Effect of essential fatty acid deficiency on maternal, placental, and fetal rat tissues.

Prolonged dietary deprivation is needed to produce essential fatty acid (EFA) deficiency. But lack of EFA also impairs reproductive function. Inclusion of small amounts of linoleic acid in the diet can overcome this difficulty; further, if large amounts of oleic acid are included in the diet, this competes with the utilization of 18:2 producing EFA deficiency. Using this approach, female rats were fed a fat-free diet containing 5% oleic acid w/w (with 2-3% 18:2) as the only source of fat for 4 months. They were mated and on the 21st day of gestation, the fatty acids of fetal tissues, placenta, maternal liver and plasma were analyzed and compared to controls on a stock diet. Fetuses from the experimental group were smaller and contained higher amounts of 18:1 and 20:3 omega 9 indicating EFA deficiency. The omega 6 fatty acids in the polar lipids of placenta of the EFA-deficient group were not significantly lower than the controls, in spite of lower concentrations in the maternal plasma, suggesting a unique capacity of the placenta to concentrate omega 6 fatty acids, which in turn may be utilized for prostaglandin synthesis needed for inducing labor and other vascular changes in the fetus.

Comparison of the barium test meal and the gamma camera scanning technic in measuring gastric emptying.

In 21 patients with nonresected stomachs and symptoms of delayed gastric emptying, obstruction was excluded by upper gastrointestinal series and upper endoscopy; all had abnormal results of barium test meal (BTM) study. Each had repeat BTM after the administration of 10 mg of metoclopramide. Each patient also had two gamma camera studies after a technetium Tc 99m sulfur colloid labeled meal; normal saline or metoclopramide was administered before each test in a blinded and random manner. Half-time (T1/2) and percentage of isotope remaining at six hours (GC6) were recorded. Ten asymptomatic controls had a gamma camera scanning study, and seven of these had a BTM. Nine of 19 patients had a T1/2 in the normal range, and in 12 of 19 patients the GC6 was in the normal range. The magnitude of retention of barium at six hours on the BTM did not correlate with the T1/2 (r = 0.076) or the GC6 (r= 0.296). Thus, these tests were not comparable in this study. By regression analysis, a significant reduction was shown in the amount of retained food and barium (P less than .01), the T1/2 (P less than .01), and the GC6 (P less than .01) after intramuscular administration of metoclopramide, indicating that both tests were able to evaluate the effects of this drug.

The relevance to family physicians of core content review: evaluation of a program of continuing education.

The Core Content Review of Family Medicine was assessed for its relevance to family practice. Three sets of questionnaires were completed and assessed by 48 experienced, certificated family physicians, of whom half were faculty members from university departments of family medicine and half were in private practice. The answers to 57% of the questions were correct. In the opinion of the panelists the answers to 62% of the questions permitted discrimination between physicians providing care of good quality and those providing care that was less than adequate. The panelists rated 78% of the questions as being educationally useful. Two thirds of the questions were deemed relevant according to a composite criterion. Examples are given of the questions considered most relevant and of those considered least relevant.

Association of participation in a nutritional program for the elderly with nutritional status.

The nutritional status of 547 participants of the Title VII Nutrition Program for the Elderly was assessed by anthropometric, biochemical, and dietary means. The nutriture for vitamins A and C was positively associated with frequency of participation in the program using biochemical and dietary methods of assessment. A significant smaller percentage of persons who participated in the program on a regular basis had inadequate intakes of riboflavin and thiamin. Fewer persons had anemia and low serum iron concentrations than was reported in an earlier baseline study. However, in this group of persons frequency of participation was not associated with anemia or serum iron-concentrations. Participation in the program was not significantly related to the dietary intake of energy, saturated fat, and cholesterol or to the incidence of obesity when assessed using triceps skinfold thickness, ponderal index, body mass index, and percentage of desirable weight. It can be concluded that the Title VII Nutrition Program for the Elderly is associated with improvement in the nutritional status of the participants.

Ultrastructural evaluation of inflammatory carcinoma of the breast.

A biopsy of a confirmed case of inflammatory carcinoma of the breast was examined by light and electron microscopy, and the results were compared with the estrogen receptor content. Although the tumor appeared undifferentiated by light microscopy, fine structural analysis revealed some evidence of differentiation. Few cells showed signs of secretory activity. The perilymphatic stroma contained few isolated neoplastic cells and some foci of tumor aggregates. In the latter, attempts to organize were made evident through the formation of a common lumen. Estrogen receptor content was determined by titration and sucrose gradient methods; both gave results indicating the absence of estrogen receptor in the biopsy specimen. The ultrastructural features of the tumor were consistent with the receptor data. Collectively, the evidence suggested that response to endocrine therapy by this patient was unlikely, and alternative means of treatment were adopted. We suggest that electron microscopic studies of such lesions may be useful in diagnosis and treatment planning.

The effects of coherin on patients with idiopathic delayed gastric emptying.

Ten patients with idiopathic delayed gastric empyting were studied in a randomized double-blind fashion to examine the effects of coherin versus placebo upon their symptoms and objective measurements of gastric emptying. The results showed no significant effect of coherin on either symptom scores or gamma camera 99Technetium gastric emptying studies in this group of patients. There is a need to develop other agents in treatment of this poorly understood condition.

Nurse training and staffing in the neonatal intensive care unit.

Most nurses are unfamiliar with the highly sophisticated level of technology that must be utilized in the delivery of care in an NICU. This article has presented the measures that are utilized at one medical center to provide the nursing staff for its NICU. The presentation focused on the organization, selection, and development of a nursing staff that is capable of delivering a sophisticated level of care to sick neonates. However, it should be emphasized that it is essential to be alert to changes in patient therapies and related technology that may well alter the practice of neonatal intensive care nursing.

Metoclopramide therapy in fifty-five patients with delayed gastric emptying.

Fifty-five patients with delayed gastric emptying and the symptoms of nausea, vomiting, postprandial bloating and early satiety were treated with metoclopramide. Obstruction was excluded by upper endoscopy and standard upper gastrointestinal series. None were on medication known to retard gastric emptying. All patients had an abnormal barium burger radiologic study. Twenty-one patients had had previous vagotomy and drainage procedure, five had diabetic gastroparesis and 29 had idiopathic delayed gastric emptying. Metoclopramide significantly decreased the symptom scores of the surgical and idiopathic patients. When all patients were analyzed together, there was a significant improvement in both the metoclopramide and placebo treated patients. When, however, the improvement on metoclopramide was compared to the improvement on placebo, there was a significant metoclopramide effect beyond the placebo effect. Thus, metoclopramide is an effective agent in treating the symptom-complex of patients with delayed gastric emptying.

Metoclopramide therapy in patients with delayed gastric emptying: a randomized, double-blind study.

Twenty-eight patients with delayed gastric emptying as measured by an abnormal barium "burger" were treated with metoclopramide in a randomized, double-blind fashion. Five had diabetic gastroparesis, four had undergone vagotomy and pyloroplasty, and 19 were idiopathic. Patients received either metoclopramide or placebo for a three-week period and symptoms were scored prestudy, at weekly intervals, and at termination of the study. Ten of 17 patients on metoclopramide and four of 14 on placebo decreased their symptom score to a level below entry criteria, indicating a significant metoclopramide effect when compared to placebo. The mean total sumptom score for the metoclopramide group was 18.4 prestudy and 7.2 poststudy while for placebo was 19.1 prestudy and 12.9 poststudy. Although improvement on placebo was significant, these patients were still symptomatic. The improvement on metoclopramide was significantly greater than the improvement on placebo. Metoclopramide is an effective agent in treating the symptom complex of delayed gastric emptying.

Autoimmune implications of immune complexes in clinical variants of hepatitis B.

Immune complexes (IC) were investigated in sera from 208 individuals with various clinical types of viral hepatitis diagnosed by clinical and laboratory criteria, including liver biopsy. Immune complexes were assessed by platelet aggregation (PI A) and by radioimmunoassay (RIA). The data were related to autoimmune phenomena (especially rheumatoid factors) and to the role that the IgM class of hepatitis B (HB) antibody might have in IC formation. Although the highest frequency of P1 A was in the few sera from patients with cirrhosis or hepatoma, the next highest was in sera from acute hepatitis patients (71%), and the lowest in sera from chronic active (57%) and chronic persistent (46%) hepatitis patients. A proportional number of patients with IC's were positive for hepatitis B surface antigen (HBs). A parallel prevalence was noted between P1 A and autoantibodies, with anti-Ig's being found more frequently in sera from acute hepatitis and chronic active hepatitis patients. The relationship between RIA results for complexes and RIA results for anti-IgG was inverse, as though anti-IgG interfered with IC reactivity by RIA. Anti-IgM pre-incubated with sera increased the amount of P1 A in sera from patients with acute hepatitis as well as in those from patients with chronic persistent hepatitis, suggesting a more frequent IgM involvement in IC's in these diseases than in chronic active hepatitis. Whereas liver cell damage in acute and active hepatitis may reflect elevated autoantibodies, the IgM class of HBs antibody may be involved in acute as well as chronic persistent hepatitis.

Membrane lipid modifications: biosynthesis and identification of phosphatidyl-N-methyl-N-isopropylethanolamine in rat liver microsomes.

In previous studies on the modification of polar head groups of membrane phospholipids with the unnatural base analog, N-isopropylethanolamine, we reported an unidentified phospholipid in addition to phosphatidyl-N-isopropylethanolamine in the various membrane fractions of rat liver. The structure of this phospholipid has now been identified as phosphatidyl-N-methyl-N-isopropylethanolamine by nuclear magnetic resonance spectroscopy, and by chromatographic and enzymic analysis. In addition, we found that when rats were injected intraperitoneally with the N-methyl-N-isopropylethanolamine, 19% of the liver microsomal phospholipid was phosphatidyl-N-methyl-N-isopropylethanolamine.
