The authors report their experience about transhepatic cholangiography with skinny needle and external-internal drainage. Results seem to be satisfactory, regarding low frequency of complications, diagnostic reliability of cholangiography and therapeutic palliative efficacy drainage. It is possible to conclude that drainage, as palliative technique in neoplastic patients, is superior or almost equivalent to surgical palliative derivation.
The hepatoportal circulatory changes which occur after porto-systemic shunts have been evaluated in 55 cirrhotic patients studied by post-operative arterioportography. In every one of these cases the shunt was patent. After side to side portocaval shunt (28 patients) the arterioportography shows the complete drainage of the splanchnic blood into the inferior vena cava. A reversed flow was observed in 20 patients. After conventional spleno renal shunt (eight patients) a maintained hepatopetal flow was present only in early angiographic controls, while in later controls all splanchnic blood flow was towards the renal vein, with evidence of reversed portal flow. Even after mesocaval shunt (ten patients) the splanchnic flow was hepatofugal. Instead, after distal splenorenal Warren shunt (two cases) the hepatopetal portal flow seems to be unaffected as evidence by angiography 15 days post-operatively. An increased of the hepatic artery diameter and its intrahepatic branches was observed in 20 out of 27 controls. This hepatic artery "hepertrophy" is related in increased hepatic artery blood flow which seems to be, according to Burchell [5], an important factor in the reestablishment of the liver circulation after portocaval shunt.

BACKGROUND: The clonal origin of myeloproliferative disorders has been clearly demonstrated and it is known that reactive thrombocytosis occurs as a non specific response to various inflammatory or neoplastic conditions. Only a few papers have discussed the topic of myeloproliferative diseases in blood donors. MATERIALS AND METHODS: We report 8 cases of myeloproliferative diseases (3 polycythemia vera and 5 essential thrombocythemia) in blood donors out of a total of 44 myeloproliferative disorders diagnosed in our Department during the last 5 years on the basis of the criteria established by the Polycythemia Vera Study Group criteria. As controls we considered 61 patients with reactive thrombocytosis referred to our Department in the same period of time. The estimated odds ratio was calculated according to standard methods. RESULTS: The prevalence of blood donors with myeloproliferative disorders was 18.1%, while that of donors with reactive thrombocytosis was 3.2%. The estimated odds ratio was 6.56 with a 95% confidence interval between 1.07 and 17.3. No other single factor except blood donations was frequent in the past history of these patients. CONCLUSIONS: Our data seem to indicate that both thrombocytosis and erythrocytosis resembled primary forms in these subjects; however, none of them suffered serious thrombotic and/or hemorrhagic symptoms. Our study indicates the importance of paying due attention to the blood cell counts of blood donors.
The bacterial flora was studied in the bronchial, pharyngeal secretions and sputum of 80 children with Cystic Fibrosis (CF) and of 21 children with other chronic lung diseases, treated at the Pediatric Clinic (Respiratory Unit), University of Genoa. Staphylococcus aureus was found in 52% of patients, Pseudomonas aeruginosa in 48%, Proteus in 5%. According to the age, the patients have been distinguished into four groups; in the 6-10 and 11-15 year-old group Staphylococcus aureus was the commonest pathogen isolated, while Pseudomonas aeruginosa was found much more commonly in the 16-20 year-old group. Details are given of antibiotic-sensitivity for single strains grown. Intensive care programs, based on specific antibiotic-sensitivity, for in-patients and out-patients, are reported: positive results in treatment courses with Gentamicin, Cephalosporins and Tobramycin are presented.
Generally cat-scratch disease is a benign inflammatory adenopathy. The Authors describe an atypical form of this disease, characterized by persistent fever and splenic granulomatosis requiring a diagnostic and therapeutic prolonged effort. They point out the important role of new immuno-fluorescent techniques to exactly identify the bacterium--Bartonella henselae--causing cat-scratch disease and suggest to include cat-scratch disease among the causes of unknown origin fever.

We report the results of a multicenter study on the diagnosis and interventional therapy of hepatocellular carcinoma (HCC). The first aim--diagnosis--was to evaluate the sensitivity of 4 imaging techniques, namely ultrasonography (US), Computed Tomography (CT), digital arteriography (DSA) and Lipiodol CT (LCT), in HCC detection. The accuracy of these techniques was also investigated in tumor staging, which is important for treatment planning. Two hundred ninety patients underwent this imaging protocol. The patients were classified by tumor spread into three groups, namely group 1 (single HCCs < 5 cm), group 2 (multifocal HCCs with max. 3 nodules or tumor volume < 80 cc), group 3 (multifocal HCCs with more than 3 nodules and/or tumor volume > 80 cc). US and CT diagnosed more cases as group 1 and fewer cases as group 3 than DSA and LCT; the latter two techniques gave a similar classification. With LCT as the gold standard, US and CT understaged 27.9% and 26.5% of cases, respectively. Even though LCT is known to have 53% sensitivity, it is currently the most sensitive preoperative investigation and therefore the best tool for treatment planning. In surgical patients, however, intraoperative US, with its nearly 100% sensitivity, is suggested. The second aim--treatment--consisted in assessing the therapeutic efficacy of intraarterial chemoembolization (CEAT) versus percutaneous ethanol injection (PEI) in non advanced HCC and of CEAT versus no treatment (NT) in advanced HCC. Treatment efficacy was evaluated with the following randomized protocols: PEI versus CEAT in group 1, PEI versus CEAT in group 2 and CEAT versus NT in group 3. The data were analyzed relative to 215 patients for 6 to 30 months. The Kaplan-Meier method was used to calculate survival rates, which were, at 24 and 30 months, 72% and 72% for PEI and 72% and 52% for CEAT in group 1, 52% and 28% for PEI and 70% and 50% for CEAT in group 2 and finally 30% and 20% for NT and 45% and 30% for CEAT in group 3. In group 1, PEI appeared markedly superior to CEAT. In group 2, the difference between PEI and CEAT was not statistically significant; the results in this group indicate that CEAT should be considered when three nodules are present because of PEI invasiveness in these cases. In group 3, CEAT results were definitely better in the first two years, but there was no difference with NT patients at the end of the third year. Therefore, CEAT is indicated in advanced HCC because it improves the survival rate in the first 24 months. After this period, the survival time is not modified by treatment.

A correlation has been found between the concentration of isoleucine tRNA in reticulocytes and isoleucine content of rabbit and sheep hemoglobins: in rabbit reticulocytes, which synthesize a hemoglobin containing 8 residues of isoleucine per molecule, the isoleucine accepting activity is more than twice as high as in sheep reticulocytes, which produce a hemoglobin devoid of isoleucine.
