[Partial trisomy of the long arm of chromosome 2 by malsegregation of a maternal translocation t(2;7)(q321;p22)].

A patient with partial trisomy 2q due to the malsegregation of a balanced maternal (2;7) translocation is reported. The proposita, who died a few hours after birth, presented the characteristic clinical features: microcephaly, prominent forehead, hypertelorism, depressed nasal bridge, upturned nostrils. Her karyotype was 46,XX,der(7),t(2;7)(q321;p22) mat.

[Child mortality in the Hauts de Seine district].

The authors present the results of the analysis of mortality statistics among the children who were born in 1977 in the French department of Hauts-de-Seine. The data were obtained from 8th day of life health certificates, from information given by the registry office and from hospital records. The results establish the department in an excellent position: child mortality rate: 7.34%, early neonatal mortality: 2.81%, late neonatal mortality 1.45% and post-neonatal mortality: 3.08%. The causes for child mortality are analysed and suggestions concerning possible improvements are made.

[Incidence, prognosis and prevention of septicaemias in patients under treatment for acute leukaemia (author's transl)].

Septicaemias are frequent and severe in patients with acute leukaemia under aplastic treatment. The present study concerns 69 such patients: 29 with acute lymphoblastic leukaemia (ALL), and 40 with acute non-lymphoblastic leukaemia (ANLL). All were treated in single rooms in the same hospital and in similar conditions. The overall incidence of septicaemia was 62%; it was 60% in patients with recently diagnosed ALL and 68% during relapses. More than 34% of ALL patients and 82.5% of ANLL patients had one or several episodes of septicaemia. Among the 74 pathogens isolated 50% were Gram-positive organisms, 45% Gram-negative organisms and 5% Candida spp.. The first episodes of septicaemias were predominantly caused by Gram-positive spp. (61%) and the subsequent ones by Gram-negative spp. (60%). The primary infection could only be diagnosed in 19% of the cases and was most frequently located in the digestive tract or perineal region. The most common focal complications were lung infections (18 cases), skin infections (12 cases) and septic shock (15 cases). Seventy-four p. cent of the patients survived with prompt and potent antibiotic therapy. Death occurred in 26% and was clearly related to the following factors: chemotherapy of relapsed leukaemia and/or blastic aplasia and/or successive episodes of septicaemia. The incidence and severity of septicaemias in leukaemic patients will only be reduced by improved prophylactic measures against infection and by less pronounced and shorter chemotherapy-induced granulocytopenia.

Acute Disease↗

[Myoglobinuria following anesthesia (author's transl)].

In a 11 1/2 year-old boy, acute myolysis with myoglobinuria followed anesthesia and had a favourable outcome. Biological signs of myolysis, without clinical symptoms seem to be frequent after Halothane-succinylcholine anesthesia. They are even more frequent in children, especially when succinylcholine is repeatedly given during Halothane anesthesia. Patient clinical symptoms (visible myoglobinuria, pain and/or paralysis) are rare and occur chiefly in children with chronic muscular disease, in whom they may be the presenting symptoms.

[Neonatal cardiac tumour: diagnostic and therapeutic problems. One case (author's transl)].

We report a case of cardiac tumour, presenting as cardiac failure during the first hours of life and refractory hypoxia. The diagnosis was made with difficulty by angiography which revealed a filling defect in the right atrium, whilst echocardiography had shown only a localised pericarditis. The tumour (which extended into the pericardium) was found to fill the right atrium and appeared to extend towards the venae cavae and the left atrium, contraindicating any attempt at excision. The newborn infant died on D7 of a strictly intra-atrial well-limited fibromyoma.

[Antibiotic therapy, intestinal microbial pullulation and risk of infection in children].

The effect of antibiotic therapy on the intestinal flora was studied qualitatively and quantitatively in 41 infants. The results have been compared with 27 normal children of the same age and background. Antibiotics were responsible for the suppression of sensitive strains and for their replacement by resistant organisms but above all to a rapid multiplication of the intestinal flora. Colistin and pristinamycin caused these changes when given orally. Ampicillin when given both orally and parenterally but Colistin and the aminoglycosides when given parenterally did not have any effect. Fourteen cases of secondary septicaemia due to resistant organisms were observed but other factors were also important, namely the young age of the patients and intestinal problems (stasis and diarrhoea).
