Bladder carcinoma arising from regenerated urothelium over lyophilized dura patch.

Lyophilized human dura was used for augmentation cystoplasty in a woman who had a scarred bladder because of multiple resections and fulgurations of superficial bladder tumors. Thirty months later, two sessile tumors developed from the urothelium regenerated over the dura patch, together with infiltrating recurrences elsewhere in the bladder, and a radical cystectomy was performed. This case supports the concept that implantation of tumor cells is a factor in the recurrence of superficial bladder carcinoma.

Altered 1-14C arachidonic acid metabolism in arterial wall from patients with renal cell carcinoma.

The metabolism of 1-14C arachidonic acid (AA) by arterial wall in patients with renal cell carcinoma and in control patients undergoing nephrectomy was investigated by a high pressure liquid chromatography (HPLC) system. No differences in 1-14C AA uptake and in the total amount of metabolites were found between the two groups, whereas the amounts of cyclooxygenase and lipoxygenase pathway (COP and LOP) metabolites produced by patients with renal cell carcinoma were significantly lower and, respectively, higher than those produced by the control group. The COP/LOP ratio was 7.2 +/- 5.5 in the control group in comparison to 1.9 +/- 0.5 in renal cell carcinoma patients. The decrease in COP metabolites was due to a markedly reduced synthesis of prostacyclin (PGI2), with no changes in thromboxane B2 (TxB2), prostaglandin F2 alpha (PGF2 alpha) and prostaglandin E2 (PGE2) production. The changes in PGI2 and 12-hydroxy-eicosatetraenoic acid (12-HETE) (metabolite of LOP) vascular production were not related to tumor dimension. The decrease in PGI2 synthesis may represent a factor favoring metastasis and thrombosis in neoplastic patients.

Presence of activated T-cells with a T8+ M1+ Leu 7+ surface phenotype in invaded lymph nodes from patients with solid tumors.

The lymphocyte surface phenotype of lymph nodes from patients with larynx or urinary bladder carcinoma was investigated by using a panel of monoclonal antibodies. The phenotype pattern of lymphocytes from lymph nodes invaded by malignant cells (as assessed by histopathology) was different from that of the cells from noninvaded or normal control nodes. Although the proportion of natural killer cells or macrophages was similar in the 3 groups of lymph nodes, invaded lymph nodes contained a higher proportion of T-cells and a lower B-cell percentage. Furthermore, cells from invaded nodes comprised 15-20% of T3+ T8+ cells that coexpressed the M1 marker and, to some extent, also the Leu 7 marker. A large proportion of cells with multiple markers were activated, as shown by the expression of Tac and HLA-DR antigens. In 2 patients activated T8+ cells expressing also M1 and Leu 7 markers infiltrated the tumor site. The presence of these activated cells both in involved nodes and tumor mass may indicate that they originate in response to cancer.

Removal of renal cell carcinoma extending to the supradiaphragmatic vena cava with the aid of cardiopulmonary bypass.

A woman with renal cell carcinoma confined within Gerota's fascia but extending to the supradiaphragmatic vena cava was successfully treated with the aid of cardiopulmonary bypass. The approach was a subcostal chevron incision coupled with a midline sternotomy. The rationale and indications for aggressive surgery of renal cell carcinoma with massive caval extension are discussed.

Xanthogranulomatous pyelonephritis: clinical experience with 62 cases.

A review of 62 patients who underwent surgical treatment for xanthogranulomatous pyelonephritis is presented. Polar resection was performed in 3 cases with localized disease, while the remaining 59 patients were treated by nephrectomy. Preoperative diagnosis is still tentative, but the recent advances in imaging techniques and urinary cytology will provide greater accuracy in the future.

[Our experience in the surgical treatment of oncocytoma of the kidney].

Five patients, two men and three women age range 38 to 84 years, were treated for oxyphil cell adenoma of the kidney. Clinical signs were present in three cases and tumor size varied between 7 and 15 cm. Treatment was by nephrectomy in 4 cases and ablation of adenoma in the fifth patient, the tumor being located in the only functioning kidney in the latter. Close follow up surveillance for 32 months failed to detect any recurrence. Clinical and pathologic features of this neoplasm and therapeutic choice in these cases are discussed below. Although radical surgery has been employed in the past as principal therapy, more precise pre- and perioperative diagnosis should allow more frequent use of conservative surgery: partial nephrectomy or tumor excision.

Pyelovesicostomy: an alternative to ureteropelvic junction-plasty in pelvic ectopic kidneys.

Pyelovesicostomy was performed in 2 cases of hydronephrosis in pelvic kidneys secondary to ureteropelvic junction obstruction. In the first patient, who presented with a solitary kidney, the procedure was done after failure of a dismembered pyeloplasty, while in the second patient the procedure was performed electively. Both patients had sterile urines and stable renal function, although some dilatation persisted in the first case. The indications and the functional aspects of this surgical solution are discussed.

Leiomyosarcoma of the prostate.

3 cases of prostatic leiomyosarcoma, observed over a 15-year period, are reported. 2 patients underwent radical surgical treatment only and succumbed to metastatic diffusion after 13 and 29 months, respectively. After palliative surgery, a 3rd man received subsequent radiation therapy for pelvic and pulmonary recurrences, and is alive with tumor after 6 years and 8 months. The diagnostic and therapeutic aspects of this uncommon and aggressive prostatic neoplasm are discussed.

A new colpocystourethropexy for female stress urinary incontinence.

A new technique for the treatment of female stress urinary incontinence is described. It consists of colposuspension obtained by suprapubic placement of two double monofilament sutures in a spiral fashion on the lateral vaginal walls, which are fixed proximally to a Cooper's ligament and tied distally one to the other over the rectus fascia. This technique has been successfully employed in a preliminary series of 26 patients and it appears to be easy to perform and with a wide application range.

Vaginal calculi associated with vesicovaginal fistulae.

Three cases of endovaginal stones associated with vesicovaginal fistulae are described. One fistula was of congenital origin, and the other two were recurrent post-hysterectomy fistulae. Other possible causes of this unusual stone location are neurogenic bladder, ectopic ureteral orifice and vaginal outlet obstruction. The radiologic appearance and the surgical treatment of vaginal calculi are discussed.

Serum beta 2 microglobulin levels in patients with renal cell carcinoma.

Serum B2m concentrations were evaluated preoperatively in 40 patients with renal cell carcinoma and normal renal function, as assessed by serum creatinine less than 1.4 mg/dl, and compared with those of 23 age-matched controls. Mean value +/- SD was 3,088 +/- 966 ng/ml for renal cancer patients, while controls had a value of 1,800 +/- 240 ng/ml. Statistical analysis, performed by Student t test, revealed a very high degree of significance (p less than 0.0005). No statistically significant differences were found between groups of patients classified according to tumor stage and cell type. Seventy percent of renal cell carcinoma cases had preoperatively elevated serum levels of B2m.

Treatment of male urethral calculi.

We treated 14 male patients for urethral stones during a 17-year interval. The calculi were located in the posterior urethra in 7 patients and in the anterior urethra in 7. Anatomical alteration of the lower urinary tract was an important predisposing factor, since 11 patients had history of bladder and urethral surgery or disease, 2 had an associated neurogenic bladder, 6 had urethral strictures or diverticula and 3 had concomitant bladder stones. Urethroplasty was performed in 5 cases, surgical removal of the stone in 3, retrograde manipulation inside the bladder in 4 and electrohydraulic endourethral lithotripsy in 2. The latter technique appears to be particularly effective for endoscopically accessible stones.

Chylous ascites after retroperitoneal lymphadenectomy: successful management with peritoneovenous shunt.

A case of chylous ascites occurring after retroperitoneal lymphadenectomy for testicular carcinoma is described. After failure of conservative therapy, the patient was successfully treated with peritoneovenous shunt. The pathophysiological and therapeutic aspects of this rare complication are described; in the world literature only 7 cases of chylous ascites following retroperitoneal lymphadenectomy are reported.

Granulomatous orchitis: clinical and pathologic aspects.

3 cases of granulomatous orchitis are described. Treatment consisted of radical inguinal orchiectomy in 2 cases, and biopsy followed by steroid therapy in the 3. The clinical and pathologic aspects of this rare inflammatory disease of the testis are reviewed.

Clinical aspects of augmentation enterocystoplasties.

Augmentation enterocystoplasty was performed in 28 patients with tuberculous, interstitial, radiation and chronic bacterial cystitis and hypertonic neurogenic bladder. Better results were observed in tuberculous contracted bladders where the extent of bladder resection and the bowel segment used were not related to the surgical outcome. In the other forms, the ileocecal segment is preferable. In our experience patients with neurogenic bladder did not benefit from the procedure. Radiographic and urodynamic studies are valuable for an accurate follow-up; radioisotope scan is also useful in selected cases. Anatomical and surgical causes of failure are discussed. Pharmacologic treatment is useful in partial surgical failure and provides indications for further bladder neck surgery.

Inverted papilloma of the ureter.

The clinical and pathological findings of a case of inverted papilloma of the ureter are reported. Treatment consisted of partial ureteral resection, followed by end-to-end anastomosis. This uncommon lesion can be considered as a benign neoplasm, since recurrence and malignant degeneration are quite exceptional. Therefore a plea is made for conservative surgical treatment, although careful long term follow-up is advisable.
