[Etiological study of low fertility in eastern Gaboon. II. Results according to the geographical distribution and the ethnic group (author's transl)].

An epidemiological study was conducted in Haut-Ogooué and Ogooué-Lolo, and included 875 men and 673 women. The purpose of this article is to precise the geographical distribution of the factors related to low fertility, and to verify the stability of the etiological relations according to the region, the place of residence and the ethny. Among men, hydroceles and epididym lesions are more frequent in Ogooué-Lolo, in small villages, and in forest areas. Microfilariae Loa-loa and D. perstans has the same distribution. In both regions and what ever the type of residence (town or village), the epididym lesions are more frequent in non fertile men, microfilariae is linked with genital lesions and is more seldom found in men who are the fathers of more than two children. Among women, leukorrheas, pelvic pains, and adnexal masses are related to low fertility, what ever the place of residence is.

[Epidemiologic evaluation of health program in perinatology. I. Region rhône-Alpes 1972-1975 (author's transl)].

In 1971, a Health Project was established in order to organize some actions destined for the prevention of perinatal mortality and morbidity. A first examination of these actions was studied in the Rhône-Alpes Region by comparing the results of two surveys, one carried out in 1972 and the other in 1975, both on a representative sample of births. This article successively examines the evolution of morbidity and prevention. The authors note an important improvement in the quality of prevention and a slight progress in the results: perinatal morbidity declined as well as certain signs of fetal distress, while on the other hand, the term and birth weight showed no significant change. In the second part of this article the authors studied the relationships between the socio-cultural characteristics of the parents and the use of prevention measures. They found that certain social inequalities observed in 1972 had decreased while some others increased, new measures seem to be adopted sooner by the higher social classes when they were first envisioned for the interest of better protecting the lower social classes. However, the inequalities diminished for certain fundamental elements of prenatal surveillance, such as the respect for the minimum number of prenatal consultations.

Possible teratogenic effect of phenothiazines in human beings.

A prospective survey was carried out by the French National Institute of Health and Medical Research (INSERM) in 12 University hospitals in Paris and included 12,764 women; 189 gave birth to babies with unequivocal, non-chromosomally based, malformations. There was a significant excess of malformed infants when women had taken phenothiazines during the first three months after the last menstrual period. This association did not seem due to chance or to bias and could not be explained by the influence of the outcome of previous pregnancies. These data are to be added to results of recent surveys showing a significant increase of malformations in children exposed in utero to anticonvulsants, carbamates, benzodiazepines, tricyclic antidepressants, and amphetamines.

Natural history of ventricular septal defect. A study involving 790 cases.

The development of 790 untreated patients affected by ventricular septal defect (VSD) has been the object of a 25-year study. Of these patients, 72% had had at least one catheterization; 13% had several. The mean observation interval is six years, and the average age at the latest data is 19.5 years. This study covers 4717 patient-years. For the entire population, the incidence rate or aortic regurgitation is 6.3% (4.3 for 1000 patient-years) and that of bacterial endocarditis is 3.7% (2.4 for 1000 patient-years). Twenty-five patients died, 15 of them between the ages of one and 39. Of the 499 cases with several clinical examinations, 71% remained stable. In 21%, changes suggesting some level of closure developed; in 7%, infundibular stenosis began to evolve and in 1% pulmonary vascular disease began to appear or became accentuated. These different rates are studied and discussed in relation to patients' age, VSD type, and various follow-up characteristics.

Radical mastectomy versus radical mastectomy plus internal mammary dissection. Five-year results of an international cooperative study.

From 1963 to 1968, the international group collected 1580 cases of breast cancer, randomized into two therapeutic groups: radical mastectomy and extended mastectomy. The data were processed on the UNIVAC 1107 computer of the I.N.S.E.R.M. Computing Center. No significant difference was observed between the two groups in the overall five-year survival rate. However, a more detailed analysis, according to certain prognostic features, showed that extended mastectomy improved the results in one subgroup: cancers of inner or medial quadrants, axillary N+. Within this group the difference was highly significant for a smaller subgroup (190 patients) including only tumors T1 and T2. In conclusion, there is no indication for extended mastectomy in any cancers of the outer quadrants or in those of the inner or medial quadrants without axillary involvement. A limited indication for extended mastectomy may be provisionally retained for T1 and T2 cancers of the inner or medial quadrants with axillary involvement.

[Feto-infantile births and deaths. Statistical data].

After a period of stability between 1968 and 1971 the birth-rate was lowered slightly in 1972-1973. The first projections worked out by INSEE lead us to predict that there will be a considerable drop in the number of births in 1974 (800, 000 as against 855,000 in 1973). Perinatal mortality has dropped rapidly since 1969. The rate for France is now better than that for Scotland or the Federal German Republic and is catching up on England. It is however still far behind the rates for the Scandinavian countries and Holland and Belgium.

[Results of a survey on breast feeding in maternal-child welfare centers of the 14th arrondissement of Paris].

An inquiry was held in the 14th ward of Paris, concerning 1 352 infants observed in centres of maternal care, and studying the conditions of breast-feeding. At birth 56% of the metropolitan French children were breast-fed. The frequency was higher in the educated classes, in primiparal and in women who had attended pre-natal relaaxation classes. The effects of these features can be added up. Women who gave birth in a public hospital and women with healthy newborn children were also more likely to be breast-fed. The duration of breast-feeding was not related to previous features. Breast-feeding was more frequent and more prolonged in immigrants than in the French metropolitan population.

A comparative study of the efficiency of hydroxyprogesterone caproate and of chlormadinone acetate in the prevention of premature labor.

The efficacy of caproate of hydroxyprogesterone and acetate of chlormadinone in preventing premature labore was compared in a controlled trial. The survey was based on 211 pregnant women with a high risk of premature delivery discovered during clinical examination. There are no significant differences between the two groups in either length of gestation, delay between the beginning of treatment and delivery or other parameters related to prematurity. The absence of evidence suggesting any significant difference between the two treatments can have three possible causes (which are discussed): the methodology, the inefficacy of the two products or the equivalent efficacy of the two products.
