group, fibrosis proliferation could be seen in peri-bronchial area, especially in the bronchi under grade VI. Respiratory bronchiole and alveoli were also involved in 11 cases. The main component of collagen in fibrosis area was identified as type III collagen by sirius red stain, reticular fiber could also be seen by Gomori stain. The type III collagen in alveoli distributed interruptedly. Image quantitative analysis showed: the integral optical density (A) of type III collagen was much higher than that of type I collagen in peri-bronchial area (P < 0.01); Both A of type III and type I collagen in peri-bronchial area were higher than that in alveolar area (P < 0.01). Mucous purulent plugs, interrupted elastic fibers in bronchial wall and chronic inflammation accompanied with lymphocytes infiltration in fibrosis area could also be seen. CONCLUSIONS: COPD induced pulmonary fibrosis distributed mainly in peri-bronchial area, especially in the bronchi under grade VI. The main component of proliferative collagen was type III collagen. The genesis of pulmonary fibrosis might be associated with chronic inflammation accompanied with lymphocytes. Peri-bronchial fibrosis, interrupt of elastic fibers in bronchial wall caused the bronchial lumen narrowed or collapsed making both airway obstruction and ventilation insufficiency deteriorated.

[The tissue oxygenation status and the effects of mechanical ventilation on oxygen dynamics in COPD patients with hypercapnia].

OBJECTIVE: To assess the tissue oxygenation status and the effects of mechanical ventilation (MV) on oxygen dynamics in COPD patient with hypercapnia. METHODS: Gastric intramucosal pH (pHi) measured by gastric tonometer was examined in 19 patients before and after MV. RESULTS: pHi calculated before MV was 7.18 +/- 0.06; After receiving MV, pHi increased gradually until above 7.32 in 4 days; during weaning, there were no significant changes in pHi before and after discontinuation. The correlation analysis showed that there was no correlation between pHi and pHa (r = 0.352, P > 0.05), PgCO(2) and PaCO(2) (r = 0.421, P > 0.05), suggesting that pHi measurement would not be affected by hypercapnia. CONCLUSIONS: Our study showed that tissue oxygenation insufficiency existed in COPD exacerbation stage. Gastric pHi may be used as the parameter to estimate tissue oxygenation status and a predictor of weaning from mechanical ventilation.

[Relationship between PDGF-A, bFGF and proliferation of smooth muscle cells in vein grafts].

OBJECTIVE: To detect the relationship between platelet-derived growth factor-A (PDGF-A), basic fibroblast growth factor (bFGF), and proliferation of smooth muscle cells in vein grafts. METHODS: An animal model of autogenous vein graft was established by transplanting internal branch of the jugular vein to the common iliac artery by end-to-end anastomosis. 25 rats were used. The grafted veins were harvested at 6 h, 2 d, 1 w, 2 w and 4 w respectively after the operation. The expression of PDGF-A and bFGF in different stages after grafting procedure was observed immunohistochemically. RESULTS: The expression of PDGF-A peaked at 1 w, paralleled with proliferation of SMC, and was significantly higher than that at 6 h (P < 0.01). The expression of PDGF-A diminished gradually. At 4 w, it was significantly lower than that at 1 w (P < 0.01). bFGF demonstrated bimodal pattern of the expression: the first peak of expression occurred by 2 d and the second peak by 2 w. The difference was significantly compared with other stages. CONCLUSIONS: The expression of PDGF-A 1 contributed to migration and proliferation of SMC. Except for contributing to proliferation of SMC, bFGF perhaps took part in endothelization.

[Relationship between GSTM1 genotype and susceptibility to senile cataract].

OBJECTIVE: To study the relationship between the glutathione s-transferase gene deletion and cataract formation. METHODS: Blood cells of total of 77 cases with senile cataract and 76 controls were detected for GSTM1 gene, and the subcapsular epithelial cells of 22 cataract lenses were also detected for GSTM1 gene. RESULTS: The GSTM1 gene deletion rate in cataract group was 53.25% and that in the control group was 46.05%, they being not significantly different statistically (chi(2) = 0.750, P > 0.05, OR = 0.75). GSTM1 gene deletion rate in the subcapsular epithelial cells of 20 cases was basically consistent with that in blood cells. CONCLUSION: GSTM1 gene deletion is not related to senile cataract formation.

[Relationship between retinal and glomerular microvascular lesions in experimental diabetes in rat].

OBJECTIVE: To evaluate the relationship between diabetic retinopathy (DR) and diabetic nephropathy (DN). METHODS: The thickness of retinal capillary basement membrane (RBM) and glomerular basement membrane (GBM) and their anionic sites were quantitatively analyzed by electron microscopy and immunohistochemical methods. RESULTS: RBM and GBM became gradually thickened with the development of diabetes. The thickened basement membrane was accompanied with decrease of anionic sites in number. CONCLUSION: DR and DN share partly the pathological process, and there are also some incoordinate phenomena in the microvascular lesion.

[A case-control study on Guillain-Barre syndrome in children of North China].

OBJECTIVE: This study aimed at exploring the risk factors for Guillain-Barre syndrome (GBS). METHODS: A case-control study design was used with 51 cases of GBS, diagnosed based on their symptoms, signs and electrophysiological examinations and exclusion of poliomyelitis and other acute flaccid paralysis, and 51 controls matched on age, sex and resident village. Serum IgG antibodies specific for Campylobacter jejuni were determined for all the subjects by enzyme-linked immunosorbent assay (ELISA) with a preparation of surface antigen of C. jejuni C(1) strain isolated from the patients and prevalent in north China. Each case and control were interviewed with his/her parents or guardians by a trained interviewer using an ad hoc questionnaire, including his/her demographic information, socioeconomic status, onset of the illness, and potential risk factors in their environment and personal hygiene. Data were analyzed with SAS software release 6.04 in a microcomputer. RESULTS: GBS was associated with a few factors, such as residential areas (45 of the 51 cases living in the rural areas, accounting for 88.2% of the total), polio and hepatitis B vaccine immunization before onset of the illness (with ORs of 7.27 and 3.14, respectively), no hand washing after defecation and before meals (with an OR of 6.15) and getting cold and going to the river or lake site before onset of the illness (with ORs of 13.75 and 12.20, respectively). Infection with Campylobacter jejuni associated strongly with the illness (with an OR of 9.5, P < 0.001). Thirty-five of the 51 cases had precursor symptoms before onset of the illness (68.6%). CONCLUSION: It suggests that occurrence of GBS may correlate to infection with Campylobacter jejuni and poor personal hygiene in children.
