[Exclusive parenteral nutrition in children. Influence of continuous or cyclic intake on the utilization of food].

A continuous measurement of the energetic consumption and a nutritional evaluation were performed over 24 hours in two comparable groups of children under cyclic or continuous total parenteral nutrition (TPN). Over the 24 hours, energetic consumptions and nitrogenous retentions were equal in both groups. In cases submitted to continuous TPN, a similar utilization of nutriments was observed during the day and night periods, with global predominance of carbohydrate oxidation and of lipogenesis. In cases submitted to cyclic TPN, night and day utilization of nutriments was significantly different, with important utilization of glycogen and lipid stocks during the day. Thus, cyclic TPN allows a more important mobilization of the energetic stocks and physical activity during the day than continuous TPN and might change the quality of the weight gain and avoid an unnecessary storage of lipid deposits.

Sinus pauses in early human malnutrition during waking and sleeping.

7 malnourished infants were studied before and after nutritional rehabilitation through long term polygraphic recordings during waking and sleeping. High amounts of sinus pauses (R-R interval comprised between 20 and 90% of the preceding interval) have been found in malnourished infants, mainly during quiet sleep. After the nutritional rehabilitation there is a dramatic reduction. The high amount of sinus pauses in infants suffering from severe malnutrition may reflect a disturbance of neurovegetative regulation, which is enhanced by sleep.

Intestinal malignant lymphoma in a child with familial celiac disease.

A case of intestinal malignant lymphoma occurring in a 10-year-old North African girl with celiac disease is reported. Celiac disease was diagnosed at age 4. A gluten-free diet was started and was maintained for the next 6 years. Seven months after the patient returned to a normal diet, clinical symptoms and subtotal villous atrophy relapsed. Within the next month an abdominal malignant lymphoma was diagnosed. After chemotherapy the tumor disappeared but recurred 6 months later, involving the colon and the liver. Death occurred shortly thereafter. A brother, two years older, also with proven celiac disease, is presently on a strict gluten-free diet. Both children shared the same histocompatibility antigens (HLA A2-B17/A29-B12, identity at the D locus). Whether or not relapse of celiac disease played a role in the appearance of the tumor remains highly speculative; however, the case of our patient suggests that familial involvement and, in some instances, ethnic origin may constitute risk factors predisposing to malignancy.

[Hyperammonemia during total parenteral nutrition in children].

Twenty-one children presenting with severe malnutrition were fed with total parenteral nutrition using 2 different aminoacids solutes containing 19% and 42% branched chain aminoacids respectively. With the 42% branched chain aminoacids solute a dysregulation of ammonia metabolism and changes in protein synthesis were observed. Several factors probably accounted for these changes. An excessive intake of branched chain aminoacids in children with malnutrition, under parenteral nutrition, is therefore iatrogenic and inappropriate.

[Malignant histiocytosis of the intestine. An anatomo-clinical case report].

Clinico-pathological case report of a 11 year-old girl who successively presented persistent abdominal pain, pneumoperitoneum, repeated intestinal hemorrhages, superficial intestinal ulcerations, first localized on the jejunum and later extended to the all gut. After a course of 13 months, the child died from diffuse and repeated hemorrhages. The morphologic studies revealed a diffuse and homogeneous cellular infiltrate made of large round "lymphomatous" cells scattered within the lamina propria and submucosa. Initially this superficial cellular infiltrate was overshadowed by accompanying inflammatory cells and was not recognized as a tumoral process. Later on the dissemination of abnormal cells to the entire ileon, mesenteric lymph nodes, spleen and liver and the cellular appearance confirmed its neoplastic nature and allowed to consider this process as a malignant histiocytosis of the intestine as described by Isaacson and Wright. This case seems to be the first case reported in childhood.

[Ileo-cecal tuberculosis. A difficult diagnosis].

An ileocaecal tuberculosis suspected in an Algerian girl was confirmed only by operative findings and by the presence of caseum at histological examination. Mycobacterium tuberculosis could not be cultured from the lesions. Diagnostic problems between tuberculous enteritis and Crohn's disease are discussed.

Developmental trend of quiet sleep is altered by early human malnutrition and recovered by nutritional rehabilitation.

Thirteen infants malnourished from birth have been recorded with polygraphic techniques continuously for 24 h. The developmental trend of Quiet Sleep was found to be altered; infants older than 4 months had a diminution of Quiet Sleep with shorter phases. Ambiguous Sleep was increased. These findings reflect the effects of malnutrition on central nervous system development and could have implications for growth and for anabolic processes. Nutritional rehabilitation performed through an intracave catheter led to an increase of Quiet Sleep and to a decrease of Ambiguous Sleep.

Blood and urinary amino acids in children receiving total parenteral nutrition.

Plasma concentrations and urinary outputs of amino acids were estimated in nineteen patients receiving intravenous hyperalimentation to evaluate the adequacy of dosage and composition of the infusates for the maintenance of normal blood concentrations of essential amino acids. The use of high concentrations of branched chain amino acids seems to be appropriate for valine and isoleucine but not for leucine. The high concentration of cysteine in the infusates used induces a very high urinary excretion of cysteine and cystine and are ineffective to bring the decreased plasma cystine levels back to normal.

Sleep related growth hormone and prolactin secretion in children during constant rate enteral nutrition.

The aim of this study was to investigate the nocturnal secretion of growth hormone and prolactin in a particular model where nutrients are delivered continuously. Six children with severe intestinal diseases undergoing total constant rate enteral nutrition for 1.5 to 8 months have been studied; all children had a normal nutritional status at the time of the recording. Sleep patterns were studied by the usual polygraphic methods from 10 p.m. to 8 a.m. Blood samples were taken every 20 min through an indwelling catheter for growth hormone and prolactin plasma level determination. Several growth hormone peaks were observed with a peak always secreted in connection with stage III-IV of the first cycle. This early peak was significantly higher than the following ones. Nocturnal patterns of prolactin secretion showed individual differences characterized by a series of episodic releases which consisted of a few long rises (4 patients) and several small fluctuations; no correlation was found with the sleep patterns; no increase in the level throughout the night was observed. Loss of the rhythmicity of alimentation does not alter the secretion of growth hormone during sleep.

[Hereditary pancreatitis. Two new kindreds ].

An acute pancreatitis was observed in 2 children belonging to two unrelated families in which a total of 8 kindreds also had pancreatitis. Evolution was so severe that it demanded parenteral feeding and partial pancreatectomy with Wirsung-intestinal shunting. The authors stress: the rarity of this condition (which has a dominant autosomal transmission), the lack of specificity, the severity in which pseudocysts predominate, the further occurrence of both diabetes and external pancreatic deficiency and finally, the risk of pancreatic carcinoma.

[Compensatory growth in children nourished artificially].

The features of catch-up growth were studied, with the aid of anthropometric measurements, in 59 malnourished children, under continuous enteral or parenteral nutrition. Normal weight was reached after 4 months and normal height after 10 months. Protein compartment was normalized after 4 months; so was the fat tissue compartment, for which, however, further increase occurred up to the 6th month. This evolutive profile, observed under artificial nutrition, does not differ from the catch-up observed under spontaneous oral feeding. The excess in fatty mass partially accounts for the high energetic cost, half reduced however in artificial feeding. The authors emphasize the risk of excessive energetic intake in artificial feeding and underline the importance of anthropometric monitoring of the lean body in the modulation of the intake.

Weight changes and sleep organisation in infants.

We have studied the relationship between weight changes and polygraphically recorded sleep, during a 24 hour period, of 26 infants aged between 2 and 11 months. Weight changes (in percentage within a period of 24 h) have been found to be correlated with the following sleep parameters: total amount of ambiguous sleep, percentage of time spent in cycle of the total sleep time, mean length of the cycles. We speculate that this relationship could be important when weight variations do not proceed as expected only on the basis of the clinical conditions and the caloric intake.

[Effect of constant rate enteral nutrition on gastrin secretion in children (author's transl)]].

The aim of this work was to study gastrin secretion in children receiving constant rate enteral nutrition (CREN) then avoiding intermittent stimulation induced by meals. Induced hypergastrinemia was performed in 12 children during CREN and compared with that in 8 children with intermittent oral feeding as a control group. Before stimulation by protein load there was no significant difference between the 2 groups; 20 minutes after stimulation, there was no change in the serum gastrin concentration in the CREN group, but a dramatic increase in the control group. The hypothetic role of gastrin and/or intestinal factors is advanced to explain the inhibition of gastrin secretory response during CREN. A progressive substitution from continuous to discontinuous intakes is suggested to avoid weaning complications of this nutritional technique.

Sleep patterns in infants under continuous feeding from birth.

Continuous parenteral or enteral feeding of infants with major gastrointestinal problems (prolonged diarrhoeas, inflammatory digestive illnesses, anatomical anomalies necessitating surgical intervention, etc.) brings about a prolonged modification in nutritional and environmental conditions. The consequence of eliminating the feeding rhythms of the waking-sleeping rhythm in 16 infants between 1 and 8 months old were studied. In most of the infants the duration of sleep was close to that given by Kleitman in normals. Slightly decreased in 4 of the infants, these durations never showed significant differences from normative values. No delay was observed in the maturation of sleep patterns. The percentage and mean length of periods of quiet sleep were practically identical for all infants but a number of them showed large amounts of ambiguous sleep. The fact that none of these values evolved with age should be stressed. Paradoxical sleep periodicity was similar to that of normal infants, while the quiet sleep-paradoxical sleep cycles were shorter. In conclusion, the absence of feeding rhythms appeared not to modify greatly the sleep maturation and organization.
