Enzymatic responses to skin flap elevation following a delay procedure.

Alterations in enzyme activities of glucose metabolism were determined in the distal portion of the skin flaps of guinea pigs elevated following 3, 7, 14, or 21 days of the delay period. Hexokinase, pyruvate kinase, lactate dehydrogenase, and glucose 6-phosphate dehydrogenase activities were increased during the delay period, whereas isocitrate and malate dehydrogenases exhibited little alteration. Increases in glycolytic enzyme activities observed during the delay procedure were diminished in the flaps elevated during initial 3 days of the delay period, but were maintained or increased further in the flaps elevated at 7 to 21 days. Despite high levels of enzyme activities during the early period of the delay, the flaps elevated during this period exhibited partial necrosis with a low glucose level and decreased enzyme activities. It is concluded that tissue glucose level and its utilization are crucial factors for flap survival.

QSAR of acetylchol inesterase inhibitors: a reexamination of the role of charge-transfer.

The binding of acetylcholinesterase (AChE) inhibitors to the enzyme is well correlated with substituent constants like logMR, epsilon pi, epsilon sigma, mu, D for series of aromatic methylcarbamates and aromatics. From a correlation study, it appears that charge-transfer effects can be quantitatively separated into steric, electronic and indicator variables (the number of lone pair electrons). Therefore, the interaction of AChE inhibitors with the enzyme can be attributed to the summation of various intermolecular forces as well as to charge-transfer and hydrophobic interactions alone.

Potential pitfalls of quantitative burn wound biopsy cultures.

We present our experience with clinical correlation of quantitative cultures. A total of 285 cultures in 18 patients were carried out according to the technique of burn wound biopsy culture. The predominant organism by far was Pseudomonas aeruginosa, followed by mixed Pseudomonas and Staphylococcus and Staphylococcus alone. There were two areas of divergence from classic correlations between biopsy results and the clinical course of the patient: data are presented from clinical burn wound sepsis and falsely low quantitative cultures, and from clinically aseptic patients pursuing a benign course with falsely high quantitative cultures. We believe that culture results must be interpreted in the light of total body surface area involved rather than as an isolated finding. Quantitative burn wound cultures are a useful guide to the management of burn patients, but must be interpreted in conjunction with clinical findings.

Metabolic adaptations in delayed skin flaps. Glucose utilization and hexokinase activity.

The distribution of glucose and hexokinase activity was determined in the epithelial tissue of delayed bipedicled skin flaps in guinea pigs. The periods of "delay" were 1, 3, 7, 14, or 21 days. The flap survival was maximal (100% of the flap) when the flap elevation was performed either 7 or 14 days following the "delay" procedure. When the flap elevation was performed 1, 3, or 21 days following the "delay" procedure, the result was partial necrosis. A differential distribution of epithelial glucose was found within the bipedicled flaps. The lowest glucose level (30% of normal) was at a distance of 2 to 3.5 cm from the end of the caudal pedicle during the first day after the "delay" procedure. This decreased glucose content recovered toward normal levels during the later part of the "delay" period. The bipedicled flaps exhibited increased hexokinase activity during the 3-week period of the "delay," and the responses of hexokinase activity and tissue glucose levels to the "delay" procedure were reciprocal in the caudal half of the flaps.

Cerium nitrate-silver sulfadiazine cream in the treatment of burns: a prospective, randomized study.

In a prospective, randomized study of 34 patients with thermal burns, treatment with the topical agent Cerium nitrate-silver sulfadiazine was compared with therapy with silver sulfadiazine alone. Despite randomization, there was a significant difference in mean burn size, being larger in the silver sulfadiazine group. There was no significant difference between the two groups in clinical course, mortality, septic deaths, or quantitative microbiology of the burn wound. It is concluded that from these preliminary data no clearcut superiority for one cream over the other can be demonstrated at this time.

High tension electrical injury of the upper extremity.

A group of 18 patients received a high tension electrical injury of the upper extremity. The over-all results were dismal. Opportunity for improvement appears to lie in a regimen of rapid and complete removal of devitalized skin and muscle, avoidance of contamination and desication and the early provision of blood supply and wound coverage with pedicled flap coverage.

Enzymes of glucose metabolism in palmar fascia and Dupuytren's contracture.

Several enzymes participating in glucose metabolism and some of the acid hydrolases were assayed in palmar fascia and Dupuytren's contracture with fluorometric microanalytical methods. The enzyme activities of glucose metabolism were lower in normal palmar fascia than in dermis. The fascia of Dupuytren's contracture exhibited a general increase in the enzyme activities of glucose catabolism. Little alteration was found in alanine aminotransferase and UDP-glucose dehydrogenase activity in the lesion. Lysosomal hydrolytic enzyme activities were increased five to ten times in Dupuytren's tissue. The dermis overlying Dupuytren's contracture exhibited an increase in the enzyme activities of glucose catabolism, but to a lesser degree than did the fascia of the lesion. The epidermis of involved palmar skin displayed normal enzyme activities.

Alveolar-arterial oxygen gradient in the burn patient.

1)A presumptive diagnosis of inhalation injury by the occurrence of a "closed-space" burn and the presence of facial burns, singed nasal vibrissae, ronchi and wheezes as clinical criteria was often in error in our study group. 2) A-a O2 gradient and change in A-a gradient in our group of patients was of value in predicting the development of pulmonary dysfunction.
