Congenital spinal cord haemangioblastoma: another cause of spinal cord section syndrome in the newborn.

A newborn infant with negative perinatal history and characteristic clinical findings of upper cervical spinal cord section is described. Metrizamide myelography performed on the 7th and 22nd days of life was negative. Peroneal somatosensory evoked responses showed a conduction block at the cervical level. Necropsy revealed a haemangioblastoma extending from levels C1 to C5.

Evaluation of cutaneous absorption and safety of idoxuridine after topical administration.

The absorption of a preparation of the antiviral drug 5-iodo-2'-deoxyuridine (idoxuridine or IDU] has been assessed in rats. A 40% w/v solution of 3H-IDU in dimethyl sulfoxide (DMSO) was topically applied to rats; cutaneous absorption was determined by morphometric evaluation of autoradiographic preparations of the skin. Passage of the drug to the bloodstream was followed using radioisotopic techniques. In addition, the potential irritative action of the same preparation of IDU was investigated in a more suitable animal model. A predictive guinea pig sensitization study was performed according to the Magnusson and Kligman method. 3H-IDU was detected in the epidermis of the rat 60 min after local application. However, it was not possible to determine noticeable levels of the radiolabelled drug in the circulating blood. Experiments on guinea pigs showed slight transient erythematous responses in three animals of the test group following first challenge, and in one animal of the test group and one of the control group, following the second challenge. The overall results of the sensitization test were negative. These results give experimental support for the safety of the IDU preparation studied.

Effect of cold-restraint stress and zinc acexamate on gastric mucus production in intact glands.

Gastric mucus content was morphometrically evaluated in gastric glands of normal and cold-restraint stressed rats. Variations induced by treatment with zinc acexamate (200 mg/kg p.o.) were also investigated. Stress decreased the glycoprotein content in glands located in areas of injury. However, in intact glands from the same animals, the glycoprotein content was increased and the proportion of sulphated macromolecules greatly augmented. Zinc acexamate reduced the severity of damage in stressed rats. Although it augmented mucus content it prevented the modification in sulphated macromolecules in these rats. These findings are discussed in relation to the role of gastric mucus in preventing gastric damage.

Anti-ulcer and membrane stabilizing actions of zinc acexamate.

The effects of zinc acexamate on stress and reserpine ulcers as well as on gastric mast cells degranulation and membrane stability were evaluated in the rat. Zinc acexamate (100 mg/kg) has demonstrated an inhibitory effect on cold-restraint stress and reserpine-induced ulcer in a dose-dependent manner. Pretreatment of rats, prior to cold restraint stress, reduced gastric mast cell degranulation. Zinc acexamate (10(-4) M) inhibits Triton X-100 release of beta-glucuronidase in isolated hepatic lysosomes. These observations suggest that ulcer protective actions of zinc acexamate may be exerted in part through enhancing gastric mucosal resistance by stabilizing biological membrane integrity.

[Perinatal aspects of the children of heroin addicts].

Maternal, perinatal and neonatal factors of 27 newborns of heroin addicted mothers were reviewed between 1980 and 1984. Results were compared with a control group of 2,587 normal newborns. A progressive increase in number of cases was observed in the last years. Mean age of heroin-addicted mothers was significant lower than control group (p less than 0.001). Only 18.5% were under obstetrical control for at least 4 times (p less than 0.001) and 40.7% had antecedents previous abortions (p less than 0.025). Four cases of maternal syphilis were diagnosed and also four mother were positive to HBsAg; statistical difference in both cases was highly significant with respect to control group (p less than 0.001). A higher index of premature rupture of membranes (11.1%, p less than 0.001) and cesarean sections (25.9%, p less than 0.001) was appreciated. No significant difference was observed attending to fetal presentation, instrumental delivery, Apgar test scores and pH of umbilical artery (p greater than 0.05). A higher incidence of preterm (18.5%, p less than 0.01) and small for gestational age babies (44.4%, p less than 0.001) was ascertained. Pregnancy control and no smoking habit might influence birth weight (p less than 0.001). Neonatal withdrawal syndrome was present in 44.4% and was related to mother's last dose and secondary to degree of heroin abuse (p less than 0.025).

Antiulcerogenic activity of zinc acexamate in different experimental models.

The antiulcerogenic activity of zinc acexamate (ZAC; Laboratorios Viñas, S.A.) has been tested in several models of gastric injury induced by acid hypersecretion, prostaglandin blockade and disruption of the gastric barrier. Lowest doses which have demonstrated to significantly prevent gastric damage ranged from 10 to 100 mg/kg depending on the experimental model used. The benefit obtained with this compound was always dose-dependent. These findings would support the hypothesis that ZAC acts by a complex inhibition of several of the mechanisms involved in the development of peptic diseases.

Cancer incidence in Murcia, Spain, in 1982: first results from a population-based cancer registry.

This report presents the first findings of the Murcia Cancer Registry, a population-based cancer registry set up in May 1981 in the Murcia region of southeast Spain (955,487 inhabitants). Descriptive epidemiological methods have been applied to study cancer incidence in 1982. The validity of cancer registration has been assessed for both completeness and accuracy. Altogether 1,987 cases were registered in 1982, the crude (all-ages) annual incidence rates per 100,000 being 238 in males and 179 in females. Excluding nonmelanoma skin cancer, which seems to be incompletely registered, age-standardized rates for Murcia are very similar to those of other registries in Spain in males, but lower in females. The risk was higher in males than in females for all sites and for the great majority of specific sites, especially larynx, oesophagus and bladder. The age-standardized rate for cancer of the larynx was 26 times higher in men than in women. Lung cancer was the most common cancer in males: the risk was 8 times that in females. Lung cancer rates were higher, in both males and females, than in other Spanish registries. Cancer of the larynx was the second most common site in males when either truncated rates (35-64 years) or cumulative incidence rates up to 64 years of age were used. The age-standardized rate (18 per 100,000) supports previous studies suggesting that the risk for this cancer in Spain and other Mediterranean countries is very high. The lung/larynx rate ratio in men was two. Cancer of the breast is the most common cancer among women, as in other registries in Spain and in most other countries. The age-standardized rate (29.4 per 100,000) is lower than breast cancer rates elsewhere in Spain. This difference may be partly explained by incompleteness of ascertainment in Murcia. The incidence rate for cancer of the cervix uteri was 4.9 per 100,000, excluding carcinoma in situ. Despite the limitations of the data, cervical cancer incidence in Murcia is likely to be similar to that in other regions of Spain.

The effects of maternal cigarette smoking on placental morphology, histomorphometry, and biochemistry.

We related cigarette smoking during pregnancy to the macroscopic and microscopic morphology, histomorphometry, and biochemistry of the placenta among participants in The Prenatal Project, a randomized, controlled trial of nutritional supplementation during pregnancy. The aim was to ascertain if the harmful effects of cigarette smoking on the fetus were reflected, and possibly mediated, by changes in the placenta, and whether such changes might be modified by the nutritional supplements, parallel to the nutritional reversal of birthweight depression among heavy smokers. Maternal smoking was associated with increased alkaline ribonuclease (RNase) levels, possibly resulting in disturbed protein synthesis. More smokers had placentas with villous hyperplasia on histologic examination. This was accompanied by biochemical evidence of significantly increased cellularity (increased DNA/gm of placental mass and decreased protein/DNA ratios) and (nonsignificant) trends to reduced syncytial villous cell size on histomorphometry. There were joint effects of cigarette smoking and use of either form of nutritional supplementation on the fibrin and stoma components of the villi, but such changes are not necessarily pathologic. The significant reduction in pathology of the stem villi among smokers was confined to those who were nutritionally supplemented, particularly heavy smokers. This change may thus have reflected the protective effects of supplementation on the birthweight of the offspring of these heavy smokers.

Paraneoplasic precocious puberty. Report of a new case with hepatoblastoma and review of the literature.

A new case of precocious puberty secondary to production of human chorionic gonadotropin (HCG) by an hepatoblastoma was studied in an 8-month-old infant. A review was made of the 35 cases of paraneoplasic precocious puberty previously reported in the literature. The most frequent cause is hepatoblastoma, which was responsible for 18 cases. There are nine reports of mediastinal teratoma, six of which were observed in patients with Klinefelter Syndrome. In another six cases, the paraneoplasic syndrome was attributed to intracranial tumoration, and two of these patients showed teratoma with areas of choriocarcinoma, whereas germinoma was suspected for clinical reasons in another three. Lastly, one case of presacral teratoma and one of retroperitoneal carcinoma were reported. The differential characteristics of paraneoplasic precocious puberty are: almost exclusive occurrence in males, because of tumoral production of HCG, moderate or absent increase of testicle size, hyperplasia of Leydig's cells without spermatogenesis, and rapidly progressive signs of puberty.

Compartmentalization of iron in sickle cell anemia--an autopsy study.

In autopsy studies of two sickle cell anemia (SCA) patients, iron was totally absent from all organs in one case in which the serum ferritin had been low, while the other patient demonstrated hepatic, splenic, and renal siderosis but no iron stores in the bone marrow. Serum ferritin had been very high in this case. These findings suggest that in SCA bone marrow biopsy may be an unreliable means to determine iron stores, but rather the ferritin level should be used.

[Communication in functional and dysfunctional families with adolescent children. II. Nonverbal communication].

This research focuses on family communications during the critical period of children's puberty provoked change. Two groups of families are compared: one group of 15 "functional" families (no psychological counseling has been requested) and one group of 15 "dysfunctional" families (a psychological counseling has been requested for one of the adolescents). The micro-analysis of the non-verbal communication reveals structural differences between the family systems. This fits with the differences revealed by the analysis of the verbal communications.

[Communication in functional and dysfunctional families with adolescent children. III. Evaluation of the relation of verbal and nonverbal communications].

This study analyzes communication in families with one or more adolescent children, thus undergoing a critical period of transition. Two groups are compared: a group of 11 "functional" families (no child referral for psychological problems) and a group of 11 "dysfunctional" families (one adolescent child referral for psychological problems). The analysis of relationships between verbal and non-verbal communication indicates that more inter-channel congruence is achieved in "functional" families; this holds for individual as well as for inter-partners interactions. The results of the previously published separate analysis of verbal and non-verbal communication are briefly resumed. The findings of the study as a whole are interpreted in terms of the General Systems Theory with regard to the function of "parental framing" of the child's development.

[Action of 13-cis-retinoic acid and aromatic retinoid on several cutaneous parameters in hyperseborrheic rats].

Castrated rats submitted to androgenic stimulation present hyperseborrhea with an increase in foamy cells associated with an increase in cutaneous lipids and a change in the number of epidermal cell layers. 13-cis-retinoic acid and aromatic retinoid (Ro 10-9359) were given orally during 17 days (10 mg/kg) to castrated rats which had received a testosterone implant. Cyproterone acetate was chosen as the antiandrogenic drug; it was given at doses of 10 mg/kg i.m. once every other day. As opposed to cyproterone acetate, the two substances studied had no effect on the seminal vesicle and ventral prostate. 13-cis-retinoic acid and retinoic aromatic acid increased the total number of epidermal layers: In the case of 13-cis-retinoic acid, the increase involves especially the nonnucleated cell layers, whereas with retinoid aromatic acid it involves the nucleated cell layer. The number of sebaceous glands is not changed by either products, on the other hand, the number of foamy cells is considerably decreased and the weight of cutaneous lipids is decreased as well.

Perforated diverticulum of the terminal ileum. A previously unreported cause of suppurative pylephlebitis and multiple hepatic abscesses.

We describe the case of a 58-year-old man who presented to the hospital with central abdominal pain, nausea, fever, chills, and dyspnea. While in the hospital, jaundice appeared and the liver function tests revealed features of both cholestasis and hepatocellular injury. He developed gram-negative septicemia and died on the sixth hospital day. Autopsy disclosed a perforated terminal ileal diverticulum and a contiguous mesenteric abscess. There was also severe phlebitis of mesenteric venous radicles which extended superiorly to the intrahepatic portal venules and veins. The portal veins were surrounded by multiple hepatic abscesses that varied in size from microscopic to 2.5 cm. This appears to be the first report in the world literature of suppurative pylephlebitis and hepatic abscesses resulting from a perforated ileal diverticulum. The subject of small bowel non-Meckelian diverticulosis is reviewed because of the rarity of this condition and the diagnostic challenges it poses.

Mycotic aneurysms of the pulmonary arteries in intravenous drug addicts. Report of three cases and review of the literature.

During the postmortem study of pulmonary lesions in intravenous drug addicts, three patients were found to have bilateral multiple mycotic aneurysms of the peripheral pulmonary arteries in association with embolic pneumonia. Two of these patients had infective tricuspid endocarditis, and one had hemoptysis. This report draws attention to the occurrence of mycotic aneurysms in this unusual location and suggests that these pulmonary lesions may be more common than currently believed.
