[Computer-assisted cardiotocography analysis within the scope of continuous electronic labor monitoring with simultaneous use of noninvasive and invasive methods of registration].

In frame of a simultaneously study with noninvasive and invasive registration technic by application of 2 fetal monitors with following computer aided CTG-analysis at 22 patients we found in relation to the quantitative CTG-parameters DF (fetal condition), D (dip area), BI (bradycardia index) as well as BR (bradycardia residuelle) in part distinct differences for the range of distribution in spite of missing statistical significance (p less than 0.05) which point to insecurity in trigger signals for the noninvasive method. As exceptionally dangerous turned out to be false trigger signals in range of medium-term heart rate changes in attendance to auto-correlation US-DOPPLER-technic. The noninvasive registration of labour pains impressed forcibly for some time about longer CTG-phases to be unsuitable for the exactly judgment of uterine activity (Montevideo-unit). In conclusion the noninvasive method in supervision of labour should be considered more critical than earlier particular under forensic points of view.

[Prenatal rubella diagnosis].

Primary rubella infections during the first trimester of pregnancy are an indication for the termination, although not all of fetuses will be infected. Concerning this situation a prenatal rubella diagnostic in 55 pregnant women was carried out to demonstrate a fetal infection. Simultaneously or separately, as methods the determination of rubella IgM antibodies in fetal blood, the demonstration of viral RNA by DNA-RNA-hybridization and the cultivation of rubella virus from amniotic fluid samples were used. In 7 of 40 women the investigation of amniotic fluids showed a positive reaction of the hybridization, in 2 cases a rubella virus could be isolated. IgM rubella antibodies were found in 5 fetuses. Because of its insufficient effectivity at present the prenatal rubella diagnostic should be applied only in special situations, e.g. in cases of a wanted child.

Expression of oncogenes in human breast cancer specimens.

More than 60 breast cancer specimens were screened for their expression status of 25 different proto-oncogenes. The screening method is based on in vitro synthesis of a radioactive cDNA copied from the total cellular RNA of tumor tissue. This cDNA is hybridized to cloned oncogene probes which are immobilized to a GeneScreen membrane. Frequently multiple oncogenes were found expressed although expression levels were rather moderate. 25-30% of the analyzed tumors showed significant expression of either erbB, src, raf1, lck or H-ras. Although neu expression--an oncogene believed to be particular relevant as prognostic parameter for mamma carcinoma--was screened for most of the tumors with a heterologous gene probe, expression signals could be detected in about 20% cases. The only notable correlation with classical clinical parameters such as tumor size and proliferation stage, hormone receptor status and different DNA indices was the observation that tumors lacking the progesterone receptor frequently express multiple oncogenes. Advantages and limitations of the cDNA/dot-blot screening for oncogene expression are discussed.

Haemodynamics of the postphlebitic syndrome.

The venous function has been assessed after deep vein thrombosis (DVT) by Doppler, strain gauge plethysmography (55 patients) and exercise plethysmography (10 patients) for a mean period of 63 weeks. Venous volume and venous outflow remain significantly lower throughout the study, whatever the site of thrombosis and the initial therapy (Heparin, local or general Urokinase). There are no significant correlations between clinical and functional parameters except for patients with proximal obstruction and popliteal valvular incompetence. Exercise plethysmography evaluates the importance of the calf pump in the postphlebitic syndrome. Static plethysmographic measurements prove to be unreliable for the long term prognosis whereas associated dynamic tests should be a better way to assess the haemodynamic changes after DVT and to control the efficiency of the prevention of the post-phlebitic syndrome.

Decreased gastric prostaglandin E2 synthesis in patients with gastric ulcers and in smokers.

BACKGROUND/AIMS: Prostaglandin inhibits gastric acid secretion and exerts protective action on the mucosa of the stomach. We studied the synthesis of prostaglandin E2 (PGE2) to determine the role of PGE2 in peptic ulcer disease. MATERIALS AND METHODS: Mucosal biopsies from 22 persons were obtained and incubated. PGE2 was then determined by radioimmunoassay. RESULTS: Compared to healthy subjects, patients with gastric ulcers or gastritis show diminished accumulation of PGE2 in the incubation medium. Other factors like age, gender, and alcohol consumption were also investigated, however, they do not influence endogenous prostaglandin production. In contrast, PGE2 synthesis was found to be decreased in smokers. CONCLUSIONS: Our data indicates a deficiency in endogenous PGE2 synthesis as one cause of gastric ulcerations and offer an explanation of the higher incidence of gastric ulcer disease in subjects with nicotine abuse.

Three extremely thermostable proteins from Sulfolobus and a reappraisal of the 'traffic rules'.

Three cytosolic enzymes from the extremely thermoacidophilic archaeon Sulfolobus acidocaldarius (DSM 639) have been investigated: adenylate kinase, pyrophosphatase and superoxide dismutase. The latter was isolated from S. acidocaldarius cells, the others were heterologically overproduced in Escherichia coli. Long-term thermostability, flexibility, catalytic activity, and thermal denaturation were investigated by biochemical and physical methods. Superoxide dismutase is hyperthermostable over several days. The other enzymes have Tm values between 87 degrees C - 98 degrees C depending on conditions and reveal long-term stability in the range of hours. On the basis of sequence alignments, core structures were defined and compared to mesophilic homologues selected by growth temperature of organisms from 25 degrees C to 88 degrees C. The data set confirms none of the simple sequence based 'traffic rules' previously proposed by others. Some aspects of thermostability based on molecular modeling studies are discussed which remain to be proved by the 3D structures. All three enzymes could be crystallized.

Alteration of prostaglandin E2 and leukotriene B4 synthesis in chronic inflammatory bowel disease.

BACKGROUND: Eicosanoid mediators play an important role in the pathogenesis of chronic inflammatory bowel disease. MATERIALS AND METHODS: The synthesis of prostaglandin E2 and leukotriene B4 in biopsied colonic specimens from patients with inflammatory bowel disease was compared with that from healthy controls. In contrast to surgical resection, biopsy by colonoscopy enabled patients with milder disease to be investigated. RESULTS: In subjects with Crohn's disease or ulcerative colitis, the synthesis of PGE2 was significantly (p < 0.05) increased, whereas synthesis of LTB4 remained unaltered. In addition, no differences in PGE2 and LTB4 production were found in different age groups or sex. CONCLUSION: We conclude that prostaglandin E2, compared to leukotriene B4, is the dominant eicosanoid in moderate inflammatory bowel disease.
