Retrosternal goiter and chylothorax: case report.
We describe a case of chylothorax associated with a retrosternal goiter. Such an association has not been previously reported. The chylothorax was probably caused by compression of the thoracic duct and brachiocephalic vessels.
Prevalence of aspirin intolerance in asthmatics treated in a hospital.
Seventy-four asthmatic patients seen in a hospital were challenged with aspirin. Aspirin sensitivity was detected in 16% of patients using anamnestic data and in 19% by a provocation test. Sensitivity and specificity of anamnesis were 71 and 97%, respectively.

Spontaneous pneumothorax in cryptogenic fibrosing alveolitis.
The prevalence of spontaneous pneumothorax was studied in 82 patients with cryptogenic fibrosing alveolitis. In 46 patients the disease affected only the lung ('lone' fibrosing alveolitis); in the remaining 36 patients the interstitial lung disease was associated with several systemic diseases ('associated' fibrosing alveolitis). 3 patients with the 'lone' form had a spontaneous pneumothorax. This figure represents a prevalence of 3.6%. We did not observe this complication in any patient with the associated form. Suction drainage was ineffective in the 3 patients. One of them died as a consequence of pleural infection. In the other 2 subjects thoracotomy, resection of blebs and pleurectomy were required.
Effect of naloxone on arterial gases in chronically obstructed patients with acute respiratory failure.
In a randomized double-blind design 10 patients with decompensated COPD received intravenous naloxone (2 mg) and placebo. Arterial gases were monitored at 15, 30 and 60 min. No significant changes were observed in pCO2 after naloxone as compared to placebo.
Ventilatory control in diabetes mellitus.
The ventilatory control system was evaluated in a group of 40 diabetic patients (20 with autonomic neuropathy (AN), and 20 without autonomic neuropathy) and 20 controls. The ventilatory increase in response to transient hypoxia was less in diabetics without AN than in controls, but even weaker in diabetics with AN. This pattern was repeated in the ventilatory response to hypercapnia. There was no correlation between the presence of abnormal respiratory responses and the duration of diabetes.
