Glaucoma of institutionalized geriatric patients.

362 glaucomatous eyes of 213 institutionalized geriatric patients 66-100 year of age (83.9 +/- 6.9 year) were studied. Of the eyes with open angle glaucoma (77%, 279/362) 51% had capsular glaucoma (CG). Of the secondary glaucomas (9%) half were neovascular glaucomas. The visual acuity (VA) of 13 patients could not be determined. More than half of the patients (114/200) were visually handicapped (VA in the better eye less than 0.3). 55 out of 114 patients were considered blind (VA less than 0.05 in the better eye). In the glaucomatous eyes the mean VA was 0.17, binocular VA for near was 0.28. 28 out of 110 eyes had Bjerrum scotoma, 21 eyes had complete arcuate scotoma and 41 eyes had a residual field in the temporal periphery. In the present study, the causes of blindness were distributed as follows: glaucoma was the only cause in 25%; cataract was one of the causes in 66% and macular degeneration in 24%. Because glaucoma is a major cause of visual impairment in the elderly population, and it is frequently discovered by chance, attention should be paid to this condition in persons over 65 years of age.

Trabeculectomy with the use of sodium hyaluronate. One year follow-up.

The effect of sodium hyaluronate (Healon) on the post-operative course 1 week, 1 month and 1 year after trabeculectomy is reported. Forty-six eyes of 38 patients with open-angle glaucoma (16 simple, 16 capsular and 14 secondary glaucomas) were operated with the use of sodium hyaluronate. The study was prospective. For comparison, 46 eyes of 38 age-, sex- and diagnosis-matched patients with glaucoma operated during the pre-Healon period were selected. Patients in the study group were operated by the same surgeon and in the control group by different senior staff members. IOP decrease was essentially the same in both groups. No differences in post-operative pressure level existed between the different types of glaucoma. Extremely shallow or flat chamber occurred more frequently in the control series: 15 versus 9. Statistical analysis could not be applied. The trend in favour of Healon is discussed. Pressure rise greater than or equal to 30 mmHg 2-7 days after the operation was diagnosed in 8 eyes of the Healon group and 13 eyes of the control group. IOP rise was relieved by argon laser treatment of the fistula during the first week post-operatively in 6/8 eyes of the Healon group in order to facilitate aqueous drainage. Sodium hyaluronate facilitated the technical performance and prevented collapse of anterior chamber and AC haemorrhages during the operation. Sodium hyaluronate seems to prevent excessive early filtration and hypotony.(ABSTRACT TRUNCATED AT 250 WORDS)

Electroretinography in Labrador retrievers given ketamine-xylazine anesthesia.

Electrophysiologic examination of scotopic and photopic retinal function was done in 31 young healthy Labrador Retrievers and 6 Labrador Retrievers with ophthalmoscopic signs of generalized progressive retinal atrophy (PRA). Scotopic responses to 4 stimulus intensities (b-wave threshold and 0.65-, 1.60-, and 2.59-log relative units above b-wave threshold, tests 1, 2, 3, and 6, respectively) were determined, using single-flash stimulation. Alternating current recorded single-flash c waves (stimulus intensity 3.18-log relative units above b-wave threshold, test 4) and 30-Hz photopic flicker (stimulus intensity 1.92-log relative units above b-wave threshold, test 7) responses were recorded. Results of the single-flash recordings from 31 healthy dogs were analyzed statistically. The method used allowed for a separation of rod and cone function. Tests 3, 6, and 7 were most useful in determination of retinal disease, when results of the healthy young dogs were compared with results obtained from generalized PRA-affected dogs. Various doses of ketamine and xylazine in dogs with ophthalmoscopically normal fundi had no significant effects on a- and b-wave amplitudes when dogs were given single-flash stimuli 1.60-log relative units above b-wave threshold (test 3). Three 3-year-old dogs, with ophthalmoscopic signs of generalized PRA, had greatly reduced a- and b-wave amplitudes and unaltered or slightly shortened a- and b-wave latencies. One 3-year-old dog with normal-appearing fundi had responses similar to those of dogs with generalized PRA of the same age. On repeated examination 6 months later, this dog had ophthalmoscopic and angiographic signs of generalized PRA.(ABSTRACT TRUNCATED AT 250 WORDS)

Intraocular lens implantation in exfoliation syndrome and capsular glaucoma.

The intra-operative difficulties and post-operative complications after posterior chamber lens implantation are described in 30 eyes with exfoliation syndrome (including 19 eyes with capsular glaucoma (CG), 30 age- and sex-matched patients with non-exfoliative eyes comprised a comparison group. Three eyes of the control group had simple glaucoma. The highest risk rate for intra-operative difficulties and post-operative complications was found in the CG group. 8/19 eyes had intra-operative difficulties and 7/19 post-operative complications corresponding 3/11 eyes and 2/11 eyes, respectively, in the exfoliation (E) group. Eyes in the control group had a low rate of complications, 3/30 intra-operatively and 2/30 post-operatively. In 63% of the CG and E eyes the visual acuity was 0.5-1.0 1 month post-operatively compared with 70% in the control group. Exfoliation syndrome is not a contraindication for posterior chamber lens implantation. Special attention should be paid to IOL selection. Complications due to insufficient mydriasis, weak zonules and possible undiagnosed lens subluxation, should be taken into account on planning the operation.

Adverse reactions to fluorescein angiography.

Adverse reactions to ophthalmic patients during 9909 fluorescein angiographies during 9 years were registered. Nausea (4.6%) and vomiting (1.3%) were the most common untoward reactions. Allergic skin manifestations occurred in 48 patients, and 5 patients complained of shortness of breath. 56 patients (0.6%) felt dizzy during or immediately after the investigation. Nine patients complained of chest pain, three of whom developed myocardial infarction. Sixteen patients collapsed during the procedure. One healthy male, 42-year-old, collapsed after the injection of fluorescein during angiography, and electrocardiogram showed an asystole of 24 seconds. Otherwise, the electrocardiograms registered on 100 consecutive patients did not reveal any systematic changes in heart rate or rhythm during fluorescein angiography.

Trabeculectomy with the use of sodium hyaluronate. A prospective study.

The effect of sodium hyaluronate (Healon) on the post-operative course and IOP 1 week and 1 month after trabeculectomy is reported. Twenty-nine eyes of 23 patients with open-angle glaucoma (10 simple, 11 capsular and 8 secondary glaucoma) were operated with the use of sodium hyaluronate. The study was prospective. For comparison, 29 eyes of 21 age- and sex-matched patients with glaucoma, operated during the pre-Healon period were selected. Patients in the study group were operated by the same surgeon and in the control group by different senior staff members. The method for trabeculectomy is described. IOP decrease was 56 and 54% in the H- and control group. No differences in post-operative pressure level existed between the different types of glaucoma. Shallow or flat chamber occurred frequently in both groups, but had a greater incidence in the control group. Sodium hyaluronate seems to prevent early excessive filtration. In five eyes of both groups choroidal detachment was simultaneously present. The incidence of choroidal detachment was the same as the overall incidence of this complication in this clinic. Post-operative pressure rise of greater than 30 mmHg after restoration of a flat chamber was more frequent in the control series. Sodium hyaluronate is useful in trabeculectomy. It prevents collapse of the anterior chamber and hyphema during the operation, it stabilizes early post-operative pressure and prevents excessive filtration combined with shallow or flat anterior chamber. It does not prevent choroidal detachment after trabeculectomy.

Optic neuritis during lactation.

The condition called "lactation optic neuritis" has been previously considered a clinical entity of its own. Four women, who developed optic neuritis within 1-12 months while breast-feeding their infants, were investigated ophthalmologically and neurologically in order, to find specific clinical features for this condition. The course of the disorder was similar to classic optic neuritis without lactation. The clinical history and laboratory findings in three of the four patients suggested a demyelinating disorder. It is possible that the decreased immunosuppressive activity just after pregnancy induces the manifestation of an underlying demyelinating disease. The existence of "lactation optic neuritis," however, is questioned as a separate entity of its own. Lactation together with decreased immunosuppression may merely act as a provocateur in the onset of optic neuritis, which in many cases is the first clinical manifestation of incipient multiple sclerosis.

The ophthalmological findings in autosomal recessive severe juvenile arteriosclerosis.

Severe juvenile arteriosclerosis (JAS) is a rare, fatal disease with probably autosomal recessive inheritance. Precocious arterial murmurs and calcification of aorta and major arteries are associated with anaemia, growth retardation, prolonged systolic hypertension, gradual renal failure, gastrointestinal perforations, and early death. The ophthalmological findings in 5 boys and 1 girl between 7 and 17 years of age were retinal vascular tortuosity of main vessels, pre-capillary arterioles and post-capillary venules, sausage-like bulging of central main arterioles and microaneurysms of arterioles of the central fundus. Narrowing and local constriction was a typical feature of peripheral arterioles. The combination of findings documented by fluorescein angiography in 4 children was typical for severe juvenile arteriosclerosis.

Psychiatric reactions complicating intracapsular cataract surgery. A 5 years follow-up.

This follow-up study describes the 47 patients out of 1506 intracapsularly extracted cataracts (1505 patients) who presented with a psychotic reaction post-operatively. Matched non-psychotic cataract patients served as a control group. The mean age of both groups of patients was 78 years at time of surgery, female/male ratio was 39/8, and the mode of anaesthesia was local analgesia/general anaesthesia 16/31. The surgery had been performed in 1977-1979 and this follow-up examination in autumn 1983. Twenty-seven patients of the study group and 16 of the controls had died during the interval period. The difference between the groups was statistically significant. The patients of the study group were statistically significantly more dependent on nursing, and their mental performance, according to an orientation test, was more reduced, compared to the controls. Visual prognosis in the operated eye was unfavourable in the study group, versus the controls, mainly because of visual deprivation due to mental debilitation. Pre-operative vision did not predict future psychotic reaction.

Fluorescein angiography in homozygote and carrier state of progressive retinal atrophy of the poodle; comparative aspects with human retinitis pigmentosa.

Fluorescein angiographic findings in 7 carriers and 7 homozygotes of a recessively inherited disease, generalized progressive retinal atrophy of the poodle, are described in detail and comparisons with 24 healthy control poodles are presented. Two angiographic types of the disease could be distinguished. These are: Type 1, a diffuse, widespread choriocapillaris atrophy associated with retinal vascular and depigmentation changes and Type 2, a predominantly retinal involvement combined with focal choroidal atrophic patches lineating major vessels. These findings share similarities with the observations made of the human disease, retinitis pigmentosa. Unaffected carriers of the disease showed characteristic fundus changes in angiography: patchy, hyperfluorescent depigmentation changes and often minor narrowing and irregularities of the arterioles and poorly visible capillary bed.

Further delineation of the Cohen syndrome; report on chorioretinal dystrophy, leukopenia and consanguinity.

Six new patients with the Cohen syndrome are reported from Finland and 25 published cases from elsewhere are reviewed. New findings are consanguinity among two pairs of parents, granulocytopenia, and marked ophthalmological changes: decreased visual acuity, hemeralopia, constricted visual fields, chorioretinal dystrophy with bull's-eye-like maculae and pigmentary deposits, optic atrophy, and isoelectric electroretinogram. Previously known features of the Cohen syndrome (non-progressive mental retardation, short stature, microcephaly, peculiar facies, slender hands and feet, floppiness, delayed puberty) are confirmed or revised. The ophthalmological features merit attention in the previous and future suspected cases of the Cohen syndrome. Autosomal recessive inheritance can be taken for granted.

Ocular findings in four siblings with pseudoxanthoma elasticum.

In a family consisting of 8 surviving siblings, pseudoxanthoma elasticum was diagnosed in 4 sisters. One of them had a severe visual handicap, and another a slight decrease of the visual acuity because of the ocular lesions. The other 2 sisters also had fundal lesions, but they were visually symptom-free. Great variation was also found in the appearance of the ocular lesions in the affected siblings. In addition to the angioid streaks, the fundal findings in the probands affected included changes of the retinal pigment epithelium, exudative maculopathy, optic disc drusen and 'fire-work'-pattern of drusen of the Bruch's membrane. A similar variety of alterations was also found in the histopathology of the skin.

Laser trabeculoplasty in open angle glaucoma.

Laser trabeculoplasty was performed in 72 patients (81 eyes) with open angle glaucoma. A positive correlation between pre-laser pressure level and pressure reduction was found. In 23 eyes with POAG and an average pre-treatment pressure of 26.4 mmHg, post-laser IOP 6 months later was 21.6 (18% reduction) and 1 year later 21.8 mmHg (17% reduction). In 24 eyes with CG and an average pre-treatment pressure of 32.9 mmHg, post-laser IOP 6 months later was 23.8 mmHg (28% reduction) and 1 year later 22.8 mmHg (31% reduction). All eyes needed additional medical therapy for glaucoma control, and 8 eyes had to be operated. Three eyes had complications. One eye had a pressure rise on the 2nd post-operative day, and two eyes had a haemorrhage from the chamber angle.
