Immunohistochemical detection of hepatitis C virus-infected hepatocytes in chronic liver disease with monoclonal antibodies to core, envelope and NS3 regions of the hepatitis C virus genome.

The localization of hepatitis C virus-infected hepatocytes in the human liver remains unclear despite the development of a serological assay for the antibody to hepatitis C virus. We studied their localization immunohistochemically with monoclonal antibodies to core, envelope and NS3 antigens of hepatitis C virus. We examined 48 liver biopsy samples from C100-3 antibody-positive patients with chronic liver disease (chronic persistent hepatitis, 5 cases; chronic active hepatitis, 41 cases; cirrhosis, 2 cases) and 12 liver biopsy samples from C100-3 antibody-negative patients with chronic liver disease (type B chronic hepatitis, 8 cases; alcoholic liver disease, 4 cases). In the C100-3 antibody-positive group, positive immunostaining for core antigen, envelope antigen and NS3 antigen was found in 23% (11 of 48), 24% (11 of 45) and 24% (11 of 46), respectively. Negative results were obtained in the C100-3 antibody-negative group. Hepatocytes with positive staining were scattered in the lobules, and they were found in the same regions irrespective of whether the antibody to core antigen, to envelope antigen or to NS3 antigen was used. Each positive cell was strongly stained in the cytoplasm; these decorations disappeared after absorption of the primary antibody with purified antigen. mean ALT levels in the patients with positive immunostaining for core, envelope or NS3 antigen (174.8 +/- 105.7 U/L) tended to be higher than in those with negative immunostaining (142.0 +/- 93.8 U/L). On histological evaluation of liver specimens with a scoring system of the histological activity index, intralobular inflammation and fibrosis had higher scores for samples with positive rather than negative immunostaining (p less than 0.05).(ABSTRACT TRUNCATED AT 250 WORDS)

In situ localization of spermatogenic cell-specific glyceraldehyde 3-phosphate dehydrogenase (Gapd-s) messenger ribonucleic acid in mice.

Northern blot analysis with a cDNA clone for mouse spermatogenic cell-specific glyceraldehyde 3-phosphate dehydrogenase (Gapd-s) previously identified transcripts in isolated round spermatids, but not in isolated pachytene spermatocytes, strongly suggesting that Gapd-s transcription first occurs in haploid germ cells. In the present study, in situ hybridization was carried out with a [35S]-labeled anti-sense RNA probe of Gapd-s to define more precisely the temporal expression and spatial distribution of Gapd-s mRNA in adult mouse testes. Gapd-s transcripts were not detected in spermatogonia, primary spermatocytes, secondary spermatocytes, or spermatozoa. However, they were abundant in round spermatids and condensing spermatids. Transcripts were not detected in somatic cells of the testis, in oocytes in pre-antral or antral follicles, or in skeletal muscle. Grain counts were used to define when the Gapd-s gene was expressed during round spermatid development in adult mice. The number of silver grains first exceeded background levels in step 4-6 spermatids (early cap phase of spermiogenesis) and were high in step 7-15 spermatids (acrosome and elongation phases of spermiogenesis). However, they returned to background levels in step 16 spermatids, the final step of spermiogenesis in the mouse. In 20-day-old juvenile mice, Gapd-s transcripts were first detected in round spermatids during the early cap phase of development. These observations confirmed that Gapd-s mRNA is expressed during the post-meiotic phase of male germ cell development, demonstrated that Gapd-s transcripts are present in step 4-15 spermatids, and established that transcription begins in round spermatids during the early cap phase of spermiogenesis in both juvenile and adult mice.

Methacholine inhalation challenge in children with idiopathic chest pain.

Bronchial reactivity to inhaled methacholine (MCH) was evaluated in 32 patients with 'idiopathic' chest pain. Each pain was recurrent in nature. The incidence of cases with a provocative concentration causing a 20% fall in the forced expiratory volume in 1 sec (PC20) of 10 mg/ml or less was 62.5% (20 cases), while it was only 11.1% (three cases) in 27 healthy controls. Seventeen patients had no personal history of allergic diseases, elevated serum IgE level or positive house dust mite-specific IgE antibody. Among these 17, eight (47.1%) had a PC20 of 10 mg/ml or less, the incidence of which was also higher than that of the healthy controls. During the challenge, eight patients complained of chest pain similar to that experienced before. The present results indicate that bronchial hyper-reactivity is an important cause of 'idiopathic' chest pain. Patients with unexplained chest pain should be considered for inhalation challenge.

[A 4 degrees C-1 min method of cold water immersion test for peripheral circulatory function in fingers].

To assess the validity of a new simplified cold water immersion test (4 degrees C-1 min method) for peripheral circulatory function, comparison was made with the conventional method (10 degrees C-10 min method). These two different methods of cold immersion test were applied to 23 patients with vibration disease and 24 healthy men. Observation was made on finger skin temperature by a thermistor and complaints in the hand by a 5-step self-reported scale method every minute during the test. The patterns of recovery of skin temperature after cold immersion in each group were similar in both methods. Pain in the hand in the 4 degrees C-1 min method was less than that in the 10 degrees C-10 min method. The recovery rate at 5 min in the patients with Raynaud's phenomenon was lower than that in those without Raynaud's phenomenon in the 4 degrees C-1 min method (p < 0.01). However, no significant differences were noted in 10 degrees C-10 min method. The results suggest that the new method is feasible in detecting the response of vasodilation after immersion. In the recovery rate at 5 min after immersion, near values of the sensitivity and specificity were observed between 50% cut-off values in the 4 degrees C-1 min method and 30% value in the 10 degrees C-10 min method. Thus, the 4 degrees C-1 min method is considered to be more useful to evaluate the physiological response after cold immersion than the 10 degrees C-10 min method.

[A case-control study on factors relating to discontinuation of domiciliary care for the bedridden elderly in a metropolitan area].

A case-control study was conducted to examine factors relating to discontinuation of domiciliary care for the bedridden elderly in Shinagawa-ku, Tokyo. Cases were bedridden residents aged 65 years and over who had abandoned home care and applied for admission to live in a special nursing home for the aged between April and September in 1990 after being recipients of welfare allowances for disabled elderly. Controls were bedridden residents who continued to be given home care and matched to cases by sex, age and beginning month of the receiving of allowances. Among 50 cases and 94 controls interviewed, we obtained responses from 31 cases (62%) and 60 controls (64%). The main results were as follows: 1. During the home-care period, ADL (activities of daily living) of cases, especially walking ability, deteriorated more severely than in controls. Night delirium also appeared more frequently in cases. 2. The primary caregivers of cases were older than those of controls. Remarkable differences between cases and controls were observed in the family structure, the number of family members and the number of sub-caregivers. Cases tended to live alone or live with a spouse only, and with smaller number of family members and caregivers. 3. Case lived more frequently in houses with small numbers of rooms and without rooms of their own. 4. As regards utilization of domiciliary care services, cases used dispatch of home helpers more frequently and used day services less frequently.

Quantitative analysis of the tracking of blood pressure and relative factors in childhood: Shimane Heart Study.

1. In order to clarify the predictive factors of cardiovascular diseases caused by hypertension and hypercholesterolaemia in adults, a longitudinal epidemiological study was started in 1978. 2. This paper reports the results of research carried out from a quantitative analysis of the tracking of blood pressure and anthropometric parameters in children and adolescents using a tracking index. The index was calculated according to the changes in blood pressure quintiles for the whole population during the observation period. 3. The study population consisted of 5148 Japanese children living in the city of Izumo, who were examined every 3 years between the ages of 6 and 15 years old. 4. Although systolic blood pressure tracked weakly during childhood and adolescence, there was no significant relation between the tracking of systolic blood pressure and a family history of hypertension. 5. The tracking indices of the anthropometric parameters were better than those of systolic blood pressure, but the index of the left ventricular mass was similar to that of systolic blood pressure. 6. In conclusion the tracking of blood pressure might be determined by the tracking of bodyweight and height.

Chronic alpha 1 blockade in a patient with Eisenmenger syndrome.

In an 18-year-old male with Eisenmenger syndrome cyanosis and erythrocytosis were increasing. The erythrocytosis diminished following oral bunazosin and phlebotomy was not needed during the treatment. When bunazosin was stopped, the erythrocytosis increased, but when it was resumed, the erythrocytosis and general fatigue diminished.

Intracellular pH and sodium-proton exchange activity of lymphocytes in stroke-prone spontaneously hypertensive rats.

1. At the age of 20 weeks, intracellular pH (pHi) of circulating lymphocytes suspended in HCO(3-)-free NaCl media was not significantly different between stroke-prone spontaneously hypertensive rats (SHRSP) and normotensive Wistar-Kyoto rats (WKY). 2. The initial recovery rate of pHi in lymphocytes tended to be greater in SHRSP than in WKY after the addition of 60 mmol/L or 120 mmol/L of NaCl, but there was no statistically significant difference. 3. The H+ equivalent efflux rate, which was a true reflection of Na(+)-H(+) activity, was significantly greater in SHRSP than in WKY (P less than 0.05). The difference in H+ equivalent efflux rate was not due to the difference in cellular buffering power between the two groups (P greater than 0.05). An increased Na(+)-H(+) exchange activity may play a partial role in the pathogenesis of hypertension.

Structure and organization of the hepatitis C virus genome isolated from human carriers.

Hepatitis C virus (HCV) is a major causative agent of posttransfusion non-A, non-B hepatitis, which often develops into malignant chronic diseases, including liver cirrhosis and hepatocellular carcinoma. We have cloned from human carriers overlapping cDNAs (9,416 bp) covering the entire coding region of the HCV genome. The latter encodes a 3,010-amino-acid polyprotein. In addition, there are 332 and 54 bases of 5' and 3' noncoding sequences, respectively. Our HCV strain has a 77% nucleic acid identity to the HCV strain cloned by workers at Chiron Corporation. The hydrophobicity profile of the putative polyprotein is similar to those of flaviviruses, but it has limited amino acid homology to polyproteins of flaviviruses and other viruses, indicating that HCV is at most distantly related to flaviviruses.

Orbital tumor in an infant with acute monocytic leukemia associated with chromosome 5q-,t(9p-11q+),11p+.

We examined an 8-month-old infant with left proptosis, chemosis and orbital mass. A computed tomographic scan revealed a left orbital tumor. An anorectal nodule was also found. Results of the hematologic study showed poorly differentiated acute monocytic leukemia (M5 according to the French-American-British classification). Chromosomal analysis disclosed 46, XX,5q-,t(9p-;11q+),11p+. We believe that our patient represents a rare case of orbital tumor in acute monocytic leukemia associated with chromosomal abnormalities.

Expression of the epidermal growth factor receptor in developing fetal mouse palates: an immunohistochemical study.

Epidermal growth factor (EGF) stimulates the growth of various tissues and, therefore, EGF receptor expression in fetal tissues may play a key role in organogenesis. We have examined immunohistochemically the ontogeny and localization of the EGF receptor in the fetal mouse palate during in vivo and in vitro palatogenesis using the anti-human EGF receptor rabbit antibody. Immunoreactive products against the EGF receptor were observed in the palatal tissue examined on days 12, 13, and 14 of gestation. On days 12 and 13, the immunoreactive products were predominantly positive on the oral and medial edge epithelia but were minimal on the epithelium of the vertical shelf. The EGF receptor immunoreactivity was less intense in the posterior palate as compared with the midpalatal region. In the fusing palate of day 14 fetuses, the cells forming the midline epithelial seam were continuously positive for EGF-R immunoreactivity. The mesenchyme of palatal shelves also showed regional heterogeneity and temporal sequence in EGF receptor expression. The localization of the EGF receptor in fetal mouse palates cultured in a serumless medium generally simulated that observed in vivo.

A cDNA clone encoding a peptide highly specific for hepatitis C infection.

A random primed lambda gt11-cDNA library was constructed from donors plasma presumably infected by blood-borne non-A, non-B hepatitis (hepatitis C:HC) agent and immunoscreened with serum pooled from patients with acute or chronic HC. Twelve lambda gt11-cDNA clones encoding antigens associated with HC infection in Japan as well as in the USA were isolated. Of these one clone consisting of 114 nucleotides and showing a discrete band on an immunoblot analysis, was extensively studied. The clone is not derived from the host DNA encoding one polypeptide specific and highly sensitive for serum from patients with HC and has no homology to the nucleotide sequences of known human viruses including hepatitis A,B and D viruses, Ebstein-Barr virus, coxsackievirus, immunodeficiency virus type 1 or Japanese encephalitis virus. These results suggest that this clone is derived from the genome of HC agent.

Distally based lateral calcaneal flap.

In our cadaver dissection, the connection between the lateral calcaneal artery, the lateral tarsal artery, and the branch of the lateral plantar artery was corroborated, indicating possible reverse blood flow in the lateral calcaneal artery. The distally based lateral calcaneal flap has been successfully used in 2 patients. Vascular anatomy, indication, and technical points of the surgery as well as advantages and disadvantages of the flap are discussed.

Increased Na(+)-H+ exchange activity in cultured vascular smooth muscle cells from stroke-prone spontaneously hypertensive rats.

Na(+)-H+ exchange activities were studied in vascular smooth muscle cells (VSMC) obtained from stroke-prone spontaneously hypertensive rats (SHRSP) and normotensive Wistar-Kyoto rats (WKY). The cytoplasmic pH of VSMC was detected by 2',7'-bis(carboxyethyl)-carboxyfluorescein (BCECF), which was used as a fluorescent pH probe with a fluorescent microplate reader; this enabled us to measure the cytoplasmic pH of cells attached to multiwell culture plates. We measured cytoplasmic pH recovery in quiescent VSMC after acid loading with 20 mmol/l NH4Cl in HCO3(-)-free buffer. This recovery was inhibited either by removal of extracellular sodium or by addition of 5-N-ethyl-N-isopropyl amiloride (EIPA). The initial recovery rate was dependent on extracellular sodium concentration. Therefore, this change in cytoplasmic pH was presumed to be due to amiloride-sensitive Na(+)-H+ exchange. The Na(+)-H+ exchange activity represented by the cytoplasmic pH recovery rate was significantly higher in VSMC from SHRSP than in WKY cells at extracellular sodium concentrations greater than or equal to 20 mmol/l. In contrast, the steady-state cytoplasmic pH of quiescent VSMC from SHRSP was lower than that of WKY cells in HCO3(-)-free buffer. These findings seemed to contradict each other, and suggested that cytoplasmic pH was regulated not only by Na(+)-H+ exchange but also by other complicated mechanisms.

Relation between blood pressure, left ventricular mass and pulsed Doppler parameters in healthy school children.

We analyzed the relations between blood pressure in sitting and supine positions, left ventricular mass (LVM) and Doppler aortic, pulmonary and mitral flow velocity measurements in 163 healthy school children. Systolic blood pressure in a supine position correlated significantly with aortic acceleration (ATc) and ejection time (ETc), corrected by the square of R-R interval, pulmonary AT and peak flow velocities. Moreover, the systolic blood pressure in a sitting position correlated with pulmonary AT and LVM. LVM correlated with pulmonary ATc, the ratio of AT to ET and average acceleration, and aortic ETc and peak flow velocity. These data suggest the following: (1) the posture influences the relation between blood pressure and flow velocity, (2) pulmonary hemodynamics are influenced by systemic blood pressure in healthy children and (3) the development of LVM may be dependent not only on blood pressure but also on cardiac work in childhood.

Changes of left ventricular isovolumic relaxation time with growth in children.

We investigated the changes of left ventricular isovolumic relaxation time (LVIRT) with growth and the factors influencing them during childhood, in a 10-year follow-up study. We measured the body height (BH), body weight (BW), lean body weight (LBW), heart rate (HR), blood pressure (BP), left ventricular muscle volume index (LVMVI), and LVIRT corrected by the preceding R-R interval (IRT/R-R), of 187 healthy children at 3-year intervals (6, 9, 12, 15 years old). IRT/R-R were prolonged with growth in boys and girls. At the ages of 12 and 15, the IRT/R-R of girls were longer than those of boys. Significant correlation coefficients for IRT/R-R on systolic and diastolic, BW, and LVMVI were 0.28, 0.31, 0.20, 0.28, respectively. These data suggest that (1) IRT/R-R is mainly determined by the diastolic BP, (2) left ventricular diastolic function of children develops with growth of body size, especially BW, and (3) left ventricular early diastolic function in adolescents is related to sexual maturation.
