Characterization of antibiotic-resistant bacteria in rendered animal products.

Antibiotics are used in food animal production to treat diseases and also to improve performance. Antibiotics are not used on all farms, and antibiotic resistance is occasionally found on farms that do not use antibiotics. Rendered animal protein products are often included in poultry feeds and could potentially serve as a source of antibiotic-resistant bacteria. One hundred sixty-five rendered animal protein products from cattle, poultry, and fish were aseptically collected from poultry feed mills. Fifty-five percent of the poultry meal samples had detectable levels of gram-negative bacteria ranging from 40 to 10,440 colony-forming units/g of sample. Poultry meal and meat and bone meal had the greatest number of samples with bacteria resistant to five or more antibiotics. A high percentage of feed samples (85%) contained bacteria resistant to amoxicillin, ampicillin, clavulanic acid, or cephalothin, whereas few samples contained bacteria resistant to ciprofloxacin, kanamycin, or trimethoprim/sulfamethoxazole. Acinetobacter calcoaceticus, Citrobacter freundii, and Enterobacter cloacae were the most commonly isolated antibiotic-resistant bacteria. Isolation for Salmonella was also performed, with 14% of the meat and bone meal samples containing Salmonella sp. Only one of the meat and bone meal isolates, Salmonella livingstone, was resistant to five or more antibiotics. Many of the antibiotic-resistant bacteria contained integrons, genetic elements that mediate multiple drug resistance.

[Our experience with Kufs disease: familial form and sporadic case].

Our experience on Kufs' disease includes a familiar, neurovisceral form and a sporadic case. The familiar form shows an autosomal, dominant inheritance, and belongs to Berkovic's type B. A variability on the age of onset was observed, being more protracted in women. The sporadic case belongs to Berkovic's type A. Her EEG showed a periodic, short-interval, activity for a long period of illness. NMR findings were nonspecific for the diagnosis. All cases showed granular osmiophilic deposits, but not storage characterized by fingerprint profiles. Visceral storage in many cases on Kufs' disease suggests the utility of visceral biopsy evaluation for the diagnosis during life.

[Hemodynamic study in hypertensive cardiopathy under ischemia induced by atrial stimulation in the absence of fixed coronary obstructions].

With the purpose to study the haemodynamic changes that occur with myocardial ischaemia induced by atrial pacing (AP) in hypertensive heart disease, we studied 7 patients with such condition, all of them with a long time history of systemic hypertension, electrocardiographic signs at rest of left ventricular hypertrophy and ST-segment depression, at least of 0.5 mm. All the patients showed normal coronary arteries in angiocardiogram. AP was started 10 beats above the basal heart rate with increments of 10 beats every 2 minutes until a ST-segment depression at least of 2 mm was obtained which occurred in all the cases studied. After every 2 minutes of AP a simultaneous 12-leads electrocardiogram recording and left ventricular and aortic pull-back pressure were obtained. At the desired end point the AP was abruptly stopped and the same parameters were registered at 3, 5, 10 and 15 minutes until recovery. During AP the left ventricular systolic pressure (LVSP) did not show any significant change, with the exception of a patient who experienced angor pectoris during the proceeding. The left ventricular end-diastolic pressure (LVEDP) increased in 3.4 +/- 1.7 mmHg, change that was statistically significant (p less than or equal to 0.01) but not hemodynamically important since only in one patient it increased above the normal levels (from 13 mmHg basal to 17 mmHg during AP). In contrast, LVEDP markedly rose above normal when AP was stopped. It is concluded that neither LVEDP nor LVSP play an important role in the genesis of the ST segment depression seen in these patients. It is showed that, similar as in patients with obstructive coronariopathy, these cases work on a depressed Starling curve during AP and its recovery for what is thought that the functional meaning of ischaemia for both entities is similar no matter that their pathogenetic mechanisms are different.

Mustard allergy: three cases of systemic reaction to ingestion of mustard sauce.

Very few cases of hypersensitivity due to ingestion of mustard have been described in the literature, although this spice enjoys widespread use. We present three cases of anaphylactic reactions to ingestion of a small amount of mustard sauce. In our patients we performed skin prick tests with mustard and with the rest of the vegetables belonging to the Cruciferae family. We also performed skin prick tests with common pneumoallergens. We measured specific serum IgE to mustard using the CAP System (Pharmacia). Oral challenges with the other vegetables of the Cruciferae family were performed in the patients with no evidence of previous tolerance. Skin prick tests and specific serum IgE determinations to mustard were positive in all three patients. The tests with the rest of the vegetables belonging to the same family were negative. In conclusion, we consider that skin prick tests and the measurement of specific serum IgE to mustard are good methods for the diagnosis of mustard hypersensitivity. We did not detect cross-sensitivity with other vegetables of the same family or with pneumoallergens.

Relapsing focal myositis: a case report.

We present a 9 year old boy with a focal myositis presenting as a pseudotumor on the right calf. Other smaller masses were present in the thigh and pretibial area on the same leg. The principal lesion recurred after two surgical excisions. Corticosteroid treatment was unsuccessful. However, methotrexate therapy resulted in a progressive regression of the lesions.

Effects of macrophage depletion at different times after treatment with ethylene dimethane sulfonate (EDS) on the regeneration of Leydig cells in the adult rat.

Testicular macrophages were selectively depleted in the right testes of adult rats by an intratesticular injection of dichloromethylene diphosphonate-containing liposomes (Cl2MDP-lp), whereas the left testes were injected with 0.9% NaCl and served as control. Before or after Leydig cell destruction with ethylene dimethane sulfonate (EDS), treatment with Cl2MDP-lp/NaCl was given at different times to study the requirements of macrophages in the different stages of Leydig cell regeneration. On day 30 after EDS treatment, new Leydig cells were abundant in the left, macrophage-containing testes. However, in the right, macrophage-depleted testes, the number of Leydig cells was related to the time elapsed between EDS treatment and macrophage depletion. When macrophages were depleted on day 10 before or on days 4 or 10 after EDS treatment, new Leydig cells were nearly absent at 30 days. However, when macrophages were depleted on days 16 or 22 after EDS treatment, Leydig cells were found at 30 days, but their numbers were equivalent to the number of Leydig cells that were already present in EDS-treated animals at the time the macrophages were depleted. These results indicate that macrophages are needed for the differentiation of Leydig cells from mesenchymal precursors, as well as for the proliferative activity of the newly formed Leydig cells, possibly through the secretion of essential growth factors.

[Intraparotid facial neurilemmoma].

Neurilemmomas or schwannomas are known to arise from the sheath of peripheral nerves. Out of the cranial nerves, the eighth nerve is the most frequently involved. Neurilemmomas arising from the facial nerve are are, and they usually affect the intratemporal portion of the nerve. Its location in the intraparotid portion is very uncommon. The case of a patient having an intraparotid mass is shown. After surgery it was found to be a neurilemmoma of the facial nerve. These tumors manifest as an asymptomatic mass in the parotid gland. Most of the times, diagnosis is made postoperatively. It is necessary to remove all the tumor with the caution to preserve the original nerve.

[Laryngeal reinnervation: transposition of neuromuscular pedicles].

Following Tucker's technique, a muscle-nerve pedicle implantation in the denervated posterior cricoarytenoid muscle was performed in five dogs. Results were evaluated after six months by videolaryngoscopy, evoked muscle action potentials and histomorphometric studies. Partial motion recovery of the reinnervated side was observed endoscopically in three dogs. Histomorphometric evaluation showed evidence of muscle reinnervation in all of the five dogs. However, no muscle action potentials were obtained in the posterior cricoarytenoid muscles after stimulation of the ansa hypoglossi branch. These data suggest that the ansa hypoglossi neuromuscular pedicle does not result in reinnervation, and that reinnervation of the posterior cricoarytenoid muscle may occur via other neuromuscular pathways.

Serum sickness due to bovine serum albumin sensitization during in vitro fertilization.

Of 32 women included in the in vitro fertilization (IVF) program in our hospital in 1987, in whom a medium containing bovine serum albumin (BSA) (Menezo's medium) was employed for rinsing follicles, 5 (15%) developed a symptom complex compatible with serum sickness within 8-12 days after oocyte retrieval by echographic puncture. All the patients had specific IgG antibodies against BSA, and intradermal skin testing with BSA and Menezo's medium were positive. We could not demonstrate the presence of specific IgE against BSA in serum by RAST, probably due to the presence of high levels of specific IgG antibodies, which can interfere in the RAST procedure. Statistical analysis showed that the volume of Menezo's medium was significantly greater (p < 0.05) in patients developing the disease. The risk of disease development is directly related to the amount of heterologous protein administrated.
