Results and relevance of urodynamic studies in the management of urinary incontinence in women over eighty.

Urinary incontinence among the elderly is a major problem that is often neglected. The results of urodynamic studies in a group of 32 women greater than 80 years old with incontinence revealed that detrusor instability was the primary cause of incontinence in 23 (72%), while 9 (28%) patients demonstrated signs of sphincter incompetence. Urodynamic study results were normal in four patients. Medical treatment on a self-regulated dose and bladder training were effective in controlling incontinence in 25 (78%) of the study group. It is suggested that, although detrusor instability is the major cause of incontinence in elderly women, urodynamic studies should be performed for accurate diagnosis to ensure optimal therapeutic results.

Page kidney phenomenon in kidney allograft following peritransplant lymphocele.

A case of kidney allograft dysfunction in a recipient with a prior lymphocele is described. We attribute it to a Page kidney phenomenon caused by a constrictive pericapsular fibrosis. Surgical exploration and excision of the fibrotic tissue were followed by the recovery of renal transplant function. To our knowledge, only 1 case of Page kidney in renal allograft due to peritransplant hematoma has been described in the literature.

Value of transrectal ultrasonography for diagnosis and treatment of prostatic abscess.

A new diagnostic and therapeutic application of transrectal ultrasonography (TR-US) is described. TR-US demonstrated one or more well-defined hypoechogenic areas in the prostate gland of 6 patients who presented with clinical signs of urinary tract infection and had only partial response to antibiotic therapy. The presence of a collection of pus in the prostate was confirmed in 4 of 6 patients by transperineal aspiration guided by TR-US, and by prostatic massage or transurethral prostatectomy in the other 2 patients. Ultrasonography also confirmed the resolution of abscesses in 5 patients.

Incidental small renal tumors accompanying clinically overt renal cell carcinoma.

We searched 66 kidneys with renal cell carcinoma for subcapsular or intraparenchymal small nodules in the apparently normal-appearing portion of the kidney. Differentiation between adenoma and carcinoma was done according to histological characteristics. Of the 66 kidneys 20 (30 per cent) contained a total of 58 small nodules ranging from 1 to 15 mm. in diameter. In 9 kidneys the lesions were consistent histologically with carcinoma, in 7 with adenoma and in 4 with carcinoma plus adenoma. Thus, 13 of the 66 kidneys (19.7 per cent) contained small carcinoma. In view of the high incidence of small carcinoma accompanying clinically overt renal cell carcinoma, we suggest that the indications for partial nephrectomy in the management of renal cell carcinoma should be reevaluated.

Extracorporeal shock wave lithotripsy using a new, compact and portable unit.

We report on the use of a new compact, inexpensive, portable shock wave lithotriptor. To date 40 treatments on 37 patients with different calculi in the upper urinary tract have been performed at our institution with excellent results and no complications. More patients currently are being treated with this machine at additional centers with equally good results.

A computer-based program for the follow-up of prostatic cancer patients.

A computer-based record program for the follow-up of prostatic cancer patients is described and its practical applications discussed. The system enables the user to record personal data, medical history follow-up (including laboratory and imaging tests), treatment and the response to it. The program can generate reports as a summary letter to the referring physician, follow-up notes on a specific date, laboratory test results on a chronological base, etc. It has the capability to analyse statistically the medical database accumulated with the aid of SPSS (Statistical Package for Social Sciences). The program, which is designed for a micro-computer, is conducted on the IBM Personal Computer. By providing instant access to all comprehensive information, the system is a valuable tool for the correct staging of disease and for systematic follow-up; most of all, it is a "real-time instrument" for the analysis of medical results.

Bladder diverticula in elderly females with urgency, dysuria and incontinence.

Bladder diverticula are usually encountered in the male. Their occurrence in women is scarcely appreciated. The association of bladder diverticula with urinary tract infection, dysuria, urgency and incontinence is reported in a series of 11 elderly female patients. The etiology of this finding is discussed based on urodynamic evaluation of these patients.

Sequential study of macrophage migration inhibition factor in renal cell carcinoma patients.

Preliminary investigation of cell-mediated immunity (CMI) in 49 patients harboring renal cell carcinoma (RCC) by the migration inhibition factor (MIF) tests as compared to 47 patients with simple renal cysts or normal kidneys established the specificity (85%) and reproducibility of this test. A retrospective analysis of the results of the MIF test was conducted in 35 patients followed for a minimum 20-month period. A marked difference was observed between the group of patients with localized disease who remained disease free and those with metastatic lesions and disease progression. The first group sustained their positive response of the MIF tests for a longer time during the study period, while the latter had a decrease of the MIF test response shortly after surgery. This difference may indicate that immune response as reflected in the positive MIF test has a role in RCC patients' defense mechanism.

Renal and bladder function recovery after prostatectomy in patients with a chronic residual urine of above 1,000 ml.

Among 750 patients who underwent surgery because of benign prostatic hyperplasia, 26 (3.5%) were found, before surgery, to have a huge residual urine of over 1,000 ml without classic prostatic symptoms. Dilatation of the upper urinary tract was found in 22 (84.6%) of these 26 patients. After surgery, almost two thirds of the patients remained with the anatomical damage (dilatation) of the upper tract. Before surgery all the patients had elevated serum creatinine. After release of the obstruction 50% of the patients were left with irreversible renal damage (as determined by serum creatinine). Although anatomical changes of the urinary bladder remained in all the patients, a satisfactory clinical recovery with a fairly good detrusor contraction and almost no residual urine was found after surgical removal of the obstruction. Awareness of the entity described here followed by early treatment can prevent progressive renal failure and detrusor decompensation.

Safety precautions in the management of patients with pacemakers when electrocautery operations are performed.

The hazards related to ten patients who underwent 11 urologic surgical procedures during a 36 month period were taken into consideration. Electrocautery was used in all instances and there were no complications. All of the pacemakers were preset for ventricular pacing and sensing inhibited or multiprogrammable and were reprogrammed during the operation to ventricular pacing fixed rate mode or magnet mode, with very satisfactory outcome. To avoid life-threatening situations arising from pacemaker inhibition or ventricular fibrillation during cautery for transuretheral resection, three basic factors were considered: 1, appropriate type of pacemaker; 2, the proper grounding of all medical electronic equipment used during the procedure, and 3, the directing of the complete flow of the electrocautery circuits substantially below the level of the pacemaker unit.

Pregnancy and Cushing's syndrome: example of endocrine interaction.

Pregnancy rarely occurs in women with Cushing's syndrome, and when it does, fetal mortality and morbidity are very high. We describe a 30-year-old woman who was found to have severe Cushing's syndrome in the 22nd week of her first pregnancy, after a year of unsuccessful attempts to conceive. The patient had the majority of the symptoms and signs characteristic of the syndrome. Laboratory examinations revealed hypokalemia of 2.7 mEq/l, serum cortisol 39.5 micrograms/dl without diurnal variation, free urinary cortisol 1,850 to 3,500 micrograms/24 h, 17-hydroxycorticosteroids (OHCS) 52.5 mg/24 h, 17-ketosteroids (KS) 12 mg/24 h, and ACTH 29 pg/ml. No suppression was observed upon dexamethasone administration (2 and 8 mg). Ultrasound examination of the adrenal glands revealed a left adrenal tumor with a diameter of 4.2 cm. An adrenocortical adenoma was successfully excised in the 24th week of pregnancy. During the 37th week of pregnancy, she delivered a normal baby girl. Postoperatively, the patient was put on maintenance therapy. One year after delivery, mother and child are in perfect health.

Pharmacologic, nonhormonal treatment of impotence. Evaluation of associated factors.

A group of impotent patients previously treated with Strychiomel was evaluated. Strychiomel is a compound of three drugs, yohimbine, thioridazine (Mellaril), and strychnine sulfate, each of which has a specific effect. The results suggest that the compound drug may be beneficial in the majority of patients (66.7%) when impotence occurs without hormonal or severe vascular or psychologic problems. Other factors such as age or presence of other simultaneous diseases do not appear to be significant when treatment with this multiaction drug (Strychiomel) is utilized.

Malignant melanomas of female urethra.

Primary melanoma of the female urethra is extremely rare. Treatment by surgery, radiation, and chemotherapy has been uniformly ineffective. We herein report on 2 patients with primary malignant melanoma of the urethra treated with immunotherapy and chemotherapy. The first patient died four years after the initial diagnosis, and the second died after two years. Chemo-immunotherapy may be considered as an additional, palliative form of treatment in the management of primary melanoma of the female urethra, but cystourethrectomy should be the initial form of treatment.
