[What Swiss psychiatrists think of schizophrenia].

The author has made an inquiry with all the swiss FMH psychiatrists, the subject being their opinion on the origins of schizophrenia. 69% answered. The majority of psychiatrists questioned, think that it is impossible to establish priorities between the psycho-genetical determinatives on one hand and hereditary and neuro-biochemical components on the other. The author was able to establish a ststistically significative correlation between the age of the psychiatrists and their opinion on the origins of schizophrenia. On the other hand, their opinion on the origins of schizophrenia was not influenced by the fact that a member of their family was schizophrenic.

[Diagnosis and first aid in stroke during the prehospital phase].

In the GDR the apoplexy occupies the 4th place in the statistics of causes of death. It is an acute pathological process and has a firm place in emergency medicine (urgent home visit service and urgent medical aid). The symptomatology is multiform and under ambulatory conditions it may lead to considerable differential-diagnostic difficulties. In the present paper references are given taking into consideration the anamnesis (factors of risk), the clinical examination of the patient, first differential-diagnostic aspects, therapeutic methods resulting from this and suitable possibilities of hospitalisation.

[Pre- and postoperative manometeric findings in the esophagus in proximal selective vagotomy].

15 patients with duodenal ulcer underwent clinical and manometric studies to assess distal esophageal function before and after proximal selective vagotomy. Position, pressure and function of distal esophageal high pressure zone were not altered by the operation. No rationale has been found for the trend to combine proximal selective vagtomy with antireflux procedures.

[Problems of bilinguism in psychotherapy].

The authors have attempted an introductory study of problems which are inherent to psychoanalytically oriented psychotherapy and psychoanalysis conducted in the second language of a bilingual therapist. This situation seems to be more complex than is usually admitted in current literature. The main problem encountered by the authors in their personal practice stems from the fact that the process of identification becomes complex when the therapist is confronted with regressive and/or progressive shifts in the course of the treatment.

Carbon-14 tracer studies in rat-liver perfusion experiments under conditions of gluconeogenesis from lactate and pyruvate.

The intracellular events in the metabolic pathway of gluconeogenesis from lactate and pyruvate in liver tissue were assumed to be understood. Nevertheless the results of several 14C-tracer experiments gave rise to the postulation of still unknown intracellular interactions under this condition. We have tried to contribute to the solution of this problem by using different 14C labeled tracers such as [1-14C]lactate or pyruvate and [2-14C]lactate or pyruvate, [14C]bicarbonate and [1-14C]octanoate in perfusion experiments with livers from rats under conditions of gluconeogenesis from lactate and pyruvate. The 14C labeling patterns of intracellular metabolites as malate, citrate, phosphoenolpyruvate, phosphoglycerate and newly synthesized glucose were analysed under different conditions. A comparison with values calculated by using metabolic models, based on the generally accepted concepts of intracellular interactions, showed some fundamental discrepancies, which justify the postulation of still unknown intracellular interactions in the pathway of gluconeogenesis from lactate and pyruvate.

Carbon-14 tracer studies in the metabolism of isolated rat-liver parenchymal cells under conditions of gluconeogenesis from lactate and pyruvate.

In rat liver perfusion experiments under conditions of gluconeogenesis from lactate and pyruvate 14C-labeling patterns of metabolites with (1-14C)-labeled and (2-14C)-labeled lactate or pyruvate, [14C]bicarbonate and [1-14C]octanoate as tracers, have been obtained, which do not agree with generally assumed reaction schemes. The experiments have been repeated with incubations of isolated rat-liver parenchymal cells. The results demonstrate that the discrepancies between expected and analysed 14C-labeling patterns of metabolites were still existent. From this observation it may be concluded, that 14C-labeling patterns of metabolites are indicative for the existence of still unknown metabolic relationships in liver parenchymal cells. Furthermore the results of our experiments prove that conclusions based on the exclusive analysis of metabolite levels are of limited value for studying intracellular events, because of uncharacterized compartmentations, which become evident in 14C-tracer studies. It cannot be answered by our studies, whether the apparent existence of differently labeled species of citrate, oxoglutarate or acetyl-CoA, represent intracellular compartmentation or whether it is the result of metabolic heterogeneity of liver parenchymal cells.

[Cancer mortality in patients of psychiatric hospitals].

Studies on cancer mortality in psychiatric patients, especially schizphrenics, are reviewed. The divergences between these studies may be partially explained by the different statistical methods employed. It is difficult to compare the populations observed, due to the influence of such elements as the method of sampling, the period of observation of psychotic patients, diagnostic methods and criteria etc. On the whole, it is concluded that no major difference could be demonstrated with certainty between a non-selected population of psychotic patients (excluding psychoorganic cases) and the general population. With regard to schizophrenics, the hypothesis of a lower mortality from cancer cannot be ruled out on the grounds of studies carried out at the Lausanne psychiatric clinic. However, reports on cancer incidence have not yet confirmed this hypothesis and further studies are needed to verify it. Various factors are discussed which may modify cancer risk in psychiatric patients in hospital such as tobacco, food, sexual activity, neuropleptics, and biochemical and immunological factors. Lastly, the psychosomatic hypothesis of carcinogenesis is discussed in the framework of this review.

A mechanical momentum in ultrastructural development of the heart.

The question of whether mechanical moments participate in ultrastructural development of the heart cannot be decided by examining heart tissue samples during embryonic life, for during maturation processes in vivo mechanical factors are always present. The significance of mechanical moments can only be investigated in a system which allows a distinction to be made between pulsations of heart myocytes under conditions of contraction against and under conditions of contraction without mechanical stress. This double possibility is provided by cell culture. An example in which mechanical influences are absent are the so-called "minihearts"; these are cell agglomerations which are detached from the culture substrate. Their cell contractions do not act against any mechanical resistance. In the electron microscope they show abundant filaments, Z-line material and membrane specializations. The highest degree of development, however, does not exceed that characterizing the cells with which the cultures were started. Even some dedifferentiation and degeneration are apparent in the minihearts. No intercalated discs can be found even after 12 weeks, provided that the culture had been started with really isolated cells. The representatives in culture of a myocyte's connection with the heart's mechanical action are the cords, along which attached points alternate with free strands. The pulsation of these strands is restrained during each contraction by their ends, which are fixed on the culture substrate. Thus, the myocytes contract against a resistance, and in this respect their pulsations resemble those of the heart, in which the myocytes' contraction acts against the pressure of the blood. The myocyte culture under these mechanical condition, after a culture time of 10 weeks, produces electron micrographs of mature cells with reduced cytoplasm, aligned mitochondria and fibrils and intercalated discs.

[The technique of proximal selective vagotomy (author's transl)].

A standard technique of proximal selective vagotomy is described (PSV) that has proved reliable and safe in a broad multicenter clinical trial. We proceed in six steps: (1) determination of the antral-fundic boundary by anatomic criteria; (2) preliminary vagomotor electrotest; (3) identification of the nerve trunks and branches to be preserved; (4) dissection of the lesser curvature; (5) dissection of the cardia and of the intraabdominal esophagus up to 6 cm above the cardia; and (6) intraoperative assessment of completeness by the vagomotor electrotest. PSV is usually carried out without a drainage procedure.

[Evaluation of treatment with hemineurine in delirium tremens].

The authors make a sum-up of all cases of delirium tremens admitted to the Lausanne University Psychiatric Clinic during the period 1970-1974. All were treated by intravenous and oral chlormethiazole (hemineurine), its technique being described. The need to restablish the fluid and electrolitic balance is stressed. Side effects are not important and there is a shortening of the acute phase, as compared to other techniques. Mortality is nil. The authors suggest that chlormethiazole is an indication of choice in the treatment of delirium tremens and are surprised that this substance has not yet taken its place in the U.S.A.

[Repeated sessions of sleep deprivation in the treatment of depression].

30 depressed patients were submitted to repeated sessions of sleep deprivation. The results obtained in a previous study were confirmed: after 36 hours of wakefulness there is an important overall improvement, albeit followed by a partial or total relapse the second day after. There are no counter-indications, even for elderly subjects with psycho-organic symptoms.

[Considerations on psychotherapy and sociotherapy in the aged].

Psychotherapy of the elderly has assumed increasing importance in recent years. W. Schulte, G. BENEDETTI, BEREZIN and others have expressed the view that the indication and methodology of such therapy must take into consideration the limitations imposed by age-related factors such as rigidity, concentration difficulties and a tendency to negate the proximity of death. Insight into the finiteness of life, brought about by the approach of death, is seen by the author as a basic problem in any psychotherapy of the elderly. However, the therapist should beware of unwarranted optimism. The practical procedure occasionally resembles that of "symbolic realization". Sociotherapy, however useful it may be, also involves the danger of camouflaging the problems surrounding the end of life. sabove all, the author sees a need to improve geriatric institutions and criticizes the lack of human warmth frequently observed in the treatment of the inmates of old people's homes.

Aging of schizophrenic patients as seen through the Rorschach test.

The authors have repeated, on the average 29.7 years later, Rorschach test in 11 schizophrenic men and 19 schizophrenic women. This work was carried out in the course of a follow-up study of schizophrenics more than 65 years of age. Analysis of the records showed that the only differences between the first and second examinations were increases in animal and popular responses in old age. The authors have tried to relate these findings to the general trends of schizophrenic development in senescence.
