Effects of extrinsic positive end-expiratory pressure on cardiopulmonary function in patients with chronic obstructive pulmonary disease.

OBJECTIVE: To choose one optimal extrinsic positive end-expiratory pressure (PEEPe) for ventilated patients with chronic obstructive pulmonary disease (COPD) and to compare two methods for choosing the optimal level of PEEPe. METHODS: Ten ventilated patients with COPD were included in the study. First, static intrinsic positive end-expiratory pressure (PEEPi, st) was measured when PEEPe was zero, and the PEEPi, st was called PEEPi, stz. PEEPe at 0%, 40%, 50%, 60%, 70%, 80%, 90% and 100% of PEEPi, stz, respectively, were applied randomly. Respiratory mechanics, hemodynamics, and oxygen dynamics were recorded 30 minutes after the level of PEEPe was changed. RESULTS: When PEEPe was not higher than 80% of PEEPi, stz, no measurement changed significantly. When PEEPe was increased to 90% and 100% of PEEPi, stz, PEEPi, st, peak inspiratory pressure, plateau pressure, pulmonary capillary wedge pressure and central venous pressure increased significantly, P < 0.01. Cardiac output and left ventricular work index decreased significantly, P < 0.01. Oxygen delivery decreased significantly, P < 0.05. When PEEPe was increased to 100% of PEEPi, stz, the right ventricular work index decreased significantly, P < 0.05. CONCLUSION: Eighty percent of PEEPi, stz was the upper limit of PEEPe. The results of the two methods used to set the level of PEEPe were identical.

Apoptosis of renal tubular cells in congenital hydronephrosis.

OBJECTIVE: To assess apoptosis in congenital hydronephrosis and discuss its clinical significance. METHODS: Apoptosis was detected in 15 kidneys from children with congenital hydronephrosis (5 mild hydronephrosis, 5 moderate hydronephrosis and 5 severe hydronephrosis) using the electronmicroscope, in situ gap labeling of fragmented nuclear DNA and DNA fragmentation analysis. RESULTS: Apoptosis was seen in kidneys from children with congenital hydronephrosis. Margination of nuclear chromatin was identified and rounded apoptotic bodies were seen. The mean apoptotic index was 0.0941 +/- 0.017 in severe hydronephrosis, 0.0325 +/- 0.0169 in moderate hydronephrosis, and 0.0021 +/- 0.0031 in mild hydronephrosis. There was a significant difference between severe and moderate hydronephrosis (P = 0.0005), as well as between moderate and mild hydronephrosis (P = 0.0154). Moreover, with an increasing degree of hydronephrosis, the number of apoptotic cells also increased. Five kidneys with severe hydronephrosis and one kidney with moderate hydronephrosis showed typical apoptotic bands. CONCLUSION: Apoptosis might participate in damaging kidneys in children with congenital hydronephrosis.

Increased expression of Hsp70 and co-localization with nuclear protein in cells infected with the Hantaan virus.

OBJECTIVE: To investigate the effect of Hantaan virus infection on the expression of stress genes. METHODS: Techniques of virus infection, Western blot, immunohistochemistry, dual-immunofluorecsence staining, laser scanning confocal microscopy, RNA dot blot and in situ hybridization were used. RESULTS: Expression of heat shock protein 70 (Hsp70) was observed in cells infected with HTV as well as the translocation of Hsp70 from cytoplasm to nucleoli following virus infection. The variable distribution of Hsp70 was related to the various time after infection. Double-label indirect immunofluorescence of nuclear protein (NP) and Hsp70 in infected cells demonstrated co-localization of these proteins in the cytoplasm. CONCLUSION: Overexpression of Hsp70 can be induced directly by Hantaan virus, which may be associated with virus protein assembly. The Hantaan virus proteins co-localize with, and possibly form a physical complex with cellular Hsp70 in infected cells.

Detailed deletion mapping on chromosome region 9p21 in human periampullary neoplasms.

OBJECTIVE: To further define the extent of chromosome 9p21 deletion in periampullary neoplasms. METHODS: The loss of heterozygosity at 5 microsatellite polymorphic markers on chromosome 9p21 was detected by polymerase chain reaction (PCR), polyacrylamide gel electrophoresis (PAGE) and silver staining in 35 specimens of periampullary neoplasms and their matching blood samples. RESULTS: Fifty percent (4/8) of pancreatic cancer cases showed the loss of heterozygosity at one or more microsatellite loci, with the more frequent sites of D9S974 (37.5%) and D9S942 (28.6%), and some showing consecutive allelic loss. Sixty-two point five percent (5/8) of ampullary carcinoma cases showed loss of heterozygosity at one or more of the loci, frequent site of loss being D9S942 (42.9%) and the next most frequent being IFNA (37.5%) and D9S171 (37.5%). Loss of one locus was observed in 14.2% (1/7) of insulinoma. CONCLUSION: The minimal common region of chromosome deletion in periampullary neoplasms is defined between the D9S974 and D9S942 loci within a 15 kb interval in 9p21, suggesting the involvement of a novel tumor suppressor gene in their carcinogenesis.

Efficacy of continuous tracheal gas insufflation in spontaneously breathing canine with acute lung injury.

OBJECTIVE: To investigate the effects of continuous tracheal gas insufflation (CTGI) combined with biphasic intermittent positive airway pressure (BIPAP) ventilation on dogs with spontaneous breathing. METHODS: Eight canine models with oleic acid induced lung injury and spontaneous breathing were ventilated in a random order by Evita 2 (Drager Inc., Germany) in modes of BIPAP (BIPAP group) and BIPAP with CTGI flow rate of 3, 6 and 9 L/min (T3, T6 and T9 groups), respectively. The setting parameters of BIPAP were fiction of inspired oxygen 60%, inspiratory to expiratory ratio 1:1, respiratory rate 20 and positive end expiratory pressure 5 cm H2O. Arterial and mixed venous blood gas, lung mechanics, systemic and pulmonary hemodynamics status were monitored at the same level of PaCO2 obtained by adjusting peak inspiratory pressure of BIPAP. RESULTS: Peak inspiratory pressure in the T6 group (14 +/- 4 cm H2O) and in the T9 group (11 +/- 3 cm H2O) were significantly lower than that of BIPAP (20 +/- 5 cm H2O, P < 0.01), but there was no significant difference among the T3, T6 and T9 groups or between the T3 and BIPAP groups. PaO2, mean artery blood pressure, mean pulmonary artery pressure, pulmonary artery wedge pressure, cardiac output, oxygen delivery and oxygen consumption all remained unchanged in four different conditions. CONCLUSIONS: Using BIPAP combined with CTGI does not cause asynchrony between ventilator and spontaneous breathing, but significantly decreases airway pressure with no influence on hemodynamics and oxygenation. Therefore, BIPAP with CTGI may be a useful support technique, especially in cases where the airway pressure should be limited.

[Increase of EGFR expression by Epstein-Barr virus LMP1 in nasopharyngeal carcinoma cells].

OBJECTIVE: To investigate the effect of LMP1 on the expression of epidermal growth factor receptor (EGFR) in HNE2-LMP1 cells. METHODS: Stable transfectant HNE2 cell line expressing LMP1 (HNE2-LMP1) or its mutants (HNE2 del 187-351, HNE2 1-231, HNE2 1-187) were used as cell models. The expression of EGFR was detected by Western blot method. The characteristics of EGFR expression were analyzed when HNE2-LMP1 LMP1 cells were transiently transfected with TRAF1, 2, 3 or TRAF1, 2, 3 dominant negative mutants (DN-TRAF1, 2, 3). The growth characteristics of HNE2-LMP1 and vector controls were analyzed on serum-free media supplemented with EGF. RESULTS: Stable expression of LMP1 in HNE2 cells increased the expression of EGFR. LMP1 CTAR1 domain, being identical to the TRAF interaction domain, was essential to the induction of EGFR. The CTAR2 domain did not induce expression of EGFR. Overexpression of either TRAF1, 3 or an dominant negative mutant of TRAF1, 3 inhibited the expression of EGFR, while TRAF2 or negative mutants of TRAF2 did slightly so. In HNE2-LMP1 cells, LMP1 expression increased the proliferative response to EGF while the vector control cells exhibited very low level of viability and did not proliferate. CONCLUSION: The induction of EGFR by LMP1 CTAR1 may be an important component of EBV infection via an TRAF1, 2, 3 mechanism in the epithelial cells and may contribute to the development of epithelial malignancies such as nasopharyngeal carcinoma.

[Changes of plasma level of neuropeptide Y in patients with pregnancy induced hypertension].

OBJECTIVE: To investigate the changes and its clinical significance of plasma neuropeptide Y (NPY) concentration in patients with pregnancy induced hypertension (PIH). METHODS: Plasma NPY levels were detected by radioimmunoassay in 30 patients with PIH, 20 normal non-pregnant women and 23 normal pregnant women. The PIH group was subdivided into mild, moderate and Severe subgroups, and the NPY concentration was also measured in these subgroups respectively at admission and one week after delivery. RESULTS: The plasma NPY levels in patients with PIH [(164.16 +/- 68.32) ng/L] were significantly higher than those of normal non-pregnant women and normal pregnancies [(86.60 +/- 20.65) ng/L, (82.42 +/- 12.46) ng/L, P < 0.01, respectively]. There was significant difference among plasma NPY levels among the patients with mild, moderate, and severe PIH at admission (P < 0.01). At one week after delivery the concentrations of plasma NPY were significantly decreased in the moderate and severe subgroups compared with the value measured at admission (P < 0.01). Moreover, the NPY levels in patients with severe PIH after delivery were still higher than those of normal non-pregnant women. CONCLUSIONS: The results suggested that the level of NPY in plasma is increased in women with PIH. Elevated plasma NPY levels may play a key role in the development of PIH.

[The change of plasma von Willebrand factor and endothelin levels in patients with pregnancy induced hypertension].

OBJECTIVE: To study the association of plasma von willebrand factor (vWF) and endothelin-1 (ET-1) levels in patients with pregnancy induced hypertension (PIH). METHODS: Plasma vWF and ET-1 were studied by ELISA method and immunoradiological method respectively in 36 patients with PIH, 18 normal pregnant women and 19 normal non-pregnant women. RESULTS: Circulation levels of vWF and ET-1 were increased with increasing severity of the PIH. In mild PIH patients, the levels of plasma vWF [(135.9 +/- 30.9)%, (P > 0.05)] weren't significantly higher than those in healthy pregnant women [(131.6 +/- 39.2)%, (P > 0.05)], but the levels of ET-1 [(63.7 +/- 4.8) pg/L] were significantly higher than those in healthy pregnant women [(47.1 +/- 4.7) pg/L, (P < 0.01)]. There were significant differences between groups of moderate-severe PIH [vWF (174.4 +/- 35.4)%, ET-1 (92.6 +/- 19.1) pg/L]. There was a positive correlation between the plasma vWF level and ET-1 level. CONCLUSION: The results suggested that plasma vWF and ET-1 concentration could be used as indicators for the severity of PIH.

[The role of insulin-like growth factor-I gene expression abnormality in pathogenesis of diabetic peripheral neuropathy].

OBJECTIVE: To explore the role of insulin-like growth factor-I (IGF-I) gene expression abnormality in neurotrophic causes of diabetic peripheral neuropathy. METHODS: Diabetes was induced in Sprague Dawley rats by alloxan. Various parameters were measured as follows: IGF-I mRNA by reverse-transcriptase polymerase chain reaction (RT-PCR), IGF-I peptide by enzyme-linked immunosorbent assay (ELISA), electrophysiological parameters of nerves by evoked electromyogram, morphometric evaluation of sciatic nerve under light microscope and transmission electron microscope. RESULTS: During the early diabetic stage, IGF-I mRNA (0.430 +/- 0.031 vs 0.370 +/- 0.016 +/- 0.280 +/- 0.010, P < 0.01, < 0.001) and IGF-I peptide contents [(0.430 +/- 0.031) ng/mg vs (30.06 +/- 3.60 +/- 23.71 +/- 2.70) ng/mg, P < 0. 01, < 0.001] in sciatic nerve tissue reduced in diabetic rats with hyperglycemia and varied with severity of diabetes. These parameters were further down regulated in diabetic rats with less duration of diabetes and non-diabetic control rats [IGF-I mRNA 0.320 +/- 0.021 - 0.230 +/- 0.060; IGF-I peptide (28.8 +/- 3.30 - 19.51 +/- 1.80) ng/mg]. They also correlated with nerve functional (sensory nerve conduction velocity: r = 0.714, P < 0.001, amplitude of evoked potentials: r = 0.716, P < 0.001, respectively) and structural abnormality (axonal size r = 0.81, P < 0.001) of the sciatic nerve. No difference was found in the above parameters between diabetic rats with euglycemia and non-diabetic control group. CONCLUSION: IGF-I gene expression in tissues was down-regulated from the early diabetic stage and the down-regulation varied with the severity and duration of diabetic state. The decrement in IGF-I level might contribute to the initiation and development of diabetic neuropathy via autocrine or paracrine pathway.

[The effects of extrinsic positive end-expiratory pressure on work of breathing in patients with chronic obstructive pulmonary disease].

OBJECTIVE: To study the effect of extrinsic positive end-expiratory pressure(PEEPe) on work of breathing in chronic obstructive pulmonary disease (COPD) patients and its mechanism. METHODS: 10 ventilated patients [4 males and 6 females with mean age of (70.1 +/- 5.5) years] with exacerbation of COPD were admitted into the study. All the patients' cardiopulmonary functions were stable, and they could receive pressure support ventilation (PSV). Patients with sever heart dysfunction, hepatic dysfunction, renal dysfunction or fever were ruled out. Each patient had an esophagus balloon pressure sensor inserted, and the esophagus balloon was properly positioned using "occlusion test". Flow sensor was placed at the end of tracheal tube opening. Then esophagus balloon pressure sensor and flow sensor were connected with Bicore CP-100 pulmonary monitor. Dynamic intrinsic positive end-expiratory pressure (PEEPi, dyn), difference of esophagus pressure (dPes), work of breathing patient (WOBp), work of breathing ventilator (WOBv), pressure time product (PTP), tidal volume (Vt), respiratory rate (RR), minute ventilation (MV), peak inspiratory flow rate (PIFR), peak expiratory flow rate (PEFR), expiratory airway resistance (Rawe), fraction of inspiration (Ti/Ttot) and so on were measured. PEEPi,dyn was measured by esophagus balloon technique. Electrocardiogram and noninvasive blood pressure modes were added to HP M1165A monitor. Heart rate (HR) and blood pressure (BP) were also measured. Arterial blood was analyzed to measure pH, partial pressure of oxygen in arterial blood (PaO(2)), partial pressure of carbon dioxide in arterial blood (PaCO(2)) and saturation of arterial blood oxygen (SaO(2)). At first, PEEPi, dyn was measured when PEEPe was zero, which was called PEEPi, dynz and was set as baseline. Then PEEPe was set randomly, which was 0%, 40%, 60%, 80% and 100% of PEEPi, dynz respectively. Other parameters remained stable. All the indexes were measured 30 minutes after the level of PEEPe was changed. RESULTS: WOBp decreased as PEEPe increased, from (1.08 +/- 0.49) J/L of PEEPe-0% of PEEPi,dynz to (0.57 +/- 0.32) J/L of PEEPe-100% of PEEPi, dynz, and significant difference was found between each level of PEEPe and the baseline (P < 0.01). But we didn't find significant difference between PEEPe-80% of PEEPi,dynz and PEEPe-100% of PEEPi, dynz. WOBv increased significantly as PEEPe increased, from (1.50 +/- 0.47) J/L of PEEPe-0% of PEEPi, dynz to (1.82 +/- 0.56) J/L of PEEPe-80% of PEEPi, dynz and (1.85 +/- 0.48) J/L of PEEPe-100% of PEEPi,dynz(P < 0.01). The total WOB (WOBp + WOBv) remained steady. PTP trended to decrease, from (153.6 +/- 76.8) cm H(2)O x s x m(-1) of PEEPe-0% of PEEPi, dynz to (86.9 +/- 45.9) cm H(2)O x s x m(-1) of PEEPe-100% of PEEPi, dynz; the difference of PTP between PEEPe-40% of PEEPi,dynz and PEEPe-0% of PEEPi, dynz was significant(P < 0.05), the differences between other levels of PEEPe and PEEPe-0% of PEEPi, dynz were very significant (P < 0.01), but no significant difference was found between PEEPe-80% of PEEPi, dynz and PEEPe-100% of PEEPi, dynz. PEEPi,dyn also decreased significantly as PEEPe increased, from (7.70 +/- 1.42) cm H(2)O of PEEPe-0% of PEEPi, dynz to (2.10 +/- 0.57) cm H(2)O of PEEPe-100% of PEEPi, dynz, very significant differences were found between each level of PEEPe and the baseline (P < 0.01). Significant positive linear correlation was found between DeltaWOBp and DeltaPEEPi, dyn were(r = 0.609, P < 0.01, n = 10). dPes decreased as PEEPe increased, from (15.20 +/- 4.16) cm H(2)O of PEEPe-0% of PEEPi, dynz to (8.20 +/- 4.13) cm H(2)O of PEEPe-100% of PEEPi, dynz, and very significant differences were found between each level of PEEPe and the baseline (P < 0.01). But the difference between PEEPe-80% of PEEPi, dynz and PEEPe-100% of PEEPi, dynz wasn't significant, even dPes of PEEPe-100% of PEEPi, dynz was higher than that of PEEPe-80% of PEEPi, dynz. Vt didn't change markedly. RR, MV and f/Vt increased slightly. Both PIFR and PEFR didn't change markedly. At the same time, HR, BP and blood gas analysis were investigated, but no significant change was found. CONCLUSION: PEEPe can reduce WOBp significantly and improve the coordination between the patient and ventilator. It is shown that decrement of pressure difference between alveolar and central airway leads to decrement of WOBp.

[The engulfment effects of macrophage on apoptotic eosinophils treated by anti-Fas monoclonal antibody in vitro].

OBJECTIVE: Expression of the Fas antigen and induction of apoptosis in eosinophils (EOS) by anti-Fas antibody in vitro were investigated. METHODS: The purified EOS from healthy donors were cultured for 72 h in the presence of human IL-5 and with or without anti-Fas monoclonal antibody (MPhiAb) at various concentrations (1 approximately 1 000 ng/ml). The changes of eosinophil viability, percentage of apoptosis, and fragmented DNA electrophoresis were observed. The eosinophils treated by Fas MPhiAb for 24 h, 48 h, 72 h were cultured with monocyte-derived macrophages, the positive percentage of macrophages which engulfed EOS was calculated. RESULTS: The anti-Fas MoAb suppressed IL-5-mediated EOS survival in dose- and time-dependant manner. High concentration of IL-5 (10(6)U/L) could not inhibit anti-Fas MoAb-mediated apoptosis. The percentage of apoptosis was (35 +/- 6)% after 24 h treated with Fas MoAb (P < 0.01), and increased to (96 +/- 3)% after 72 h (P < 0.01). The percentage of macrophages which engulfed eosinophils was significantly higher than control (P < 0.01). CONCLUSION: The anti-Fas MoAb can efficiently induced eosinophils apoptosis, apoptotic EOS induced by anti-Fas MoAb could be engulfed by macrophages.

[Histopathological changes of lung and right ventricle induced by high concentration of carbon dioxide in rats].

OBJECTIVE: To evaluate if high concentration of carbon dioxide causes any histopathological changes of the lung and right ventricle in rats. METHODS: Wistar rats were divided into control group (Group A, n = 21) and hypercapnic group (Group B, n = 30). Group B received mixed gas exposure (8% CO(2), 21% O(2), 71% N(2)), 7 hours daily for 4 weeks. Arterial blood gas, superoxide dismutase (SOD), lipid peroxide (LPO) in the tissue of the lung and right ventricle were determined. Histopathological changes of the lung and right ventricle were investigated by light microscope, transmission and scanning electron microscope. RESULTS: Decompensated respiratory acidosis [pH 7.21 +/- 0.04, PaCO(2) (56 +/- 3) mm Hg] developed in Group B. SOD in the lung [(463 +/- 174) U/g protein] and ventricle tissue [(235 +/- 94) U/g protein] in Group B were significantly lower than those in Group A (P < 0.01), while LPOs [(8.1 +/- 3.2) nmol/ml and (17 +/- 5) nmol/ml] in group B were significantly higher than those in Group A (P < 0.01). The index of pulmonary capillary permeability in Group B (37 +/- 4) was also higher than that in Group A (28 +/- 4, P < 0.001). Tissue edema in the lung and heart, endothelial cell damage of the small blood vessels and severe myofibril damage of the right ventricle, pulmonary microthrombus formation and increased pulmonary capillary permeability were found in Group B. CONCLUSION: Hypercapnic rat model is successfully reproduced by exposure of 8% CO(2); high concentration of CO(2) causes severe damage of the lung and right ventricle as well as the whole body.

[Preliminary study on the rules of streptococcus mutans reactive oxygen species metabolism].

OBJECTIVE: To study the rules of Streptococcus mutans reactive oxygen species metabolism and its enzymatic basis, and to investigate its significance intermediate to plaque microbial ecology. METHODS: Electron spin trapping method was used to measure reactive oxygen species produced by Streptococcus mutans in the different incubation conditions, and evaluate the efficicay of NADH to hypoxanthine-xanthine oxidase and Fe(2+)-H2O2 system. RESULTS: Electron spin resonance (ESR) spectrum of DMPO-O2- and DMPO-OH. and superoxide dismutase were obtained in Streptococcus mutans culture fluid, these spectrum were strengthened by exogenous sugar, and disappeared when methylene blue was added to culture fluid, but that spectrum was not significantly changed by NADH. ESR spectrum of hypoxanthine-xanthine oxidase and Fe(2+)-H2O2 system were significantly reduced or disappeared by NADH. CONCLUSION: These results suggest that there are O2- and OH in Streptococcus mutans metabolism, and they may come mainly from the reaction catalyzed by NADH oxidase.

[Ecophysiological response of Pinus koraiensis seedlings to elevated CO2].

Studies on the ecophysiological response of Pinus koraiensis seedlings were conducted in open-top chambers which were controlled with CO2 concentration of about 500 and 700 mumol.mol-1. The result shows that there were different responses in net photosynthetic rate and stomatal conductance at high CO2(500 and 700 mumol.mol-1) and ambient CO2 concentrations. Under 500 mumol.mol-1 CO2, the carboxylase activity of RuBP was the highest, the photosynthesis showed "up-ward", the average daily net CO2 assimilation was the greatest, and the contents of chlorophyll and soluble sugars were the highest. While under 700 mumol.mol-1 CO2, the photosynthesis showed "down-ward" and was lower than that of the control, and the enzyme activity and the contents of substances were the lowest.

Ocular histopathological changes after eyeball enucleation induced by corneal trauma.

OBJECTIVE: To observe the ocular histopathological changes after eyeball enucleation induced by corneal trauma. METHODS: Light microscopic examination was done on 117 eyeball specimens enucleated after corneal trauma (18 with corneal fissure and 99 with corneal perforating trauma). RESULTS: Acute, subacute or chronic inflammatory changes, and fibrous membrane formation were observed in well-closed corneal wounds, whereas inflammation, atrophy and scar were observed in the focal tissues. But at the late period, secondary glaucoma, retinal detachment, endophthalmitis and eyeball atrophy resulted in blindness. Corneal fistula was observed in those with inadequate cure of wounds caused by ingrowth of corneal epithelium, embedment of iris and vitreous body, and large area of centrally located tissue deficiency of the corneal. A high incidence of endophthalmitis was noted due to the presence of corneal fistula. Severe inflammation was observed in the anterior segmental tissues with fibrous infiltration in the anterior chamber, which might result in rapid destruction of the eyeballs. CONCLUSIONS: Ocular pathology varies with the difference of the position, form, size and closing conditions of the corneal laceration after trauma.
