[A pathological study of lens subcapsular epithelial proliferation].

OBJECTIVE: To observe the histopathological changes of lens subcapsular epithelial proliferation and analyze their etiology. METHODS: Sixty-four enucleated eyes with lens subcapsular epithelial proliferation were observed with light microscopy and analyzed retrospectively. Among all the cases of lens subcapsular epithelial proliferation, there were 36 ones enucleated because of trauma, 10 absolute glaucoma, 6 corneoscleral staphyloma, 5 intraocular tumor, 2 after cataract extraction, 4 endophthalmitis, 1 cataract combined with ocular atrophy (diabetes type II). RESULTS: Proliferated lens subcapsular epithelial cells were atypical, irregularly arranged, and extended to the posterior subcapsule. In cases with lens capsular rupture or partial absence, there were subcapsular fibro-connective tissue metaplasia, scar tissue formation, cortical collapse, local liquefaction and calcification. CONCLUSION: Lens subcapsular epithelial proliferation is caused by some intraocular pathological conditions. In cases with endophthalmitis and ocular trauma, its severity is the most serious.

[A survey and treatment of blindness in Gejiu City of Yunnan Province].

OBJECTIVE: To calculate the percentage of blindness in the mass and evaluate the current conditions of treatment for cataract. METHODS: In accordance with the national unified standards and procedures, a clue survey of the people aged under 40 years and a general survey of the people over 40 years in Gejiu city, Yunnan province were carried out. In the mean time, operations were performed for the patients with cataract. RESULTS: Of the total 361 214 persons being surveyed, there were binocular blindness in 1 037 cases (0.29%) and monocular one in 983 cases (0.27%). Of the cases with binocular blindness, 593 cases (57.18%), and of the monocular blindness, 617 cases (62.77%) could be treated. The leading causes of 1037 cases with binocular blindness were cataract (51.98%), injuries of the eye (19.96%), corneal opacity (16.00%), glaucoma (10.03%), refractive error and other ocular diseases (2.03%). The average prevalence rate in the female cases was higher than that of male cases (chi(2) = 53.48, P < 0.01). Blindness was associated with older age There were variations in the prevalence rates in different aboriginals and in cases with different occupations. Cataract was the main disease to cause both the monocular and binocular blindness. Among the patients with cataract 539 cases (90.98%) of binocular blindness and 500 cases (81.03%) of monocular blindness were curable. The cataract operation was performed on 738 cases with cataract, including 396 cases with binocular blindness and 342 cases with monocular blindness. At postoperative 3 months, the rate of the best corrected visual acuity (eye sight > 0.05) was accounted for 97.73%, the rate of the eye sight > 0.3, 86.36% and the rate of wearing glasses (including implantation of intraocular lens), 85.60%. CONCLUSION: The prevalence of blindness is associated with the aboriginal, occupation, age and the sex. The leading cause of blindness is cataract. So the surgical restoration of vision in cases with cataract is the main measure in the prevention of blindness.

[Studies of the clinicopathological changes of eight patients with lipid storage myopathy].

OBJECTIVE: The clinicopathological analysis of eight patients with lipid storage myopathy are presented. The pathogeny and therapeutic effect are probed into. METHODS: Eight cases of lipid storage myopathy diagnosed by muscle biopsies with microscopic and electron-microscopic examination are analyzed. Quadriceps or biceps were biopsied. Muscle samples were stained with routine histology and histochemical enzyme and inspected by microscopy. Thin sections were stained with uranyl acetate followed by lead citrate prior to examination in a electron microscopy. Also, the therapeutic drugs of eight patients were evaluated. RESULTS: Vacuole or crack of muscular fibers involved all eight patients. Sudan Black B and Oil Red O stains demonstrated increase of lipid droplets within muscle fibers. Ultrastructural examination revealed numerous lipid droplets dispersed throughout the residual myofilaments. Three cases with pathologic changed muscular fibers occupying less than 1/5 were belong to low-grade, two cases (between 1/5 to 1/3) were moderate, three cases (more than 1/2) were severe. There was one case accompanying glycogen storage disease. One case was concomitant with deficiency of cytochrome C oxidase. After prednisone treatment, seven cases had greatly improved and one case failed to respond to. Treatment using vitamin B(2) together with other vitamins brought about a striking effect. Carnitine was very effective on the patients with system deficiency of carnitine. CONCLUSIONS: The pathogeny of lipid storage myopathy is varied. The confirmed diagnosis is depend on pathological features of muscle biopsy. Treatment with prednisone, carnitine, vitamins and food containing carnitine rich is very effective. It should be select the special treatment method if the pathogeny is clear.

[Chromosome t (8; 21) and t (15; 17) in a patient with acute myeloid leukemia].

OBJECTIVE: To report an acute myeloid leukemia (AML) patient with both chromosome t (8; 21) and t (15; 17). METHODS: Chromosome specimen was prepared by short-term culture of bone marrow cells, karyotype analyses by R-banding technique, and fusion genes by reverse transcriptase-polymerase chain reaction (RT-PCR). RESULT: Karyotype analyses showed the appearance of 45, X, -X, t (8; 21), t (15; 17), and RT-PCR assay revealed AML1/ETO and PML-RARalpha fusion gene transcripts. CONCLUSION: AML with both t (8; 21) and t (15; 17) might be recognized as a a novel subtype of acute leukemias.

[High efficient expression of recombinant human platelet factor IV in Pichia pastoris].

OBJECTIVE: To express recombinant human platelet factor 4 (PF4) in methanol yeast. METHODS: PF4 cDNA amplified by PCR was cloned into the mating factor alpha (MFalpha) signal sequence, downstream the alcohol oxidase 1 (AOX 1) promoter of the pPIC9 vector. The reconstructed vector was subsequently integrated into the methanol yeast P. pastoris strain GS115. The highly expressing strain was selected and the protein sequences and their biological activities were assayed. RESULTS: The amino acid sequence of rhPF4 was the same as that of natural one. The rhPF4 can neutralize the anticoagulation effect of heparin in a dose-dependent manner. CONCLUSION: The amount of rhPF4 expressed in methanol yeast meets well the scale for manufacturing. The biological activities of the rhPF4 were the same as that of natural PF4.

[Inhibition of platelet function by L-arginine.L-aspartate salt].

The effects of L-arginine.L-aspartate salt (DR) on platelet aggregation, adhesion and release were investigated. Platelet aggregation induced by adenosine 5 -diphosphate (ADP) was significantly inhibited (P<0.01) by intravenous injection of DR (15 mg/kg) in rats or oral administration (15 mg/kg) in rabbits, the inhibitory effect on rabbit platelet aggregation lasting for more than 8 h (P<0.01). Platelet aggregation induced by ADP, collagen or thrombin in rats was all markedly inhibited by 7.5, 15 or 30 mg/kg of DR (bid for 3.5 d, ig, P<0.01). Platelet adhesion to foreign objects was inhibited by 30 mg/kg of DR (ig). Bleeding time in rat tails was prolonged by 30 mg/kg of DR (P<0.05). Furthermore, PGI(2) released from the vascular wall was increased in DR-treated rats (P<0.05), however, TXA(2) released from platelets was not affected. These data demonstrate the inhibitory effect of DR on platelet function, suggesting that its action target may be different from that of acetylsalicylic acid, and that the increase of PGI(2) release may be responsible partly for this effect. It is suggested that DR may probably be used as a new agent for regulating platelet function.

The Barnes-Jewish Hospital/Washington University Renal Transplant Program: comparison of two eras 1991-1994 and 1995-2000.

The first cadaveric transplant at Barnes-Jewish Hospital/Washington University was performed in 1963, the first living related transplant in 1965, and the first living unrelated transplant in 1983. Changes in the renal transplant program initiated in 1993 and 1994 resulted in many improvements over the past decade. Our comparison of 2 modern eras of transplant, 1991-1994 and 1995-2000, showed the following: 1. No significant differences in patient and donor characteristics. 2. Trends toward greater use of living donors (p = 0.07), older cadaveric donors (p = 0.084) and particularly cadaveric donors > 55 years of age (p = 0.09). 3. Decreasing mean CIT: 19.2 hours vs. 14.2 hours (p < 0.001). 4. Decreasing use of donors with CIT > 24 hours: 22% to 3%, (p < 0.001). 5. Decreased rate of DGF: 13% vs. 8% (p = 0.044). 6. Decreased rate of symptomatic CMV: 35% vs. 14% (p < 0.001). 7. Decreased rate of PTLD: 3.5% vs. 0.5% (p = 0.004). 8. Decreased one-year rate of acute rejection: 41% vs. 15% (p < 0.001). 9. Current one-year rate of acute rejection < 8%. 10. Decreased length of initial hospital stay: 12.7 days to 8.0 days (p < 0.001). 11. Decreased length of hospital in the first year after transplant: 10.6 days vs. 6.4 days (p < 0.001). 12. There were no improvements in patient and graft survival at one and 3 years. a. one-year patient survival rates: 95% vs. 96%. b. 3-year patient survival rates: 90% vs. 90%. c. one-year death-censored graft survival rates: 91% vs. 94%. d. 3-year death-censored graft survival rates: 87% vs. 88%.

[Study on FM100-HUE color vision in male workers exposed to lower concentration of carbon disulfide].

The effect of CS2 on color vision in workers exposed to CS2 was explored. The color discrimination in 191 exposed and 93 non-exposed male workers was studied by a Farnsworth Munsell 100-HUE (FM100-HUE) test. The results showed that the time weighted average (TWA) concentration of CS2, determined by Japan GC-17A gas chromatography, was (14.75 +/- 6.89) mg/m3 in a long rayon workshop and (16.30 +/- 5.42) mg/m3 in a short rayon workshop. Higher FM100-HUE color vision score was often in the exposed group (106.9) than in the controls (78.3). The discrimination to green and blue zone was also significantly impaired (P < 0.05). It is concluded that the color vision was disturbed in workers exposed to CS2. The workers should be followed by health surveillance.

[Investigation of the tongue squamous cell carcinoma prognostic factors].

OBJECTIVE: The aim of this study was to investigate the factors which influence the prognosis of the tongue squamous cell carcinoma (TSCC). METHODS: The Cox's proportional hazard model was used to analyze 121 cases of TSCC treated by the affiliated dental hospital of West China University of Medical Sciences in the period from 1990 to 1994. Thirteen parameters (age, sex, occupation, primary tumor site, clinical stage, tumor differentiation, clinical therapy, cervical lymph node metastasis, surgical margin status of the primary tumor, amount of blood transfusion, smoking, drinking, reconstruction during operation) were evaluated. RESULTS: The monofactor analysis showed that factors influencing the postoperative survival of TSCC included primary tumor site, the surgical margin status of the primary tumor and the cervical lymph node metastasis (P < 0.05). Multifactor analysis demonstrated that two of these thirteen relevant factors influenced the postoperative survival of TSCC independently. They are successively the surgical margin status of the primary tumor and the cervical lymph node metastasis (P < 0.05). CONCLUSION: This study suggests that it is critical for surgeons to resect the primary tumor and the metastatic cervical lymph nodes thoroughly to improve the survival rate of TSCC patients.

[An experimental study on effects of pingyangmycin on vessels].

OBJECTIVE: The aim of this study was to investigate the mechanism of Pingyangmycin sclerotherapy for cavernous hemangiomas. METHODS: Totally 9 rabbits (one as the control) were selected to be injected with Pingyangmycin and sodium morrhuate into the auricularis posterior vein, and then these veins were examined histologically 2, 7, 14 and 21 days after injection respectively. RESULTS: After injection, Pingyangmycin nonspecifically made the endothelia and veins defective, and induced proliferation of endothelial cells and smooth muscle cells after 7 days. Further, the veins became sclerostenosis 21 days after injection. On the other hand, sodium morrhuate caused thrombosis quickly and the vascular cavity disappeared. Finally, the thrombus became fibrogenesis. The local skin could be observed swollen and necrosis. CONCLUSION: Pingyangmycin is a better sclerosant for the therapy of cavernous hemangioma.

[An experimental study of effects of injection of pingyangmycin on activities of enzyme of endothelial cells].

OBJECTIVE: The objective of this study is to investigate the mechanism of acute trauma of endothelium after injection of Pingyangmycin. METHODS: A total of 42 adult Wistar rats were divided randomly into experimental groups (24), positive control groups (12) and normal control group (6), and the rats in the experimental or the positive control groups were injected with Pingyangmycin or sodium morrhuate into the mesenteric veins respectively, and then the enzyme activity of hepatic endothelial cells and hepatocytes was examined histochemically 0.5, 1, 2, 4, 8 and 24 hours after injection. RESULTS: After injection, the activity of 5'-nucleinase(5'-N) in hepatic endothelial cells and that of lactic dehydrogenase (LDH) and succinodehydrogenase (SDH) in hepatocytes decreased gradually, but that injected Pingyangmycin was less than that injected sodium morrhuate. CONCLUSION: The trauma caused by Pingyangmycin is less than that caused by sodium morrhuate, but the reaction of trauma caused by both is nonspecific.

[A retrospective clinical study of 6539 cases of malignant oral-maxillofacial tumors].

OBJECTIVE: The aim of this study is to obtain the general information of post-operation malignant oral-maxillofacial tumor patients in forty-seven years. METHODS: A total of 6539 patients with malignant oral-maxillofacial tumors in the hospital of stomatology, west china university of medical sciences, from 1953 to 2000 were retrospected. RESULTS: The average age of patients with malignant oral-maxilloficial tumor is 48.7 years old. The ratio of male to female is 2.31:1. The most common original malignant tumors in this oral-maxillofacial region are malignant oral tumors, malignant maxillary tumors, malignant mandibular tumors, and malignant sialoma successively. As to the original tissues of tumors, there were 5869 cases of epithelial tumors, and made up 89.75% of all the investigated patients. Tumors came from lymphatic and hematopoietic systems and mesoderm were much less those came from epithelia. There were 4177 cases of squamous cell carcinomas and the percentage was 63.88%. CONCLUSION: The incidence rate of malignant tumors in oral-maxillofacial region tends to increase, and the average age of patients also rose. The ratio of male to female decreases gradually. Epithelial tumors, especially squamous cell carcinomas are very common tumors happening in this region.

[Expression of vascular endothelial growth factor in oral squamous cell carcinoma].

OBJECTIVE: The aim of this study was to estimate the expression of vascular endothelial growth factor(VEGF) and intratumoral microvessel density (IMVD) in oral squamous cell carcinoma(SCC). METHODS: A total of 60 cases of oral squamous carcinoma were immunohistochemically stained with a SP method by using monoclonal antibodies of VEGF and CD34. RESULTS: There was no significant difference in the expression of VEGF in different TNM stages and lymphe node metastases of oral squamous cell carcinoma. The expression of VEGF showed no relation with pathologic grades, but IMVD expression differed significantly in SCC. The carcinoma in grade I and IV of IMVD differed significantly in expression of VEGF. CONCLUSION: It indicates the change of IMVD may resulted from the change of VEGF in oral SCC.

[Expression of VEGF-C and its correlation with cervical lymph nodes metastasis of oral cancers].

OBJECTIVE: Vascular endothelial growth factor C (VEGF-C) is known to be capable of inducing cell proliferation of lymphatic endothelia and development of lymphatic vessels. Thus, in the current study the expression of VEGF-C was investigated to understand the mechanism of lymphatic metastasis of oral cancer. METHODS: VEGF-C protein expression in benign and malignant oral lesions was investigated with an immunohistochemical staining assay, followed by light microscopic examination and image analysis. RESULTS: Immunohistostaining of VEGF-C was observed in normal, benign and malignant epithelia, as well as in some lymphatic vessels. The VEGF-C positive rate was significantly higher in squamous carcinomas than in normal tissues and benign lesions. It was also noted that in squamous carcinomas the expression level of VEGF-C correlated well with pathological grades and stages of lymph node metastasis, but not with clinical grades. CONCLUSION: These findings suggest that VEGF-C may promote lymphatic metastasis of oral cancer, and it is possible to be used as a predictor for the lymphatic metastasis and prognosis of oral cancer.

[Preparation of cucurbitacinBE polylactic acid nano-particles for targeting cervical lymph nodes].

OBJECTIVE: Anti-cancer nano-particles for targeting cervical lymph nodes were designed and prepared on the basis of the physiologic and anatomic properties of the peri-oral cancer lymphatic capillaries. METHODS: The biodegradable polylactic acid (PLA) was selected as the carrier material, and cucurbitacinBE (CuBE) with very high anticancer activity against oral squamous cell carcinoma was used as the model drug. According to the criteria of the diameter of the nano-particles, drug loading (DL) and embedding ratio (ER), the preparing technology was optimized by uniformly designing experiment on the basis of single-factor experiment. CucurbitacinBE poly-lactic acid nano-particles (CuBE-PLA-NP) was prepared with the method of emulsion solvent evaporation, and then CuBE-PLA-NP lyophilization injection was prepared by using 10% manninositose as the supporting agent. RESULTS: The diameter distribution for optimized CuBE-PLA-NP lyophilization injection ranged from 47 nm to 120 nm, and the average diameter was 85 nm. DL and ER were 23.03% +/- 0.47% and 93.00% +/- 0.10% respectively. The release study of the thermostatics also demonstrated that Higuchi equation was adequate to fit the release curve of CuBE-PLA-NP. CONCLUSION: CuBE-PLA-NP can be used as the release agent, as CuBE-PLA-NP can enter peri-oral cancer lymphatic capillaries for targeting cervical lymph nodes by the peri-cancer submucosal injection. Therefore, it can also used for animal and clinical experiments.

[Acute toxicity and local stimulate test of cucurbitacinBE polylactic acid nano-particles of targeting cervical lymph nodes].

OBJECTIVE: The aim of this experiment was to study acute toxicity and local stimulation of cucurbitacinBE poly-lactic acid nano-particles (CuBE-PLA-NP) for targeting cervical lymph node. METHODS: Various doses of CuBE-PLA-NP lyophilization injection and cucurbitacinBE (CuBE) were hypodermically or veno-injected into the mice, and then the drug toxicity and side effects were observed; 50% lethal dose (LD50) was counted. CuBE-PLA-NP lyophilization injection and CuBE were also injected into the quadriceps femoralis muscle of rabbits to observe the local stimulate responses of drugs. RESULTS: The fifty percent lethal doses of subcutaneous and venous CuBE-PLA-NP in mice were 16.74 mg/kg +/- 1.83 mg/kg and 5.60 mg/kg +/- 0.83 mg/kg respectively, that were higher than those of CuBE which were 8.63 mg/kg +/- 1.00 mg/kg and 2.50 mg/kg +/- 0.30 mg/kg respectively. Local stimulate response in rabbit quadriceps femoralis muscle after CuBE-PLA-NP and CuBE injected are 0 grade and 0.1 grade respectively. CONCLUSION: The acute toxicity of CuBE-PLA-NP is lower than that of CuBE. Both CuBE-PLA-NP and CuBE are adequate for local injection, but the local stimulation of CuBE-PLA-NP is smaller than that of CuBE.

[Expression of TNF alpha and VCAM-1 in nasal polyps and relation with eosinophil infiltration].

OBJECTIVE: To study the expression of TNF alpha and VCAM-1 in nasal polyps tissue and its significance. METHOD: Paraffin sections of nasal polyps and chronic rhinitis samples were studied with immunohistochemical technique. RESULT: Expression of TNF alpha and VCAM-1 were stranger in nasal polyps tissues than in controls. There was positive relationship between the expression of TNF alpha and that of VCAM-1(r = 0.833), and expression of VCAM-1 coincide with eosinophil infiltration(r = 0.746). CONCLUSION: It is suggested that TNF alpha may up-regulate the expression of VCAM-1 in vessel endothelium, and prompt adhesion and migration of eosinophils, TNF alpha and VCAM-1 may play important role in the pathogenesis of nasal polyps.
