Coronary vasomotor tone: a heterogeneous entity.

Changes in coronary vasomotor tone are undoubtedly involved in the pathogenesis of many ischaemic coronary syndromes. The term coronary artery 'spasm' should be limited to the situations characterized by focal coronary constriction sufficient to cause transient total or sub-total coronary occlusion. Mild coronary artery constriction is also observed in many other situations, without even causing coronary artery occlusion. We present suggestive evidence that the mechanisms underlying coronary artery 'spasm' and 'physiological' coronary artery constriction are different.

First recognized community outbreak of haemorrhagic colitis due to verotoxin-producing Escherichia coli O 157.H7 in the UK.

The first recognized outbreak of haemorrhagic colitis due to Escherichia coli O 157.H7 in the United Kingdom affected at least 24 persons living in East Anglia over a 2-week period. The illnesses were characterized by severe abdominal pain and bloody diarrhoea of short duration. Eleven patients were admitted to hospital and there was one death. Patients were mainly adult women who had not eaten out of the home in the 2 weeks before onset. Unlike previously reported outbreaks hamburgers were not the vehicle of infection, and a case-control study suggested that handling vegetables, and particularly potatoes, was the important risk factor.

Familial 'sleep apnea plus' syndrome: report of a family.

We describe a familial disorder consisting of sleep apnea, anosmia, colorblindness, partial complex seizures, and cognitive dysfunction. The phenotypic expression of the syndrome suggests an autosomal dominant inheritance with incomplete penetrance.

Pathologic features of cytomegalovirus retinopathy after treatment with the antiviral agent ganciclovir.

Ganciclovir is a new antiviral compound (also called BW B759U, DHPG, BIOLF-62, and 2'NDG) that has been used for the treatment of cytomegalovirus (CMV) retinopathy in immunocompromised patients (bone marrow recipients or acquired immune deficiency syndrome [AIDS] victims). The authors studied the eyes of three AIDS patients with CMV retinopathy who died while receiving ganciclovir chemotherapy. Gross, microscopic, and ultrastructural studies of these cases showed varying degrees of retinal scarring and active CMV lesions at the margins of the scars. CMV antigens were localized in cells at all layers of retina at the border of the lesions and in isolated cells in a perivascular location within histologically normal appearing retina. These areas probably represent sites of recrudescence when the drug is discontinued. In situ hybridization using a cloned complementary DNA (cDNA) probe of human CMV corroborated the immunocytologic localization of the virus. Ultrastructural studies showed megalic syncytial cells containing mostly capsids exclusively in the cell nucleus. The cytoplasmic electron-dense membrane-bound bodies that have characterized untreated cases of CMV retinopathy were absent in the treated cases. An attempt to isolate CMV in tissue culture from the vitreous and retina of one of the cases yielded a negative result. Our results indicate that ganciclovir does not effectively eliminate CMV from the retina nor does it suppress expression of all viral genes. Ganciclovir appears to function by limiting viral DNA synthesis and subsequent packaging of viral DNA into infectious units, thereby acting as a virostatic chemotherapeutic agent.

Treatment of cytomegalovirus retinitis with ganciclovir.

Eighteen immunocompromised patients with cytomegalovirus (CMV) retinitis were treated with ganciclovir, an investigational antiviral drug. CMV retinitis in association with acquired immune deficiency syndrome (AIDS) developed in 17 patients; CMV retinitis developed in one patient after cardiac transplantation. Fourteen patients responded to ganciclovir treatment with improvement in CMV retinitis. In 11 patients, the response was classified as complete; in three patients, the response was partial. Continued improvement in the retinitis was often seen while the patient was on maintenance treatment. Maintenance ganciclovir therapy was required; relapse occurred in five of seven patients in whom ganciclovir treatment was interrupted. The major limiting toxic side effect of ganciclovir was neutropenia, which necessitated temporary discontinuation of ganciclovir in five patients but was reversible in all cases. Ganciclovir appears to be an effective therapy for CMV retinitis, but chronic maintenance therapy is required.

Psychoanalytic supervision and the larger truth.

If one asks, then, what is the larger truth in supervision and where is it to be found, the answer is that it is to be found in the history and character of the student; in the intersubjective encounter between the student and the supervisor; in the secret use of supervision by the student as he creatively searches for himself. However one chooses to define truth or to speak to a theory of supervision, it is important to note that supervision is not purely an educational activity nor a therapeutic one. It lies somewhere in between and therefore requires some unique theoretical formulations.

Asynchronous expression of the immediate-early protein of herpesvirus saimiri in populations of productively infected cells.

The time course of virus replication in cultures of permissive cells infected with high multiplicities of herpesvirus saimiri (HVS), a gammaherpesvirus, is protracted relative to the replication of herpes simplex virus (HSV), an alphaherpesvirus, under similar conditions. The basis for this difference was investigated by quantitative immunofluorescence microscopy exploiting monoclonal antibodies specific to the HVS 52 000 mol. wt. immediate-early polypeptide (IE 52K) and to delayed-early (DE 51K, DE 110K) and late (130K and capsid proteins) gene products to measure the timing of gene expression in individual cells of infected cultures. The timing of the transitions from IE to DE and from DE to late protein synthesis occurred at proportionately different intervals in the growth cycle of HVS, relative to that of HSV. In particular, the DE to late transition occurred relatively later in HVS infections. However, asynchrony in the events leading to the expression of the first class of virus proteins (IE 52K) was the main source of the extended course of HVS replication in populations of infected cells. This asynchrony was not modified significantly by infection at different stages of the host cell-cycle and was reduced, but not overcome, by very high applied multiplicities of infection. Double-antibody staining revealed a positive correlation between the accumulation of high concentrations of parental virus particles at perinuclear sites and early detection of HVS IE 52K gene expression. Both of these events remained sensitive to a microtubule poison (colcemid) for many hours after infection with HVS, whereas the rapid and synchronous expression of the IE 175K protein (ICP4) in HSV-1-infected cells was insensitive to post-infection exposures to this drug. We conclude that significant differences in early stages of virus entry and intracellular processing which precede immediate-early gene expression are largely responsible for differences between the replicative cycles of these representatives of gamma- and alphaherpesviruses in cultures of permissive cells.

Feeding premature infants with human milk or preterm milk formula. Effects on postnatal growth and on circulating concentrations of intermediary metabolites, amino acids, and regulatory peptides.

Results of a comprehensive longitudinal study comparing the effects of feeding healthy preterm infants with human milk or a specially adapted formula designed for the preterm infant are reported. 10 healthy infants were given human milk from birth, and 9 similar infants were given a formula which contained 80 kcal, 1.8 g protein, and 4.5 g fat per 100 ml. Anthropometric measurements were made weekly as were routine haematological and biochemical variables together with plasma amino acid and gastrointestinal regulatory peptide levels and metabolic fuel concentrations. Infants receiving the formula demonstrated a significantly greater growth velocity compared with infants receiving human milk. There were no significant differences between the two groups in routine haematological or biochemical variables measured nor in plasma insulin or blood glucose, lactate, pyruvate, or ketone body concentrations. Plasma amino acid profiles, however, did demonstrate some significant differences between the two groups with higher methionine and threonine levels in the formula-fed infants. Plasma motilin, enteroglucagon, neurotensin, cholecystokinin, gastric inhibiting polypeptide, and pancreatic polypeptide levels all demonstrated significant postnatal surges, with significant differences between the two groups in plasma gastric inhibiting polypeptide and pancreatic polypeptide concentrations.

A single major immediate-early virus gene product is synthesized in cells productively infected with herpesvirus saimiri.

A major virus-specific immediate-early (IE) polypeptide with an apparent mol. wt. of 52 000 (52K) was synthesized immediately after the removal of a cycloheximide block applied to owl monkey kidney cell cultures at the time of infection with high multiplicities of herpesvirus saimiri (HVS). The IE 52K polypeptide was phosphorylated and accumulated rapidly and efficiently in the nucleus of infected cells. Monoclonal antibodies to IE 52K and to a major non-structural delayed-early (DE) DNA-binding protein with similar electrophoretic mobility (DE 51K) were used to show that the IE 52K protein and DE 51K proteins are antigenically distinct. The monoclonal antibody to the IE 52K protein of HVS strain 11 reacted in immune precipitation and immunofluorescence tests with polypeptides of similar mol. wt. present as nuclear antigens in cells infected with heterologous strains of HVS and herpesvirus ateles.

Isolation and characterization of monoclonal antibodies to structural and nonstructural herpesvirus saimiri proteins.

An improved screening procedure was applied to identify hybridomas secreting antibodies to herpesvirus saimiri-specified polypeptides among the products of fusions between SP2/0 myeloma cells and spleen cells from mice immunized with purified virus particles or virus-specific DNA-binding proteins. Twenty-four monoclonal antibodies were isolated with specificities for 13 different virus-specified polypeptides (or complexes of polypeptides), including the major capsid protein of the virus (150K), the 160K and 130K structural proteins, a 108K structural phosphoprotein, structural glycoproteins, the nonstructural early 76K protein, early nonstructural DNA-binding proteins of 48 to 51K and 110K and the major immediate-early protein of 52K. Antibody to the virus 76K protein precipitated a host protein of 62K, and a number of antibodies specific for host proteins were also isolated. Antibody to the 52K immediate-early polypeptide precipitated the delayed-early 76K protein, whereas the antibody to the 76K protein did not precipitate the 52K polypeptide. These observations suggest the presence of epitopes common to virus and host proteins and an antigenic site common to an immediate-early and a delayed-early virus protein. The antibodies were used to examine the sites of intracellular accumulation of virus polypeptides, the formation of complexes of structural proteins, and the postsynthetic processing of virus proteins. The present collection of monoclonal antibodies provides a set of reagents with specificities for members of each of the major kinetically or functionally distinct classes of virus gene products.

Dramatic pyrexic and cardiovascular response to intravaginal prostaglandin E2.

The efficacy of prostaglandin E2 (PGE2) for the induction of labor in pregnancies complicated by IUFD or missed abortion has been demonstrated. Gastrointestinal and pyrexic side effects are frequently observed. The following report describes two patients with documented IUFD who experienced a dramatic pyrexic and cardiovascular response to the intravaginal PGE2. In addition, evidence will be provided to account for this unusual pharmacologic reaction, as well as provide a method of managing this unusual side effect.
