Diseases of prostate treated by local microwave hyperthermia.

A new method is described for the treatment of diseases of the prostate with local hyperthermia applied transrectally by microwaves. A series of 500 treatment sessions given to 74 patients with 14 controls, is reported. Thirty patients had advanced cancer of the prostate, 37 had benign hyperplasia and were unsuitable for surgery, and 21 had chronic prostatitis. Marked objective and subjective improvement was noted in the majority in twenty-four months of follow-up.

Combined hormone-chemotherapy for metastatic prostatic carcinoma. Eleven-year follow-up.

Thirty-six patients with histology-proved Stage D2 carcinoma of the prostate were treated with a combination of bilateral orchiectomy, diethylstilbestrol 3 mg/day, and chemotherapy (5-fluorouracil and cyclophosphamide) soon after diagnosis was established. The combined therapy was well tolerated by the patients, and complications were not severe and of a transient nature. The majority of patients showed a subjective and objective improvement: 75 per cent of patients had relief of bone pain, and 80 per cent reported relief in urinary symptoms. There was regression or stabilization of the primary tumor in 82.2 per cent. Disappearance or stabilization of osteoclastic lesions on bone scans was noted in 55.5 per cent of patients. The cumulative survival rate at eleven years is 55.5 per cent.

Leiomyosarcoma of kidney.

The case of a seventy-one-year-old woman with leiomyosarcoma of the kidney capsule is presented. Despite lack of invasion into surrounding tissues and few mitoses, the tumor recurred thirteen months after nephrectomy. A full subjective response and an almost complete objective regression lasting eight months were achieved using a short course of combination chemotherapy. The literature on leiomyosarcoma of the kidney is reviewed.

Decreased semen quality in a male infected with malaria.

A 33-year-old male of proven fertility suffered six attacks of malaria while resident in an African country. For this he received anti-malarial drugs. Semen analysis performed after the fourth attack, and repeated during the following 2 years after his return to Israel, revealed severe oligozoospermia, necrozoospermia and occasionally even azoospermia. Immunological examination of the patient revealed an inverse ratio of T-cell subsets and mast cell degranulation in response to palludrin. Twenty-five months after the last attack of malaria a significant improvement was found in semen quality and there was an increase in the ratio of T-helper to T-suppressor cytotoxic cells. These observations indicate that although malaria and its treatment may affect spermatogenesis, recovery may be expected eventually.

Further studies of a transplantable rat pheochromocytoma.

Additional histopathological and immunocytological studies were completed on the serially transplanted Warren rat pheochromocytoma. Special efforts were made to characterize features of the primary tumor common to pheochromocytomas, as well as features of the primary tumor commonly found in neuroblastoma. In summary this study found evidence for a composite primary tumor exhibiting a dual differentiative expression of both pheochromocytoma and neuroblastoma. We feel this reflects a possibility that this primary tumor arose from a common progenitor cell in the neural crest.

Changes in the sexual behavior of couples after prostatectomy. A prospective study.

Sexual behavior after prostatectomy was observed in 90 couples, with a follow-up of 3-6 years. Retropubic prostatectomy was performed on 53 patients (58.7%), transvesical prostatectomy on 12 patients (13.7%), and transurethral prostatectomy on 25 patients (27.7%). In order to have a reliable figure of the sexual behavior after surgery, all the couples underwent personal interviews separately before and 3-6 years after surgery. The interview included 4 main subjects: erection allowing sexual intercourse; antegrade ejaculation; orgasm, and libido. The separate identical double interviews were performed in order to achieve a high reliability in answer to sexual performance, this being the main unsolved question in previous studies. The results of the interviews were assessed by a computer-processing program and statistically analyzed. Our results imply that prostatectomy has an unfavorable influence on sexual activity: 33% of the potent preoperative patients became impotent after surgery.

Experience with bacillus Calmette-Guérin in patients with superficial bladder tumors. Value of monocyte activation in intravesical bacillus Calmette-Guérin therapy.

Twenty-four patients with transitional cell carcinoma of the bladder, who had failed thiotepa therapy, were subjected to immunotherapy with intravesical bacillus Calmette-Guérin (BCG) by a modification of the Morales protocol. The results show a marked decline in the number of recurrent tumors following BCG immunotherapy. From a cumulative recurrence of 1 in 8.3 months, recurrences declined to 1 in 82 months. In parallel with the clinical follow-up, the level of the in vivo activation of peripheral blood monocytes of the treated patients, as a result of immunostimulation with BCG, was monitored by the quantitation of oxidative burst products released by the cells. The oxidative burst capacity of peripheral blood monocytes were found to undergo an increase of 80-140% in 5 of 10 patients tested. The patients with a high level of activation of peripheral blood monocytes did not have tumor recurrence while 3 of the 5 patients who did not show an increase in the oxidative burst had recurrent tumors. The possibilities for utilizing the oxidative burst test as an early indicator for distinction between patients who will respond to BCG immunotherapy and nonresponders is discussed.

Mixed carcinoma of the prostate with penile metastases and priapism.

A case report of mixed prostatic carcinoma metastasizing to the penis is presented. A review of the literature revealed only 55 cases of prostatic carcinoma metastasizing to the penis report to date. To the best of our knowledge, no such unusual case has been reported before.

Active specific immunotherapy of renal cell carcinoma patients: a prospective randomized study of hormono-immuno-versus hormonotherapy. Preliminary report of immunological and clinical aspects.

Early results of a prospective, randomized trial of active, specific immunotherapy adjunctive to nephrectomy in all stages of RCC are presented. Forty-three patients with median followup of 30 m, who were randomly allocated to either immuno-hormonotherapy arm (IMT), or hormonotherapy alone (HT), are evaluated in terms of progression-free interval (PFI) and overall survival by life table method. Immunotherapy consisted of autologous irradiated tumor cells (AITC), admixed with bacillus Calmette-Guérin (Glaxo) administered by the intradermal and endolymphatic route. Clinical results of this study show only a trend for advantage of the experimental (IMT) arm over the control (HT) arm, this trend did not reach statistical significance level: prolongation of disease free period in stages I-III with localized disease (p less than 0.1) and prolongation of survival in patients with metastatic disease (p less than 0.07). A correlation was established between induction of cutaneous delayed hypersensitivity (DTH) to AITC and prolonged PFI and survival: patients with positive DTH had a significantly better course of disease than those who could not be converted to positivity after repeated immunizations. Positive in vitro leukocyte migration inhibition against autologous tumor preparations correlates well with positive in vivo cutaneous DTH. Some immunological aspects of active immunization with autologous tumor cells are discussed.

The value of Rowatinex in the treatment of ureterolithiasis.

A randomized, double-blind study was carried out on forty patients with acute renal colic in order to evaluate the efficacy of Rowatinex in the treatment of patients with ureteric stones. Patients were randomized to Rowatinex and placebo tablets. In the Rowatinex-treated patients there was a significantly higher rate of expulsion of stones which were greater than or equal to 3 mm in diameter when compared with the placebo group (61% vs. 28%, respectively). A higher success rate (spontaneous stone expulsion and/or disappearance of dilatation of the collecting system on IVU) was noted in the Rowatinex group when compared to the placebo group (78% vs. 52%, respectively), although this was not statistically significant. No significant side effects were noted during the treatment with Rowatinex.

Histopathological observations in the canine prostate treated by local microwave hyperthermia.

A large series of repeated experiments were performed applying localized microwave hyperthermia to the prostate in dogs using a new water-cooled skirt-type antenna [1], operating at 915 MHz, as part of a new hyperthermia apparatus being developed for the treatment of the prostate in humans. The prostate gland of 20 male dogs was heated repeatedly under general anesthesia, at temperatures between 40 degrees C and 47 degrees C, and for different lengths of time up to 10 h. The prostate and other tissues were evaluated histopathologically following treatments. Invariably, all treatments by hyperthermia of the prostate caused a mononuclear inflammatory infiltration in the interstitium and polymorphonuclear infiltration in the glandular elements. Permanent tissue damage was found to be time-and temperature-dependent. Heating at 42.5 degrees C (+/- 0.5 degrees 5C) for up to 1.5 h was found to be harmless and could be safely repeated with our equipment. This study was part of a preclinical evaluation of a new antenna and apparatus.

A comparative evaluation of lymphoscintigraphy versus lymphangiography and computerized tomography scanning in diagnosis of lymph node metastases in advanced bladder cancer.

The potential of lymphoscintigraphy to detect lymph node metastases compared to lymphangiography and computerized tomography scanning was evaluated in 26 patients who underwent radical cystectomy for invasive bladder cancer. Four-view images of the abdominoperineal area were taken 90 to 120 minutes after 99mtechnetium-rhenium sulfide was injected into 2 interdigital spaces in each foot. Results of lymphoscintigraphy interpretation correlated with surgical and histological findings: a correct diagnosis was made in 61.5 per cent of the patients, while 23.1 per cent had false positive and 15.4 per cent had false negative results. Although computerized tomography was the most accurate method to detect lymph node metastases (correct diagnosis in 73.1 per cent of the patients) no significant difference was found among the 3 diagnostic methods. False positive interpretation of lymphoscintigraphy was twice as common as that of the radiological studies (23 versus 11.5 per cent). The possibilities that may cause image variation interpreted as a false positive result are discussed. Because lymphoscintigraphy is an easier and less time-consuming study than lymphangiography, the former method is suggested to be an additional and sometimes (for example if short-term followup studies are required) preferred modality to evaluate the extent of lymph node involvement in cases of invasive bladder cancer.
