Epidemiology of human ancylostomiasis among rural villagers in Nanlin County (Zhongzhou village), Anhui Province, China: age-associated prevalence, intensity and hookworm species identification.

Hookworm infection has traditionally been highly endemic to Anhui Province, China. Zhongzhou village in southwestern Anhui was identified as an endemic focus of infection caused by the hookworm Ancylostoma duodenale. This species was found to predominate over Necator americanus in ratios ranging from 35:1 based on the recovery of third-stage infective larvae (L,) to 21:1 based on the recovery of adult hookworms after anthelmintic chemotherapy. The overall prevalence of Ancylostoma infection in Zhongzhou was 33.2% with a greater prevalence among males. Unlike the age-associated prevalence patterns for N. americanus in Hainan and other southern Chinese provinces that show increasing prevalence with age and the highest prevalence among the elderly, the age-associated prevalence for A. duodenale in Zhongzhou exhibited a peak in middle-aged adults with subsequent decline. The age-associated intensity pattern exhibited a similar trend although the most of the hookworm infections were light or moderate infections as defined by quantitative egg counts.

Killing effect of interleukin-2 and interferon-alpha activated leukemic bone marrow in remission on K562 leukemic cells.

OBJECTIVE: To evaluate the killing effect of interleukin-2 (IL-2) and interferon-alpha (IFN-alpha) activated bone marrow cells on K562 cells. METHODS: Semi-solid colony and 3H-TdR incorporating method were used. RESULTS: Bone marrow from leukemia patients in remission was activated in vitro with IL-2 for 3 days. The activated bone marrow (ABM) displayed killing effects of 0.31-2.30 logs on K562 cells. This killing effect was further increased to 0.30-3.15 logs when IFN-alpha added with IL-2 to the marrow for activation. IL-2 alone or in combination with IFN-alpha showed no inhibition of CFU-GM and K562 cells. Compared with IL-2 or IFN-alpha alone, the combination of the two cytokines could more effectively maintain the killing effect of ABM on leukemic cells. CONCLUSIONS: IFN-alpha can augment the purging effect of IL-2 ABM and combination of the two cytokines can effectively maintain the cytotoxicity of ABM.

DNA vaccine pCD-Sj32 and its efficacy of protective immunity against infection of Schistosoma japonicum.

OBJECTIVE: To study protective immunity afforded by murine immunization with DNA vaccine of Schistosoma japonicum (S. japonicum) as measured by reduction in worm burden and host antibody, cytokines. METHODS: DNA vaccine pCD-Sj32 was constructed, identified and expressed. pCD-Sj32 could induce substantial protective immunity against infection of S. japonicum in BALB/c mice. The best efficacy can be produced with one injection of 100 micrograms DNA into the quadriceps muscle, combined with challenge for 8 weeks after immunization. T lymphocyte subsets of CD8+, IL-2. TNF and IFN-gamma of experimental animal could play important roles in regulating immune functions of schistosomiasis. RESULTS: High titre of specific antibody IgG could be induced by vaccinated with pCD-Sj32, and antibody can mediate macrophage to produce ADCC effects in vitro. CONCLUSION: pCD-Sj32 may represent a new approach to developing subunit vaccine.

[Clinical characteristics and psychotropic therapy of depressive and anxious symptoms of patients with somatic diseases].

OBJECTIVE: To explore the characteristics of depressive and anxious symptoms and the drug therapy response of patients with somatic diseases. METHODS: All the subjects were inpatients in a general hospital. The depressive and anxious symptoms met with the following conditions: hospital anxiety and depression scale (HAD) > 7 scores, Hamilton Depression Scale (HAMD) > 12, Hamilton Anxiety Scale (HAMA) > 10, Clinical Global Impression (CGI) > or = 3. RESULTS: In patients 200, 195 received the therapy, and 5 discontinued. The severity of depressive symptom in descending order was tumor group, blood disease group, endocrine disease group, depressive group, cardiovascular disease group, renal disease group. The effects of Seroxat (serotonin selective reuptake inhibitor, SSRI): showed that the overall efficacy against depressive and anxious symptoms, measured with 50% reduction rate in endpoint scales scores, was 61.6% and 61.5% respectively. CONCLUSION: Different characteristics of emotional symptoms existed among different somatic diseases. Seroxat is effective in treating emotional problems of patients with somatic diseases.

[Psychosocial risk factors of Alzheimer's disease].

OBJECTIVE: To explore the relationship between psychosocial factor and risk of Alzheimer's disease(AD). METHODS: A cohort study of people 55 years and older was followed up 10 years after a baseline screening. 5,055 elderly subjects in a district of Shanghai were stratified clustering sampled from community. Information about psychosocial factors was collected in 1987. The clinical diagnoses of dementia and AD were made according to DSM-III-R and NINCDS-ADRDA criteria. 4,896 non-demented persons were followed in 1997. The incidence of AD was calculated and the relationship between the psychosocial factors and the AD was analyzed by Logistic model. RESULTS: 1,203 subjects were resurveyed in 1997 and among them 124 new cases of dementia including 81 cases of AD were identified. By computing the relative risk(RR) of psychosocial factors to the AD, many psychosocial factors were found to be correlated with AD including blue collar occupation, no job, no reading or writing, no taking community activities, no gardening in leisure, bad psychological feeling and lower education (RR > 1). The items with high PARP > 50% were not working, no job, retirement, no taking community activities, no reading or writing, blue collar occupation and no gardening in leisure. MMSE total score is a good predict for AD. CONCLUSION: Psychosocial factors play an important role in the development of Alzheimer's disease.

[Effects of chloroquine on insulin sensitivity in insulin-resistant rats].

OBJECTIVE: To investigate the mechanism of chloroquine improving insulin sensitivity. METHODS: The changes in insulin sensitivity and the effects of chloroquine were observed in insulin-resistant (IR) rats induced by high-fat feeding. RESULTS: In IR rats, the glucose infusion rate (GIR) determined by euglycemic clamp and 2-deoxyglucose (DG) uptake in the skeletal muscle were lower than those in normal rats, but the activity of insulin degradation of liver (LIA), fasting plasma insulin (FINS) and plasma lipids were higher. In the high-fat fed rats with chloroquine administration, GIR and 2-DG uptake in the skeletal muscle were increased, and LIA, FINS and some plasma lipid indices were decreased compared with the IR rats. CONCLUSION: Chloroquine can improve the insulin sensitivity and hamper the onset and development of insulin resistance.

Quantitative assessment of Parkinson's disease deficits.

OBJECTIVE: To quantitatively analyze the tremor and rigidity due to Parkinson's disease. METHODS: 38 patients with Parkinson's disease (PD) ranging in age from 45 to 72 years and 211 normal subjects aged from 16 to 76 years were investigated. The frequency and range of tremor, the muscle tone of the upper limbs in elbow were detected by a computerized video motion detecting system and a new invented apparatus which can detects skeletal muscle tone. RESULTS: For the PD patients, the frequency of resting tremors was detected in 4 to 6 per second. For extensor and flexor in the PD patients, the value of muscle tone was higher than that of normal subjects and the value of muscle tone in flexor was higher than that of extensor. The rigidity increased gradually with repeat passive movement. The curves of rigidity were shown on computer screen or printed out. The data of rigidity were compared with the M-A Scale. A patient who was suspected to suffer from PD above by the equipments and found the muscle tone was higher than normal. In another PD patient the rigidity was obvious at one side and the muscle tone in "normal side" was also high. These equipments were used to record changes of rigidity and tremor in one more PD patient taking with different drugs in order to see the drug effect. CONCLUSION: Quantitative methods are useful to analyse the motion disorders due to PD.

[Acute lymphoblastic leukemia complicated with tumor lysis syndrome--four cases report].

OBJECTIVE: To report four acute lymphoblastic leukemia(ALL) patients who developed tumor lysis syndrome(TLS) after initial chemotherapy. METHODS: The clinical features and blood biochemical changes of four patients with TLS were analysed. RESULTS: TLS was characterized by hyperuricemia, hyperkalemia, hyperphosphatemia and hypocalcemia. Three cases had renal dysfunction and the other died of acute renal failure. After allopurinol therapy and alkalinization of urine, the blood biochemical parameter became normalization in all the three cases. CONCLUSIONS: TLS can be effectively controlled by early recognition and administration of allopurinol therapy and alkalinization of urine.

[Pure red cell aplasia after allogeneic peripheral blood stem cell transplantation: a case report and literature review].

OBJECTIVE: To report a case of post-allogeneic peripheral blood stem cell transplantation (allo-PBSCT) pure red cell aplasia(PRCA) and the treatment outcome. METHODS: A patient with acute non-lymphoblastic leukemia(M2a) was treated with allo-PBSCT. His post-PBSCT PRCA was treated with plasmapheresis(2-3 times weekly) and Epo(3,000 U/day, subcutaneously). RESULTS: Reticulocyte count (Ret) recovered to 0.01 at day +131 and hemoglobin reached 110 g/L at day +154. Erythroid cells in the bone marrow recovered to 0.25, and BFU-E and CFU-E within normal range. This normalized hematopoiesis remained for over 5 months after the cessation of Epo. CONCLUSION: Post allo-PBSCT PRCA can be successfully treated with plasma exchange and Epo.

[Clinical application of impulse oscillometry].

OBJECTIVE: To discuss the clinical significance of impulse oscillometry(IOS) to determine the resistance of large and small airways and the compliance of lung. METHODS: The IOS indices of 40 normal cases, 50 cases of chronic obstructive pulmonary diseases (COPD) and 60 cases of pleural effusion were determined at 5-35 Hz. Their FEV1/FVC and maximal expiratory flow-volume curve (MEFV curve) were measured in conventional method at the same time. RESULTS: Compared with normal cases, the peripheral resistance of the 50 patients with COPD rose obviously and the central airway resistance rose in some of them, the resonance frequency increased, but the peripheral reactance descended at the same time (P < 0.01). Resistance curve show markedly nonlinear decreasing course with increasing frequency. The resistance increased and the reactance descended in end-expiration. FEV1/FVC < 50%, MEFV curve showed severe obstructive shape. 60 patients with pleural effusion were found the resistance increasing and reactance descending to negative values in end-inpiration at low frequencies (inverse values relation at low frequencies). FEV1/FVC were nearly normal, MEFV curve showed restrictive dysfunction shape only in half of them. CONCLUSIONS: IOS can reflect variation of airway resistance and lung compliance. The future development of IOS could be prosperous.

[Clinical study on acute upper respiratory tract infection treated with qingkailing injection].

OBJECTIVE: To explore the effect of Qingkailing injection (QKLI) in treating acute fever. METHODS: Four hundred cases of acute upper respiratory tract infection were divided into three groups treated with QKLI 120 ml/d, 160 ml/d and 200 ml/d respectively, and compared with a control group treated with lincomycin 1.8 g/d. RESULTS: The markedly effective rate of QKLI was 84.14% which was better than that of lincomycin (75.83%). The mean initial time of QKLI (12.6 h) was shorter than that of lincomycin (17.6 h). Effect of QKLI on patients with disease course within one day was better than that within 3 days. No significant difference revealed between effects in groups treated with different dosage of QKLI. The effect of QKLI in lowering white blood cell count was similar to that of lincomycin. For patients with positive throat swab culture of pathogenic bacteria, QKLI displayed good antibacterial action in vivo. CONCLUSION: QKLI is a highly effective with rapid action drug for treatment of acute respiratory infection.

[Abnormal dynamic electrocardiogram in patients with acute cerebral infarction].

OBJECTIVE: To assess the effect of lesions of insular cortex on cardiac damages in patients with cerebral infarction. METHODS: 24-hour dynamic electrocardiogram recordings was measured were performed in 74 patients with hemispheric cerebral infarction in the acute phase and in 39 control subjects. RESULTS: Supraventricular arrhythmias were significantly increased in groups of right insular infarction as compared with groups of lesions in other regions and with the control. A significantly increased rate of ST segment abnormality was found in groups of left insular infarction as compared with groups of lesions in other regions and with the controls. CONCLUSION: The effect of hemispheric cerebral infarction on heart function are mainly correlated with destruction of insular or regions adjacent to the insular. Increased rate of supraventricular arrhythmias is associated with right insular infarction. While increased rate of ST segment abnormality is associated with left insular infarction.

[Methodology of DNA fingerprinting and its application in identification of M. tuberculosis].

OBJECTIVE: To explore methodology of DNA fingerprinting technique and its application in identification of strains of M. tuberculosis. METHODS: This research was based on different copy numbers and location of chromosome DNA IS6110 in M. tuberculosis genome. After DNA of M. tuberculosis were cut by endonuclease PvuII, the products were transferred to nylon membrane, then detected and hybridized by enhanced chemiluminescent (ECL) labeling techniques. RESULTS: Fifty clinical isolates from different TB patients had unidentical DNA fingerprinting patterns. DNA fingerprinting patterns showed a great difference from sputum and pus specimens of the same patient. H(37) R(v) ofloxacin resistant and sensitive strains had an identical DNA fingerprinting pattern. CONCLUSIONS: Identification of M. tuberculosis strains is feasible by DNA fingerprinting technique.

[The value of 99mTc-MIBI image axillary lymph node in selecting operative method for breast cancer].

OBJECTIVE: To retain axillary lymph node not infiltrated by cancer cell. METHODS: Breast cancer was diagnosed by fine needle aspiration cytology (FNA) or tumor biopsy before operation. Axillary lymph node was examined by surgery and (99m)Tc-MIBI image. Pathological examination of the axillary tissue after operation is used as gold standard in comparison of the two examination methods. RESULTS: The sensitivity, specificity, and accuracy of surgical examination are lower than those of (99m)Tc-MIBI image (P < 0.05). CONCLUSIONS: (99m)Tc-MIBI image combined with surgical examination can be used as a method for retaining axillary lymph node.

[Relationship between PDGF-A, bFGF and proliferation of smooth muscle cells in vein grafts].

OBJECTIVE: To detect the relationship between platelet-derived growth factor-A (PDGF-A), basic fibroblast growth factor (bFGF), and proliferation of smooth muscle cells in vein grafts. METHODS: An animal model of autogenous vein graft was established by transplanting internal branch of the jugular vein to the common iliac artery by end-to-end anastomosis. 25 rats were used. The grafted veins were harvested at 6 h, 2 d, 1 w, 2 w and 4 w respectively after the operation. The expression of PDGF-A and bFGF in different stages after grafting procedure was observed immunohistochemically. RESULTS: The expression of PDGF-A peaked at 1 w, paralleled with proliferation of SMC, and was significantly higher than that at 6 h (P < 0.01). The expression of PDGF-A diminished gradually. At 4 w, it was significantly lower than that at 1 w (P < 0.01). bFGF demonstrated bimodal pattern of the expression: the first peak of expression occurred by 2 d and the second peak by 2 w. The difference was significantly compared with other stages. CONCLUSIONS: The expression of PDGF-A 1 contributed to migration and proliferation of SMC. Except for contributing to proliferation of SMC, bFGF perhaps took part in endothelization.

[Orbito-zygomatic approach for removal of tumors of the skull base].

OBJECTIVE: To evaluate the advantages and indications of orbito-zygomatic approach for removal of tumors of the skull base. METHODS: Fifteen patients with tumors of the skull base were operated on using obito-zygomatic approach. The advantages and indications of the approach were evaluated. RESULTS: In 7 cases of meningioma, total removal of the tumor was achieved in 6 cases, and subtotal removal in 1. In 4 cases of craniopharyngioma, the tumor were totally removed in 3 cases, and subtotally removed in 1. The tumor were totally removed in 2 cases of trigerminal neurilemoma and 1 case of germinoma. There was one operative death. All patients recovered except 3 who had minor extraocular parasis. CONCLUSIONS: This approach ensures additional space, and less cerebral retraction for easy access to skull base tumor, especially the large neoplasm of middle cranial base with or without orbital or posterior cranial base extension.

[Timing of vitrectomy for proliferative diabetic retinopathy in cases with type II diabetes mellitus].

OBJECTIVE: To determine the timing of vitrectomy for proliferative diabetic retinopathy with severe vitreous hemorrhage, retinal traction and even tractional detachment resulted from fibrovascular membranes in type II diabetes mellitus. METHODS: According to the preoperative duration of vitreous hemorrhage and severity of diabetic retinopathy, 78 eyes of 64 cases having undergone vitrectomy were divided into 2 groups for the comparison of postoperative visual acuity. RESULTS: After 9 months of follow-up, in the group with preoperative vitreous hemorrhage of or= 0.3, while in the group with preoperative vitreous hemorrhage of > 6 months' duration 5.3% of cases reached such a level (P < 0.01). After one year, in the group with preoperative vitreous hemorrhage of or= 0.5, while in the group with preoperative vitreous hemorrhages of > 6 months' duration, 7.1% (P < 0.05). In the group of vitreous hemorrhage with or without local tractional retinal detachment, 35.7% had visual acuity of >or= 0.5, while in the group with large area of tractional retinal detachment, 16.7% (P < 0.05). CONCLUSION: Earlier vitrectomy is preferable for type II diabetes mellitus with vitreous hemorrhage, and the operative treatment should not be abandoned for the patients with vitreous hemorrhage of longer duration and severe tractional retinal detachment.
