Plasma tocopherol concentrations in response to supplemental vitamin E.

Normal healthy volunteers were studied after various doses of dl-alpha-tocopherol were ingested on different schedules. Administration of 440, 880, or 1320 mg (400, 800, or 1200 IU) of dl-alpha-tocopherol as a single dose resulted in elevation of plasma alpha-tocopherol concentrations that peaked at 12-24 h. Chronic administration of dl-alpha-tocopherol (440, 880, or 1320 mg/d for 28 d) resulted in a steady state that occurred by days 4-5 of supplementation. Discontinuation of the treatment after day 28 was associated with a decline of plasma alpha-tocopherol, which returned to the pretreatment concentrations between 12 and 20 d. The plasma elevation of alpha-tocopherol was affected by dietary fat intake. Individuals consuming a high-fat diet showed significantly greater plasma alpha-tocopherol concentrations as compared with those fed a low-fat diet. The results of this study indicate that plasma kinetics of alpha-tocopherol are similar when supplements of 440, 880, or 1320 mg dl-alpha-tocopherol are given to normal, healthy individuals.

Deep fascia of the foot. Anatomical and clinical considerations.

The deep fascia of the foot lies beneath the subcutaneous tissue and surrounds the intrinsic foot muscles. Depending on its location, the composition of the deep fascia varies. In some areas it is thin, while in other areas it is greatly thickened to form retinacula and the plantar aponeurosis. Selected clinical considerations that relate to the deep fascia of the foot are described. These include the following: plantar fasciitis, infection, compartment syndrome, calcaneal fracture, and neuroma.

Serum liver enzyme and histopathologic changes in calves with chronic and chronic-delayed Senecio jacobaea toxicosis.

Progressive changes in serum enzyme activity and liver histologic features were monitored in calves fed tansy ragwort (Senecio jacobaea)-contaminated pellets. The experiments were designed to simulate natural intoxicant ingestion conditions in relationship to the dose and duration of exposure to the toxic plant to correlate early laboratory diagnostic changes with the natural progression of the disease, thereby facilitating early diagnosis and intervention by veterinary clinicians. Eight calves were fed tansy ragwort and 4 additional calves served as controls. In group 1, 4 calves were continuously fed dried tansy ragwort mixed in a pelleted feed at a 5% concentration by dry weight until terminal liver disease developed. Serum liver enzyme (alkaline phosphatase, glutamate dehydrogenase, and gamma-glutamyltransferase) activities were monitored at weekly intervals in these calves and in the 2 controls. In group 2, 4 calves were fed the same contaminated feed for only 60 days, with return to normal feed for the duration of the trial. Two additional calves served as controls. Their liver enzyme activities were monitored every other week in conjunction with percutaneous liver biopsies. All 8 calves fed tansy ragwort-contaminated pellets developed terminal hepatopathy in either a chronic pattern (n = 6) or a chronic-delayed pattern (n = 2), with the onset of a moribund state or sudden death at 11 to 17 weeks and 27 to 51 weeks, respectively. The calves were euthanatized when classic terminal signs of hepatic encephalopathy first became evident. The clinicopathologic patterns of chronic and chronic-delayed toxicoses were typical of over 5,000 cases of field tansy toxicosis diagnosed at the diagnostic laboratory. Serum glutamate dehydrogenase was the first enzyme to increase in most animals, with a short-term increase to peak values followed by a rapid return to normal. This enzyme change was followed by increases in alkaline phosphatase and gamma-glutamyltransferase. Serum enzyme changes preceded development of recognizable histologic lesions. Vacuolar changes in hepatocyte nuclei, biliary hyperplasia, and fibrosis sequentially developed in liver biopsy specimens from each animal, whereas megalocytosis was not a predominant feature until necropsy. On the basis of our findings, we suggest that the optimal tests for diagnosis of pyrrolizidine alkaloid intoxication should consist of liver biopsy and determination of concurrent serum liver-enzyme activities.

[Helicopter and emergency physician at the accident site. Medical comparison between altitude and flat land emergency care].

In-field intensive care reduces lethality and morbidity in severe trauma. We analyze and compare 400 medical helicopter rescue flights in the region of Basel and 325 medical air-rescues in the mountains of Interlaken. The presence of a physician in the helicopter is based on an interventional concept, with clinical diagnosis, monitoring and early intensive in-field care. Compared to the mountains, we find a predominance of road accidents (44%) in the region of Basel, with more head injuries (47.3%) and more polytrauma (19.4%) and thoraxtrauma (20.5%). In the mountains, with a majority of sport accidents (65.5%), pelvis and extremity injuries (73.4%) prevailed. As a consequence of inaccessible accident sites in the mountains, 23% of the rescues had to be performed by winch. An endotracheal intubation was performed in 35.5% of the patients in the Basel region, and in 11.6% in the mountains. Fixation of fractures and hemostasis was performed in both regions in 22% and 12% of the cases. In spite of high rates of severe conditions in both regions (57% in Basel, 17% in the mountains), the low mortality prior to and during air transport (2.5% in the Basel and 2.7% in the Interlaken region) confirms the necessity of the concept of early in-field intensive therapy. This concept allows good analgesia, is save and comfortable for the patients. The presence of a physician in the air rescue team has to be recommended not only for rescues in the plain, but also in the mountains.

[Mechanical trans-sutural colorectal anastomosis in the treatment of cancers of the rectum. Apropos of 70 cases].

The reduction in the acceptable length of distal margin below the tumor makes low anterior resection more suitable for rectal cancers. The technical difficulties of low anastomosis are reduced by performing stapled colorectal anastomosis through the suture. Seventy patients were submitted to this procedure. There was no operative mortality, while the reported morbidity was 14%. Sixteen patients underwent preoperative radiotherapy of 30 Gy without any subsequent leakage. This technique is discussed in relation to the hand-sewn colo-anal anastomosis.

[Surgical treatment of pyloric stenosis of ulcerous origin. Apropos of 68 cases].

The study turns about 68 cases of post ulcerous pyloric stenosis operated from 1970 to 1990, that means 8.5% of the duodenal ulcers (796 cases) operated during this period. The stenosis was entire or strangled in 27 cases (39.7%) with an important gastric dilatation in 34 cases (50%). The treatment has been composed of a gastroenterostomy (30 cases--44%), most of the time with a troncular vagotomy, of a pyloroplasty with vagotomy in 20 cases (29.5%) and of an inferior polar gastric resection in 18 cases (26.5%). The operative mortality was 2 cases (3%). The at distance-results (average follow-up of 7 years) show that 71% of the re-examined patients (45 cases) are classified Visick 1 or 2 and 9% visick 4. Ones concludes to the value of troncular vagotomy with gastroenterostomy which can be realised in all of the occasions and gives good functional results when the vagotomy with pyloroplasty seems to lie more frequently in state to uncertain functional results.

Purification and characterization of a 7Fe-ferredoxin from Rhodobacter capsulatus.

A ferredoxin was purified anaerobically from Rhodobacter capsulatus grown photoheterotrophically with excess ammonia. This ferredoxin, called ferredoxin II (FdII), had a molecular weight of approximatively 15,000 by gel filtration and 14,000 by SDS polyacrylamide gel electrophoresis indicating that it is monomeric. Its absorption spectrum (oxidized form) exhibited maxima at 280 nm and 400 nm; the A400/A280 ratio had a calculated value of 0.55. Chemical determination of its iron and sulfur atom content, the value of the extinction coefficient at 400 nm (epsilon 400 = 26.8 mM-1 cm-1) and EPR spectra indicated that ferredoxin II contained one [3Fe-4S] and one [4Fe-4S] cluster. Upon reduction with excess dithionite only the [3Fe-4S] cluster became reduced. The reduction of both clusters was achieved by using 5-deazaflavin as photocatalyst. Ferredoxin II was also purified from bacteria grown under nitrogen limiting (nif derepressing) conditions. In in vitro assays, ferredoxin II catalyzed electron transport between illuminated chloroplasts and nitrogenase.
