The present study involved 143 patients with confirmed Cushing's syndrome, including 100 with Cushing's disease, 20 with adrenal adenoma and 23 with adrenal carcinoma. Out of 120 assessable patients 68% had arterial hypertension and the majority of these (78%) presented with adrenal carcinoma. Blood pressure returned to normal in 85% of hypertensive cases after Op'DDD therapy or adrenalectomy. Excessive cortisol production is probably the main cause of hypertension, but the precise pathophysiological mechanism is controverted. Serum lipids were studied in 88 patients, 40% of whom were found to have combined hyperlipoproteinaemia, this abnormality being particularly frequent in patients with malignant adrenal tumour. Serum cholesterol levels returned to normal after adrenalectomy. However, they were further increases during Op'DDD treatment (p less than 0.01) and only became normal after withdrawal of the drug. Cerebral vascular accidents were uncommon (6%) and might have been due to cobalt irradiation of the hypothalamus-pituitary region.
Two cases of pneumoperitoneum following status epilepticus are reported. In both patients perforation of a hollow viscus was excluded by laparotomy and status epilepticus had been accompanied by haematemesis. An increase in upper digestive tract pressure resulting from high abdominal pressure during seizures might have caused leakage of air into the peritoneum through lacerations in the lower oesophageal mucosa. Awareness of this mechanism of non-surgical peritoneum would avoid unnecessary exploratory laparotomy in these seriously ill patients.

To quantify the influence of continuous positive pressure ventilation (CPPV) on hepatic plasma flow (HPF) in 8 critically ill patients submitted to controlled ventilation, we measured simultaneously HPF by the indocyanine green dye (IGD) clearance and cardiac output (CO) by the thermodilution technique, while increasing PEEP from 0--20 cm H2O. CPPV induced a significant decrease in HPF which was related to the level of PEEP. A linear correlation was documented between HPF and CO; HPF remained a constant fraction of CO during CPPV. These results suggest that the decrease in CO is the most likely mechanism of the fall in HPF during CPPV.
Acute coronary obstruction occurred in two patients during coronary angiography. In one case the obstruction was in the left main coronary artery; in the other it was close to the origin of the left anterior descending artery. In both cases acute cardiac ischaemia ensued, with electromechanical dissociation and collapse, which was not reversible by resuscitation. Rapid disobliteration of the occluded coronary artery was done with a guide-wire pushed through the obstruction via the coronary catheter. The recanalisation was completed by an intracoronary perfusion of streptokinase in one case. In both cases recovery was rapid and spectacular. The occurrence of acute ischaemia during coronary angiography should suggest accidental coronary occlusion. If a thromboembolic origin is suspected, transluminal disobliteration should be attempted. It is simple and can reverse a dangerous condition.

The acetylation state of the blocked NH2-terminal end of arginine kinase was characterized by the occurrence of 1 mol acetyl group/mol protein; a 5-residue segment corresponding to the amino-terminal portion was isolated from a pronase digest of the enzyme and its amino acid sequence determined as N-acetyl Ala-Asx-Ala-Thr. Arginine kinase is terminated at the carboxyl end by the sequence Lys-GluMetOH; this particular 3-residue sequence is repeated three times in the overall structure of the protein and is present in three CNBr fragments. One of these, a peptide of 14 amino acid residues, was identified in the course of this study and its amino acid sequence determined. Its location at the COHO-terminal end of the enzyme was recognized on the basis of investigations carried out with des-MetOH-Glu-arginine kinase, a specific proteolytic derivative. The alignment of the eight CNBr-fragments which constitute the arginine kinase molecule was established according to the sequential and compositional properties of seven unique tryptic methionyl peptides isolated from the whole protein. The alignment was confirmed by using BNPS-skatole fragments of the enzyme as another protein source.
Kluyvera is proposed as a new genus for the group of organisms formerly known as Enteric Group 8 (synonym = API group 1). Strains of Kluyvera share the properties of most members of the family Enterobacteriaceae: they are gram-negative rods, motile with peritrichous flagella, catalase positive, and oxidase negative; they grow on MacConkey agar, ferment D-glucose with the production of acid and gas, and are susceptible to many antibiotics. Strains are usually indole positive, methyl red positive, Voges-Proskauer negative, citrate positive, H2S (triple sugar iron) negative, urea negative, phenylalanine deaminase negative, lysine decarboxylase positive, arginine dihydrolase negative, and ornithine decarboxylase positive. Kluyvera strains ferment many of the sugars and polyhydroxyl alcohols used in identification. By deoxyribonucleic acid-deoxyribonucleic acid hybridization, strains of Kluyvera were divided into three groups. Kluyvera ascorbata is proposed as the type species for the genus. Most strains of K. ascorbata have been isolated from clinical specimens. K. cryocrescens is proposed as the second species. It was occasionally isolated from clinical specimens, but it was isolated more commonly from the environment. Kluyvera species group 3 was heterogeneous, but was distinct from the two named species by deoxyribonucleic acid hybridization. This group was rare, so no species name will be proposed at this time. K. ascorbata can be differentiated from K. cryocrescens by its positive ascorbate test, inability to grow at 5 degrees C in a refrigerator, and smaller zones of inhibition around carbenicillin and cephalothin disks. The test normally used for identification does not clearly differentiate these two species. Kluyvera species are probably infrequent opportunistic pathogens. The most common source is sputum, where they are probably not clinically significant. Five strains have been from blood cultures. More information is needed about the incidence and clinical significance of the genus Kluyvera.
The authors present a disposable device which may be used for skin tests studying delayed cellular hypersensitivity with 7 antigens and a glycerin control. With the aim of more objective evaluation of the results, they suggest the establishment of a score taking into account the number of positive reactions and the degree of the reactions. Finally, they describe the results of a large multicentre study involving 830 subjects considered to be healthy and divided into 4 groups according to the response to the tests.
